Influence of ambroxol on lung tissue penetration of amoxicillin.

Lung amoxicillin levels were assessed in tissue obtained from 16 patients who underwent lung resection due to different pulmonary diseases. In this double-blind placebo controlled study patients were allocated to two groups in randomized order; one group received amoxicillin 1000 mg t.i.d. (n = 8) and the other amoxicillin 1000 mg + ambroxol 60 mg t.i.d. (n = 8). A trend towards higher antibiotic lung tissue levels was observed in patients who received the antibiotic together with the mucolytic agent. The ratio pulmonary tissue/serum amoxicillin levels reached a significant difference being higher in the amoxicillin plus ambroxol group than in the other one (0.411 +/- 0.04 vs 0.672 +/- 0.07; p less than 0.01), even if the amoxicillin plasma levels were lower. A possible effect of ambroxol on lung tissue penetration of amoxicillin is suggested and discussed.

Influence of ambroxol on amoxicillin levels in bronchoalveolar lavage fluid.

Amoxicillin levels were measured in bronchoalveolar lavage (BAL) fluid samples obtained from patients who after randomization were treated in double-blind fashion either with amoxicillin 1000 mg p.o.t.i.d. or with amoxicillin 1000 mg + ambroxol 60 mg p.o.t.i.d. Antibiotic levels were higher in the group receiving ambroxol (0.32 +/- 0.02 micrograms/ml; n = 8) than in the other one (0.19 +/- 0.02 micrograms/ml; n = 6). This difference was statistically significant (p less than 0.001). Comparisons of protein concentrations in BAL fluid samples and of amoxicillin plasma levels did not show significant differences in the two groups. These results seem to prove that ambroxol is able to increase the antibiotics concentration in the lungs, although the mechanism of action is still unclear. In addition it could be shown that the BAL technique is suitable for exploring the lung concentration of antibiotics.

Ambroxol plus amoxicillin in the treatment of exacerbations of chronic bronchitis.

Twenty-three patients with exacerbations of chronic bronchitis were divided in two groups in a randomized fashion receiving either amoxicillin 1500 mg/d (n = 13) or amoxicillin 1500 mg/d associated with the mucolytic drug ambroxol 90 mg/d (n = 10). The improvement in cough, expectoration difficulties and sputum purulence was statistically more evident and occurred earlier in the ambroxol + amoxicillin group than in the amoxicillin only group. Although amoxicillin plasma and sputum levels were similar in both groups, the differences in daily sputum volume, which was also statistically greater in patients receiving ambroxol, suggests that this drug favours the bronchial mucus clearance of the antibiotic which could be related to the more favourable clinical evolution. No changes were observed in lung function tests and blood gases.

[Clinical results in the treatment of chronic obstructive bronchitis with ambroxol in comparison with bromhexine (author's transl)].

Preclinical studies of trans-4-[(2-amino-3,5-dibromobenzyl)-amino]-cyclohexanol hydrochloride (ambroxol, NA 872), the metabolite VIII of bromhexine, show that it is superior to the basic substance with respect to its bronchosecretolytic properties. In a double blind study 30 patients suffering from chronic obstructive bronchitis received in randomized order 36 mg bromhexine/d or 45 mg ambroxol/d, each over a period of 4 weeks. The following parameters were studied: mean bronchial flow resistance, forced expiratory volume, static lung volumes, arterial blood gases as well as clinical and laboratory results. Bromhexine was not associated with a change in lung function parameters. Ambroxol was associated with a 25% reduction of the average bronchial flow resistance. The forced expiratory volume improved over the period on the average by 14%. Patients with slight arterial hypoxemia had an increase in the partial pressure of arterial oxygen. The patients found both substances facilitated expectoration. Further studies appear necessary to confirm the improved respiratory performance under the influence of ambroxol.

Effect of ambroxol on human phagocytic cell function.

