[Seroepidemiological study of urogenital schistosomiasis in Algeria: the focus of Khemis el Kechna].

Two foci of schistosomiasis are known to be active in Algeria, first in the north of the country, at wadi El Hamiz in relation with the dam of El Hamiz river in the commune of Khemis el Kechna; and the other in the south, in the Tassili N'Ajjer region (Djanet, Ihérir, Tamadjert). In relation with a preparatory phase of a control programme of the disease a sero-epidemiological study was performed in the focus of Khemis el Kechna in 1987-1988; 1,876 parasitological samples and 1,760 serological samples were collected. The parasitological prevalence was 4.90%, the serological prevalence was 15.6% with a wider distribution by age.

[Abnormal hemoglobins in Algeria].

The existence of frequent hemoglobin abnormalities raises a Public Health problem in Algeria. The presence of genes for hemoglobin S, hemoglobin C and thalassemia in various regions of the country causes severe congenital hemolytic anemias: thalassemia, sickle-cell anemia, S thalassemia, association of HbS and HbC. These diseases are often invaliding requiring frequent admissions to hospital and have a high social cost. We have also observed certain rare hemoglobins, of which some, hemoglobin Setif, hemoglobin D Ouled Rabah were described for the first time in Algerians. They are not pathogenic in the heterozygous state.

[1st cases of alpha-thalassemia in Algeria: 12 cases of hemoglobinosis H].

These first cases of hemoglobinosis H show that alpha-thalassemia is not a simple curiosity in this part of the Western Mediterranean. They are not localised to a single part of Algeria as the areas from which the patients came were more than 150 km apart. They suggest that the enquiry should be continued by other means to determine the prevalence and pathological incidence.

[Incidence of cancer in the wilaya of Setif, Algeria].

Preliminary results are presented for a general population-based cancer registry in Setif, Algeria, for 1986-88. Standardised incidence rates for all sites, excluding non-melanoma skin cancer, are 70.1 for men and 59.9 for women: these rates are lower than those reported for most populations. The most frequent cancers are lung, stomach and liver in men, and cervix, liver and breast in women. Nasopharyngeal carcinoma is frequent in both sexes. Incidence of cancers of the gallbladder and extrahepatic bile ducts in women appears particularly high. These results represent the first detailed incidence data for all cancers in an Algerian population.

[Fetomaternal rhesus immunization in Algeria].

The frequency and modalities of immunisation in the rhesus system were studied in 847 rhesus negative. Algerian women, and in 247 foreign women. 33% of the Algerian women and 8% of the foreign women were immunised. The high muliparity still frequent in Algeria is one of the causes of this difference. The specificity of the antibodies, the mode of foetal involvement, and the antibody titre in relation to the number of pregnancies and the month of the pregnancy are indicated.

[Idiopathic thrombopenic purpura in Algeria].

The study in Algeria over a period of 3 years of 90 cases of idiopathic thrombopenic purpura out of 409 hemorrhagic syndromes, showed the following peculiarities: the chronic forms are as frequent in the child as in the adult and we did not observe any subacute forms. Except in one case the use of prednisone at a dose of 2 mg/kg gave no result in patients who were not improved by a dose of 1 mg/kg.

[Epidemiologic survey and serodiagnosis of contagious agalactia of small ruminants in Eastern Algeria].

Contagious agalactia is largely present all over the world and affects most of the African countries. It plays an important economic role in terms of subsequent milk and meat losses. One third of the 15 millions of sheep living in Algeria is present in the Eastern regions. A survey was conducted in order to confirm the existence of the disease and to evaluate the prevalence. A total of 372 sera and 80 milk samples was collected from suspected herds i.e. herds presenting either mastitis and arthritis symptoms or mastitis and keratitis symptoms. It was possible using several serological tests to detect the positive animals and to compare the specificity and sensitivity of the tests.

Kidd and MNS gene frequencies in Algeria.

Red blood cell phenotyping using micro-methods was carried out for Kidd (JK) and MNS systems in Algeria. The number of subjects tested was 3495 and 4388 respectively. Gene frequencies were estimated by the maximum likelihood method. The distribution of JKa seems to follow a West-South East-South gradient, whereas the MNS haplotypes show a heterogeneous distribution. Our results indicate an intermediate status of the Algerian population between Negroids and Caucasoids: JKa and MS haplotypes are medium, while MSU and NSU show low frequencies.

[Isolation of Enterovirus 70 during an outbreak of acute hemorrhagic conjunctivitis in Algeria in 1973].

The Enterovirus 70 was isolated in Algeria in 1973 during an outbreak of acute hemorrhagic conjunctivitis. The main epidemiological features were similar to those previously described in other countries: involvement of the adults especially, mainly those living in communities, irrelevance of the different socio-economical levels of the population. The spring waters may have played a role in the transmission of the disease. The virological study draws the attention on the optimal conditions for the isolation of the virus and shows that the strains are antigenically related to those isolated in Morocco during 1971. The Enterovirus 70 was associated with adenoviruses and it was not possible to evaluate the respective role of the two groups of viruses in the determinism of the epidemic. The detection of the residual antibodies in sera collected in 1974 indicates that 58% of a group of 50 adults have antibodies against the Enterovirus 70. It is not yet possible to ascertain whether those antibodies are specific or only reflect an antigenic crossing between the Enterovirus 70 and one or several already known enteroviruses.

[Epidemiology of juvenile insulin-dependent diabetes in Algeria (Wilaya of Oran)].

