Density dependence of pulmonary resistance: correlation with small airway pathology.

The density dependence of maximal expiratory flow is not an effective test of the site of airway narrowing in obstructive lung disease. We hypothesized that the density dependence of pulmonary resistance (DD,RL) would be more closely related to the degree of airway narrowing and peripheral airway pathology in smokers. We measured maximal expiratory flow at 50% vital capacity (V'max50) and lung resistance (RL) breathing air and 80% helium-20% oxygen, and calculated density dependence of V'max50 and RL in 40 patients who had moderate airflow obstruction and in 10 normal subjects. We compared the density dependence of RL and V'max50 with the degree of airway obstruction and bronchiolar pathology scores in 27 patients with resected lung specimens. There were no differences in DD of V'max50 or RL between normal subjects and patients, and no relationship between the degree of obstruction or the bronchiolar pathology score and the DD of these measurements. There were significant relationships between V'max50, RL and the bronchiolar pathology scores. In conclusion, lung resistance and maximal expiratory flow are related to the severity of peripheral airway pathology, but there is no relationship between the severity of obstruction or the severity of peripheral airway pathology and the density dependence of maximal expiratory flow or lung resistance.

Advances in automation for experimental pathology.

The establishment of the National Center for Toxicological Research (NCTR), Jefferson, AR, to study long term effects of low doses of potentially toxic substances, including carcinogens, resulted in the initiation of chronic experiments requiring histophatologic studies of large numbers of animals. Processing the tissues and recording and analyzing the massive amount of pathologic data from these studies required innovative automated technics. The result was a unique computerized pathology data system of NCTR. The system encompasses the development of training programs for necropsy techincians, histology technicians, and pathology tissue screeners; the use of automated equipment for processing, embedding, sectioning, and staining tissue; the use of prelabeled cassettes, slides, and pathology forms; the use of mark-sensitive forms for the collection of gross and microscopic findings; the use of data collection terminals for animal identification, animal weights, and organ weights; the collection of gross and microscopic data on a Mod Comp III minicomputer; and the storage, analysis, and generation of pathology reports and tables over a data link from an IBM 370-158 computer. Some of the features of this system may be applicable in other toxicology laboratories and medical pathology departments.

[Relationship between bone fluoride content, pathological change in bone of aborted fetuses and maternal fluoride level].

Relationship between bone fluoride content, pathological change in bone of aborted fetuses and maternal fluoride level was studied in 46 pregnant women and their inducedly-aborted fetuses. Results showed fluoride content in fetal femur averaged 368.2 micrograms/g, and 41.4% of the bone with pathological change. Fluoride levels in maternal urine and amniotic fluid and fluoride content in fetal femur and pathological change in fetal femur appeared a positive correlation between them. Femur fluoride content and pathological change of bone in fetuses born to mothers with mottling teeth were significantly greater than to those without them. Pathological change in fetal femur presented dose-response relationship with their bone fluoride content. When the latter reached greater than 500 micrograms/g, pathological changes occurred in 90% of the bone.

[The value of anatomo-pathologic examination of the placenta in areas of endemic malaria and low socio-cultural levels].

OBJECTIVES: Determine the sensitivity of the pathology examination of the placenta as a screening examination for malaria and the consequences of this infection on prematurity and birth-weight. METHODS: Eighty placentas were examined at the Mjunga, Madagascar dispensary at the beginning of the rainy season. The aspect of the placenta was compared with a malaria index and to malaria disease state as a function of parity and anti-malarial prophylaxis used by the mother as well as with the state of the infant. RESULTS: Among the placentas examined, 41.3% were considered normal and abnormal or clearly pathological in 58.7%. Estimating the gestational age on the basis of the histological examination of the amniotic cells was in agreement with the gestational age calculated from the last cycle in 53 cases and in disagreement in 8 cases. The percentage of cases of malaria discovered by the pathology examination (20%) was greater than that after thick swab screening (10%). 75% of the mothers has Plasmodium falciparum infection at the time of delivery and 13.8% of the mothers with negative thick drops had malaria lesions of the placenta. The parity of infected mothers was similar to non infected mothers. All the premature newborns had pathological placentas included 12.5% with malarial lesions. 90% of the hypertrophic newborns had pathological placentas included 50% with malarial lesions. No case of congenital malaria was observed. CONCLUSION: Pathology examination of the placenta is as sensitive as blood drop tests for screening for malaria. The histological examination of amniotic cells can give a good estimation of gestational age in developing countries.

Missed pathology following laparoscopic cholecystectomy: a cause for concern?