The in vitro effects of ambroxol on phagocytic and microbicidal activities of human neutrophils and monocytes have been studied. Preincubation of neutrophils with ambroxol (10 to 100 ng/ml) had no effect on their phagocytic or microbicidal capacities. On the other hand, when monocytes were preincubated for 24 hours with ambroxol a significant increase of their microbicidal activity was observed, without effect on the phagocytic properties.

[The pharmacokinetics and bioequivalence of various dosage forms of ambroxol].

An intraindividual comparative single-dose study was carried out under carefully controlled conditions on 12 healthy volunteers in order to establish the bioavailability of trans-4-(2-amino-3,5-dibromobenzyl)-amino-cyclohexanol (ambroxol) the active principle of newly developed tablets and drops, in comparison to a commercial i.v. preparation. In an additional single-dose, cross-over study on 12 healthy volunteers the bioequivalence of ambroxol was investigated after administration of a newly developed vs. a commercial dose-equivalent, sustained-release dosage form. Following off-line derivatisation using formaldehyde to the corresponding tetrahydroquinazoline compound, ambroxol was assayed from plasma by high-performance liquid chromatography. A 2-compartment model was taken as a basis for the calculation of the plasma concentration curves and the pharmacokinetic parameters following intravenous injection of the drug. After i.v. administration, the terminal elimination half-life, the apparent volume of distribution and the total plasma clearance were determined to be 3.72 h, 1.52 l/kg and 565 ml/min, respectively. From the tablet and drop formulations the systemic availabilities were calculated to 73 and 81%, respectively; the mean transit times were determined to be 6.8 and 5.4 h, respectively. Both sustained-release dosage forms investigated are bioequivalent.

[Induction of osmiophilic lamellar bodies by corticosteroids, Fenoterol and Ambroxol in lung tissue cultures from human fetuses].

Organ cultures were set up from human fetal lung originating from the first trimester of gestation (8-12 weeks) and followed their morphological differentiation in the absence and in the presence of Dexamethasone, Betamethasone, Ambroxol and Dexamethasone + Fenoterol. We were especially interested in studying whether multilamellar bodies and tubular myelin structures which are characteristic of the human surfactant, could be demonstrated in the cells after different treatment. Dexamethasone and Betamethasone promoted directly the development of the multilamellar bodies and the tubular myelin structures in the type II cells. - Furthermore, Ambroxol (Bromhexin-metabolite VIII) produced a prominent stimulatory effect in the synthesis of the surfactant in a concentration of 12 ng/ml. Fenoterol has probably no stimulatory effect on the surfactant production. In cultures treated with 10 ng/ml Fenoterol + 10 ng/ml Dexamethasone the benefit effect of Dexamethasone in stimulation of the multilamellar bodies was impaired. Our investigation shows that the human fetal lung is able to respond to the action of glucocorticoids and Ambroxol as early as in the first trimester of gestation by accumulating phospholipids.

Cord blood prolactin and thyroid hormone levels after antenatal administration of betamethasone or ambroxol for prevention of respiratory distress syndrome (RDS).

Several studies showed that ambroxol is efficacious in enhancing fetal lung maturation, provided large doses are given. We investigated cord blood levels of PRL and thyroid hormones in 31 infants born after antenatal administration of betamethasone, in 31 infants born to mothers treated with ambroxol for prevention of RDS and in 27 infants whose mothers had not received any drug enhancing fetal lung maturity. The only significant difference (p less than 0.05) between the groups was noted in T3 mean levels that were higher in the ambroxol group than in the betamethasone group between the 33th to 36th week of gestation. Hormone levels did not appear significantly different in infants who developed RDS rather than in infants who did not.

Influence of ambroxol on the bronchopulmonary level of antibiotics.

Antibiotic levels in bronchopulmonary tissue following oral administration of ampicillin, erythromycin and amoxycillin dosed 50 mg/dg were compared with those obtained by the same treatments n0 mg/dg p.o of trans-4-[(2-amino-3,5-dibromo-benzyl)amino]cyclohexanol (ambroxol, Mucosolvan) in several groups of rats. The addition of ambroxol was correlated with an increase of 234% in the pulmonary average concentration of ampicillin, 27% in that of erythromycin 27% in that of amoxycillin; the statistical evaluation of these differences resulted in less than 0.05 in each case. It may be concluded that ambroxol, through a mechanism still unclear, can increase the antibiotic levels in the lungs of the rat.

