Mapping of motor cortex gyral sites non-invasively by transcranial magnetic stimulation in normal subjects and patients.

Transcranial stimulation offers the opportunity to replace a standard clinical method of invasive brain cortical mapping. A figure '8' magnetic coil with a 1-3-cm area of stimulation is combined with a new method of creating magnetic resonance imaging (MRI) scans which represent the cortical surface (mapping cuts) and thus establishing common landmarks between the MRI and the scalp for cortical mapping. This assigns gyral sights to scalp sites of transcranial stimulation. We report the development and evaluation of these techniques in 6 normals and 10 patients. Cortical maps were shown to enlarge during voluntary contraction. They were variable between individuals, as reported for direct cortical mapping. Maps showed a degree of short and longer term consistency within an individual. The maps were influenced by lesions. They also provided clinical information of the relation of functioning cortex to brain tumors that could influence clinical management including surgical decisions. A consideration of safety of the method in comparison to direct brain stimulation indicates that it falls within existing parameters for electrical stimulation of the brain, and could reduce the risks of the present standard by lowering charge density and the need for invasive methods. Broader application could lead to development of functional imaging methods, which would be a useful preoperative planning tool and investigative method.

Electroencephalographic abnormalities in 351 Nigerians with epilepsy.

A review of the electroencephalographs of 351 epileptic patients admitted to the Ahmadu Bello University Teaching Hospital, Kaduna from March, 1982 to November, 1984 is presented. Those under 20 years age group predominated in the cohort studied. The frequencies of generalised epilepsy and partial epilepsy were almost equal. Partial epilepsy with complex symptomatology was common and petit mal was relatively rare. The inter-ictal EEG record was normal in 35.6% who were usually young and presumably suffered from idiopathic epilepsy. Differences existing in the EEG records reported and those reported in Nigeria and elsewhere are discussed. The need for an epidemiological study of the general population in the Northern Nigeria is emphasized.

[Diagnosis of epilepsy in private practice].

Careful recording of family and personal history is all-important in the diagnosis of epilepsy. If possible, a description of seizures should be obtained from an observer. Occasional seizures are to be distinguished from real epilepsy. The indications for cranial computerized tomography (CCT) are discussed: the indication is imperative in fresh partial epilepsies with elementary symptoms (some 30% brain tumors) and less so in fresh partial epilepsies with complex symptoms (some 5-10% brain tumors). In late epilepsy (onset after age 20-30, negative family and personal history), CCT is always worth considering.

Single photon emission computed tomography with 99mTc hexamethyl propylene amine oxime (HMPAO) and focal motor seizures.

The proper localization of regions of interest (ROIs) in Single Photon Emission Computed Tomography (SPECT) is an essential task. The limited spatial resolution makes it difficult to define regions by their structure. The particular problems are: 1) the difficulty to find the appropriate slice and its orientation in space; 2) the individual variation of the brain in dimension and shape. Referring to the basic work of Talairach and Szikla (1967), research is conducted to overcome these methodical problems in interpreting cranial computed tomography by using a proportional localization system. We analyzed four cases with focal motor seizures of different aetiology (tumour, hemorrhagic infarction, intracerebral hematoma, multifocal leukencephalopathy) by 99mTc HMPAO SPECT. The accumulation of the radiopharmacon was measured in ROIs which were delineated with the aid of the above mentioned system. The regions were selected in accordance to the autoradiographic study of penicillin induced epileptic seizures in animal experiments. Dependent on the severity of the clinical manifestation we were able to document the involvement of the supposed primary focus and of several related zones. We suggest that this approach should prove some usefulness in the examination of the interdependency of different brain areas.

Influence of flunarizine on hippocampal epileptic afterdischarges in the rat.

Epileptic afterdischarges elicited by electrical stimulation of the dorsal hippocampus in freely moving rats were not significantly changed by flunarizine administration in comparison with control sessions in which the animals received the solvent only. On the other hand, flunarizine significantly reduced the number of wet dog shakes, the main automatisms accompanying limbic afterdischarges.

[Complex focal seizures as a cause of long-term paranoid psychoses in advanced age].

Many organic diseases of the brain--including intoxications, inflammatory or neoplastic disorders--can cause acute psychosis. Even if the psychotic symptoms last for several years, the possibility of an organic cause should be borne in mind. We report the case of a patient with a paranoid disorder for two years before admission. The EEG showed generalized 1-2 Hz spike and wave complexes. These epileptic activities--and also the paranoid symptoms--ceased completely after administration of an antiepileptic medication.

[A case of intracranial osteochondroma originating with psychomotor epilepsy].

A 28-year-old woman was admitted to our clinic due to psychomotor epilepsy. Craniogram, CT scan demonstrated a calcified lesion in the frontal, parasagittal region. MR proton images showed a mixed hyper-and hypodense lesion. The tumor originated from the frontal bone and was totally removed using airdrill safely. Histological findings revealed osteochondroma originating in the frontal bone is rare, and diagnostic procedures and surgical management of this tumor are discussed.

[Gamma-vinyl-GABA: first experiences in Italy. II].

The long term efficacy of GVG was studied in patients with drug resistant CPC and CPC plus secondarily generalized seizures. GVG was started as monotherapy followed by add-on therapy with carbamazepine (CBZ), phenobarbital (PB), valproate (VPA). The results suggest: 1) loss of efficacy of gamma-vinyl-GABA during long-term use, and 2) at present, GVG appears to be an effective safe antiepileptic drug for initial monotherapy.
