Clinical profiles in fibromyalgia patients of the community mental health center: a predictive index of psychopathological severity.

INTRODUCTION: In recent years we have seen an increasing demand for mental health care in patients with fibromyalgia and psychiatric symptoms, although it is not clear if the symptoms are primary or secondary to the presence of the syndrome. This fact has led mental health providers to think that there would be some psychological factors influencing the vulnerability of suffering this painful syndrome, because its etiology is quite non-specific. Bradley et al. (1978) identified different psychopathological profiles within chronic pain syndromes with the MMPI, which were subsequently adapted by Yunus et al. (1991) for fibromyalgia. This present work studied the clinical profile in patients with fibromyalgia. METHOD SAMPLE: 75 patients with fibromyalgia from the community mental health center and 55 healthy subjects. Tools: STAI-E/R, BDI, MMPI-2, MMPI-2 personality disorders, MMPI-2 PSY-5. STATISTICAL ANALYSIS: descriptive statistics and mean comparison (Student's t test). Confirmatory cluster analysis. Discriminative analysis of subgroups. RESULTS: Two different patterns were obtained: group A (32 %) with a typical chronic pain profile (CP) and group B (68 %) with a psychological maladjustment profile (PM). With the discriminative analysis, we obtained the coefficients of the discriminative canonical functions that maximize the differences between both groups. CONCLUSIONS: We confirmed Bradley's classification, obtaining two different psychopathological patterns in the fibromyalgia syndrome sample we studied. We obtained an index of psychopathological profile in fibromyalgia, which would form a new scale, from MMPI-2 for discriminating psychopathological severity in fibromyalgia.

Coagulation profiles in neonates--if the specimen clots repeat the test.

OBJECTIVE: The aims of this study were to determine in two institutions the frequency of clotted coagulation profile specimens in neonatal patients, the proportion of patients who had the test repeated if the coagulation profile specimen clotted and the number of abnormal results in the tests that were repeated. METHODS: The number of neonatal coagulation profiles (i.e. prothrombin time and activated partial thromboplastin time) were identified from the pathology databases at the Royal Women's Hospital and at the Mater Mother's Hospital. Each specimen that clotted and the number of tests that were repeated were identified and counted. Data from the tests that were repeated were analysed and compared with normal ranges for neonates. RESULTS: There were 746 specimens that were sent for a coagulation profile of which 45 (6.0%) clotted. Of the 45 clotted specimens, 31 (69%) were repeated. From the 31 repeated specimens, 6 clotted, 1 was an 'insufficient' specimen, and 24 were able to be processed. From the 24 processed specimens 17 (71%) had an abnormal result, 11 of which had a mildly elevated prothrombin time and the other six had a severe clotting abnormality. CONCLUSIONS: If a neonatal patient's coagulation profile specimen clots it should not be assumed that the infant's clotting is normal, and another specimen should be sent for analysis.

Secondary amenorrhea and lipid profile.

The aim of this study was to evaluate the lipid profile in women with amenorrhea in reproductive age and to investigate possible impact of type of amenorrhea on lipid profile by comparing lipid profile in normogonadotropic and hypergonadotropic women with amenorrhea. This cross-sectional study included 23 amenorrheic and 12 eumenorrheic women under 40 years old. Each woman underwent the measurement of body mass index, serum levels of FSH, LH, estradiol, progesterone, total cholesterol, HDL, VLDL, LDL, and triglycerides. According to FSH levels women with amenorrhea were divided in two groups: hypergonadotropic (FSH>25 IU/L) (n=9) and normogonadotropic (FSH>25 IU/L) (n=14). There were no differences in BMI, estradiol, progesterone, cholesterol, HDL, LDL, VLDL and triglycerides between amenorrheic and eumenorrheic women. Women with normogonadotropic amenorrhea had higher plasma concentrations of triglycerides (P=0.027) and VLDL (P=0.262) and lower levels of HDL (P=0.014) than women with hypergonadotropic amenorrhea. In hypergonadotropic women with amenorrhea was found positive correlation between progesterone and HDL (P=0.043). The normogonadotropic women with amenorrhea had higher levels of VLDL and triglycerides and lower levels of HDL than hypergonadotropic women with amenorrhea. These results suggest that the type of amenorrhea is important factor which may modify lipid profile in women with amenorrhea. The changes in lipid profile in normogonadotropic women with amenorrhea could be a risk for cardiovascular diseases.

