Comparison of live attenuated and inactivated influenza vaccines in schoolchildren in Russia: I. Safety and efficacy in two Moscow schools, 1987/88.

The performance of two doses of cold-adapted live attenuated vaccine versus one dose of whole-virus inactivated vaccine was compared in 8-15-year-old schoolchildren in two schools in Moscow, Russia, during the winter of 1987/88. Both vaccines gave rise to low frequencies of associated febrile or systemic reactions, but the inactivated vaccine, delivered by jet injector, did cause small local reactions in about half of the children. Immunogenicity was higher for both vaccines in antibody-free children, and higher levels of serum antibody were detected following use of inactivated vaccine. During the winter, influenza A (H3N2) and influenza B viruses circulated in Moscow. A clear outbreak of (H3N2) virus occurred in both schools, and infections with type B virus also occurred in one school. The influenza A/Philippines/2/82 (H3N2) component of both vaccines exhibited protective efficacy of about 40% (p < 0.05) against serologically proven infection caused by the antigenically drifted A/Sichuan/2/87 (H3N2)-like epidemic viruses in one school. In another school where illnesses associated with antibody rise were documented, efficacy was seen for both vaccines in reduction of illnesses, and of illnesses with serological evidence of infection, but statistical significance was not achieved.

Alcohol consumption patterns among pregnant women in the Moscow region of the Russian Federation.

Data regarding the prevalence and patterns of alcohol consumption among pregnant women in the Russian Federation is lacking. As part of a longitudinal pregnancy outcome study being conducted in the Moscow Region of Russia, in the 5-month period from January through May 2005, pregnant women in four prenatal care facilities were screened for self-reported alcohol consumption in the month around the time of conception and in the most recent month of pregnancy. Among the 413 respondents, 347 (85.0%) reported some alcohol consumption during one of the two time periods, and 193 (51.9%) of these drinking women reported some alcohol use in the most recent month. Of particular concern was the pattern of drinking, with 75 (20.2%) of drinking women reporting at least one episode of five or more drinks around the time of conception, and 153 (41.1%) of drinking women reporting at least one episode of three or four drinks during that same time period. Furthermore, this same pattern of heavier episodic drinking was reported by 18 (4.8%) and 39 (10.5%) of drinking women, respectively, in the most recent month in pregnancy before the screening interview. These data indicate that pregnant women in these areas of the Moscow Region present an important opportunity for education and intervention for alcohol-related birth outcomes.

Characterization of Borrelia burgdorferi sensu lato isolated in Moscow province--a sympatric region for Ixodes ricinus and Ixodes persulcatus.

The relationships between Borrelia species and the vector ticks, Ixodes persulcatus and Ixodes ricinus were examined in a molecular epidemiological study. We conducted a survey in the Moscow region which is a sympatric region for both species of tick. We examined 630 unfed I. ricinus and I. persulcatus, ticks collected from four different regions around Moscow within an area of 250 km2. Eighty-four ticks were culture positive (13.3%) and the prevalence rate varied in each region from 5.7% to 42.3%. No difference was found between the total prevalence rate for both species. Eight Borrelia afzelii-like variant isolates from I. ricinus and Clethrionomys glareolus were identified as B. afzelii by flagellin gene and 16S rDNA sequence analyses. Most isolates from I. ricinus were identified as Borrelia garinii type 20047 and B. afzelii. Two isolates were identified as Borrelia burgdorferi sensu stricto (s.s.) and Borrelia valaisiana, respectively, but no B. garinii type NT29 was found. In contrast, isolates from I. persulcatus were identified as both types 20047 and NT29 of B. garinii, and B. afzelii. No B. burgdorferi s.s. isolate was found among isolates from I. persulcatus.

Molecular epidemiology of hepatitis B, C, D and E viruses among children in Moscow, Russia.

