Time to pregnancy--a model and its application.

Biological fertility is poorly measured by the number of children born in industrially advanced societies. The time elapsing from when a couple decides to have a child to clinically recognizable pregnancy is a useful alternative. Time to pregnancy can be collected in broad categories in large samples. A model for condensing important information from such data is presented, which fits several large samples of reported waiting times. It is shown that multiparous women conceive more quickly than primiparous women.

Language development in children with recurrent acute otitis media during the first three years of life. Follow-up study from birth to seven years of age.

From a cohort of 113 children, followed prospectively from birth during the first three years of life regarding different aspects of acute otitis media (AOM), two study groups were selected for the present investigation: 13 children with recurrent AOM (rAOM, i.e. at least six episodes of AOM during a 12-month period), and 29 children without any AOM episode. The purpose of this study was to analyse the possible effects of early onset rAOM on language development as assessed at four and seven years of age at phoniatric and linguistic examinations performed blindly. There were no differences between the two groups on any of the linguistic analyses performed, although the rAOM group manifested a somewhat better performance on auditory discrimination tests at four years of age. The results of the present study show that rAOM during the first three years of life, in otherwise healthy children, does not cause a detectable delay of language development at four and seven years of age.

Long-term effects of childbearing on mortality: evidence from pre-industrial Sweden.

This paper presents an analysis of the impact of childbearing history on later-life mortality for ever-married men and women using historical micro-level data of high quality for southern Sweden. The analysis uses a Cox proportional hazards model, estimating the effects on old-age mortality of number of births and timing of first and last births. By studying the effects of previous childbearing on mortality by sex and social status, we also gain important insights into the mechanisms relating childbearing to mortality in old age. The results show that number of children ever born had a statistically significant negative impact on longevity after age 50 for females but not for males. Analysis by social group shows that only landless women experienced higher mortality from having more children, which seems to indicate that the main explanations are to be found in social or economic conditions specific to females, rather than in the strictly biological or physiological effects of childbearing.

Stability of Bayley Motor Scale scores in the first year of life.

The purpose of this study was to determine whether Bayley Motor Scale scores were stable in infants during the first year of life, that is, whether each individual infant's score was essentially the same at each of the five test ages. Subjects were 15 low-risk and 8 high-risk infants with Bayley Scales of Infant Development Motor Scale scores at 2, 3, 4, 8, and 12 months. All high-risk and most low-risk infants also had outcome scores at either 24 or 36 months. For the group as a whole, and for all infants with "normal" outcomes, subjects did not change ranks over the six test ages (ie, high-scoring infants' ranks remained high and low-scoring infants' ranks remained low), but scores varied significantly from test to test. Scores of infants with "non-normal" outcomes did not vary significantly from test to test. The highest and lowest scores of 75% of the infants with normal outcomes and 57% of the infants with non-normal outcomes differed significantly. Four patterns of variations of Bayley Motor Scale scores over the five test ages of the first year after birth were identified. The first pattern included three infants with the most stable scores. The three remaining patterns were different from each other, but a consistent feature of each was a peak at 4 months. Finally, scores at 3 and 8 months were best able to correctly identify outcome. A single Bayley Motor Scale score may not reflect an infant's true abilities; some factor may be inflating scores at age 4 months.

Urinary sodium excretion in young infants: role of gestational and postnatal ages.

Fractional urinary sodium excretion (FENa) and urinary sodium excretion (UNaE) were determined 88 times in 42 healthy, appropriate weight-for-gestational age infants. Gestational ages (GA) were 28-41 weeks; birthweights were 930-4135 g. Nineteen preterm infants were studied serially a total of 59 times between 1 and 55 days after birth. During the first 4 days after birth, multiple hierarchical regression analyses showed that FENa and UNaE were inversely related to GA (P less than 0.001). Postnatal age (PN) was not significantly related to FENa or UNaE. However the effect of GA on FENa and UNaE was not significantly greater than the effect due to postnatal age (PA) (P = 0.31 and P = 0.80, respectively). UNaE (1.6 +/- 0.2 mmol/kg per day) was significantly greater than sodium intake (1.1 +/- 0.2 mmol/kg per day) at 2.6 +/- 0.2 days (P less than 0.05). Longitudinal studies extending beyond 4 days indicated that GA and PA had interactive effects on FENa and UNaE; hence the contribution for a given GA was dependent on PA (and vice versa). Sodium intake remained stable (average 1.8 mmol/kg per day) beyond 7 days after birth and was consistently greater than UNAE (P less than 0.01). It is suggested that in healthy preterm infants beyond 2 weeks PA, a sodium intake of 2-3 mol/kg per day may be adequate to meet the renal sodium losses.