Antenatal ambroxol usage in the prevention of infant respiratory distress syndrome. Beneficial and adverse effects.

A bromhexine metabolite ambroxol, is a relatively new promoter of fetal lung maturation. The data on its efficacy and side effects in humans are not yet as many as those of corticosteroids. We found that in 24 premature labor patients ambroxol reduced the incidence of respiratory distress syndrome when compared with the control group, consisting of 58 patients. There was no concomitant disorder in any patient that would have contributed to the fetal lung maturation. We also observed septic morbidity to be less frequent in the ambroxol group. Thyroid hormone levels were within normal range both in maternal and fetal circulation. There were no side effects attributable to the drug. Maternal liver and renal function test results did not differ significantly throughout the treatment.

[Effect of intravenous ambroxol hydrochloride on lung function and exercise capacity in patients with severe chronic obstructive pulmonary disease].

The effect of Ambroxol hydrochloride on spirometry, pulmonary gas exchange and exercise capacity in patients with COPD were evaluated. Twenty patients mean age 59 +/- 9 yr presenting with severe airways obstruction were divided by random number to two groups: treated (L) and control (K). L patients received iv infusion of one gram of Ambroxol in 500 ml of 0.9% saline daily, for 5 days. K patients received infusions of 500 ml 0.9% saline at the same intervals. Blood gases, capnography and incremental exercise test on cycloergometer were performed before treatment and after the fifth dose of the drug or placebo. Before treatment spirometry, blood gases, capnometric++ indices and exercise tolerance were similar in both groups. After treatment in patients from L group an increase in VC from 1.78 +/- 0.61 L to 2.04 +/- 0.58 L (p < 0.01), FEV1 from 0.96 +/- 0.43 L to 1.11 +/- 0.49 L (p < 0.01), MEF25 from 1.36 +/- 0.93 L/sec to 1.61 +/- 0.96 L/sec (p < 0.01) was found. End-tidal carbon dioxide in 60 sec hyperventilation test decreased from 3.7% to 2.9% (p < 0.001). There were no differences in exercise tolerance and blood gases after treatment. In the control group all studied variables remained unchanged. We conclude that Ambroxol improves airways patency in patients with severe COPD.

The effect of Ambroxol on bleomycin-induced changes in phospholipid composition of rat lung surfactant.

The influence of Ambroxol on bleomycin-induced changes in lung surfactant phospholipid composition in rats was examined. Bleomycin alone (intratracheally 10 mg/kg, single dose) produces a significant fall in phosphatidylcholine (especially in disaturated species) and phosphatidylglicerol content in both lavage and postlavaged lung tissues surfactants. Ambroxol (8 days, 2 mg/kg s.c. per day) normalizes these changes. Thus, Ambroxol has an salutary effect on bleomycin-induced changes in lung surfactant phospholipids.

Electrophysiological investigation of the effects of ambroxol on the transepithelial Na+ ion transport pathway in airways.

Isolated tracheal wall was studied by the Ussing method during selective inhibition of single, electrogenic, transepithelial ion transport pathways with either amiloride (for Na+) or bumetanide (for Cl-). It was shown that ambroxol influences the Na+ transepithelial transport pathway which is partly responsible for the hyperpolarization after mechanical stimulation of the mucosal surface of tracheal wall. Ambroxol augments the Na+ current during the presence of bumetanide in the experimental system, but diminishes it during the washing of amiloride from the measuring chamber. These bidirectional effects of ambroxol provide indirect evidence that the drug is a partial agonist for the memberance receptors which form part of the intrawall control system responsible for integration of airwall function with the transepithelial ion transport.