In order to precise the prevalence and incidence of IDDM in a well characterized population, we have ascertained between 1979 and 1988 all juvenile diabetic patients living in the department of Oran (Wilaya of Oran), Algeria, aged less than 20 years at the date of Dec. 31th 1988 who had onset of the disease before 15 years old. Cross comparisons of the data obtained both by a IDDM registry and a questionnaire sent to the practitioners show an excellent correlation. A total of 173 families was studied, including 155 Simplex Families (SF), and 18 Multiplex Families (MF). The prevalence was 0.27 p. 1000 among children aged less than 15 years. The annual incidence (mean = 4.4 p. 100,000) increased regularly from 1.6 p. 100,000 in 1981 to 8.1 p. 100,000 in 1988. The prevalence of IDDM among siblings was 3.8%. The birth order of index cases (first affected siblings) was lower in MF (3.0 +/- 0.4) than both in index cases of SF (4.0 +/- 0.3; p less than 0.01) and in secondary affected siblings of MF (4.8 +/- 0.05; p less than 0.001). The frequency of 2nd degree consanguineous marriages (between first cousins) was greater among the IDDM families compared with control families: 25% vs 11% (RR = 2.9; p less than 0.001).

Herpetic and chlamydial genital infections in Algiers, Algeria: value of the Papanicolaou staining.

The prevalence of herpetic and chlamydial genital infections was studied in 309 women attending an outpatient gynecology clinic (Algiers, Algeria). Cell culture, IF on smears and Papanicolaou staining were the methods used for HSV diagnosis, serology by the MIF method and Papanicolaou staining were used for Chlamydia trachomatis diagnosis. The prevalence of herpetic genital infection was estimated at 7.2% by cell culture, 89% by IF method, and 3.9% using the Papanicolaou staining. Specific anti-Chlamydia trachomatis IgG with a titer greater than or equal to 16 were found in 35.7% out of 95 sera, and 18% out of the 95 women had an IgG titer greater than or equal to 64. If we consider IgG titer greater than or equal to 64 as significant for an active chlamydial genital infection, the concordance rate between the Papanicolaou staining and the MIF method is of 48%. However, the Papanicolaou method appears as insensitive for both the HSV and Chlamydia trachomatis diagnosis. The relative specificity of this method for HSV diagnosis accounts for its use as a screening test.

[Practical methods for rabies control in Algeria].

Control measures currently in use in Algeria have not given the results expected. Suggestions are made of the eradication of rabies in rural areas as well as in urban ones by concentrating on the dog and adapting the legislation.

[Epidemiology of rabies in Algeria].

An epidemiological study on rabies in Algeria has been carried out on data provided by the "Institut national de Santé publique" and the "Institut Pasteur d'Algérie". It showed that no region is unhurt. The animal reservoir is of the domestic type, essentially the dog. In 40% of cases, bites are due to stray dogs.

[Phage types of Vibrio cholerae and non agglutinable Vibrio isolated in Algeria from 1975 to 1977].

1 285 strains of Vibrio cholerae and 556 strains of non agglutinable Vibrio were isolated either from stool specimens or water in Algeria from 1975 to 1977. These strains were phage-typed with Basu and Mukerjee's 5 phages for the Vibrio biotype El Tor and with Mukerjee's phage IV for the Vibrio biotype cholerae. The results obtained are discussed in terms of choleraphages used for the diagnosis and epidemiological study of cholera.

[Some aspects of the evolution of cholera in Algeria from 1971 to 1975].

Taking as a basis information collected in their laboratory, the authors study certain aspects of the evolution of cholera in Algeria during the different outbreaks from 1971 to 1975. 6 301 strains of vibrio-cholerae (mainly serotype Ogawa) were isolated either from cholera patients or carriers. All but two of strains were sensitive to all antibiotics tested. Most of the Wilayate (districts) were affected during these outbreaks which took place from June to December; the majority of cases were observed from August to October but principally in September.

[Pseudo-exfoliative fibrillopathy in eastern Algeria. Epidemiologic and statistical study].

The systematic examination of 1440 patients of more than 40 years old divided into two groups of 607 hospitalised patients and 833 consultants has revealed the exceptionally high frequency of the pseudo-exfoliative disease in the East of Algeria. In fact this syndrome is find in 25% of the cases. The analysis relates the different epidemiology criteria: age, topography, geographic factors and biomicroscopic characters. The association to the glaucoma of this syndrome forms the subjects of a study which details particularly its frequency, its different clinical forms, the factors of the intraocular pressure and the evolution under treatment. The authors have compared this study to those wHich have been described in literature.

[Deficiency rickets: the current situation in France and Algeria].

Persisting vitamin D deficiency rickets in France results from the climatic, environmental and geographic situation of this country. Although systematic administration of vitamin D supplements to infants greatly reduced the prevalence of rickets among infants, clinical and/or biological signs of vitamin D deficiency are still found in children and adolescents, mainly during the winter and in populations vulnerable for economic, cultural or religious reasons. Signs of vitamin D deficiency are also found, during the winter-spring seasons, in pregnant women and their newborns living in urban areas. Such vitamin D deficiencies could be overcome by vitamin D supplementation to susceptible populations. In Algeria, vitamin D deficiency rickets present a continuing public health problem. The persisting high incidence of rickets among children appears to result mainly from economic and cultural factors. Vitamin D supplementation and health education are mandatory to reduce the prevalence of vitamin D deficiency among pregnant women and the occurrence of vitamin D deficiency rickets in infants, whether breast-fed or not.
