[Circadian periodicity of blood coagulation system indices and the hormonal activity of the adrenals in healthy persons (under conditions of rest and of stressed work)].

13 patients with the heart ischemic disease (HID) were examined in conditions of a relative rest (hospital) and 20 patients - during an intensive day work. Within the 24-hr rhythm of gemocoagulation parameters, 3 phases were distinguished (conventionally): I a tendency to hypercoagulation at 0-3-6 a.m.; II the most safe period at 9 a.m.; III an increased ability of coagulation at 12-3-6-9 p.m. During the intensive day work, hypercoagulative shifts become more obvious and the anticoagulating system becomes activated at the I and the III phases. Within the 24: hr rhythm of the plasma fibrinolytic activity, 2 phases were distinguished (conventionally): I low activity at 0-3-6-9-12 a.m.; II sufficient activity at 3-6-9 p.m. During the intensive day work fibrinolytic activity increases, and the character of 24-hr rhythm preserves. Within the 24-hr rhythm of the adrenal hormonal activity, 3 phases were distinguished (conventionally): I a reduced activity at 0-3-6 a.m.; II high activity at 9 a.m.; III high enough activity at 12-3-6-9 p.m. In conditions of intensive day work the 24-hr rhythm of the adrenal hormonal activity does not change.

[Chemical properties of plasma fibrinogens and fibrins from normal and cobalt-treated rabbits].

The sulfitolysis products of fibrinogens from normal and cobalt-treated rabbits (5 mg Co2+/kg b.w.) were resolved by ion exchange chromatography on CM-cellulose columns. The elution patterns of both fibrinogens showed a distinct heterogeneity of gamma-chains. Furthermore, a gamma-chain derivative from cobalt fibrinogen could be distinguished electrophoretically from the corresponding one of normal fibrinogen because of its reduced electrophoretic mobility. Normal and cobalt fibrinogen did not differ from each other in their N-terminal (Val2 Ala2 Tyr2) and C-terminal (Pro1-2 Val4-5) amino acid compositions related to a subunit structure of A alpha 2 B beta 2 gamma 2, and their carbohydrate contents - neutral hexoses 1,21% (1,26%), N-acetyl hexosamines 1,16% (1,05%), N-acetyl neuraminic acid 1,19% (1, 13%), values for cobalt fibrinogen in parentheses. The main amounts of carbohydrates are bound to gamma- and B beta-chains, The BrCn cleavage products from cobalt fibrinogen and its gamma- and B beta-chains showed other electrophoretic properties than the corresponding derivatives from normal fibrinogen. But BrCN split products of A alpha-chains of both fibrinogens were electrophoretically very similar. Spectrographic investigations of the S-sulfoderivates demonstrated a diminution of the absorption maximum near 282mn of gamma and B beta derivatives of cobalt fibrinogen. A alpha-chains of both fibrinogens were not different from each other. Using autoradiography the highest 58Co binding could be found in the gamma-chain with a reduced electrophoretic migration velocity, whereas B beta-and gamma-chains with unchanged electrophoretic mobility bound only small amounts. A alpha-chains of cobalt fibrinogens were apparently not loaded with 58Co. gamma-chain and alpha-chain cross-links could be observed in normal fibrins stabilized by factor XII, however, in cobalt fibrins a gamma-dimer formation was demonstrable without participation of gamma-chains with reduced electrophoretic mobility. A distinct alpha-chain cross-link could not be demonstrated either. From these and other investigations on molecular weights of both fibrinogens it was assumed that earlier observed changes of physicochemical properties and biological behaviour of cobalt fibrinogen might result from a complex binding of cobalt ions on specific structures of the fibrinogen molecule.

In vitro studies of the influence polyester materials with a different degree of surface wettability have on blood haematological parameters and coagulation and fibrinolysis system parameters.

The paper presents the studies of the influence polyester knitings with a different degree of surface wettability have on the coagulation system, fibrinolytic system and blood haematological parameters in in vitro studies. The studies of Human whole blood were performed for 5, 15, 30, 60, 90, 180 and 240 min. In view of the obtained results in has been reported that a hydrophobic surface knitting does not cause important quantitative changes in any of the selected parametes of the coagulation system and fibrinolysis system. A hydrophilic surface polyester knit causes important lengthening of aPTT value and reduced factor XII, IX and VIII activity. A hydrophilic surface polyester knitting causes a bigger decrease in leukocytes and blood platelets and a bigger increase in beta thromboglobulin in relation to the hydrophobic surface knitting. Neither hydrophobic surface knit nor hydrophilic surface knitting cause changes in the erythrocyte system.