Although many have recorded the incidence of complications after laparoscopic cholecystectomy, few have discussed the possibility of missing intra-abdominal pathology after this procedure. We have evaluated the first two years, September 1990-September 1992, of laparoscopic cholecystectomy in our community. Readmissions within 10 months of the original surgery with another diagnosis similar to gallbladder disease were considered "missed pathology" at the original surgical procedure. For the first 12-month period, 465 laparoscopic cholecystectomies were performed. Seventy-seven patients were readmitted, with 13 of these patients having other intra-abdominal pathology. These readmissions were for carcinoma (6), inflammatory bowel disease (2), diverticular disease, esophageal varices, and appendicitis. In the second year 429 laparoscopic cholecystectomies were performed; 59 patients were readmitted, with 10 of these patients having other intra-abdominal pathology. These readmissions were for carcinoma (3), inflammatory bowel disease (2), strongyloides, peptic ulcer disease, and abdominal pain of unknown etiology (3). Although intra-abdominal pathology was found in only 2%-5% of all patients having surgery for gallbladder disease, of the patients who were readmitted for "missed pathology," 46% the first year and 30% the second year were readmitted for carcinomas. Several other diseases were found in patients whose symptoms mimicked gallbladder disease. It is therefore possible that in the zeal to perform a new procedure, other diagnoses may be overlooked.

Social pathologies in Zimbabwe.

This paper looks at the trend of social pathologies (diseases and cause of death which are more social in origin than biological) in Zimbabwe over the past decade. The rate of increase was found to be very high, even by international standards. In order to find plausible explanations for this rapid rise, the Zimbabwean situation was compared to two different populations; the Navajo Indians in the United States whose rate of social pathologies was also high and the Island of Mauritius whose level of socio-economic development was similar to that Zimbabwe but whose rate of social pathologies was very low. It was concluded that the reason for the rise in social pathologies in Zimbabwe was due to both socio-economic development and excess alcohol consumption. Efforts to combat social pathologies, however, should first be directed towards the problem of excess alcohol consumption which is more directly related to social pathologies than socioeconomic development.

Surgical results and prognostic factors of pathologic N1 disease in non-small-cell carcinoma of the lung. Significance of N1 level: lobar or hilar nodes.

The surgical outcome of pathologic N1 disease is controversial. To clarify whether pathologic N1 disease is a uniformly intermediate group or a mixed group of potentially early stage disease and advanced stage disease, we reviewed our previous cases with pathologic N1 disease. We retrospectively investigated 78 patients with pathologic N1 disease who had undergone a complete resection with mediastinal lymph node dissection during the period from April 1972 to December 1990. The cumulative postoperative survival at 5 years was 49.2%. No significant difference in the survival was found according to the following variables: sex, primary site, pathologic T factor, histologic type, type of resection, performance of adjuvant therapy. The lobar lymph nodes (Nos. 12 and 13) were only involved in 30 patients (38.5%), whereas the hilar nodes (Nos. 10 and 11) were involved in 48 patients (61.5%). The survival associated with lobar N1 disease was significantly better than that of hilar N1 disease (64.5% versus 39.7% at 5 years; p = 0.014). In lobar N1 disease, the brain was the most frequent site of distant metastasis, whereas the lungs were the most frequent site in hilar N1 disease. It was suggested that pathologic N1 disease is a mixed group of potentially early stage disease and advanced stage disease with regard to the postoperative prognosis.

[The pattern of distribution of free amino acids in human amniotic fluid related to the duration of pregnancy. Part II: Changes in concentration in pathological pregnancy (author's transl)].

18 free amino acids from 196 specimens of amniotic fluid of pathological pregnancies between the 28th and 41st week of pregnancy were measured by gaschromatography. The concentrations of the amino acids in amniotic fluid in normal and pathological pregnancies were analysed by statistical methods. There were only minor differencies of concentration of free amino acids in the amniotic fluid between normal and pathological pregnancies with the exception of hydroxyproline. This was found in a significantly higher concentration in the amniotic fluid of the pathological group. Supposing that the materno-fetal as well as the feto-maternal amino acid exchange may be reduced by the placenta insufficiency, the significantly higher concentration of hydroxyproline in the amniotic fluid during pathological pregnancies may be caused higher fetal urine excretion of hydroxyproline into the amniotic fluid. The increase of hydroxyproline in the amniotic fluid of pathological pregnancies is probably an early diagnostic parameter of an impaired transplacental exchange -- placenta insufficiency.

Pathological gambling in hospitalized substance abusing veterans.