[Evolution of the convexity of tegumental profile during mixed dentition].

More and more in the past years the orthodontist has started to be interested in having his patient at the end of the treatment not only with a perfect occlusion but also with a harmonious face. In this regard, a real asset is the facial profile angle (Milicescu), with a significant value in the treatment result by compensating other disorders or by being a disturbance factor even during a correct treatment. The information is still general, creating a harmonious or dys-harmonious face effect, relative independent of other angles. In order to appreciate the tegumental profile, we invite you to analyze the soft-tissue profile and the total facial profile (presented by Rakosi) applied to a Romanian research group considered to be relevant for the normal values. The tegumental facial profile convexity has been analyzed through a transversal study but also in dynamics, in a longitudinal study of five years corresponding to the mixed dentition period.

Comparison of K hand files and ProFiles 0.06/0.04 in simulated curved root canals prepared by students.

OBJECTIVES: The objective of this study was to compare the step-down technique in simulated curved root canals using K hand files (VDW) and ProFiles 0.06/0.04 (Dentsply Maillefer). METHOD AND MATERIALS: A total of 72 canals were blindly prepared by instructed students: 1 block with K files and a second one with ProFiles 0.06/0.04 in connection with the EndoStepper (SET). Postoperative images of the canals were taken, and an independent dentist assessed preparations for amount and position of material removed during preparation. Efficacy of the instruments is described in terms of canal shape, instrument failure, canal blockages, and preparation time. RESULTS: ProFile instrumentation provided well-centered and tapered preparations while maintaining the apical foramen; in contrast, the use of K files resulted in frequent alterations of the root canal. In the apical portion, both techniques showed unprepared areas. The coronal diameter in hand-instrumented canals was more irregular and wider than in those mechanically prepared. There was no separation of a ProFile instrument. Hand instrumentation was faster than mechanical, but the difference was not statistically significant. CONCLUSION: Under the conditions of the study, the ProFile rotary system was simple and safe to use and created better preparations than hand instrumentation.

Plasmid profiles and randomly amplified polymorphic DNA analysis of Salmonella enterica serotype Enteritidis strains from outbreaks and sporadic cases in Turkey.

The aim of the study was to investigate the characteristics of Salmonella serotype Enteritidis strains isolated from outbreaks and sporadic cases in Turkey by plasmid profiles and randomly amplified polymorphic DNA (RAPD) patterns. A total of 64 S. Enteritidis clinical strains were selected from the culture collection of the Enterobacteria Laboratory of Ankara University Medical School Department of Microbiology and Clinical Microbiology for molecular analysis using the plasmid profiles and RAPD method. Fifty-six isolates (88%) harbored one to four plasmids ranging in size from 2.5 to 100 kbp. 57 kbp plasmids were the most common plasmids, and forty-four strains (69%) carried 57 kbp plasmids alone or together with other plasmids. The outbreak strains carried the same plasmid profile: three plasmids sized 57, 40, 3.0 kbp. None of the strains analyzed displayed any RAPD bands with the primer OPB-17. By using primer p-1254, 42 strains (66%) were divided into fourteen RAPD patterns. Ten of the outbreak strains (77%) showed >80% similarity by cluster analysis program. Analysis of RAPD-PCR with primer p-1254 proved an easy, rapid and discriminative method complementing antibiogram and plasmid profiles in routine laboratories, and may contribute to the investigations of S. Enteritidis which still cause outbreaks in Turkey. This study presents the first report on S. Enteritidis isolates in Turkey investigated by plasmid profiles and RAPD methods.

Specificity, sensitivity and diagnostic predictive value of selected laboratory generated autoantibody profiles in patients with connective tissue diseases.

We tested the hypothesis that the application of 5 laboratory generated serologic profiles facilitates the interpretation of individual autoantibodies in connective tissue diseases without loss of specificity, sensitivity or diagnostic predictive value. The predetermined laboratory generated autoantibody profiles were analyzed in 423 sera from patients with clinically diagnosed connective tissue diseases. All sera positive for one or more autoantibodies to a selected panel of nuclear antigens could be segregated into one of 5 profiles. The distribution of individual autoantibodies and profiles in the sera of patients with various connective tissue diseases were compared in terms of their respective diagnostic predictive value and sensitivity. The application of these profiles improved the sensitivity and diagnostic predictive value of these tests when compared to individual autoantibodies without loss of specificity, thus aiding clinical interpretation.