Early postnatal switch in magnesium sensitivity of NMDA receptors in rat CA1 pyramidal cells.

1. Whole-cell patch-clamp recordings of iontophoretically induced N-methyl-D-aspartate (NMDA) receptor-mediated currents (INMDA) in CA1 pyramidal cells in hippocampal slices from 1- to 40-day-old rats were used to characterize developmental changes in the Mg2+ sensitivity of NMDA receptors. 2. The dose-response relations for extracellular Mg2+ blockade of INMDA indicated a high affinity binding of Mg2+ to NMDA receptors at membrane potentials more negative than -60 mV, independent of postnatal age. 3. Depolarizing the cells unblocked NMDA receptors by decreasing their affinity for Mg2+. The efficacy of depolarization in unblocking NMDA receptors markedly increased after postnatal day 4 (P4), endowing the receptors with a greater voltage dependence. 4. The NR2B subunit-specific NMDA antagonist ifenprodil reduced INMDA in pyramidal cells of all ages. The sensitivity of INMDA to ifenprodil was greatest during the first postnatal week and decreased thereafter, indicating an enhanced contribution of NR2B subunit-containing NMDA receptors to INMDA in the first week after birth. 5. In the first postnatal week, the ifenprodil-insensitive INMDA component had a lower voltage dependence than the total INMDA. In older pyramidal cells, the voltage dependence of the ifenprodil-insensitive component and the total INMDA were similar. 6. In another set of CA1 pyramidal cells, single-cell reverse transcription and polymerase chain reaction (RT-PCR) were used to characterize concomitant developmental changes in NMDA subunit mRNA expression. The mRNA for the NR2D subunit was detected during the first postnatal week in 50 % of the cells and disappeared thereafter. The proportion of cells expressing the NR2A and NR2B subunits remained relatively constant throughout the first five postnatal weeks. 7. We conclude that NMDA receptors in hippocampal CA1 pyramidal cells are effectively blocked by Mg2+ at all ages. After 4 days they become much less sensitive to Mg2+ at depolarized membrane potentials. This postnatal switch in voltage control of Mg2+ binding to NMDA receptors may be due to the downregulation of NR2D subunit expression in developing CA1 pyramidal cells.

Obstetric outcomes at the birth place in Menlo Park: the first seven years.

During its first seven years of operation, the Birth Place, a free-standing birth center in California, registered 898 women, of whom 690 (77%) were admitted in labor and 150 (17%) were referred for hospital birth prior to onset of labor. Using carefully delineated screening criteria, the center had an overall 18% intrapartum transport rate to the hospital, primarily for prolonged or arrested labor, a 3% cesarean section rate, no maternal mortality, and one neonatal death resulting from Cornelia de Lange syndrome, a congenital mental retardation-malformation syndrome of unknown etiology, which in this case was incompatible with life. Deliveries at the Birth Place were associated with low cost, a high level of maternal satisfaction, a low cesarean section rate, low neonatal mortality, and no maternal mortality.

Some marital and child-rearing characteristics of early mother-bereaved women.

A sample of 160 female, psychiatric patients from North-east Scotland whose mothers died before they were aged 11 was compared with 80 non-bereaved, female, psychiatric patients, matched by decade of birth, from the same area. Six subgroups of the bereaved patients classified according to the events which followed the bereavement were also examined. In addition, samples of 44 and 27 women from the Chichester general population, whose mothers died before age 11 and from 11 to 19 respectively, were paired by age, parental social class and age at death of father, with Chichester women who were not mother-bereaved. Comparisons were made in respect of age at marriage, relative age of subject and marital partner quality and fate of marriage, delay between marriage and birth of first child, total number of children, and ability to cope with the birth and subsequent child-rearing. The early bereaved women showed a tendency to marry younger men but otherwise their marriages were no different from those of the controls. The early bereaved had more difficulties with child-rearing, particularly when their children reached adolescence. Surprisingly, the findings for the six subgroups were not appreciably different from those of the total bereaved patients.

The postnatal development of lymphoreticular aggregates and lymph nodes in infants' lungs.

A quantitative study has been made of standardized sections taken from the right middle lobe of 316 children's lungs. Lungs showing no pathological state showed that lymph nodes are present at birth and these increase in prominence if not in numbers throughout the first year after birth. Lymphoreticular aggregates are not present among the alveoli of the normal infant lung at birth. These first appear around a week after birth and progressively increase in number throughout infancy and early childhood, being almost universally present by the age of 5 years. The development of peripheral lymphoreticular aggregates in the lungs of infants appears to be entirely environmentally and antigenically stimulated.

