The relationship between hearing disorder and performance on intelligence tests among 28 children and young adults with Down syndrome was investigated. Intellectual and audiological assessments were obtained and evaluated in a multiple regression analysis. Performance on intelligence tests by individuals with abnormal tympanograms was inferior to that of individuals with normal tympanograms. Hearing sensitivity measures were uncorrelated. Findings suggest the need for an awareness of the potential impact of middle ear disorder when assessing intellectual functioning and the importance of otological care for infants and young children with Down syndrome.
The German Registry for Hearing Loss in Children (DZH) processes nationwide data from audiological centers. Coping with the accrued data and its subsequent management and analysis requires a high degree of security and control. To establish a nationwide registry it is necessary at an early stage to take into consideration the legal requirements of the participating states. Use of the DZH as an example demonstrates how a pragmatic solution can be reached. Special issues concerning data collection, transfer, storage and deletion, coding strategies to ensure anonymity, checking for duplicate entries, data separation, and automated data analysis and data protection are explained.
Psychogenic hearing defects are more common than previously recognized. The disorder is characterized by typical behaviour, audiological signs and symptoms. Patients may show severe hearing losses as evaluated by pure-tone threshold audiometry but can understand usual conversation. Results of audiological examination may be conflicting and suggest a wide range of organic hearing disorders. Other characteristics are dramatic improvement of hearing following sham procedures or placebo medication. Detection of psychogenesis and etiologic factors are of utmost importance for successful therapy. Between 1988 and 1991, 21 children and adolescents (ages 5-16 years) with psychogenic hearing losses were studied. Typical signs as well as diagnostic and therapeutic tools are illustrated in renewal case histories. In-patient diagnosis and treatment are recommended, since improvement was achieved in all patients treated.

Recent studies have revealed marked differences in the prevalence of hearing impairment in childhood. It remains unknown whether this is caused by geographic, sociocultural or ethnic factors. In order to provide consistent data, 314 children in residence at a school for hearing impaired children of a well-defined German area were investigated retrospectively. We estimated an overall prevalence of .43/1000 of hearing impairment for this area. A striking variability between different nationalities could be observed. For German children, the prevalence was .36/1000, for Turkish 1.04/1000 and for Italian children 1.37/1000. Hereditary hearing losses and, in particular, autosomal recessive caused hearing disorders were the most common cause of hearing impairment in foreign but not in native children. One reason might be the frequency of marriages between relatives resulting from the social isolation of the foreign groups in German society.
A representative sample of Grade 1 (first year school entry) schoolchildren in Swaziland were surveyed during a single week to determine the prevalence of ear and hearing disorders: 79.8% had both normal ears and normal hearing, 16.8% had an ear disorder, but 80% of them had normal hearing. The most common disorder was impacted wax, with a prevalence rate of 74/1000. Middle ear disorders were common and the prevalence rate for children with active middle ear disease was 30/1000 (17/1000 having a hearing loss), and for children with inactive ear disease, the prevalence was 21/1000 (5/1000 having a hearing loss); 8/1000 children were found to have a sensorineural hearing loss, 5.3/1000 unilateral and 2.1/1000 bilateral. Improved treatment of acute otitis media, which is also common in the pre-school age group, could reduce the sequelae of the disorder, which has a deleterious effect on hearing and impairs educational achievement once the children enrol at school.
In recent years, we have observed causes of congenital or early onset hearing disorders that had not been recognized or known to exist before. These include drugs passed to an as-yet unborn child. Principal among them is alcohol, but others, such as cocaine, are now also occupying our attention. This article reviews the effects of such exposure on the communication skills of children who had been exposed. It is not yet clear whether there are specifically auditory effects, nor is it clear whether the communicative effects are long lasting.