BACKGROUND: It is known that the prevalence of HBV and HCV infections vary according to geographical areas. However, in Russia, an adequate level of information on the molecular epidemiology of hepatitis viruses has not been available so far. OBJECTIVES: To investigate the characterization of various hepatitis viruses in Russia, we conducted molecular-based epidemiological survey of hepatitis viruses including hepatitis B virus (HBV), hepatitis C virus (HCV), hepatitis D virus (HDV) and hepatitis E virus (HEV) among children in Moscow, Russia. STUDY DESIGN: The study population of 374 subjects (ranging in age from 1 to 14 years old) consisted of 195 patients with liver diseases and 179 patients without liver diseases. Viral DNA/RNA was determined by nested PCR. Genotyping of HBV and HCV were examined by PCR using type-specific primers. Anti-HEV antibody was assayed by ELISA. RESULTS: The infection rate of each virus among patients with liver diseases including acute hepatitis, chronic hepatitis or cirrhosis was 65.6% for HBV and 15.9% for HCV. In contrast, among non-liver disease patients, the infection rates were 14.4% for HBV and 0.6% for HCV, respectively. The most common viral genotypes were type D (85%) of HBV and type 1b (79.3%) of HCV. HDV RNA was detected in 7 of 149 (4.7%) HBV DNA-positive children tested. Moreover, testing for HEV among 341 subjects resulted in the detection of anti-HEV IgG in 62 cases (18.2%). CONCLUSIONS: Our results suggest that HBV infection is widespread in Moscow and have led to a high incidence of acute and chronic liver diseases among children in this region.

Black tea consumption and risk of rectal cancer in Moscow population.

PURPOSE: This population-based case-control study (663 cases and 323 controls) examined the effect of black tea intake on the risk of rectal cancer in Moscow residents. The Moscow population was selected for its wide range of black tea consumption. METHODS: This study used three measures of tea consumption: the volume of beverage (l/month), zavarka (tea concentrate, l/month), and dry tea (g/month). We calculated the adjusted odds ratios (ORs) and 95% confidence intervals (CIs) for these three parameters of tea intake using logistic regression. RESULTS: Greater use of dry tea was associated with lower risk of rectal cancer in women (high vs. low: OR = 0.40; 95% CI, 0.23-0.70) and in men (high vs. low: OR = 0.77; 95% CI, 0.42-1.43). The observed effect was weaker when tea was measured as zavarka (high vs. low in women: OR = 0.47; 95% CI, 0.26-0.83; in men: OR = 0.99; 95% CI, 0.52-1.96) and as beverage volume (high vs. low in women: OR = 0.68; 95% CI, 0.39-1.19; in men: OR = 1.03; 95% CI, 0.53-2.09). CONCLUSIONS: These findings support the hypothesis that black tea consumption reduces the risk of rectal cancer. The attenuation of the effect across the three measures of tea intake can be explained by an increasing degree of exposure misclassification from dry tea to zavarka and beverage volume.

Latex aeroallergen quantification in hospitals of Moscow, Russia.

BACKGROUND: Health care workers (HCWs) who use latex gloves regularly are at a significant risk for developing allergic sensitization to natural rubber latex (NRL) proteins. The airborne route of exposure presents a hazard for sensitization and subsequent allergic symptoms. OBJECTIVE: The purpose of this study was to measure the content of the NRL proteins in the air of work areas (laboratories, treatment rooms, operating rooms, examination rooms, and dentist's treatment rooms) in eight Moscow hospitals where the employees used powdered latex gloves. METHODS: The air samples were collected by a volumetric filtration method using a static air sampler at a flow rate of 3 L/second. Airborne particles were collected onto polytetrafluoroethylene filters rated 99.9% efficient at 0.3 microm. The NRL allergens were extracted from the air filter samples in phosphate-buffered saline and quantified by an inhibition immunoassay using a five-person pool of sera containing latex-specific human immunoglobulin E. RESULTS: The airborne latex allergen concentrations in 11 areas (including positive control) where latex gloves were used varied from 5 to 26 ng/m3. Conversely, in 12 areas where latex gloves were seldom used or where powdered-free latex gloves were used allergen levels were not detectable (including two negative controls). The highest concentration of latex allergen (26 ng/m3) was found in an examination room of a gynecology department. Low and moderate concentrations were detected in most operating rooms, and low concentrations in the laboratories and treatment rooms. CONCLUSIONS: This study indicates that, in general, the airborne NRL concentrations in the work areas of Moscow hospitals are rather low compared with NRL concentrations reported from American hospitals and are dependent on the intensity of latex glove usage.