[Somatometry and lipid profile in smokers. Modifications after smoking withdrawal].

BACKGROUND: The purpose of this work was to determine the somatometric and lipidic profile in a group of 90 smokers and to carry out a prospective study on modifications due to smoking withdrawal. METHODS: A somatometric and lipidic profile was performed to 90 smokers and 30 non-smokers. Afterwards smokers were included in a smoking withdrawal program. One year later all subjects who stopped smoking as well as 10 of the smokers who failed in smoking withdrawal underwent a new blood analysis and somatometric study. RESULTS: Initial somatometry showed that smokers had a significantly higher (p less than 0.05) overweight and endomorphic index, based on somatometric characteristics. In addition, smokers showed significant lower level of HDL-cholesterol (high-density lipoprotein) than non-smokers (p less than 0.001). Somatometric and lipidic profile performed one year after smoking cessation showed a statistically significant increase on HDL-cholesterol (p less than 0.05). Somatometric and lipidic profile of the subjects who failed in smoking withdrawal showed no significant changes in relation to the study made the previous year. CONCLUSIONS: From these data we suggest that modifications of lipidic profile induced by tobacco consumption can be reverted, at least in part, after smoking cessation. In addition smokers present higher overweight when theoretic weight is based on somatometric characteristics.

Abbreviated kinetic profiles in area-under-the-curve monitoring of cyclosporine therapy.

Abbreviated kinetic profiles can reduce the number of phlebotomies and drug assays, and thereby the cost of area-under-the-curve (AUC) monitoring. In the present investigation, we used two independent data sets: group 1, 101 AUC profiles from 77 stable renal-transplant patients, which included a 5-h sample in addition to the usual 0-, 2-, 4-, 6-, 10-, 14-, and 24-h samples; and group 2, 100 profiles from 50 stable renal-transplant patients before and after a change in their daily oral dose of cyclosporine. Group I demonstrated a fair correlation between cyclosporine trough concentrations and the AUC calculated from a complete set of seven concentrations (r2 = 0.820 and 0.758 for the 24- and 0-h samples, respectively). Stepwise multiple linear-regression analysis revealed that the abbreviated set of three time points (2, 6, and 14 h) explained 96% of the variance in AUC values calculated from the full set of seven samples; additional time points increased the accuracy only slightly. For group 2, we examined the difference between the observed and the predicted concentrations by linear extrapolation; the error in the observed AUC value, compared with the predicted value calculated from seven time points (-13.2% to -1.2%), was similar to the error from just three time points (-11.5% to 4.5%). Abbreviated AUC profiles involving three time points used with a model equation seem to provide a reliable alternative to full seven-point profiles.

Impact of medication profile review on prescribing in a general medicine clinic.

The effect of medication profile review by a clinical pharmacist on prescribing in a general medicine clinic was studied. Patients who were receiving five or more prescription or nonprescription medications were randomly assigned to an intervention group (n = 315) or a control group (n = 257). A clinical pharmacist reviewed the medication profile of each intervention group patient on the day before the patient's clinic visit and attached a written profile review to the medical record for study by the physician during the visit. After each clinic day, the pharmacist obtained updated records of patients in both groups and collected data on the number and cost of medications ordered before and after the visits. After the clinic visits, the average number of medications and the average monthly medication cost per patient decreased by 0.21 and $0.60, respectively, for the intervention group, compared with increases of 0.48 and $3.31 for the control group. The net result of a single profile review was a decrease of 0.69 prescription per patient, for a monthly medication cost savings of $3.91. The number and cost of medications that were discontinued were significantly higher in the intervention group. The intervention group also had significantly fewer drugs added for previously documented medical problems. Medication profile review by a clinical pharmacist reduced both the number and cost of drugs for patients receiving five or more medications.

[Molecular epidemiological markers of Legionella pneumophila in the Valencian Community (III). Peptide profiles].

Electrophoretic profiles using soluble peptides obtained from Legionella pneumophila serogroup 1 have been studied to be used as epidemiological markers. Using 8.5% polyacrylamide gels (SDS-PAGE) a unique profile common to environmental and clinical isolates has been detected showing 2 differential bands: a 150 kD and a 230 kD band present in only one clinical isolate. In order to achieve better resolution in the peptide profile 10 to 20% gradient SDS-PAGE has been used, confirming the existence of a common profile and 2 subtypes as well as another profile present in a smaller number of strains.

