The multiple Y-V plasty in linear burn scar contracture release.

The technique of the multiple Y-V plasty is described. The records of all 129 patients who underwent surgery to release burn scar contractures over a 5-year period are reviewed. Halfway through this study period the multiple Y-V plasty was introduced, resulting in a marked reduction in the number of Z-plasty and skin grafting procedures carried out. There was an associated fall in the number of wound complications seen and the length of hospital stay required.

Two unusual cases of distal interphalangeal joint Dupuytren's contracture.

Following trauma, two Caucasian males presented with Dupuytren's contracture affecting the distal interphalangeal joint of the little finger and extending onto the dorsum of the digit to insert into the nail bed. Complete excision of the cords and appropriate physiotherapy helped the patients achieve excellent postoperative results.

Surgical treatment of contracture and syndactyly of children with epidermolysis bullosa.

A new approach to the operative treatment of syndactyly and contracture of the hands of children with recessive dystrophic epidermolysis bullosa is described. It is based upon the principle of surgical release of fingers allowing spontaneous epithelialisation of skin wounds without using skin grafts. Nineteen children had operations using this method, with an incidence of recurrence of 53%. This method has the advantage of a short operating time, simple technique and limited trauma.

Long term follow-up of dermofasciectomy for Dupuytren's contracture.

During the 4-year period 1980-1983, we carried out 338 operations for Dupuytren's disease of the hand. Of these 46 (13.6%) had a dermofasciectomy and skin graft for recurrent disease. In this sub-group, 34 patients (10.1%) were operated on by the same senior surgeon (H.G.B.). Only these cases were included in this study. Average follow-up was 100 months (range 80-120 months). It was found that graft durability was good, there was no clinical evidence of recurrence beneath grafts, contracture did not exceed 15 degrees at any joint and two point discrimination in the graft was equal to, or better than, 12 mm in the palm and 8 mm in the digit.

The use of scapular skin island flaps in the treatment of axillary postburn scar contractures.

The scapular island flap easily covers the anterior and posterior aspects of the axilla, providing a smooth skin surface with good underlying subcutaneous fatty tissue. Its elastic properties are well adapted to the dynamic requirements of the shoulder joint. Performed in 20 patients for treatment of axillary postburn contractures, this flap led to a convincing improvement of the shoulder range of motion and satisfying aesthetic results.

Reversed dynamic slings: results of use in the treatment of post-traumatic flexion contractures of the elbow.

We report our experience with reversed dynamic slings used in the treatment of post-traumatic flexion contractures of the elbow. There were 21 patients treated for deformities ranging from 35 degrees to 70 degrees (average 55 degrees). The flexion deformity after treatment varied from 25 degrees to 45 degrees (average 34.8 degrees), giving an average increase in range of movement of 39.1 per cent.

Gastroschisis and congenital contractures: coincidence or syndrome?

An infant with a gastroschisis defect and severe congenital contractures of all extremities is presented. This case has offered dilemmas in judgement and in management as well. It is suggested that the occurrence of both these malformations in the same infant is probably not coincidental.

Negative halothane-caffeine contracture test in mdx (dystrophin-deficient) mice.

The genetics of malignant hyperthermia (MH) are ill-understood; however, the association of Duchenne muscular dystrophy (DMD) with MH is well known. A deficiency of dystrophin is common to both the DMD and mdx mouse, an animal model for DMD. Using muscle contracture tests for MH, we have shown that in the mdx mouse there is no MH susceptibility, suggesting the lack of a direct role of the dystrophin in the development of MH syndrome.

The C.C.A. syndrome (congenital contractural arachnodactyly): a new differential syndrome for Marfan's syndrome and homocystinuria.

The first case in the dental literature of congenital contractural arachnodactyly (C.C.A. syndrome) is presented. This newly delineated syndrome is an autosomal dominant heritable disorder of connective tissue. Its similarities to Marfan's syndrome and homocystinuria, as well as other syndromes, are discussed. The lack of cardiovascular disease, specific ocular anomalies, and mental retardation are presented in the differential diagnosis of the C.C.A syndrome with Marfan's syndrome and homocystinuria.

The open palm and digit technique in the treatment of Dupuytren's contracture.

The authors have used the open palm and digit technique for Dupuytren's contracture in fifty-four patients since May, 1973. The procedure consists in making transverse skin incisions on the palm and fingers leaving the wounds open after limited fasciectomy. The results have been surprisingly good due to the elimination of haematoma formation and finger oedema, and the excellent quality of the final scar.

Bladder contracture following intravesical BCG immunotherapy.

Intravesical BCG immunotherapy is commonly associated with irritative bladder symptoms. Bladder contracture as a result of this treatment is rare. Two cases of persistent small-capacity bladders occurring more than one year after intravesical BCG therapy are reported.

Histocompatibility antigen patterns in Dupuytren's contracture.

Thirty-seven consecutive patients with Dupuytren's Contracture were investigated by tissue typing techniques to ascertain their HLA--A, B, C and DR antigen status. Whilst no definite HLA antigen was found in association with this disease, at least one possibly significant pattern of HLA DR antigens was found in these patients. This finding was also noted in patients with scleroderma, and the scleroderma-like syndrome induced by vinyl chloride exposure.

Antibodies to collagen types I-VI in Dupuytren's contracture.

Sera from 16 patients with Dupuytren's contracture were tested for IgG and IgM antibodies to native and denatured human collagen types I, II, III, IV, V and VI. IgG antibody to at least one collagen type was found in 11/16 (69%) of these patients, compared with 27/96 (28%) normal adult blood donor controls. The prevalence of antibody to denatured type II collagen was raised, and although there was no overall increase in HLA-DR4 compared with a control population, this antibody was associated with HLA-DR4 in this patient group.

Dupuytren's contracture in the Negro.

In 1979 the first Negro patient with Dupuytren's Contracture who had no evidence of Caucasoid Admixture in his genetic make-up was reported on. Another five such cases are presented.