General Mikhail Kutuzov: one-eyed defender of Moscow.

General Mikhail Kutuzov was a charismatic Russian general, most remembered for his defense of Moscow against Napoleon. He was in and out of favor with the emperor, Alexander I, but in times of crisis, on more than one occasion he was called back to lead the Russian Forces. From the standpoint of ophthalmology, it is of interest that while fighting the Turks, Kutuzov sustained two separate severe head wounds that ultimately led to loss of sight in his right eye. A portrait of General Kutuzov hangs in the Hermitage Museum in St. Petersburg, Russia, Kutuzov's birthplace. In the portrait Kutuzov is standing with his left side facing forward, presumably to hide his disfigured right eye. He lost the Battle of Austerlitz against Napoleon and at best gained a draw at the Battle of Borodino. Nevertheless, the latter battle was the beginning of the end for Napoleon's Grand Armée. They entered a deserted Moscow, part of Kutuzov's scorched earth policy that left no food or housing for the enemy, and with the Russian winter rapidly approaching, Napoleon began his long retreat from Russia. As with the German army, the harsh winter claimed many casualties while Kutuzov added to the French misery by harassing the retreating army from the rear. Of 450,000 French soldiers, only 10,000 returned to France.

Cancer mortality among Moscow shoe workers exposed to chloroprene (Russia).

OBJECTIVES: To assess the risk of cancer among workers of a Moscow (Russia) shoe factory exposed to chloroprene (2-chloro-1,3-butadiene) (CP). METHODS: This is a retrospective cohort mortality study among 5,185 shoe manufacturing workers employed between 1940 and 1976, and followed from 1979 through 1993. Exposure to CP was assessed by linking the job history with industrial hygiene data. We calculated standardized mortality ratios (SMR) using the Moscow population as reference, and conducted an internal comparison analysis based on Poisson regression modeling. RESULTS: For the entire cohort, all-cause mortality was close to expectation and all-cancer mortality was increased. There was an increase in the mortality from liver cancer (SMR = 2.4, 95 percent confidence interval [CI] = 1.1-4.3), kidney cancer (SMR = 1.8, CI = 0.9-3.4), and leukemia (SMR = 1.9, CI = 1.0-3.3). Mortality from liver cancer and leukemia was associated with various indicators of CP exposure. A similar, although less consistent, pattern was found for kidney cancer mortality; while for the remaining neoplasms, no association was suggested with CP exposure. CONCLUSIONS: The association between CP exposure and risk of leukemia may be due to concomitant exposure to benzene. The results for liver cancer point towards a carcinogenic effect of CP.

Alcohol-related harm and alcohol consumption in Moscow before, during and after a major anti-alcohol campaign.

AIMS: To examine the rates of alcohol-related harm in relation to levels of alcohol consumption before, during and after a major anti-alcohol campaign in Moscow. DESIGN: Changes in State alcohol sale and alcohol consumption and certain forms of alcohol-related harm were observed as a function of time. FINDINGS: Following the 1985 anti-alcohol campaign, State alcohol sales decreased by 38.0% in 1.5 years in Moscow, and total consumption decreased by 28.6%. At the same time, admissions for alcohol-related mental and behavioural disorders, deaths from liver cirrhosis, alcohol poisoning and other blood alcohol positive violent deaths decreased by 63.3%, 33.0%, 50.8% and 50.9%, respectively. There was a linear correlation between medical variables and alcohol consumption during its decrease in 1985-86. An increase in blood alcohol positive violent deaths began in 1987, before the increases in other variables. Growth of total alcohol consumption began in 1987, and continued during all subsequent years, although it was especially high in 1992-93 at the time of the introduction of market reforms in Russia. Alcohol-related mental and behavioural disorders and liver cirrhosis mortality increased after a time-lag following the rise in alcohol consumption. CONCLUSIONS: The findings provide stark evidence of the potential impact of policy measures applied to general alcohol consumption on alcohol-related harm.