Intraoperative studies and studies on selected parameters of coagulation and fibrinolysis following implantation of DALLON H prostheses with greater surface wettability.

TRICOMED S.A. (Poland) has modified its own knitted polyester vascular prosthesis by change in its physical and chemical characteristics and using hydrophilia. The work has aimed at evaluation of intraoperative tightness and evaluation of change in the number of selected parameters of coagulation and fibrinolysis following implantation of a hydrophilic vascular graft into the thoracic aorta defect in piglets. In view of the experimental tests performed it has been noticed that use of DALLON H prosthesis is easy and the handling of the graft is the same as that of other knitted vascular grafts. DALLON H prosthesis does not need preclotting and it reaches immediate tightness after the blood flow has been restored. In view of the results of the blood studies after implantation coagulation activation in endogenous and exogenous system (aPTT and PT), and increase in activity of factor XII and VII. Concentration of fibrinogen was at a higher level. Thrombin III activation remained at the same level after a primary increase, but protein C activation was decreased. Plasminogen activation was higher which means that the fibrinolytic system was activated. Changes in the level of evaluated indicators were observed until day 14 after implantation of the prosthesis. On day 21st the values of the selected parameters were equal with the values prior to implantation.

[Effect of oxidized cellulose on fibrin formation and blood platelets].

BACKGROUND: Haemostyptic materials initiate and hasten blood clotting at the site of their application. The properties of haemostyptic materials are used for treatment of capillary and parenchymatous haemorrhage along with surgical treatment. Celluloses one of the biopolymers studied for a long time, suitable because of its biocompatibility and non-toxicity. METHODS AND RESULTS: In the submitted study the authors used microdispersed calcium-sodium salt of oxidized cellulose which is formed by oxidation of cellulose in position C6 (patent Alltracel Pharmaceuticals). The authors investigated the effect of oxidized cellulose on fibrin formation and platelets. Using the optic method of the surface plasmon resonance they investigated the initial stage of interaction between fibrinogen and oxidized cellulose. Oxidized cellulose retards and reduces the interaction of the immobilized fibrin monomer with fibrinogen. Fibrin formation was investigated spectrophotometrically at 350 nm. In the presence of cellulose the period of formation of fibrin gel was prolonged and its turbidity increased, depending on the concentration of the cellulose used. The platelet activation by cellulose was assessed by measuring the released serotonin. For the activation of platelets by cellulose the presence of plasma is necessary, rinsed platelets were not activated by cellulose. It was revealed that direct, interaction of rinsed platelets or fibrinogen with cellulose plays a secondary role. CONCLUSIONS: These data and the retarded activation of platelets in plasma with factor XII deficiency indicate that due to negatively charged oxidized cellulose probably activation of the contact coagulation system occurs and this leads to the activation of platelets and fibrin formation.

[Revision of the biological significance of the contact system].

Current concept of blood coagulation is divided into two stages: an "initiation" stage which is handled by tissue factor pathway, and an "augmentation" stage handled by intrinsic pathway beginning in factor XI. Recent studies have demonstrated that the contact system is a modulator for vascular biology with vascular tone regulation, anticoagulant, profibrinolytic, antiadhesive and proinflammatory functions. Changes of contact system are associated with sepsis, thrombosis, etc.

Thrombohaemorrhagic balance in diabetes mellitus.

Diabetes changes the thrombohaemorrhagic balance which exists in healthy flowing blood. This change predisposes a diabetic patient to various thrombo-embolic conditions, leading to increased mortality and morbidity of these patients. Increase in certain coagulation factors like factors XII, XI, VIII, fibrinogen and VWF, decreased fibrinolysis, increased platelet aggregation, endothelial cell dysfunction coupled with increased blood viscosity and decreased red cell deformability are some of the changes that contribute to increased thrombo-embolic incidence in this disease.

[The effect of obzidan and isobarin on the lysosomal apparatus of the neutrophilic granulocytes and on hemostasis under stress factors].