BACKGROUND: Previous studies have suggested that pathological gambling and substance abuse may be more likely to occur together than expected by chance. We examined this possibility as well as explored psychosocial and diagnostic variables that may be associated with this coincidence. METHOD: Of 276 patients who were administered the South Oaks Gambling Screen, 134 subjects were interviewed using a semistructured comprehensive psychiatric questionnaire. Data analysis utilized Student t tests or analysis of variance routines. RESULTS: The rate of comorbid pathological gambling in the sample was 33% (92 of 276). A high rate of comorbid substance abuse and pathological gambling was associated with a positive history of childhood experiences of gambling in the family group (p = .001) and with larger family size (p = .001). In addition, current alcohol consumption was significantly higher (p = .007) in the pathological gambling group. CONCLUSION: Comorbidity of substance abuse and pathological gambling is common in substance abuse patients in a VA hospital. Substance abuse treatment programs should identify patients with pathological gambling and include treatment interventions that address both problems.

[The presence of pathological processes among spanish drivers: its relevance to road safety].

BACKGROUND: The presence of pathological processes is considered to be a risk factor in road traffic accidents. The Council Directive 91/439 (29 July 1991) and the legislation prevailing in our country (RD 1467/1983 and 13/1992) specify the different diseases or deficiencies which can affect the ability to drive appropriately. The aim of this study is to assess the presence of pathological processes among spanish drivers. METHODS: A sample of Spanish drivers were selected and a self questionnaire was sent to them. 1500 questionnaires were considered to be valid. RESULTS: The health state of the majority of the drivers was shown to be good (61.8%) or very good (35.3%). 15.3% of those surveyed suffered some pathological process, of which 52.0% were considered capable of affecting their ability to drive without any danger (7.9% among all surveyed). 6.6% of the drivers with pathological processes considered that their ability to drive had decreased because of that reason, and 3.1% had stopped driving. CONCLUSIONS: The presence of incapacitating pathological processes among Spanish drivers is frequent. Health care workers should be aware of this and should inform their patients of the possible effect of a pathological process and other risk factors concerning driving ability.

[Cancer and pathologic hyperplasia of endometrial mucosa. Clinical study].

The authors carried out a tentative evaluation of the factors regarded as increasing the proneness to endometrial cancer development. Particular attention was paid to pathological hyperplasia of the endometrium. The studied material consisted of 592 women (76 with endometrial cancer and 516 with endometrial hyperplasia). The incidence of pathological hyperplasia of the endometrium was six times higher than that of endometrial cancer. Endometrial cancer was found mainly in postmenopausal women. Apart from pathological hyperplasia of the endometrium, among other factors characterizing women developing endometrial cancer, hypertension should be mentioned in the first place. In the postoperative material obtained from women operated on for endometrial cancer (76 cases) and pathological endometrial hyperplasia (244 cases) the character of pathological changes in removed ovaries in 2/3 of the cases pointed to hyperplastic non-malignant lesions. In 1/3 of the women either no pathological changes or only fibrosis and ovarian tissue atrophy were found in the ovaries.

[Role of Chylamydia in the pathology of pregnancy and chronic gynecological diseases].

For the elucidation of the role of chlamydial infection in pathological pregnancies, serological examination by the CFT with group-specific and species ornithosis antigen was carried out on 723 blood serum specimens from women with obstetrical pathology, 124 specimens from patients with various chronic gynecological diseases and 124 control blood serum specimens from women with nromal pregnancy and favourable obstetrical anamnesis. The complement-fixing antibody (CF) was detected in 72 (9.9%) serum specimens from women with obstetrical pathology and chronic gynecological diseases. No CF antibody was found in control sera. Negative results of the CFT were obtained with species ornithosis antigen in all sera. In parallel examinations of the blood serum specimens from women with obstetrical and gynecological pathology by the CFT and intradermal test, correlation of positive results was demonstrated in 62.5% of the patients. Among the women with positive reactions for chlamydial infection, in 52.8% thee pregnancy was pathological. Among 52 sero-positive pregnant women the death of the fetus was observed in the perinatal period in 21.2%. Among 111 babies born alive after previous pregnancies 55 babies (49.5%) died at the age under 1 year. Thus, chlamydial infection may possibly have causative relation to pathological pregnancy, death of the fetus in the perinatal period, and death of the babies under 1 year.

[The treatment of the cardiac patient in dentistry and oromaxillofacial surgery. II. The practical management of patients with hemodynamic pathologies].

