The usefulness of a new pediatric duodenoscope PJF in the diagnosis of neonatal cholestasis was studied in 23 infants with ages ranging from 19 to 150 days. In 22 of 23 infants the papilla was cannulated. In 13 of 14 neonates (93%) with neonatal hepatitis, the common bile duct was opacified and biliary atresia was excluded. In one of two neonates with choledochal cyst, the common bile duct was demonstrated. In six of seven neonates (86%) with biliary atresia, only the pancreatic duct was demonstrated and the diagnosis was suspected. Although absence of a common bile duct opacification does not rule out biliary atresia, ERCP with the new duodenoscope proved to be most useful in the diagnosis of normal biliary tree and served to avoid unnecessary surgery in most infants with neonatal cholestasis.

Pharmacokinetics is the study of the time course of drug absorption, distribution, metabolism, and excretion, allowing examination of the potential relationships between drug disposition and pharmacologic or toxicologic effects. The pharmacokinetics of the aminoglycosides and certain other drugs are different in patients with cystic fibrosis (CF), but this topic is controversial. Differences in disease severity between study subjects and in the methods used might explain the disparities. Understanding the fundamental principles of pharmacokinetics is necessary for the clinician to evaluate drug disposition data in patients with CF. To determine whether observed pharmacokinetic differences are attributable to CF, the investigator must consider a number of factors in the design and conduct of pharmacokinetic studies: analytical methods, study population selection, techniques for drug administration, method used to collect biologic specimens, evaluation of parallel rates and routes of drug excretion, and selection of pharmacokinetic and statistical techniques. Pharmacokinetic investigation in patients with CF should permit evaluation of the complete disposition profile for a drug, allow comparison between the experimental data and factors that characterize the disease state, and be rigorous enough to provide explanations for any observed variability in pharmacokinetics.

Extracorporeal shock wave lithotripsy has revolutionized the clinical treatment of renal calculi. A device was constructed with a spark gap for shock wave generation and a water-filled ellipsoidal reflector to focus the shock wave energy. A membrane coupled the device to the pig and an ultrasonic transducer was used for stone visualization. Initial in vitro experiments and in vivo studies using dwarf pigs demonstrated fragmentation of calculi.
A 3F electrohydraulic electrode was used with flexible ureteroscopy to treat upper urinary tract calculi in 18 patients. Of 21 procedures performed 10 involved retrograde passage of the ureteroscope and 11 consisted of antegrade procedures with percutaneous nephrostomy access. Complete fragmentation was achieved in 19 of the 21 procedures. In 1 patient bilateral renal caliceal diverticular calculi were treated with the electrohydraulic electrode used to open initially the diverticular orifice. Minor complications included retained stone fragments and small ureteral perforations. No serious complications resulted from the lithotripsy and no patient required an open operation. Flexible ureteroscopy, retrograde or antegrade, appears to offer access to upper tract stones not treated reliably with rigid ureteroscopy. Electrohydraulic lithotripsy with this small 3F probe appears to be a safe and effective means to treat upper urinary tract stones.
Druckrey analysis of the results of skin painting TG.AC transgenic mice with three different doses of benzo[a]pyrene was used to obtain quantitative estimates of the in vivo cancer chemopreventive efficacy of N-difluoromethylornithine. The percent decrease in mean rate of subvisible tumor development during the tumor latent period produced by 2000 ppm and 4000 ppm DFMO in the diet was 39% and 50%, respectively. Druckrey analysis was also successfully applied to transformations of published data on human cigarette smokers. The time before onset of lung cancer in 2% of smokers was prolonged 17 weeks for every cigarette per day not smoked. The Druckrey model, of substantiated validity because of the results in human smokers, provides a rapid quantitative screening method applicable to large numbers of candidate chemopreventive agents.
Insecticide measurements and standard World Health Organization bioassays on random samples of new unwashed, traditionally washed and up to 18 months field-used 'long-lasting' deltamethrin treated mosquito nets demonstrated a rapid reduction of efficacy under field conditions. The technology of 'long-lasting' insecticide-treatment needs much improvement.
