Application of internal fixation in the treatment for tuberculosis of the spine.

This is a study of 18 patients who had surgical treatment for spinal tuberculosis. All patients were admitted with the spinal tuberculosis and treated with a combination of anterior debridement and bone graft with posterior internal fixation using the Luque or Dick technique. The patients were able to walk within a few days postoperation. Three to seven years follow-ups show no recurrence, kyphosis and/or solid fusion.

Metatarsal neck osteotomy with rigid internal fixation for the treatment of lesser toe metatarsophalangeal joint pathology.

Metatarsalgia associated with metatarsophalangeal (MTP) joint instability and/or plantar callosity formation is a difficult problem to treat. During a 15-month period, we performed 50 osteotomies of the metatarsal neck with rigid internal fixation in 47 feet of 42 patients. Three patients were excluded from the study, leaving 47 osteotomies in 44 feet of 39 patients for review. There were 6 men and 33 women, with a mean age of 57 years. In addition to lesser MTP joint pain with or without instability, the majority of patients had first ray pathologic condition, which was also addressed at the time of surgery. All but one of the osteotomies were united radiologically at 6 weeks. The mean shortening was 4.1 mm (range, 2-12 mm), and the mean follow-up was 9 months. There were no cases of malunion, nonunion, or avascular necrosis. At follow-up, 33 patients were asymptomatic. Eight patients (nine feet) had a degree of persisting pain at follow-up (seven mild and two moderate), but the source of this pain was only the metatarsal or MTP joint that was operated on in three cases. In this article, we describe the indications, the technique, and the results of the osteotomy.

The use of the Hartshill system for the internal fixation of spinal fractures and tumors.

We present the use of our Hartshill system for the posterior fixation of fractures and tumors. We present our first 69 fractures with a minimum two year follow-up (11 cervical, 8 thoracic, 42 thoracolumbar, 8 lumbar). The Hartshill system is a simple and secure method for the fixation of the spine in such cases. Both in thoracolumbar fractures and in tumours the common problem is collapse of the anterior column so that the spine is in kyphosis. Biomechanically the solution for the reduction and internal fixation in such cases is to use a fixation system which reduces the kyphosis by creating a lordosis. This is simply achieved with the Hartshill system.

[Changes in airway space following mandibular setback using sagittal split ramus osteotomy with rigid internal fixation].

The changes in airway space following mandibular setback using sagittal split ramus osteotomy and rigid internal fixation were studied in 28 Japanese patients with mandibular prognathism. Lateral cephalometric radiographs were taken immediately before and shortly after surgery as well as 5 to 12 months postoperation. The mean setback of mandible was 8.3 mm in the right side, and 8.2 mm in the left side. The mean amount of relapse of pogonion point during the following-up period was 0.52 mm horizontally, and 0.92 mm vertically. A significant decrease in airway space width and area, espacially in the lower part of airway space was found following mandibular setback shortly after surgery. Although there was some increase both in airway space width and area during the following-up period, they did not increase to their original values. This suggests that the narrowing of airway space following mandibular setback using sagittal split osteotomy can be permanent. No significant correlation was found between the amount of mandibular setback and airway space changes, and no significant correlation was found between the amount of airway space changes and relapse of pogonion point during the following-up period.

Ultrastructure of the fibrous tissue surrounding internal fixation devices.

The possible bursal nature of the fibrous tissue that develops like a capsule over internal fixation devices was studied ultrastructurally in 13 human and four canine cases. In both species, the basic structural pattern was similar. The capsule had two distinct zones. The superficial compact zone appeared as a pseudomembrane, and consisted of elongated fibrocytes arranged transversely in a collagenous and microfilamentous matrix. In the deep zone, the cytoplasm and its constituents of connective tissue cells varied from meager to abundant. The structural organization of the capsule as well as its cellular characteristics did not conform to the specificities of an adventitious bursa. The capsular tissue did not appear to be affected by such host factors as age; implant alloy composition; the type of the underlying bone or its abnormalities; duration of implantation, or superficial or deep location of the metal device. Inflammation rarely occurred in the capsular soft tissue, including those cases where pain developed over the implants following an uneventful bone union. Because metal implants do not induce formation of adventitious bursa, pain over an uncomplicated implant is not likely to be due to a bursitis.

[Delayed diagnosis of isolated ulnopalmar dislocation of the fifth carpometacarpal joint and its treatment with open reduction and internal fixation].