When odontostomatological or surgical treatment is performed we suggest, in a first phase, to distinguish cardiac patients from the others. In a second phase a careful nosological diagnosis will be performed. Consequently, patients' medical history plays a fundamental role in both diagnostic phases. In this article the authors present the results of several years of research carried out to obtain a correct clinical and therapeutic approach for clinical and surgical Odontostomatology. After an introduction on the clinical features of heart hemodynamic pathologies the most important clinical cases are discussed: like, for example, acardiohemia, valvulopathies and heart decompensation. The principal diacritic features of hemodynamic diseases are illustrated. Essential hypertension (borderline and resident) is a sort of disease not directly related to hemodynamics pathology. Moreover a lot of clinical studies find in heart hemodynamic pathologies the principal problem caused by hypertension on heart physiology. Consequently the authors describe essential hypertension in the same part of pathologies commonly known as heart hemodynamic pathologies. In the last phase the authors illustrate the most opportune therapeutic steps corresponding to the principal pathologies above-described. These matters were dealt with from an odontostomatological point of view. The results obtained suggest the necessity of keeping to the management that was described. Actually a low percentage of accidents occurred only when the above-mentioned clinical processes were completely performed.

Graft arteriosclerosis-induced myocardial pathology in heart transplant recipients: predictive value of endomyocardial biopsy.

BACKGROUND: Ischemic myocardial pathology resulting from graft arteriosclerosis (GA) includes subendocardial myocyte vacuolization (SEMV), indicative of sublethal ischemic injury, and coagulative myocyte necrosis, acute and healing (CMN), indicative of infarction. METHODS: To assess the sensitivity and specificity of myocardial pathology resulting from GA in endomyocardial biopsy specimens, we correlated SEMV and CMN in the final three biopsy specimens (mean interval from last biopsy to death 23 +/- 20 days) with autopsy findings from 30 heart transplant recipients who survived more than 3 months and died of GA (n = 16) or of other causes (n = 14). The two groups were similar in other parameters. RESULTS: Myocardial ischemic injury was present at autopsy in all 16 patients with GA with the following distribution: SEMV (n = 13) nine right ventricle (RV) and left ventricle (LV), four LV only; focal CMN (n = 8) six RV and LV, two LV only; subendocardial infarct (n = 3) three LV only; and transmural infarct (n = 3) one RV, two LV. Ischemic injury was present in the RV of 11 of 16 patients with GA. Of patients without GA, one had SEMV at autopsy; none had infarcts. Ischemic myocardial pathology was present in 10 of 48 biopsy specimens from patients with GA compared with one of 41 biopsy specimens from patients without GA (p < 0.05). The specificity of SEMV on biopsy was 98%, but sensitivity was only 17%. The positive predictive value for ischemic injury was 92%, and negative predictive value was 51%. CONCLUSIONS: Myocardial pathology resulting from ischemia was present at autopsy in all patients dying of GA. Although more prevalent in the LV, 69% of patients had ischemic myocardial pathology in the RV, where it may be accessible to biopsy. Ischemic myocardial pathology in biopsy specimens is highly specific but far less sensitive, for diagnosing GA.

[Relationship between imaging and pathological features and clinical factors in surgical cases of temporal lobe epilepsy].

The relationships between imaging, pathology and presumed causes in surgical cases of temporal lobe epilepsy (TLE) was studied. The subject was 62 patients (33 males and 29 females) who had had no attacks for more than one year after surgery. Average age at surgery was 28.2 +/- 9.9 years. Obvious neurological abnormalities were not found in these cases. MRI, PET and SPECT were performed. Hematoxylin and eosin was used for pathological judgement. Their medical charts were used to investigate their clinical factors. Although patients suspected of having encephalitis/meningitis had been hospitalized for 2 days to three months during childhood due to disturbance of consciousness with high fever and convulsion for several days; they were not diagnosed with encephalitis/meningitis at that time, and they suffered almost no handicaps other than epilepsy for several years following their comatose episodes. [Result] (1) On MRI, mesial temporal sclerosis (MTS) was detected in 48 of 52 patients (92%); 32 (62%) had high-signal intensity on T 2-weighted images; 31 (60%) had atrophy ¿23 (44%) had high-signal intensity on T 2 + atrophy¿; 5 (10%) had calcified lesions; and 2 (4%) had cystic lesions. On PET and SPECT, abnormal cerebral blood flow was noted in 33 of 36 (92%). (2) On pathological findings (61 cases), Ammon's horn sclerosis (AHS), tumors, gliosis in lateral temporal and meningeal inflammatory finding were detected in 42 (69%), 10 (23%) and 8 (13%) cases, respectively, whereas 2 showed no abnormalities. The 2 patients with normal pathology showed both high-signal intensity and atrophy on MRI. (3) The presumed causes of TLE were encephalitis/meningitis and/or suspected of these diseases in 15 patients (24%), injuries at birth in 5 (8%), and none in 42 (68%). The presumed causes in the 43 patients with AHS were encephalitis/meningitis in 11, injuries at birth in 3, and none in 29. Of the 15 patients in whom encephalitis/meningitis was estimated as the causes of TLE, only 6 (40%) had pathological evidence of meningeal inflammatory finding. Of the 42 patients in whom cause could not be determined, 2 had pathological evidence of meningeal inflammatory finding.

