Gastric bezoars--spontaneous resolution.

Six cases of gastric phytobezoars were seen on fiberoptic gastroscopy over a period of four years during which time 1,400 gastroscopies were done. Four of these cases showed complete resolution of bezoars on repeat endoscopy during a follow-up period ranging from two weeks to one year. Our experience shows that, in the absence of gastric outlet obstruction, small bowel obstruction or other serious problems due to bezoars, they may not need surgical or aggressive medical treatment.

[Gastric bezoars].

Gastric bezoars are concretions sometimes mistaken for cancer, because of their polymorphous symptoms. Previous gastric surgery modifies anatomy and functions of the stomach and favours the bezoar deposition. Digestive endoscopy allows a correct diagnosis and therapy. A wide review of the literature is given.

Nonoperative management of persimmon bezoar. A successful modification of traditional papain therapy.

The soft phytobezoars occurring in the postoperative stomach may be readily removed by enzyme digestion and manipulation. In contrast, the persimmon bezoar (diospyrobezoar) has been persistently to such treatment and has required surgical removal. Experience with unsuccessful attempts at enzyme digestion in a number of such patients led to the experimental use of a "soaking" technic which satisfactorily dissolved two of three large persimmon bezoars and shrank the remaining foreign body sufficiently so that it could be removed endoscopically by way of the esophagus. A concomitant gastric ulcer healed uneventfully on medical therapy.

Esophageal bezoar: a rare but distinct clinical entity.

A 89 year old female patient presented with severe dysphagia and was suspected to have carcinoma of the esophagus. Endoscopy revealed an esophageal phytobezoar which passed down spontaneously after unsuccessful endoscopic extraction attempt. Barium swallow study revealed diffuse spasm of the esophagus. A review of English literature revealed only 17 previous cases of esophageal bezoar. Salient features of esophageal bezoars are discussed based on previous reports and the current case.

[Mechanical lithotripsy of a gastric bezoar].

At the First Medical Clinic of the Faculty Hospital Prague Motol the authors detected during a routine gastroscopic examination of a 71-year-old female patient the presence of a gastric bezoar. The examination was indicated on account of dyspeptic complaints. With regard to the size of the bezoar (50 x 20 mm) the authors decided for mechanical lithotripsy. It proved possible to reduce the concrement during the first session and during subsequent sessions it was broken down to small size and the fragments were eliminated per vias naturales. Several fragments were extracted. Chemical analysis provided evidence of the origin of the concrement in the biliary pathways--the concrement was pure cholesterol. ERCP did not reveal an artificial communication between the biliary system and the stomach or duodenum. It did not prove possible to visualize the gallbladder by ultrasonography. The selected therapeutic procedure was successful.

[Pharmacological studies of "reiousan" which contains bezoar and ginseng].

Pharmacological properties of "Reiousan", a crude drug preparation consisting of bezoar and ginseng, were studied. The hypotensive effect on spontaneously hypertensive rat, inhibition of platelet aggregation, acetic acid-induced writhing, stress ulcer, CCl4 and d-galactosamine-induced hepatic injury, and facilitation of recovery from neuroparalysis and muscular fatigue, protective effect on anoxic brain damages, antipyretic effect, and facilitation of learning in aged mice were observed after administration of "Reiousan". It was suggested that bezoar and ginseng act synergistically in causing the hypotensive effect, acetic acid-induced writhing and drug-induced hepatic injury, and protection from anoxic brain damages and muscular fatigue.

[Pharmacological studies of reiousan which contains bezoar and ginseng: III. Effects on experimental cerebral ischemia].

Pharmacological effects of Reiousan, a crude drug preparation consisting of bezoar and ginseng, on experimental cerebral ischemia and anoxia were studied. After administration of Reiousan, the survival time of mice subjected to hypobaric hypoxia and the gasping duration of isolated rat head tended to increase. Reiousan inhibited all the following: lipid peroxides production in rat brain homogenate, tissue swelling induced by the xanthine-xanthine oxidase system in rat brain cortical slices, rat brain swelling induced by freezing, lipid peroxides production in the rat brain after ligation of bilateral common carotid arteries, and lipid peroxides production in Mongolian gerbil brain after reperfusion following ligation of bilateral common carotid arteries. These effects may result from antioxidant activity of bilirubin, a constituent of bezoar.

[Determination of cholesterol in natural bezoar by gas chromatography].

A gas chromatographic method for the determination of free and total cholesterol in natural bezoar has been established in this report. The method is simple, specific and accurate. The free and total cholesterol contents in three kinds of bezoar are between 0.072% to 0.214% and 0.546% to 0.608% respectively.

[Study on radiosterilization of crude drug pill involving bezoar bovis--radiolysis of bilirubin and cholic acids in hydrous starch pellet (author's transl)].