We present a 21-year-old male patient with an isolated ulnopalmar dislocation of the fifth carpometacarpal joint that occurred due to a fall on the hand. Diagnosis was delayed for five weeks and closed reduction was not successful. He was treated with open reduction and internal fixation. Upon removal of fixation materials six weeks after surgery, he developed reflex symphatic dystrophy and limitation in joint movements, which disappeared following rehabilitation and medical treatment at the end of 15 weeks. Functional and radiographic results were satisfactory.

Erectile dysfunction after perineal compression in young men undergoing internal fixation of femur fractures.

Erectile dysfunction (ED) following treatment for lower limb fractures is not uncommon. The mechanism is thought to be a perineal neurovascular traction injury acquired during surgery. Four young men who sustained femur fracture underwent reduction and intramedullary nail fixation. Post-operatively each presented with varying degrees of ED. Phosphodiesterase inhibitors were used in each case and sexual function returned in all. It is essential that a sexual history be taken in all patients after internal fixation of femur fractures so that treatment can be initiated early on if required. This serious complication from traction may be avoidable with careful patient positioning during traction.

The report of the cartilage regeneration using total joint regeneration system (internal-fixator type).

A new therapeutic system called the "total joint regeneration system" is proposed as a treatment for osteoarthritis and rheumatoid arthritis. We have developed devices which allow the regeneration of damaged cartilage under noninvasive mechanical stimulation. This paper introduces the basic concepts of total joint regeneration and reports the results of the first animal experiment using an internal-fixator device using rabbits and a dog.

Clinical and functional outcomes of internal fixation of displaced pilon fractures.

The clinical and functional outcomes for patients treated with open reduction and plate fixation of displaced tibial pilon fractures were determined. A retrospective search of the authors' trauma database was conducted for AO and Orthopaedic Trauma Association Code 43 injuries (pilon fractures) in adults 18 years or older who were treated between December 1988 and December 1992. The group of 64 patients who required open reduction and internal fixation to treat their fractures make up the primary cohort for this analysis. Twenty of these cases required no fibular fixation; the remainder were mostly fixed with 1/3 tubular or 3.5-mm compression plates. Tibial fixation was done using most commonly 3.5-mm cloverleaf plates, 1/3 tubular plates, or both. Of the 64 patients treated with open reduction and internal replacement, four (5%) patients had deep infection develop. Two (7%) of 14 patients had open fractures, and two (4%) of 50 patients had closed fractures. Three of these four patients smoked tobacco products; one was also an intravenous drug abuser. Staphylococcus aureus was the organism in two cases; Enterobacter, in the other two. The infection was controlled with a free flap in two cases, with antibiotics and wound debridement in one and with an arthrodesis in one. Thirty of the 64 patients completed the Short Form-36; two of these patients had bilateral fractures. The study group had significant differences in general health perceptions, physical function, physical role function, emotional role function, social and mental function, and pain and energy levels when compared with age matched population data and patients with tibial plateau fractures. The effect of other injuries on these functional status results cannot be determined specifically.

Burst fracture of the fifth lumbar vertebra: results of posterior internal fixation and transpedicular bone grafting.

Burst fractures of the fifth lumbar vertebra are rare, and there are only a few reports on this subject, which is characterised by its unique anatomical and biomechanical features. This retrospective analysis reports on ten patients whose fractures of L5 were stabilised with a short internal fixator in combination with a posterior fusion as well as transpedicular bone grafting. The average follow-up period was 22 months. Radiometric data were surveyed and compared to the functional results. The height of the fractured vertebra remained nearly unchanged throughout the course. Loss of lordosis of 4 degrees in the upper disc space and 4 degrees in the lower disc space were observed postoperatively. At the time of follow-up, the values of segmental lumbar lordosis were significantly below the preoperative level. The narrowing of the neural canal was reduced from 57% to 28% with the surgical intervention. However, there was no correlation between the functional and the radiological outcome. Neurological deficits were documented in two patients, which declined during the course. There were no severe postoperative complications. The results of the present study demonstrate that the described surgical procedure in fractures of the fifth lumbar vertebra does not promote an anatomic restoration of the fractured vertebra, nor of the segmental lordosis. However, the clinical results do not correlate with the radiological outcome. Nonoperative treatment with early mobilisation without external support seems to be the treatment of choice.

Management of a nonunion of the distal femur in osteoporotic bone with the internal fixation system LISS (less invasive stabilization system).