Experiments on rabbits were carried out with the purpose of evaluating the participation of beta-adreno-blockader and sympatholytic agent in the reaction of liberation of lysosomal enzymes of peripheral blood neutrophilic granulocytes in response to the immobilization. It was established that the sympathetic nervous system (via alpha- and beta adrenoreceptors) produces an effect on reduction of the number of lysosomes and liberation of lysosomal enzymes that participate indirectly through factor XII in hemostasis regulation.

Effect of DDAVP on endotoxin-induced intravascular coagulation in rabbits.

We have evaluated the effect of 1-Deamino-8D-arginine vasopressin (DDAVP) on an experimental model of intravascular coagulation (DIC) induced in rabbits by injection of 20 micrograms kg-1 h-1 during 6 h of E. coli lipopolysaccharide. DDAVP significantly ameliorated the platelet drop and fibrinogen decrease (p less than 0.01) induced by endotoxin in control animals. A significant reduction in factor XII consumption (p less than 0.01) and a decrease in the generation of endotoxin induced PAI-1 activity in rabbits circulation was also observed (p less than 0.005). Moreover, fibrin deposition in kidneys of rabbits receiving DDAVP was significantly reduced as compared to control animals. Finally, the mortality rate in the control group was significantly higher than in DDAVP-treated rabbits (p less than 0.01). The hemostatic changes induced by DDAVP correlated with lower fibrin deposition and reduction in mortality rates.

[Kinetics of I-131 fibrinogen in cancer patients: pharmacological approach].

The results obtained in a previous study using 131I fibrinogen in cancerous patients suggested a local intravascular clotting precess. In order to elucidate the mechanism of fibrinogen kinetic abnormalities different drugs, including heparin, prednisone, ticlopidin, aspirin and indomethacin were administered in 68 patients and their effects evaluated by change in the 311I fibrinogen disappearance rate. The results suggest that these drugs may counteract with the early stages of coagulation (kinin-forming system, factor XII) and that abnormal 131I fibrinogen kinetic in cancer would be a non specific phenomenon.

Effectivity of heparin in assisted reproduction.

Disturbances of the embryo-maternal interaction, i.e. impaired implantation, are seen in only a minor fraction of couples. These malfunctions become evident as recurrent spontaneous abortions (RSA), or repetitive implantation failure (RIF) in cases with IVF or ICSI procedures. The antiphospholipid syndrome (APL) is the only consensus-defined syndrome associated with RSA (anticardiolipin antibodies and/or lupus anticoagulant plus clinical symptoms). Since antiphospholipid antibodies directly interfere with hemostasis (increased coagulation), heparin is an established treatment option in these cases resulting in unequivocal benefits. There is no defined antibody syndrome in RIF even if it may be assumed that it exists. Conclusive evidence for a benefit of heparin (and aspirin) in this situation is lacking as well. However, the majority of investigations including our own experience indicate that anticoagulation may be useful. Besides the extensively studied anticardiolipin antibodies, other - by far less thoroughly investigated - antiphospholid antibodies have been described. So far it is unclear if heparin may exert positive effects in women carrying these antibodies. Autoreactive immune processes may also become apparent by the emergence of further antibodies, such as antinuclear (ANA), thyreoglobulin (TGA) and thyreoperoxidase antibodies (TPO) etc. However, there is no established definition of a syndrome associated with these antibodies, TGA and TPO probably being the most relevant. - Most studies in this area including our own experience indicate that heparin may be a useful. The detection or autoantibodies per se is probably not of pathophysiological relevance if there is no ongoing pathological activation of the immune system. However, an acute autoimmune response associated with irregular antibodies may represent the pathophysiological basis of a reproductive autoimmune failure syndrome. In these cases, immune-equilibrating interventions appear to be more appropriate than heparin therapy. - Coagulation disorders, namely thrombophilia, are a frequent cause of RSA and probably RIF as well, the most relevant being antithrombin deficiency, Factor V Leiden and prothrombin mutations. Deficiencies of protein S, protein C and factor XII and XIII are of minor importance. There is a varying degree of evidence for a benefit of heparin/aspirin in these syndromes. Heparin not only reduces the abortion rate but also lowers the risk for developmental retardation, premature birth and preeclampsia. - The effects of heparin are not restricted to anticoagulation. It is directly or indirectly (e.g. via heparan sulfate proteoglycans or heparin-binding EGF) involved in the adhesion of the blastocyst to the endometrial epithelium and the subsequent invasion. Actually, prolonged heparin treatment (14 days) resulted in an increased pregnancy rate in our patient population. Shorter courses of heparin where not effective.

