Doppler ultrasonographic grading of hepatic vascular malformations in hereditary hemorrhagic telangiectasia -- results of extensive screening.

AIMS: In previous studies, the prevalence of hepatic vascular malformations (VMs) in a large Italian family with hereditary hemorrhagic telangiectasia (HHT) was examined by Doppler ultrasonography (US) as screening technique, and the relevant Doppler US findings were described and classified. Thereafter, Doppler US has been routinely used to screen HHT families for liver involvement. Hepatic VMs were evaluated and classified on the basis of Doppler US findings. METHODS: Three hundred and forty-six subjects belonging to 64 pedigrees were checked for the presence of signs of HHT. All of them underwent abdominal Doppler US screening for hepatic VMs. Vascular abnormalities were classified as minimal if the hepatic artery was dilated in extrahepatic tract only and measured > 6 mm; as moderate if the hepatic artery was dilated in both intra and extrahepatic tract; and as severe if complex changes of the arterial hepatic branches were associated with hepatic and/or portal vein dilatation. Furthermore, Doppler parameters, both qualitative (flow direction, turbulence) and quantitative (peak flow velocity and resistivity index in hepatic artery, mean velocity in portal vein, diastolic peak flow velocity in hepatic veins), were entered into our VM classification. RESULTS: HHT was found in 222 subjects, with hepatic VMs detected by Doppler US in 92 (41.4 %) (24 males, 68 females, mean age 52.2). Hepatic VMs were minimal in 11 subjects, moderate in 70, and severe in 11. CONCLUSIONS: On the basis of our proposed grading, hepatic VMs can be easily classified in subjects with HHT by Doppler US. Depending on the degree of hepatic vascular derangement, appropriate programs for follow up and/or therapy can be designed.

Incidence of alcohol abuse in the parents of medical students.

An anonymous self-report survey of the medical students at the University of Colorado Health Sciences Center assessed how many of the students' parents were alcohol abusers. The results indicated that 27% of the student body were children of alcohol abusers, a rate twice that of the general population. This finding has implications for teaching strategies, student substance abuse, prevention efforts during the medical-school years, and provision of appropriate programs through student health services.

Dietary assessment issues of concern to policymakers: statement from the Food and Agriculture Organization of the United Nations.

Since its establishment in 1945, the Food and Agriculture Organization of the United Nations (FAO) has worked with member countries to increase food production and food supplies and improve the nutritional well-being of the world population. Dietary assessment is an important component in the process of identifying groups that are nutritionally at risk and in developing appropriate programs and policies to improve nutritional well-being. However, simpler, more cost-effective methods are needed that will allow rapid analysis of survey results so that information can be readily available to policymakers at all levels. FAO continues to work toward development of methods to improve nutrition monitoring and surveillance for developed and developing countries.

MacP12: a protein property multi-profile plot program for the Apple Macintosh.

MacP12, a program for the Apple Macintosh, allows simultaneous plotting of two protein property profiles selectable from 12 built-in amino acid property scales. Various parameters such as the region to be analyzed, the size of the sliding window, the weighting function and the size of the graphical output can be easily adjusted by the user, which makes this program appropriate for diverse research questions. Since built-in scales can be simply exchanged, MacP12 is adaptable to the specific needs of the individual user.

Continuous collection of pulse oximetry data: a new, inexpensive, portable computerized method.

The availability of reliable pulse oximetry equipment has led to interest in identifying patterns of hypoxaemia in the postoperative period. Methods for the computerized collection and analysis of pulse oximetry data have been described, but these require continuous use of a relatively powerful computer system throughout both the monitoring (data collection) and analysis periods. We have designed a technique which uses a small, portable and relatively inexpensive computer unit for data collection, after which the data may be transferred to a more powerful computer for analysis. Appropriate programming and choice of software have produced a relatively "user friendly" system which can be operated successfully even with minimal computing experience. The unit has the potential to be modified to form the basis of a "medical advice system" which could be used for the "intelligent" monitoring of high risk patients.

Physical therapists as scar modifiers.

Hypertrophic scars, a common sequelae to burn injuries, represent an overgrowth of dermal components like collagen. To the burn-injured patient, the red, raised, rigid scars represent impaired function and distorted appearance. Physical therapists modify scars by treating with pressure and stretching to minimize the devastating effects. Healing time, location of the injury, condition of the unhealed and healed areas, and treatment cost influence physical therapy. This article describes scarring and provides practical guidelines for pressure materials and stretching techniques. With an appropriate program, caring for a patient with burns becomes extremely beneficial and rewarding.

