Prevalence of severe pelvic organ prolapse in relation to job description and socioeconomic status: a multicenter cross-sectional study.

The aim of this study was to determine if certain occupations or socioeconomic levels are associated with pelvic organ prolapse. Investigators at six American sites performed pelvic organ prolapse quantification examinations on women presenting for routine gynecologic care. Between September 1999 and March 2002, 1,004 patients were examined. Severe pelvic organ prolapse was defined as the leading edge being 1 cm or more beyond the hymeneal ring. The data was analyzed with the Kruskal-Wallis analysis of variance, Bonferroni test, multiple logistic regression, and descriptive statistics. The prevalence of severe pelvic organ prolapse in our group was 4.3%. Women who were laborers/factory workers had significantly more severe prolapse than the other job categories (p < 0.001). Women with annual income of Dollars 10,000 or less had significantly more severe pelvic organ prolapse than other income groups (p < 0.001). These differences persisted even when controlling for age, race, number of deliveries, body mass index >30, and smoking status (all p < 0.001). Laborers/factory worker jobs and an annual household income of Dollars 10,000 or less are associated with severe pelvic organ prolapse.

Beyond the job description: managing for performance.

Translation of individual effort into performance that effectively meets organizational goals is challenging. This article demonstrates a systematic approach that addresses both baseline performance and staff development by using personnel management concepts and relating them to existing resources available from the American Occupational Therapy Association. The systems approach described uses performance standards as a basis for staff recruitment, orientation, development, and appraisal. Also discussed are the applications to student clinical education.

Integrated care at the bedside: job description consensus.

A Nursing Career Pathway Committee developed a differentiated nursing model based upon the Midwest Alliance in Nursing competencies. Two distinct and complementary roles provide direct nursing care: the Registered Nurse in the Associate Role and the Registered Nurse in the Primary Role. Through a differentiated nursing demonstration project, the need for changes in the "charge nurse" role became apparent. Opportunities for growth and participation, regardless of position held, are presented. Agencies without "clinical ladders" will be intrigued with this fresh approach for rewarding nurses who provide direct care.

Job description for the Certified Nephrology Nurse: professional and legal aspects.

As in many European countries, in Germany, practice and responsibilities of the Certified Nephrology Nurse (CNN) are still affected by a slowly changing structure in health care-providing institutions and the developing qualification process of the nurse working in the renal field. It is evident that parallel to the development and implementation of the European Core Curriculum the scope of practice for the CNN needed to be defined and published by a group of clinical experts working in the renal nursing field. A task force composed of members of the EDTNA-ERCA and the German National Renal Care Association was set up to tackle all aspects of the professional profile. A lawyer experienced and recognised in the medical field supported the group. In this paper information about the background and content of the document is presented, as are the core areas of practice and responsibilities and areas where junior nurses or helpers may work under the guidance of CNN, also the professional and legal aspects of the CNN profile are addressed, discussing possible benefits and problems for colleagues in practice when using and implementing the documents in different structures of renal care-providing organisations. This document was published and sent to all important national Bodies in Germany, having an immediate impact in units where areas of practice and responsibilities were undefined for certified renal nurses and nurses auxiliaries.

[The job description card--a method for improved integration of the handicapped in occupational activities].

The integration of disabled individuals at their work place requires not only good educational, psychological and medical preparation but also good cooperation of the employer as contract partner. To simplify the task of the employer, a method created to serve the goals of the organization and how it structures its jobs was slightly expanded to that it can also meet the requirements of vocational rehabilitation. With the help of a computer suitable work stations are preselected from an existing inventory of available jobs. A physician examines and identifies the suitability of these jobs with the help of special descriptions which spell out demands and requirements the worker has to be able to meet. 150,000 work stations in the chemical industry were analyzed and used to prove the applicability of the procedure.