The EROP-Moscow oligopeptide database.

Natural oligopeptides may regulate nearly all vital processes. To date, the chemical structures of nearly 6000 oligopeptides have been identified from >1000 organisms representing all the biological kingdoms. We have compiled the known physical, chemical and biological properties of these oligopeptides--whether synthesized on ribosomes or by non-ribosomal enzymes--and have constructed an internet-accessible database, EROP-Moscow (Endogenous Regulatory OligoPeptides), which resides at http://erop.inbi.ras.ru. This database enables users to perform rapid searches via many key features of the oligopeptides, and to carry out statistical analysis of all the available information. The database lists only those oligopeptides whose chemical structures have been completely determined (directly or by translation from nucleotide sequences). It provides extensive links with the Swiss-Prot-TrEMBL peptide-protein database, as well as with the PubMed biomedical bibliographic database. EROP-Moscow also contains data on many oligopeptides that are absent from other convenient databases, and is designed for extended use in classifying new natural oligopeptides and for production of novel peptide pharmaceuticals.

Environmental and social factors as determinants of respiratory dysfunction in junior schoolchildren in Moscow.

BACKGROUND: The process of industrialization of the USSR has left a legacy of widespread and often poorly controlled pollution which is widely believed to have adverse implications for health, in particular for respiratory disease among children. OBJECTIVES: To assess the relationship between area of residence and respiratory function in junior schoolchildren in different districts of Moscow. METHODS: A survey was conducted of 539 children aged 6-12 years who attend school and live in one of three districts of Moscow with varying ambient pollution levels. Spirometry [forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1)] was assessed at school by trained school health staff. Parents of the children completed a questionnaire asking about respiratory function and factors potentially associated with it, as well as about social and other factors that could influence respiratory development and the health status of their children. RESULTS: There was appreciable difference in the characteristics of the children from the three districts. Children from the lower pollutant districts were generally younger, had higher parental income, and were less frequently exposed to cigarette smoke at home. They were also less likely to report heavy lorry traffic in the streets outside their homes. After adjustment for age, gender and height the FVC was 7.6 per cent (3.6-11.5 per cent) lower in children from the medium pollution district and 9.9 per cent [95 per cent confidence interval (CI) 5.6-14.0 per cent] lower in children from the high pollution district compared with those in the least polluted district (p < 0.001 for trend). These differences were little affected by further adjustment for household income or exposure to household smoking. In contrast, FEV1 showed comparatively little variation across districts. The odds of a forced expiratory ratio (FER) <75 per cent were substantially lower in the high pollution compared with the low pollution district (odds ratio 0.10, 95per cent CI 0.03-0.32 after adjustment for age, gender and height), and there was clear evidence of a trend across pollution categories (p < 0.001). The frequency of reported allergy was also lower in the high pollution district. FVC increased, and the probability of a low FER decreased, with household income. CONCLUSION: Children from areas of high environmental pollution had lower lung capacity but also smaller risk of a low FER compared with those from cleaner areas. The extent to which these differences can be attributed to environmental pollution is unclear without more detailed study. However, socio-economic deprivation, which was associated with pollution, appears to be an important determinant of respiratory function although it was associated with a lower risk of an obstructive pattern of lung function tests.

Comparative assessment of pediatric intensive care in Moscow, the Russian Federation: a prospective, multicenter study.