Prostate specific antigen and pathological features of prostate cancer in black and white patients: a comparative study based on radical prostatectomy specimens.

PURPOSE: We compared the relationship of pathological features and preoperative prostate specific antigen (PSA) levels of a consecutive series of black patients to a stage matched cohort of white patients treated during the same period. MATERIALS AND METHODS: The radical prostatectomy specimens of 40 black patients were reviewed and tumor volume was determined. Histopathological features (stage, grade, zonal distribution of cancer foci), tumor volume and prostate weight were correlated to pretreatment serum PSA levels. These parameters were compared with those of 148 white patients matched by pathological stage. RESULTS: Black patients exhibited a significantly higher incidence of seminal vesicle involvement (p=0.03) and cancers with a Gleason score of 8 or more (p=0.02), and a trend toward decreased pathologically organ confined, margin negative disease (40% black versus 53% white men, p=0.13). Although the incidences of multifocal cancer were virtually identical (90 and 82%) in the 2 groups, black patients exhibited a higher incidence of transition zone cancer foci (p <0.001). Mean prostate tumor volume, total gland weight and serum PSA level among black and white patients with pathological stage pT2-, pT2+ and pT3- cancer were not significantly different. However, with advancing pathological stage (pT3+ and pT3c) disease black patients had higher preoperative serum PSA levels on univariate and multivariate analyses despite similar total gland weight and tumor volume. CONCLUSIONS: Black patients who underwent radical prostatectomy often exhibited adverse pathological features. Two novel findings were that the distribution of cancer foci within the prostate was significantly different between black and white patients, and that serum PSA levels in patients with locally advanced prostate cancer were higher in black than in white men, despite adjustment for known variables affecting PSA. These observations suggest that differences in the biology of prostate cancer between these 2 races may exist.

Decorin and biglycan of normal and pathologic human corneas.

PURPOSE: Corneas with scars and certain chronic pathologic conditions contain highly sulfated dermatan sulfate, but little is known of the core proteins that carry these atypical glycosaminoglycans. In this study the proteoglycan proteins attached to dermatan sulfate in normal and pathologic human corneas were examined to identify primary genes involved in the pathobiology of corneal scarring. METHODS: Proteoglycans from human corneas with chronic edema, bullous keratopathy, and keratoconus and from normal corneas were analyzed using sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE), quantitative immunoblotting, and immunohistology with peptide antibodies to decorin and biglycan. RESULTS: Proteoglycans from pathologic corneas exhibit increased size heterogeneity and binding of the cationic dye alcian blue compared with those in normal corneas. Decorin and biglycan extracted from normal and diseased corneas exhibited similar molecular size distribution patterns. In approximately half of the pathologic corneas, the level of biglycan was elevated an average of seven times above normal, and decorin was elevated approximately three times above normal. The increases were associated with highly charged molecular forms of decorin and biglycan, indicating modification of the proteins with dermatan sulfate chains of increased sulfation. Immunostaining of corneal sections showed an abnormal stromal localization of biglycan in pathologic corneas. CONCLUSIONS: The increased dermatan sulfate associated with chronic corneal pathologic conditions results from stromal accumulation of decorin and particularly of biglycan in the affected corneas. These proteins bear dermatan sulfate chains with increased sulfation compared with normal stromal proteoglycans.

[A search for the sources of pathological alpha activity in the human EEG in limbic structure lesions].

Dependence of manifestations of EEG pathological activity on the electrode montage was studied in cases of brain lesions involving limbic structures by means of the topographic mapping and 3-d dipole source localization. The original program DIPVIS on the basis of BRAINLOC software allowed us to reveal the stable sources of lateralized pathologic alpha activity and demonstrate the discrepancy between its localization and the localization of the normal human alpha rhythm. The sources of pathological activity in question were predominantly localized in the mediobasal structures of the affected hemisphere in contrast to the occipital region of a healthy human. It was established that the topography of the cortical pathological activity may be determined by peculiarities of localization and, especially, orientation of its dipole generators. Afferent stimulation produced an enhancement of the pathological alpha-activity and increase in dipole source concentration in the mediobasal portion of the temporal lobe. The findings suggest that the discovered from of the alpha-rhythm reflects the reaction of activation of the hippocampal structures involved in the pathological process.