A double-blind placebo controlled trial with oral ambroxol and N-acetylcysteine for mucolytic treatment in cystic fibrosis.

The therapeutic efficacy of oral N-acetylcysteine (NAC) and ambroxol as compared with the effect of placebos was studied in 36 cystic fibrosis (CF) patients with mild to moderate pulmonary disease. The patients were randomly assigned to one of three regimens, matched on the basis of age and Chrispin-Norman scores. The trial was conducted over a period of 12 weeks. Patients were assessed clinically and by extensive pulmonary function techniques (body-plethysmography, maximal expiratory flow-volume curves, trapped air determination). Although no clinical differences could be observed between the three groups, significant impairment in the placebo group was found for trapped air and FEV1 when compared to the active groups, suggesting a therapeutic effect of ambroxol and NAC in CF.

Assay of ambroxol in biological fluids by capillary gas-liquid chromatography.

A sensitive and rapid method for the determination of ambroxol in biological fluids is described. It comprises a single extraction step, derivatization and selective determination with capillary gas-liquid chromatography (in split-mode) and electron-capture detection. The limit of quantification in plasma is ca. 3 ng/ml. The method is applied to the pharmacokinetics of ambroxol in humans.

Simultaneous determination of roxithromycin and ambroxol hydrochloride in a new tablet formulation by liquid chromatography.

A rapid and accurate liquid chromatographic method is described for the simultaneous determination of roxithromycin and ambroxol hydrochloride in a new tablet formulation. Chromatographic separation of the two drugs was achieved on a Diamonsil C(18) column (200 mm x 4.6 mm, 5 microm). The mobile phase consisting of a mixture of acetonitrile, methanol and 0.5% ammonium acetate (39:11:50 (v/v), pH 5.5) was delivered at a flow rate of 1.0 ml/min. Detection was performed at 220 nm. Linearity, accuracy and precision were found to be acceptable over the concentration range of 201.2-2012.0 microg/ml for roxithromycin and 42.7-427.0 microg/ml for ambroxol hydrochloride, respectively. Separation was complete in less than 10 min. The proposed method can be used for the quality control of formulation products.

Determination of ambroxol hydrochloride, methylparaben and benzoic acid in pharmaceutical preparations based on sequential injection technique coupled with monolithic column.

The porous monolithic columns show high performance at relatively low pressure. The coupling of short monoliths with sequential injection technique (SIA) results in a new approach to implementation of separation step to non-separation low-pressure method. In this contribution, a new separation method for simultaneous determination of ambroxol, methylparaben and benzoic acid was developed based on a novel reversed-phase sequential injection chromatography (SIC) technique with UV detection. A Chromolith SpeedROD RP-18e, 50-4.6 mm column with 10 mm precolumn and a FIAlab 3000 system with a six-port selection valve and 5 ml syringe were used for sequential injection chromatographic separations in our study. The mobile phase used was acetonitrile-tetrahydrofuran-0.05M acetic acid (10:10:90, v/v/v), pH 3.75 adjusted with triethylamine, flow rate 0.48 mlmin(-1), UV-detection was at 245 nm. The analysis time was <11 min. A new SIC method was validated and compared with HPLC. The method was found to be useful for the routine analysis of the active compounds ambroxol and preservatives (methylparaben or benzoic acid) in various pharmaceutical syrups and drops.

[Ambroxol-induced immune hemolytic anemia].

INTRODUCTION: Drug-induced immune hemolytic anemia is a rare cause of hemolytic anemia. CASE RECORD: A 68-year-old male patient developed an acute intravascular hemolysis with acute renal failure. Common causes of hemolysis were ruled out and the patient rapidly improved. An immune mechanism was confirmed by the positivity of the direct antiglobulin test with anti-IgA in the presence of ambroxol only, one of the drug the patient had received during 6 days before hospitalization. DISCUSSION: To our knowledge, this is the first report of ambroxol-induced immune hemolytic anemia. This case also underlined that the direct antiglobulin test should also be performed with anti-IgA to rule out any false negative.
