[The treatment results in inflammatory diseases of the adnexa uteri with different methods of using sinusoidal modulated currents].

Electrophoresis of 2% solution of sodium salicylate was made by impulse currents (SMC) according to transcutaneous, intracavitary, intraorgan techniques in 90 gynecological patients with nonspecific acute and chronic affections of the uterine appendages. Echography proved high efficacy of the intracavitary and intraorgan treatment. Simple performance of SMC therapy in combination with sodium salicylate solution microclysters and efficacy which was not inferior to intracavitary drug electrophoresis makes the method practicable in gynecology and physiotherapy.

[New approaches to the diagnosis and treatment of inflammatory diseases of the adnexa uteri].

The paper summarizes follow-ups of 2500 patients with inflammatory diseases of the uterine appendages, provides evidence for the benefits of laparoscopic diagnosis and treatment of all types of this condition, which makes it possible to verify the diagnosis and perform less traumatic organ-sparing operations. To use combined antiinflammatory antibiotic therapy in view of the etiology of inflammation, as well as immunomodulators is the requisite condition for treating acute types of a disease. The new approaches to the diagnosis and treatment of inflammatory diseases of the uterine appendages substantially improved therapeutical outcomes and maintained female reproductive function.

[Experimental and clinical studies of ceftazidime in the field of obstetrics and gynecology].

Ceftazidime (CAZ) was studied for its transference into adnexa uteri and uterine tissues as well as for its effects and safety on gynecological infections. The results obtained are as follows: Peak levels of CAZ were obtained in the tissues of adnexa uteri and uteri at 15--30 minutes after one shot intravenous injection of CAZ 1 g, and relatively high concentrations were maintained for several hours. In the treatment of 33 cases of gynecological infections, the clinical efficacy of CAZ was assessed as excellent in 13 cases and effective in 20 cases. As for the bacteriological effects of CAZ, 95.5% of clinically isolated organisms were eradicated. The laboratory tests performed before and after administration of CAZ revealed rise in GOT, GPT values in 2 cases and eosinophilia in 1 case. However, these cases were all mild and required no particular measures.

[Transfer of cefuzonam into female genital organs].

Cefuzonam (CZON) was given intravenously at a 1 gram dose to 24 patients prior to abdominal total hysterectomy for uterine myoma. Concentrations of CZON in the serum of cubital vein, uterine artery, and in tissues of uterus and adnexa uteri were measured by the bioassay method. Bilateral uterine arteries were clamped at 15 and 30 minutes and 1, 2, 4 and 6 hours after the administrations of the drug. Measurements were done on 4 patients at each sampling. Concentrations of CZON in the serum of cubital vein and uterine artery changed in a similar manner with respective half-lives of 52.0 minutes and 48.2 minutes. Peak concentrations in oviduct, ovary, endometrium, myometrium, cervix uteri and portio vaginalis were reached at 15 minutes after drug administration. The peak concentration in the ovary was as high as 37.5 micrograms/g, and those in other tissues varied between 17.6 and 25.5 micrograms/g. Tissue concentrations decreased gradually after their peaks in similar manners to those in serum, and their half-lives ranged between 37.4 and 53.7 minutes. From these results, it was found that CZON was transferred to female genital organs in high enough concentrations to inhibit growth of bacteria which were often isolated from the pelvic inflammatory diseases, and CZON seemed to be highly effective to clinical pelvic inflammatory diseases.

Adnexal and cul-de-sac abnormalities: transvaginal sonography.

Sixty-seven patients selectively chosen from 354 undergoing conventional transabdominal (TA) sonography for evaluation of a clinically suspected adnexal mass subsequently underwent transvaginal (TV) sonography either because the TA sonograms were technically suboptimal or because it was not possible to characterize with certainty an abnormality identified with TA sonography. TV sonography added diagnostically useful information in 25 of 28 patients with cystic pathologic changes in the adnexa uteri. Eight of 12 patients with tuboovarian abscess and nonspecific adnexal masses visualized with TA sonography had tube-shaped fluid collections characteristic of pyosalpinx identified with TV sonography. TV sonography added diagnostically useful information in all seven patients with diseases of the cul-de-sac (rectouterine fossa) and allowed differentiation of adnexal from primary uterine disease in three patients with TA sonograms on which findings were equivocal. It also expedited the diagnosis of a tubal pregnancy in ten of 14 patients and was useful in the detection of adhesions and perforated intrauterine devices. These results indicate that adjunctive TV sonography can provide important diagnostic information.

Uterine metastases from breast cancer in a patient under tamoxifen therapy. Case report.

The authors report a case of metastases of breast cancer confined to the uterus and cervical subserous leiomyoma in a postmenopausal woman under tamoxifen therapy for two years. After abnormal uterine bleeding, pathologic examination on biopsy of a cervical polyp revealed cervical involvement secondary to lobular breast cancer. The patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy. Pathologic examination of the surgical specimen revealed both invasion of breast origin and of the cervix until the isthmus, endometrium and cervical subserous leiomyoma. The adnexa uteri were not affected. The possibility of uterine metastases in patients suffering from breast cancer, either undergoing tamoxifen therapy or not, always has to be considered.

[A rare case of ovarian cancer in a patient treated for secondary sterility].

The authors described a rare case of ovarian cancer in a young woman who looked for gynecological help because of secondary sterility. There were found cysts of the both ovaries, which were conservative treated by insistence of the patient for three months. After unsuccessful gestagenic therapy there was performed a gynecological laparoscopy, which showed that on one of the ovaries there was an exophyte growths suspected for carcinoma tissue. It was offered an emergency operation, which was made only after 1,5 months because of personal causes of the patient. It was taken a sample that was with unconvincing data for ovarian cancer. The clinical experience and professionalism of the surgical team were put on the test. It was performed a radical operation with removal of the uterus, adnexa uteri, partial peritoneoectomy, total lymph dissection, appendectomy, total omentectomy. The patient was discharged from hospital in good condition and she is liable to postoperative chemotherapy.