Ontogeny of apoptosis during lung development.

Apoptosis has been shown to be involved in several processes during embryogenesis, but the ontogeny of apoptosis during lung development ahs not been studied. The goals of the current study were to determine if apoptosis occurs during lung development, and to determine the ontogeny of the changes in apoptosis that occur. We studied the ontogeny of apoptosis in vivo using lungs from 14-18-d gestation fetal rats, newborn rats, and 1-d-, 2-d-, 5-d-, and 10-d-old rat pups. Apoptosis was assessed by electron microscopy and the terminal deoxyribonucleotidyl transferase dUTP nick end-labeling assay. We compared the in vivo results with explants of 14-d gestation fetal rat lung placed in culture for 1-4 d because the biochemical development of the lung in organ culture has been shown to closely parallel the development of the lung in vivo. We found apoptosis of mesenchymal cells at the periphery of distal lung buds in early fetal lung (14-16-d gestation). Apoptosis of both mesenchyme and epithelium was present in later fetal lung (18-d gestation). There were no qualitative differences in apoptosis between in vivo fetal lung and explant cultures of fetal lung. There was a 14-fold increase in apoptosis at birth and in the first postnatal day of life (9-12% of cells) compared with fetal lung (0.6-1% of cells). This was followed by a rapid decline in the percentage of apoptotic cells to fetal levels at postnatal d 2-10. We conclude that apoptosis occurs in a spatially, temporally, and cell-specific manner during lung development. The number of cells undergoing apoptosis increases dramatically in the first day after birth.

Effect of stress administered during pregnancy on the development of fetal testes and their subsequent function in the adult rat.

When maternal stress, containing a large anxiety component, was administered during pregnancy there was a significant decrease in 3 beta-hydroxysteroid dehydrogenase (3 beta-HSD) activity in the fetal testis from days 16 to 20 of gestation, but not at birth nor in the first week after birth. However, persistent effects were found in adult males of 90 days of age. Basal testosterone concentrations in both plasma and testes and testicular 3 beta-HSD activity were significantly lower whilst basal plasma progesterone concentrations were significantly higher in the stressed group. When the stressed offspring were subjected to short-term stress (one session), their plasma testosterone concentration was significantly below that of the controls. It is suggested that suppressed gonadotrophin secretion during critical periods of development alters fetal testicular function, and that raised circulating levels of stress-induced hormones such as beta-endorphin may be responsible for changes in gonadotrophin secretion.

Early stages of myelination in the spiral ganglion cells of the kitten during development.

Myelinated cell bodies of spiral ganglion in kittens during development can first be distinguished by light microscopic observation at the end of the first postnatal week. By mean of electron microscopic observation, the first signs of myelinated perikarya can be observed around the time of birth. Myelination is preceded by the ensheathment of the ganglion cell body by two or three layers of Schwann cell cytoplasmic processes. At birth, the first sign of myelination is visible at the basal part of the cochlea with thin thickenings which contain two or three major dense lines. At this stage, these thickenings cover only a small perikaryal area. Later on, the thickness and the number of thickenings increase and cover the major part of the cell body. The onset of myelination of the processes seems to precede the perikaryal myelination.

An inducer protein may control the timing of fate switching in a bipotential glial progenitor cell in rat optic nerve.

In rat optic nerve, oligodendrocytes and type-2 astrocytes develop from a common (O-2A) progenitor cell. The first oligodendrocytes differentiate at birth, while the first type-2 astrocytes differentiate in the second postnatal week. We previously showed that the timing of oligodendrocyte differentiation depends on an intrinsic clock in the O-2A progenitor cell. Here we provide evidence that the timing of type-2 astrocyte differentiation, by contrast, may depend on an inducing protein that appears late in the developing nerve. We show that extracts of 3- to 4-week-old, but not 1-week-old, rat optic nerve contain a protein (apparent Mr approximately 25,000) that induces O-2A progenitor cells in culture to express glial fibrillary acidic protein (GFAP), an astrocyte-specific marker in the rat central nervous system.

Induction of parturition in alpacas and subsequent survival of neonates.