Increased fibrin specificity and reduced paradoxical thrombin activation of the combined thrombolytic regimen with reteplase and abciximab versus standard reteplase thrombolysis.

In patients with acute myocardial infarction treated with thrombolytics, platelet activation as well as alterations of the hemostatic and fibrinolytic systems have been described favoring early infarct-related artery reocclusion. We investigated the effects of a newer thrombolytic regimen with half-dose double-bolus reteplase (2 x 5 IU, 20 patients) combined with abciximab versus full-dose reteplase (2 x 10 IU, 18 patients) on the fibrinolytic and the hemostatic system in patients with acute ST-segment elevation (in the electrocardiogram) myocardial infarction. The thrombolytic regimen with half-dose reteplase in combination with abciximab caused in vivo a lower systemic plasminemia and a lower paradoxical activation of the contact phase of the coagulation system (measured as activated factor XII); a lower paradoxical thrombin activation/generation; and a lesser extent of fibrinogen breakdown compared with the reteplase regimen. These results could be, at least in part, a possible explanation for the observed significantly lower rates of reinfarction until 7 days after enrollment and of recurrent ischemia in the combination group in the Global Use of Strategies to Open Occluded Coronary Arteries V (GUSTO V) trial.

Five prothrombotic polymorphisms and the prevalence of premature myocardial infarction.

We studied 5 functional hemostatic polymorphisms in 281 patients with premature myocardial infarction and in 530 control subjects. The role of these polymorphisms when analyzed independently is small, if any. However, the simultaneous combination of factor XIII and prothrombin polymorphisms exacerbated the risk. (OR=12.12; p=0.028). Moreover, combinations of factor V Leiden with prothrombin, and factor XII with prothrombin polymorphisms were only identified in patients. Our results support the relevance of gene-gene interactions in myocardial infarction.

[Topic usage of bismuth subgallate as a hemostatic in tonsillectomy].

The most feared complication for the ENT specialists in the adenotonsillectomy is the immediate or mediate haemorrhage after surgery. We revise the literature and contrast experiencies with other colleage. We have found an astringent substance (bismuth subgallate), which helps us give an exact intraoperative diagnosis of the bleeding spot by the colour contrast it shows, and serves us also as a hemostatic, since it activates the XII factor in coagulation. We revise this product regarding its origen, prentation, applications and toxic effects.

Quantification of functional and inactivated alpha 2-macroglobulin in sepsis.

Alpha 2-macroglobulin (alpha 2 M) in vitro inhibits numerous proteinases that are generated during inflammatory reactions and therefore, probably plays an important role in diseases such as sepsis. To monitor the state of alpha 2 M in sepsis, we developed novel assays for functional and inactive alpha 2M. Functional alpha 2M in plasma was measured by quantitating the binding of alpha 2M to solid-phase trypsin. Inactive alpha 2M (i alpha 2M) was assessed with a monoclonal antibody, mcAb M1, that specifically reacts with a neodeterminant exposed on i alpha 2M. This mcAb in combination with chromogenic substrates was used to detect alpha 2M-proteinase complexes. Functional alpha 2M was reduced in plasma from 48 patients with clinical sepsis compared to healthy controls (p less than 0.0001). Levels of functional alpha 2M on admission and the lowest levels encountered in 23 patients with shock were lower than in 25 normotensive patients (p = 0.023 and p = 0.009, respectively). Increased levels of i alpha 2M (greater than 30 nM) at least on one occasion were found in only 4 of the 48 patients, being not different in hypotensive compared with normotensive patients, and not in patients who died compared with those who survived. Levels of functional alpha 2M correlated significantly with levels of factor XII and prekallikrein suggesting that decreases in alpha 2M at least in part were due to contact activation. Indeed, in two patients with increased i alpha 2M, complexes between alpha 2M and kallikrein were demonstrated in addition to plasmin- and thrombin-alpha 2M complexes.

On the target specificity of plasminogen activator inhibitor 1: the role of heparin, vitronectin, and the reactive site.