We present a case illustrating the successful use of the internal fixation LISS in an osteoporotic nonunion of the distal femur, where classic osteosynthesis has failed. The LISS plate with its angular stability offered the possibility to achieve excellent purchase in the severely porotic and partially destroyed bone. In combination with the use of an autologeous bone graft laterally and a strut cortical autograft medially, a mechanical support and an osteoinductive stimulus was provided and the extremity could be saved by this procedure.

Bone structure after removal of internal fixation plates.

We used single-photon absorptiometry to assess the forearm bones after the removal of internal fixation plates in 14 patients. We found convincing evidence of cortical atrophy in only one patient, in whom the plates had been removed prematurely after only 16 months. It is suggested that such plates should be retained for at least 21 months, to allow bone density to return to its prefracture level. The recommendations of the AO/ASIF group are supported.

Comparison of implant materials used in maxillofacial rigid internal fixation.

The concept of using metal plates for the alteration or repair of the craniomaxillofacial (CMF) skeleton is based on the ability to mimic the support and protective functions of bone, either temporarily or permanently, with minimal effect on bone healing, remodeling, and growth while avoiding toxicity to surrounding and distant tissues. Plating systems currently available for CMF rigid internal fixation are made from stainless steel, cobalt-chromium alloys, and titanium alloys. Each material exhibits the basic biomechanical and biocompatibility requirements to be acceptable clinically, but there are subtle and significant differences that may influence the choice of one material over another for a given clinical situation. This article reviews concepts of strength, corrosion resistance, biocompatibility, adverse reactions, and imaging characteristics for all three plating materials in addition to how these factors influence clinical choices.

Biodegradable internal fixation.

The objective of this article was to assess the value of the use of biodegradable materials in internal fracture fixation according to the current literature. Research methods included a computerized Medline search and a hand check of references of identified articles. Also, a complete reference list was obtained from the manufacturer of Biofix (Bioscience Ltd., Tampere, Finland). The reviewers abstracted descriptive information about population, materials, complications, follow-up times in clinical trials and strength of fixation, complications, and population for animal experiments. Results indicated that sterile sinus formation is mostly associated with polyglycolic acid, with rates up to 25%, and to a lesser extent, polylactic acid. Volume of implanted material and vascularity of bone appeared to affect the rate of sinus formation. Absorbable fixation was equivalent to or better than steel fixation for functional outcome refracture rate, and in transepiphyseal femoral and humoral fractures. Polyglycolic acid and polylactic acid both became toxic between 10 days and 4 weeks of hydrolysis. Polyglycolic acid had the highest initial strength at 370 MPa compared with other polymers.

Stability of acetabular fractures after internal fixation. A cadaveric study.

The rigidity of a transverse acetabular fracture in a laboratory cadaveric model immobilized by internal fixation was studied. On the application of a longitudinal load imposed on the fifth lumbar vertebra, four modes of displacement of the acetabular fracture were monitored by the use of variable impedance transducers. The rigidity of various combinations of lag screws and plates was assessed. The fracture deformations documented were generally quite small at any observation point with any of the fixation methods, and in most cases they were recoverable on load release. Anterior column lag screw fixation combined with plate fixation of the posterior column provided a degree of stability indistinguishable from that of other methods, and allowed the minimal exposure and devascularization of the pelvis. The 3.5 mm reconstruction plate, which is readily contoured to the intricate peri-acetabular bony structure, showed no significant difference in rigidity compared to the other apparently more rigid plates under study.

[Biomechanical considerations concerning the use of internal fixator systems in spinal interventions].

Three different internal fixateurs ("Dick", "Kluger", "SOCON") were investigated in an in-vitro setting. The systems showed nearly identical deformation under physiological loads. A new definition for the stabilizing capacity indicated a distinct weakness under compressive and torsional loading. In order to avoid early failure of the implant and/or the vertebra-implant interface bracing and a restricted mobilisation is recommended until healing of the fracture/fusion has taken place.

[Ventral compression spondylodesis using internal fixator instrumentation--a biomechanical study].

Pseudarthrosis is said to be a main complication in anterior interbody fusion. It is usually the result of an insufficient mechanical stability which hinders bony incorporation of intervertebral bone grafts. In order to improve stability dorsal transpedicular instrumentation should be carried out to achieve interfragmentary compression between bone block and adjacent vertebral endplate. Using human lumbar spine cadavers and a capacitive measuring mat an experimental study was completed to find out in which way an internal fixator has to be used to achieve this goal. The collected data showed that simple compression of the fixator by reducing the distance between the Schanz-screws did not produce sufficient interfragmentary compression. Preloading the fixator in kyphosis and compression in a second step improved the results significantly. Using this technique a compressive interbody fusion can be attained.
