[Development and validation of a dependence scale].

Two dimensions are identifiable in nursing hospital caring: a) nurse activities based on nurse critical evaluation, carried-out in order to achieve patients highest independence; b) nurses activities implementing routinely care and medical prescriptions. Therefore, in order to improve the appropriateness of their evaluations, nurses should measure patients level of dependence using valid and reliable instruments. The study was designed to develop and test a dependency scale for hospitalized patients, in order to better identify their needs of nursing care; the study was carried out at Udine University Hospital. The instrument developed (ADI 8) is a eight-ranks scale. A comparative validation between ADI 8, Barthel Index ans Swiss Grid (MS) proved that ADI 8 has more discriminative power for medium dependency level subjects. Moreover, ADI 8 showed less tendency to overestimate than Bartel Index. Reliability test was carried out using paired observers in blind mode. Kendall statistics proved that ADI 8 possess high reliability for each factor and overall (.80).

[Object relation with drug--case report].

Psychoanalytic exploration of the action of psychotropic drugs has a long tradition which begins in the pioneer days of contemporary psychopharmacotherapy in 1950-ties. This exploration and publications based on them theorized about influence of neuroleptics on ego defences, id drives and intrapsychic conflicts and decrease in symptomatology as a result of action of the medication on all listed above. The paper presented is the case story about psychodynamic meaning of medication with a neuroleptic drug in a new context. The patient after a psychotic episode (paranoid type) was treated with quetiapine 600 mg per day and in the same time had an individual psychotherapy. During psychotherapy her inability to establish object stability was disclosed. In that particular intrapsychic situation the patient established a relation with an omnipotent and idealized representation of the drug. Medication was an absolutely idealized object for her one that was controllable and stabile and could not abandon her. The quality of relation with the drug was based on a primary defence mechanism like splitting, idealization and omnipotence and denial.

Nurse codependency: instrument development and validation.

This study developed and evaluated the Nurse Codependency Questionnaire (NCQ) and generated initial estimates of the stability and internal consistency of responses for the questionnaire. An initial pool of 95 items, reflective of four domains of codependency, was generated from the codependency literature using a domain-referenced approach. Seven expert judges from the nursing and codependency fields calculated the content validity index (CVI) as > .80. Items were critiqued for relevance, clarity, and predicted direction of each item's correlation with the total codependency score. A convenience sample of 547 male and female nurses from Texas was recruited from a variety of professional settings to test the NCQ. Evidence of reliability and validity was sought through the use of principal factor analysis (PFA) techniques and correlation analysis. The specific domains of "codependent caretaking" and "lack of voice" represented two of the four hypothesized domains that were supported by factor analysis. Data screening and item analysis resulted in a final sample of 24 items. Test-retest reliability was .90 and internal consistency reliability was .80 for the entire scale. Reliability estimates for the "codependent caretaking" and "lack of voice" scales were .65 and .59, respectively for test-retest; and .81 and .64, respectively for internal consistency. Known groups validity was supported by each of the factors' ability to discriminate between binge and nonbinge eaters. The NCQ may be useful for identifying codependency within the nursing profession. Research is needed to determine the external factors that influence the overt expression of nurse codependency. Screening for nurse codependency may contribute to the health of the profession by providing a means for anticipatory guidance and early intervention.

Codependency in nurses. How it affects your organization.

Codependence in a staff nurse affects both direct patient care and relationships with coworkers, physicians, and supervisors. Codependent behaviors negatively impact the nurse's sphere of influence. The nurse manager's codependency has the same impact, but the sphere of influence is larger and thus, the potential for harm is greater. Codependent behaviors enacted at different levels of an organization can disrupt an entire institution or profession. The author describes how nurses' practices are affected by codependent behaviors and identifies ways in which those behaviors can be modified by good management.

Family compliance to a treatment programme for alcoholics: a prospective study of prognostic factors.

A total of 198 alcoholic subjects was recruited into a treatment programme according to Hudolin's method. The programme included three steps: (1) presentation of the programme to the alcoholic family (four meetings); (2) a therapeutic community that met weekly over 4 months; and (3) attendance at an on-Treatment Alcoholic Club located close to the family's home residence (weekly self-conducted meetings). Data on the families' compliance to the treatment programme were obtained in January 1993 and the prognostic significance of 11 variables collected during the first step was determined by the proportional hazards regression model. The cumulative proportion of family compliance after 40 months was 36.4%, with similar drop-out rates at each of the three steps of the programme. The prognostic factor which was the major determinant of family compliance to treatment was the active presence of the whole family in the first step, while a good job performance was independently associated with family compliance, being a significant prognostic factor in the second step of the programme. Alcohol abstinence was obtained in 35.9% of the whole study group. This programme provided results for alcohol abstinence which are comparable with methods that require more expensive resources.

Is adapting to others codependency or flexibility?

1. Many people use the word codependency without understanding its meaning. Codependency should be used to describe behaviors that have developed in a dysfunctional family in which priorities have become unbalanced, and energy is focused on ensuring that everything seems normal from the outside rather than focusing on the needs of individual family members. 2. Codependency may continue into a professional role and result in a manager who has low feelings of self-worth and a strong need for approval from others. 3. Flexibility is different from codependency. A flexible individual has strong and positive feelings of self-worth and is able to listen to opposing points of view and actively listen to others. Change and compromise are possible for this person.

A critical analysis of the concept of codependency.

This article addresses problems associated with conceptualizing interpersonal behaviors as addictions or diseases and pathologizing characteristics associated with women. The failure of the codependency model to focus on the unequal distribution of power and resources and the blurring of responsibility between the actor and audience are also discussed. Individual identity can be lost in the label, and the codependency model encourages separation from rather than connection with the family of origin.