Plasminogen activator inhibitor 1 (PAI-1) is the fast-acting inhibitor of both tissue-type and urokinase-type plasminogen activators (t-PA, u-PA) and is an essential regulatory protein of the fibrinolytic system. In the presence of either the protein vitronectin or the glycosaminoglycan heparin, PAI-1 is also an efficient inhibitor of thrombin. To assess whether these cofactors turn PAI-1 into a general protease inhibitor or whether their influence is restricted to thrombin, the second-order association rate constants between PAI-1 and the human plasma proteases t-PA, u-PA, plasmin, thrombin, Factor Xa (FXa), and Factor XIIa (FXIIa) in the absence and in the presence of either vitronectin or heparin are determined. In addition, the role of the PAI-1 reactive site P3 to P3' residues for the specificity of inhibition was studied by using PAI-1 reactive site mutants. Our results show that: (1) Heparin exclusively increases the rate of inhibition of thrombin by PAI-1, whereas in the presence of heparin the rate of inhibition of the other proteases is not altered; (2) Vitronectin is an obligatory cofactor for the inhibition of thrombin by PAI-1. In addition, vitronectin moderately increases the rate of inhibition by PAI-1 of u-PA and of plasmin, but does not alter the rate of inhibition of t-PA, FXa, or FXIIa; (3) Apart from the important role of the P1 residue, no consensus can be presented on the nature of other residues within the P3 to P3' region with regard to target protease specificity.

Domain 3 of kininogens contains a cell-binding site and a site that modifies thrombin activation of platelets.

High and low molecular weight kininogens (HK and LK) are able to bind to platelets to inhibit thrombin binding to and activation of platelets. The heavy chain domain on the kininogens that contains these functions has been determined. Domain 3 (D3) but not domains 1 or 2, completely inhibited 125I-HK binding to platelets (Ki = 24 +/- 7 nM, n = 4). 125I-D3 specifically bound to unstimulated platelets and human umbilical vein endothelial cells. On platelets, it was blocked by unlabeled D3 and HK but not prekallikrein, factor XII, C1s, or C1 inhibitor. Further, one monoclonal antibody (HKH13) directed to kininogens' D3 blocked 125I-HK and 125I-D3 binding to platelets. The binding of 125I-D3 to platelets was fully reversible by addition of 35 molar excess of unlabeled D3. D3 binding to platelets was saturable with an apparent Kd of 39 +/- 8 nM (n = 4) and 1227 +/- 404 binding sites/platelet. D3, like HK and LK, inhibited thrombin-induced platelet activation by preventing thrombin binding to platelets. Another monoclonal antibody (HKH12), directed to D3, which did not block HK binding to platelets, reduced HK's ability to inhibit 125I-alpha-thrombin binding. This result suggests that the region on D3 that inhibits 125I-alpha-thrombin binding to platelets is different from that which directly binds to platelets. These studies indicate that D3 of the kininogens contains both a binding region for platelets and endothelial cells and another region that inhibits thrombin-induced platelet activation.

The contact-system dependent plasminogen activator from human plasma: identification and characterization.

Apart from tissue-type plasminogen activator (t-PA) and urokinase-type plasminogen activator (u-PA), a third PA appears to occur in human plasma. Its activity is initiated when appropriate triggers of the contact system are added, and the activation depends on the presence of factor XII and prekallikrein in plasma. The activity of this, so-called, contact-system dependent PA accounts for 30% of the PA activity in the dextran sulphate euglobulin fraction of plasma and was shown not to be an intrinsic property of one of the contact-system components, nor could it be inhibited by inhibitory antibodies against t-PA or u-PA. We have succeeded in identifying this third PA in dextran sulphate euglobulin fractions of human plasma. Its smallest unit (SDS-PAGE) is an inactive 110 kDa single-chain polypeptide which upon activation of the contact system is converted to a cleaved, disulphide-bridged molecule with PA activity. The native form, presumably, is an oligomer, since the apparent Mr on gel-chromatography is 600,000. The IEP is 4.8, much lower than that of t-PA and u-PA. Although the active 110 kDa polypeptide cannot be inhibited by anti-u-PA, it yet comprises a 37 kDa piece with some u-PA related antigenic determinants. However, these determinants are in a latent or cryptic form, only detectable after denaturation by SDS. The 110 kDa polypeptide is evidently not a dimer of 55 kDa u-PA or a complex of u-PA with an inhibitor.(ABSTRACT TRUNCATED AT 250 WORDS)