Radiolysis of bilirubin and cholic acids (cholic acid, desoxycholic acid and lithocholic acid) in hydrous pellet have been investigated with following parameters, which were hydrous content, radiation dose and the dose rate, to discuss the application of gamma-irradiation for sterilization of crude drug pill involving bezoar bovis. At 774 C/kg (3.0 MR) irradiation for 5% and 10% hydrous contents pellets, the radiolysis percent of those components were less than 5%. However, the higher hydrous pellet, the radiolysis percents of those are more increase. At the same irradiated condition for 20% hydrous contents pellets, the radiolysis percents of those were 15-22%. The hydrous percent of commercial crude drug pill involving bezoar bovis are about 9%, so that the radiolysis of those components will be less than 5% on the sterilization. The radiolysis percent of bilirubin are constant to variation of radiation dose rate between 51.6-722.5 C/kg.hr (0.2-2.8 MR/hr). But, the values of cholic acids don't definite such as that of bilirubin, because of larger analytical error.

Cement bezoars of the stomach.

Cement ingestion resulting in the formation of concretions in the stomach is unusual in surgical practice. Cement bezoars that are retained in the stomach do not undergo degradation and attempts at chemical dissolution are not recommended. Early surgical removal is the only safe and effective method of treatment. Smaller concretions are excreted without incident but may cause intestinal obstruction which should be anticipated and surgically relieved.

An intraluminal surgical approach to the management of gastric bezoars.

Trichobezoars are difficult to remove endoscopically, often cause nausea and vomiting, and can result in small-bowel obstruction. A patient with a trichobezoar presented to our clinic with symptoms of partial small-bowel obstruction. Multiple attempts at flexible endoscopic removal were unsuccessful. Two large-diameter percutaneous gastrostomies with an inflatable balloon and distal foam-rubber stent to assure intragastric positioning were introduced under general anesthesia. Visualization was provided by a 0 degree panavision laparoscope placed through one of the gastrostomies. The bezoar was removed through the second gastrostomy using standard laparoscopic instruments. The patient made an uneventful recovery. This is the first reported case of percutaneous removal of a trichobezoar. We conclude large-diameter gastrostomies may serve as a port of access for numerous other intraluminal procedures.

Esophageal obstruction by phytobezoar. Rare complication of gastric bezoar.

We describe a patient in whom a gastric phytobezoar was regurgitated into the esophagus during an episode of vomiting, giving rise to sudden dysphagia. The bezoar remained impacted for 3 days during which time a sever ulcerative esophagitis due to pressure necrosis and secondary infection developed. Healing has been accompanied by esophageal stricture formation which still necessitates esophageal dilatation at intervals.

Charcoal bezoar. Small-bowel obstruction secondary to amitriptyline overdose therapy.

Nasogastric administration of activated charcoal is effective in decreasing the half-life of amitriptyline. A case is reported in which this therapy for amitriptyline overdose led to charcoal bezoar formation and small-bowel obstruction. Amitriptyline's atropinic side effects on the gut make this a possible complication in such patients.

Jejunal monilial bezoar following total gastrectomy.

A case is reported of monilial bezoar involving the jejunum following total gastrectomy in a patient who was neither severely debilitated nor immunosuppressed. This condition should be differentiated from the more commonly occurring postgastrectomy phytobezoar.

Small bowel obstruction secondary to bezoar: CT diagnosis.

Two cases of small bowel obstruction secondary to phytobezoar diagnosed by computed tomography (CT) and confirmed at surgery are presented. CT findings were dilated intestinal loops and an intraluminal mass with air bubbles retained in its interstices, resulting in a mottled appearance. We propose that definite diagnosis of small bowel bezoar can be made on the basis of these CT findings.

Acute renal failure caused by fungal bezoar: a late complication of Candida sepsis associated with central catheterization.

The authors report a case of acute renal failure caused by fungal bezoar in the renal pelvis. The patient was successfully treated with bilateral percutaneous nephrostomy drainage. He had been admitted because of necrotizing enterocolitis, at the age of 26 days. Eventually, his bowel was reduced to 40 cm of small intestine, including 5 cm of terminal ileum. Candida sepsis developed during central total parenteral nutrition, at the age of 76 days. Five weeks after the diagnosis of systemic candidiasis, sudden anuria developed, and ultrasonography showed echogenic material in both renal pelvises. Bilateral percutaneous nephrostomy catheters were placed in the renal pelvises, and local irrigation with amphotericin B was performed for 3 weeks. The renal function of the baby was completely recovered, without systemic antifungal treatment.

Small-bowel obstruction due to Bezoar following pancreas transplantation with portal-enteric drainage: a case report.

BACKGROUND: Despite recent improvements, surgical complications continue to occur frequently after pancreas transplantation, remaining the leading cause of early graft loss. Small-bowel obstruction, however, is exceedingly rare; it has not been associated with an enhanced risk of graft loss. METHODS: Intestinal obstruction occurred 7 days after pancreas transplantation due to bezoar blockage at the level of the jejunojejunostomy of the Roux-en-Y loop, which had been constructed to drain the exocrine secretions of the pancreas graft. RESULTS: CT scan promptly identified the foreign body and greatly facilitated graft rescue before duodenal rupture or the development of graft pancreatitis. Nineteen months after repeat laparotomy the patient is alive with good pancreatic endocrine function. CONCLUSIONS: In cases of pancreas transplantation with enteric drainage, obstruction of the Roux-en-Y loop may create a totally sealed system that may lead to severe duodenal dilation and eventually to duodenal rupture or graft pancreatitis.
