Breastfeeding as an intervention within diarrheal diseases control programs: WHO/CDD activities.

The Control of Diarrhoeal Diseases Programme (CDD) at the World Health Organization (WHO) is looking at ways to prevent diarrhea and has identified breastfeeding as an important factor. CDD is studying the protective effects of exclusive breastfeeding and the morbidity and mortality risks for infants exclusively, partially, or not breastfed. CDD has identified trials of interventions for breastfeeding promotion and the evaluation of approaches for the implementation of tested interventions among its research priorities. CDD also has included breastfeeding promotion information in modules in its two management training courses. The Programme plans to monitor the effectiveness of the training and develop future activities based on that feedback.

From genes to geometry: Controlling embryo models by programming genomic activation.

Embryo-like models derived from stem cells have emerged as powerful tools to study early development. In this issue, Lodewijk et al.1 demonstrate that activating just two enhancers via CRISPR activation (CRISPRa) in mouse embryonic stem cells (ESCs) can drive self-organization into structured embryo-like models, offering a genome-driven approach in stem cell and developmental biology.

Results of a multicenter trial using an intensive active exercise program for the treatment of acute soft tissue and back injuries.

Dissatisfaction with current nonsurgical treatment of acute soft tissue and back injuries initiated a search for more effective treatment. A multicenter trial, involving 12 clinics, treated 1,072 patients. Treatment consisted of intensive, time-limited exercises emphasizing mobility, muscle strengthening, work conditioning, sequence training, and appropriate education sessions. Return to full-time work was proof that the patient had recovered fully. Time off work and compensation costs were parameters used to evaluate results. A detailed study of 703 patients treated at five clinics, operational for more than 12 months, showed superior results when compared with the comparison group of 2,172 matched control subjects. The treatment group returned to work earlier and realized substantial cost savings. This treatment program has proved to be superior to the miscellaneous variety of treatment modalities used for acute soft tissue and back injuries provided to the comparison group.

Back Injuries

Replication program of active and inactive multigene families in mammalian cells.

In a comprehensive study, the temporal replication of tissue-specific genes and flanking sequences was compared in nine cell lines exhibiting different tissue-specific functions. Some of the rules we have determined for the replication of these tissue specific genes include the following. (i) Actively transcribed genes usually replicate during the first quarter of the S phase. (ii) Some immunoglobulin genes replicate during the first half of S phase even when no transcriptional activity is detected but appear to replicate even earlier in cell lines where they are transcribed. (iii) Nontranscribed genes can replicate during any interval of S phase. (iv) Multigene families arranged in clusters of 250 kilobases or less define a temporal compartment comprising approximately one-quarter of S phase. While these rules, and others that are discussed, apply to the tissue-specific genes studied here, all tissue-specific genes may not follow this pattern. In addition, housekeeping genes did not follow some of these rules. These results provide the first molecular evidence that the coordinate timing of replication of contiguous sequences within a multigene family is a general property of the mammalian genome. The relationship between replication very early during S phase and the transcriptional activity within a chromosomal domain is discussed.

[The Armenian SSR program of active screening for tumors of the digestive tract].

A procedure for identification of groups at high risk for GI tumors was introduced in polyclinics of the Armenian SSR. Complex examination of GI tract was conducted. A questionnaire, occult blood and peripheral blood fibrinogen tests were used in a population of 49,904. 6,663 subjects (13.4%) were examined endoscopically: esophagogastroduodenoscopy--4,997 and colonoscopy--1,666 cases. Malignant tumors of GI tract were detected in 0.1%, benign--in 0.5% of screenees.

Supporting evidence for an active treatment program for advanced salivary gland carcinomas.

The role of systemic chemotherapy in the treatment of salivary gland tumors remains unknown. Because of the infrequent occurrence of these tumors, only a few clinical trials with chemotherapy have been reported in the literature. The current trial presents preliminary results with the combination of cyclophosphamide, doxorubicin, and cisplatin. These results are viewed in terms of other recently reported data obtained with the same combination. The collected data strongly suggest that salivary gland tumors may be very responsive to chemotherapy.