OBJECTIVE: Comparative assessment of pediatric intensive care. DESIGN: Prospective multicenter study. SETTING: Four pediatric intensive care units in Moscow, the Russian Federation. PATIENTS: Consecutive unselected admissions (n = 583), < or = 14 yrs of age, in a 6-month period. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Effectiveness was defined as the ratio of observed to predicted mortality, based on prediction by Pediatric Risk of Mortality (PRISM) severity of illness scoring. Efficiency (on the day of admission only) depended on either a mortality risk of > 1% or the administration of intensive care unit-dependent therapies. In all four hospitals, observed mortality rates were higher than expected, with a range of standardized mortality ratios between 1.10 and 1.83 (mean 1.32). The excess mortality was found in the low- and medium-risk strata (risk of mortality of < 1% to 15%). Admission efficiency ratings did not fluctuate greatly between institutions (mean 60.4%, range 55.7 to 65.9). CONCLUSIONS: We provided a quantitative description and assessment of pediatric intensive care in Moscow. Moderate efficiency may reflect a low threshold for ICU admission due to poor nurse/patient ratios on the wards. Effectiveness in the low- and medium-risk strata is below standard, as compared with a Western reference population. Excess mortality was concentrated in the low- and medium-risk strata, and can only partially be explained by the inclusion of co-morbidity. Future analysis should focus on specific treatment protocols, protocol adherence, and the determination of infectious and therapeutic complications.

Health care in Moscow.

In the Russian Federation privatisation is affecting the health care sector as much as it is industry and commerce. That the general public support the transfer of state clinics to the private sector is a mark of their dissatisfaction with the old state run system. Doctors too see better opportunities to practise good medicine and be paid better for doing so. In Moscow the health department has set up a commission to license all clinics providing treatment, which should ensure standards of safety, training, and equipment. The Russian Federation is also trying to establish a medical insurance system to cover its citizens for health care, but in Moscow and elsewhere its implementation has been delayed by arguments and bureaucracy. In the meantime the health of Muscovites remains poor, with a high incidence of birth defects, and illnesses among the young.

[Polymorphism of angiotensinogen T174M gene and cardiovascular diseases in the Moscow population].

The groups of patients with myocardial infarction (MI) and hypertrophy of the left ventricle (HLV) (n = 45 and n = 53, respectively) and a sample of healthy individuals from the Moscow population (n = 60) were examined for T174M polymorphism of AGT gene (replacement of methionine for threonine at position 174 of the correspondent amino acid sequence). In MI patients the content of TT genotypes and T allele was significantly lower than in the control group (57.8% against 80% and 67.9 against 89.2%, respectively), whereas the proportion of M allele and TM heterozygotes was increased (32.1 against 10.8% and 37.8 against 18.3%, respectively). In patients with HLV, the proportion of TT genotype (64.2%) and T allele (77.4%) was also lower than in the control group, whereas the frequency of M allele was increased (22.6%). Our results suggest that the T174M polymorphism of AGT gene is associated with MI and HLV in the Moscow population.

[Experience in organizing ambulatory control of patients with sarcoidosis in Moscow].

Dispensary record group VIII for patients with sarcoidosis was introduced in Moscow in 1994 and a center based on the Moscow Research Production Center?? was set up to diagnose and treat such patients. In addition to advisory work, the sarcoidosis center performs organizational and methodological functions. A scheme of subgroup (A, B, and C) distribution of Group III patients by the activity and pattern of sarcoidosis has been developed to have unique follow-up methods. The diagnosis made in the patients and their dispensary registration, the efficiency of treatment, and their movement by subgroups are checked up. This system of a center-dispensary interaction revealed a dynamics in sarcoidosis incidence and prevalence in the city in 1994-1997. Evidence is provided for that it is expedient and timely to organize a follow-up of patients with sarcoidosis particularly when the share of paid medical services is increasing.

[The pattern of distribution of polonium-210 in the superficial soil layer in Moscow].

The pattern of distribution of 210Po, 226Ra, 232Th, and 40K was studied in the superficial soil layer of Moscow. There was a correlation between the soil levels of the natural uranium decay products 210Po and 226Ra. The ratio of specific 210Po, 226Ra activities was 1.3 +/- 2.6 in the soil samples. There is a close correlation between the soil levels of 210Po and its organic substance. The specific activities of the natural radionuclides in the Moscow soil were as follows: 210Po 30-50 Bk/kg, 226Ra 10-30 Bk/kg, 232Th 10-30 Bk/kg, and 40K 200-400 Bk/kg. These levels are no greater than those of those of natural radionuclides in the Earth's crust.