Immigrant policy: issues for social work practice.

Immigrants make up a significant segment of U.S. society. Immigration to the United States has been characterized by steady growth, dramatic changes in ethnic composition, and declining socioeconomic levels. The challenge for social work is to respond to the social services needs of immigrants by designing appropriate programs that will contribute to the social and economic integration of immigrants. This article provides an overview of the major policy issues relevant to social work practice with immigrants and describes the recent U.S. immigrant population. It discusses current federal policy that affects service provision to immigrants, defines immigrant eligibility for social services, outlines the major areas of need among immigrants, and considers implications for social work practice.

Differentiation lineage-specific expression of human heat shock transcription factor 2.

Differentiation of multipotential hematopoietic cells into lineage-committed precursors involves the selection and maintenance of appropriate programs of gene expression, regulated by specific transcription factors. Using human K562 erythroleukemia cells capable of differentiating along erythroid and megakaryocytic lineages, we explore the differentiation-related role of heat shock transcription factor 2 (HSF2), which belongs to a family of transcription factors generally known to regulate heat shock gene expression. We demonstrate that enhanced HSF2 expression and the acquisition of HSF2 DNA binding activity are strictly specific for erythroid characteristics of K562 cells. Our results reveal a multistep regulatory process of HSF2 gene expression. In K562 cells undergoing hemin-mediated erythroid differentiation, the increase in HSF2 protein levels is preceded by transcriptional induction of the HSF2 gene, accompanied by increased HSF2 mRNA stability. In contrast, during megakaryocytic differentiation induced by the phorbol ester TPA, expression of HSF2 is rapidly down-regulated, leading to a complete loss of the HSF2 protein. These results indicate that the determination of HSF2 expression occurs at the early stages of lineage commitment. Taken together, our data suggest that HSF2 could function as a lineage-restricted transcription factor during differentiation of K562 cells along either the erythroid or the megakaryocytic pathway.

Dealing with the news media: effective communication strategies for medical examiners.

Medical examiners have responsibilities that often result in news media coverage that can bring unwanted attention. Dealing with the news media is always a learning process, but various resources exist to help in maintaining credibility. Local offices should consider establishing proactive community relations and media relations programs. Appropriate steps should be taken to manage media relations, including the appointment of an official spokesperson and regular use of the prepared statement. This article also explores the benefits of being prepared for media interviews, offers advice for responding to the media following a mass disaster, and examines options for dealing with a media investigation. Medical examiners, coroners, investigators, and other forensic experts have responsibilities that frequently bring them in contact with the news media. Today, a crime scene investigation is not complete without media attention. We now live in an era of "CNN journalism," in which local investigations may be propelled into the national spotlight. Unfortunately, the result can be unwanted attention. What resources can medical examiners rely on to better manage the news media? What can you do to maintain credibility? There are a number of options available, all of which can easily put into practice.

Nurse endoscopist training: the next step.

Nurses have been successfully performing flexible sigmoidoscopy since the early 1970s. There are numerous studies, in both medical and nursing literature, indicating that nurses can perform this traditional physician role safely and efficiently. Both the American Society of Gastroenterology Endoscopists (ASGE) and Society of Gastroenterology Nurses and Associates (SGNA) have endorsed this practice. Programs appropriate for training the nurse endoscopist, however, have not been defined. Although similarities exist in the literature about how nurses and other nonphysician endoscopists are trained, there are also many discrepancies. If nurses are to take a proactive stance in overseeing nursing practice, objective criteria and clinical competencies need to be established. This is the next step in establishing standard acceptance by the medical community and public.

Life-style changes and coronary heart disease: the influence of nonpharmacologic interventions.

Life-style habits such as diet and exercise can have powerful affects on the development and progression of coronary heart disease. This article presents evidence supporting the use of these two modalities in the treatment of coronary heart disease. Diet is discussed in terms of cholesterol, obesity, fiber, fish oils, and antioxidants. Exercise is discussed in terms of the preparation for and the components of an exercise program. Appropriate nursing interventions are offered for use when working with patients who must modify their dietary or exercise habits.

Evaluation of stroke performance in tennis.