OBJECTIVE: To evaluate use of fluprostenol, dexamethasone, and oxytocin for induction of parturition in alpacas, and to determine viability of the newborn crias. DESIGN: Prospective, randomized, controlled trial. ANIMALS: 36 pregnant alpacas within 10 days of parturition. PROCEDURE: Animals were randomly assigned to treatment groups. Plasma progesterone and plasma and urine estrone sulfate concentrations were measured for 5 days after treatment. Clinical signs of the neonates were determined. RESULTS: Time between treatment and parturition was significantly shorter for animals that received fluprostenol than for animals in any other group. The highest dose of dexamethasone (0.5 mg) caused fetal death. None of the other treatments induced early parturition. Time between birth and first suckling, body weight, rectal temperature, pulse rate, and respiratory rate at birth and serum IgG concentration 24 hours after birth were not different between crias born after fluprostenol treatment and crias born to control alpacas. CLINICAL IMPLICATIONS: Fluprostenol was effective at inducing parturition in these alpacas, but dexamethasone and oxytocin were not. Crias born after fluprostenol treatment were similar to crias born to control alpacas.

Prenatal and postnatal development of the small intestine in the insectivore Suncus murinus.

The development of the small intestine in the insectivore Suncus murinus was noted during the period from 21 days' gestation to 20 days after birth. At 21 days of gestation, the proximal small intestine exhibited the beginning of villus formation, whereas the distal small intestine preserved the stratified epithelium. Stratified epithelium in the distal small intestine changed into a single layer by 24 days' gestation. At 26 days' gestation, each epithelial cell was immature; but by 28 days mature-looking epithelial cells were found. The shape of the villi changed from cuboid to columnar during the same period. The connective-tissue cores of the villi began to develop at 7 days after birth in the proximal small intestine and at 15 days after birth in the distal small intestine. Crypts appeared at 15 days after birth. Endocytosis of epithelial cells took place at 28 days of gestation. In the proximal small intestine, supranuclear vesicle clusters were observed first at birth; they began to decrease both in number and size at 10 days' gestation and then disappeared completely by 20 days after birth. In the distal small intestine, large supranuclear vacuoles were observed first at 28 days of gestation. Although these vacuoles invariably were found up to 15 days after birth, they also disappeared completely by 20 days. Epithelial cells showed a structure similar to those of the adult after weaning.

Prevalence of menstrual pain in relation to the reproductive life history of women from the Mayan rural community.

BACKGROUND: The prevalence of menstrual pain in Western societies is relatively high and has been shown to be related to many factors (e.g. age, menarche age, parity, menstrual cycle regularity, cigarette smoking, dietary habits). However, less data are available for traditional societies, within which there are, presumably, not as many potentially disturbing factors that could influence menstrual symptoms as in Western societies. AIM: The study seeks to determine the prevalence of dysmenorrhoea, and the factors related to menstrual pains in women with natural fertility (i.e. without hormonal forms of contraception). SUBJECTS AND METHODS: 177 non-smoking women between 18 and 45 years of age living in one Mayan village (Yaxcaba, Yucatan, Mexico) were the subjects of this study. The method of logistic regression was used with the dichotomous dependent variable being the presence or absence of menstrual pain together with such independent variables as age, age at menarche, number of pregnancies, age when giving birth to the first child, the weight at interview and BMI. RESULTS: The general prevalence of the menstrual pains was 28%. There was only one variable influencing dysmenorrhoea, namely the age at which women give birth to their first children. Those mothers who gave birth to their first child at an earlier age (at mean age of 19.4 vs 21.1 years) had a lower prevalence of dysmenorrhoea. CONCLUSION: In the studied traditional society the main factor related to menstrual pain was the age at which woman first gave birth. It is hypothesized that an earlier start to reproductive life in some way decreases the sensitivity of the uterus to prostaglandins.

Development of positive feelings in primiparous mothers toward their normal newborns. A descriptive study.

Though mothers' development of positive feelings about their newborns is the first step in maternal attachment, little research has described factors which may influence the process. This descriptive study reports normative data for primiparous mothers with normal newborns. The authors interviewed 100 primiparous mothers on the postpartum ward within 72 hours after delivery at a community hospital to ascertain factors associated with the development of positive feelings. Over one third (39%) of mothers studied reported their first positive feeling during the prenatal period, 42 percent during birthing or the first day and 19 percent on the second or third day. A majority of mothers (70%) described their strong positive feelings toward their newborns (i.e., "love") as a sense of caring, commitment and concern. Several demographic, biological and environmental factors were associated with the development of first positive feelings in primiparous mothers. One half of mothers who planned to become pregnant developed positive feelings prenatally, while 57 percent of mothers who did not plan to become pregnant reported their first positive feelings during birth or the first day. Delay in first positive feelings until after the first day was associated with labor longer than 8 hours, disappointment with the "bonding" experience, breast-feeding, and high depressive symptoms.
