Preventive dentistry--a priority in Sri Lanka.

Sri Lanka, being a developing country, cannot resort to high technology and sophisticated dentistry to achieve coverage for its whole population. The most cost effective means of controlling and preventing oral diseases has to be used and the primary health care approach with its basic philosophy of self-reliance and community participation will contribute substantially to the improvement of oral health of Sri Lankans. This paper outlines the decisions made in relation to preventive dentistry and the epidemiological data on which the goals were based.

[Preventive dentistry in France].

In France, oral prevention has recently progressed in various aspects of dental caries prophylaxis which is reviewed in this paper. A better knowledge of dental caries prevalence has been obtained at a national level and the French Government authorized in October 1985 salt fluoridation for a period of 5 years which in addition to other means of caries prevention such as fluoridated toothpastes and various local and topical procedures has increased the potentials of fluoride use in France. In addition a larger use of fissure sealants has been encouraged. It is hoped that the introduction of salt fluoridation in France will reduce the caries incidence to a DMFT not more than 3 for the 12 year old children thus fulfilling the WHO goals for oral health in the year 2000.

Five years of community preventive dentistry and health education in the county of Sutherland, Scotland.

Examination of 11-year-old Scots children who had participated in a 5-year, fortnightly dental health education and preventive dentistry programme showed that, compared with a similar age-matched group, the regimen produced mean DMF and DMFS reductions of 48.6% and 42.1%, respectively. In addition, the plaque and gingival indices differed significantly and 22.5% of test children were caries-free compared with only 7.6% of controls.

Sumter Smith Arnim, DDS, PhD (1904-1990): a pioneer in preventive dentistry.

This article reviews the life story and contributions of Sumter Smith Arnim (1904-1990), a noted author, lecturer, teacher, and researcher in preventive dentistry. Following in the footsteps of Levi Spear Parmly, MD, DDS (1790-1859), and Charles C. Bass, MD (1875-1975), Arnim developed and popularized a wide range of preventive concepts and practices, especially dental plaque control via mechanical means. Today, these innovative ideas and techniques continue to have an impact on oral health research, the practice of dentistry, and the teaching of oral hygiene to dental patients. This article traces both the development of these oral health concepts and Arnim's study of dental plaque ("microcosm"), which he characterized by means of phase-contrast microscopy and cinemicrography.

A preventive dentistry training program for caretakers of persons with disabilities residing in community residential facilities.

People with disabilities develop more dental disease than the average population and have a harder time accessing and using resources for dental treatment. It is therefore critical to prevent dental problems in this population. This article discusses the development and use of a preventive dentistry training program consisting of a videotape, workbook, instructions for trainers, and pre- and post-tests. The training program was designed to facilitate integration of dental and behavioral information into daily routines. Participants were able to demonstrate that they could learn the information in the training materials using the methodology that was employed. Further work remains to be done to revise and disseminate the materials and to demonstrate that the knowledge that the participants gained can be effectively applied in community care settings.

Needs and implementation of preventive dentistry in China.

Based on a literature review and recent surveys, the needs and implementation of preventive dentistry in China have been estimated. The average DMFT of 12- and 15-yr-old schoolchildren in Beijing were 1.61 and 2.11 respectively. The caries prevalence was higher in the coastal cities in the south. The caries rate was also higher in urban areas than in rural areas. There were some controversies in the results of fluoridation studies with one study reporting as high as 47.2% of the children to be afflicted with enamel fluorosis. The community periodontal disease status and treatment needs index (CPTNI) showed that 93% of the 12-yr-olds had bleeding, 98% had calculus and 15% had shallow pockets, with 100% of the children needing prophylaxis. There was a high prevalence of dental malocclusion with 54% of schoolchildren with dental anomalies, of whom 36% needed treatment. Much emphasis had been placed on oral pathology, oral medicine and maxillofacial surgery in the curriculum of Stomatology in Chinese dental schools.

The economics of pit and fissure sealants in preventive dentistry: a review.

Oral diseases are progressive, cumulative, and become more intricate to treat with advancement. While dental caries is an infectious transmissible disease with children being at the highest risk, primary prevention can reduce this risk. Primary prevention in dentistry is usually considered to be community fluoridated water supplies, professional fluoride treatments, and pit and fissure sealants. While community fluoridated water supplies have been proven to be cost-effective, the cost-effectiveness of pit and fissure sealants has primarily been studied in school-based programs of children from low socio-economic backgrounds. Dental sealant programs are just one way to help increase primary prevention in the oral health disparities of children. The purpose of this paper is to review the literature regarding the cost-effectiveness of pit and fissure sealants as a preventive strategy in preventive dentistry.