In the present studies, the Leuven Tennis Performance Test (LTPT), a newly developed test procedure to measure stroke performance in match-like conditions in elite tennis players, was evaluated as to its value for research purposes. The LTPT is enacted on a regular tennis court. It consists of first and second services, and of returning balls projected by a machine to target zones indicated by a lighted sign. Neutral, defensive, and offensive tactical situations are elicited by appropriately programming the machine. Stroke quality is determined from simultaneous measurements of error rate, ball velocity, and precision of ball placement. A velocity/precision (VP) an a velocity/precision/error (VPE) index are also calculated. The validity and sensitivity of the LTPT were determined by verifying whether LTPT scores reflect minor differences in tennis ranking on the one hand and the effects of fatigue on the other hand. Compared with lower ranked players, higher ones made fewer errors (P < 0.05). In addition, stroke velocity was higher (P < 0.05), and lateral stroke precision, VP, and VPE scores were better (P < 0.05) in the latter. Furthermore, fatigue induced by a prolonged tennis load increased (P < 0.05) error rate and decreased (P < 0.05) stroke velocity and the VP and VPE indices. It is concluded that the LTPT is an accurate, reliable, and valid instrument for the evaluation of stroke quality in high-level tennis players.

Factors related to self-concept of elderly residing in a retirement center.

The purpose of this study was to explore the self-concept of elderly adults residing in retirement centers as well as its relationship to other variables, including function of activities of daily living (ADL), subjective well being, etc. Selected demographic attributes (sex, age, years of education etc.) were also examined for their relationship to self-concept. The subjects were a convenience sample of people aged 65 or older who were recruited from a retirement center in the south of Taiwan (N = 42), and a control group of elderly living with their family (N = 33). The Tennessee Self-Concept Scale (TSCS) was used to measure self-concept. The Multidimensional Functional Assessment Questionnaire- Chinese Version (MFAQ) was applied to measure the function of activities of daily living. A total of 37 subjects from the retirement center group and 28 subjects from the home group completed these questionnaires. The data were analyzed using SPSS/PC software version 10.0. After multiple linear regression analysis, the results showed that the elderly residing in the retirement center had significantly lower Personal Self and Family Self scores. The variables of ADL, sex and subjective well-being had effects on the total self-concept. Female elderly individuals also revealed significant higher scores than males in the subscales of Moral- Ethical, Family Self, Social Self, Identity, Satisfaction Self. These results suggest we should design appropriate programs to increase elderly people's interaction with others and establish new social networks for them which may enhance a sense of positive self-concept.

Decubitus ulcers and rehabilitation medicine.

The approach to management of decubitus ulcers is challenging and complex. Management should be targeted to prevention. Many factors will influence the development of ulcers and their healing. These factors include physiologic as well as sociologic elements. The physician's approach must be to consider all potential contributing factors and to translate these into an appropriate program of prevention and education and treatment where necessary. The treatment program should be based on physiologic principles, but should also take into account the full patient and his psycho-social emotional needs. Patient cooperation is very important. With good medical care and full cooperation of the patient, management can achieve a healed wound and a restored patient. If either of these elements are not present, the complications may include chronic local infection with abscess formation, osteomyelitis, sepsis, amyloidosis, and death.

Barriers to health promotion activities in public hospitals.

Despite the central role hospitals have in the health care system, relatively few health promotion activities are conducted in Australian public hospitals. This study investigated the types of obstacles that were perceived to inhibit health promotion activities in hospitals. A questionnaire for self-completion was sent to medical superintendents in all public hospitals in Queensland and 112 questionnaires were returned (92.6 per cent response rate). The results indicated that lack of finance, lack of interest by relevant others, and needs (for appropriate programs, training and patient receptivity) were the barriers reported by superintendents. The barriers of 'interest' and 'needs' were related to a lack of written policies in some areas, but not directly to levels of other health promotion activities being conducted in the hospitals. Success in facilitating health promotion programs in hospitals will need to include a change in the environment, in particular the views of medical superintendents. The combination of attitude change and the availability of a motivated person (such as a health promotion officer) to lead the activities may be needed in order to produce an increase in the level of health promotion in public hospitals.

Observations on the psychopharmacology of the aged.

Psychotropic drugs are discussed under the descriptive categories of antipsychotic, antidepressive, antimanic, anti-anxiety, and cognitive acting. In the antipsychotic classification, special attention is given to side effects (extrapyramidal motor signs, tardive dyskinesias, akathisis) and to dosage for the elderly. The section on congenitive acting drugs includes some pertinent observations on cognitive ability in the aging, the importance of correct diagnosis, and therapeutic strategy. The aged are a population at risk for not only disease, but iatrongenic illness due to direct and indirect drug action. Rational therapy involves understanding the underlying dynamics of the disease and thus the selection of the most effective treatment. Psychotropic drugs in an appropriate program are valuable therapeutic assets.
