Multicenter study of incidence of Mycobacterium marinum in humans in Spain.

A retrospective survey was carried out on the epidemiological monitoring of the isolation incidence of Mycobacterium marinum in 21 laboratories, in an attempt to gain an insight into the frequency of its isolation in Spain. From 1991 to 1998, 39 cases (22 male and 17 female) of bacteriologically confirmed M. marinum infection were accumulated. The majority of the cases (35) were fish related. The clinical presentation usually suggested sporotrichosis, with the majority of skin eruptions appearing on the hand (32 cases). For treatment, minocycline was recommended as the drug of choice in 12 cases, rifampicin in eight cases and clarithromycin and ethambutol in seven cases.

Deep water running: an effective non-weightbearing exercise for the maintenance of land-based running performance.

Deep water running (DWR) has become a well-recognized from of cardiovascular conditioning for injured athletes and has been used successfully to maintain running performance. DWR provides for decreased stress and weightbearing to injured tissue and joints, allows for maintenance of cardiovascular fitness and a training effect, and offers greater specificity of exercise in relation to running. During a 22-month period, 181 active duty Army soldiers, placed on temporary profiles for injuries that precluded them from their regular weightbearing physical fitness activities, participated in a DWR program. Injuries to the back, knee, and ankle were the most common reasons for referral to the program. This article reviews the physiological characteristics of DWR, specifics of DWR program design, DWR mechanics, and the advantages of DWR over other aerobic forms of exercise to maintain land running performance in military personnel on temporary non-weightbearing profiles.

Responding to fecal accidents in disinfected swimming venues.

These recommendations are solely for management of fecal accidents in disinfected recreational water venues. The recommendations do not address use of other nonchlorine disinfectants because there is limited pathogen inactivation data for many of these compounds. Because improper handling of chlorinated disinfectants could cause injury, appropriate occupational safety and health requirements should be followed.

Responding to aquatic facilities.

The primary and fundamental responsibility of a local EMS system is to provide prehospital care to the citizens of the community. Therefore, each community must have a system in place to ensure continual, efficient prehospital care. There are a variety of levels of EMS education, which include first responder, EMT-Basic, EMT-Intermediate and EMT-Paramedic, along with other components, including the emergency medical dispatcher, other healthcare providers and specialty care centers. In order for a community to provide and ensure continual, efficient prehospital care, the components must work together in a well-planned and coordinated effort. If lifeguard personnel are expected to respond to respiratory, cardiac and spinal-injury incidents within their aquatic facilities, they must be included within this comprehensive community EMS system. Therefore, fire, rescue and other prehospital EMS agencies must coordinate their efforts and training responsibilities with the local lifeguard services in their area.

[Cryptosporidium y Giardia in surface water].

The aim of this work was to determine Giardia and Cryptosporidium levels in surface water used either as recreational or drinking water source, and to estimate the relationship with bacteriological and physicochemical parameters determining water quality. This study was carried out during 22 months, in Santa Fe and Santo Tomé cities (Santa Fe Province, Argentina). Cryptosporidium from 100% of the recreational samples (44-2404 oocysts/100 l) and from 92% of water sources (< 20-539 oocysts/100 l) was detected, while Giardia was found in lower concentrations and frequencies: 72% (< 20-670 cysts/100 l) and 31% (< 20-65 cysts/100 l), respectively. Statistically significant relationships were found between these parasites and organic matter, turbidity and bacteriological indicators of contamination (Escherichia coli, streptococci, Enterococcus spp, Pseudomonas aeruginosa). These relationships, however, vary according to the sampling place, the source and levels of contamination.

["Banal dermatomycoses" that cannot be overlooked].

Two young adults presented an interdigital intertrigo and a dyschromic nail alteration, respectively. They were adept of intense sport swimming in pools. Lesions had been misdiagnosed as athlete's foot and onychomycosis, respectively. Several antifungal treatments had been tried without success. After many months of inadequate management, the dermatological examination revealed the presence of two malignant melanomas corresponding to the eroded acrolentiginous and nodular subungueal types, respectively.

[A contribution to the treatment of otomycoses (author's transl)].

It is reported about the treatment of 39 cases of otomycosis with the local specificum Clotrimazol (chemical term: Bis-phemil-(2 chlorphenyl-)1 imidazol-methan), on the market as "Canestan". In 35 cases complete and permanent healing was observed within a period of 8-12 days. In 2 cases the treatment took 3 weeks. Since irritations of the concha or the cheek below the concha respectively happened only in 2 cases, the compatibility can be regarded as good.