The Epstein-Barr virus (EBV) in human pathology. II. Serologic profiles of EBV infections.

Antibodies to EBV induced intracellular antigens, (VCA, EA, NA) and VCA-IgM antibodies have been investigated to define EBV serologic profile of 245 individuals. This profile was a first studied in EBV primary infections. In infectious mononucleosis, the efficiency of EBV serodiagnosis is lower than Paul Bunnel Davidsohn reaction (PBD) : primary infection profile is only characterised in 80% of the positive PBD infectious mononucleosis (IM). On the other hand, EBV antibodies are preponderant for diagnosis in other clinical manifestations of EBV primary-infection were PBD is not alwasy positive (47%). EBV antibodies of patients with various diseases and antibodies of normal subjects show different profiles. By interpretation of these profiles one discuss the possibility to characterize reinfection, and either latent or active persistant infection.

Relationship of condylar bone profiles and dental factors to articular soft-tissue thickness.

The relationship between articular soft-tissue thickness and condylar bone profiles, compact bone thickness, and deviation in form was studied in central sagittal sections from 53 human temporomandibular joints. Because of the absence of normal standards, several definitions of thickness were used. The angular bone profiles had a higher probability of having thicker soft tissue in the superior sector (59%) than the other profiles. An ANOVA to test for soft-tissue thickness differences between profiles approached significance for the angular profile in the superior sector (P = 0.049). Neither compact bone thickness nor deviation in form had an observable relationship with articular soft-tissue thickness, probably because of the dynamic state of the articular tissues in young adult temporomandibular joints. The distribution of categories of articular soft-tissue thickness was not related to dental attrition; however, there was some skewing toward reduced soft-tissue thickness in the anterior sector in the group with loss of molar support.

[Basic geometrical principles of profile harmony as an aid in planning and simulating maxillofacial surgery].

Dividing the facial profile into three sectors according to the principle of the golden section with one circle around the auditory meatus as the center provides a basis for a harmonic profile. An additional important factor for the general harmony of the profile is the nose. Based on a great number of measurements, computer programs allow preoperative profile simulations and the assessment of surgical feasibility. As numbers and the resulting numerical relationships play a fundamental role in the classification of the human body, it is possible to express beauty in terms of geometry. It is, however, only at certain definite numerical relationships of these three facial sectors to each other that a harmonic profile of the human head is produced. The resulting constructive ideals of beauty provide aids and guidelines in plastic surgery.

An index for objective evaluation of the soft tissue profile.

Statistical analysis of the relationship between human perceptions and the physical characteristics of a sample of Japanese profiles led to the development of the Profile Aesthetic Index. The Profile Aesthetic Index is a composite of seven physical variables (five angles and two distances) measured on soft tissue profiles. The correlation between the index score and the mean rating score given by orthodontists and dental students was r = .80. Clinical validity of the Profile Aesthetic Index was examined by comparing the index scores before and after the surgical-orthodontic treatment of 50 patients. Potential uses and limitations of the index are discussed.

The fetal biophysical profile score: current status.

The cumulative experience with fetal biophysical profile scoring as a method for antepartum fetal risk assessment is now extensive. The cumulative data indicate that the method is sensitive for recognizing both the normal and compromised fetus. Moreover, the method appears to offer the advantage of grading various degrees of fetal compromise. The additional information gained by real-time ultrasound scanning (gestational age determination, fetal morphometrics, and fetal anomaly screening), while not an integral part of fetal biophysical profile scoring, nevertheless remains a critical aspect of antepartum fetal assessment. These data are collected simultaneously with the fetal biophysical profile score. It is impossible to separate cleanly the advantages of fetal biophysical profile scoring in isolation of this additional information collected. It would seem that such an attempt at separation is artificial since these data in combination provide more key information to the physician to guide fetal management. It seems reasonable to effect continued modification and improvement of the existing fetal biophysical profile score as a means of antepartum fetal risk assessment.

Plasmid profiles as an epidemiological marker for Salmonella enterica serotype Enteritidis foodborne outbreaks.

The incidence of enteritidis serotype of Salmonella enterica in salmonellae infections has steadily increased in Spain from 27.1% in 1982 up to 63.4% in 1987. Given this high incidence, we have studied the plasmid profiles of Enteritidis isolates to subclassify them. Different profiles were observed in 50 isolates. In 13 Enteritidis serotype outbreaks, up to 5 different plasmid profiles were found. Each outbreak correlated with a single plasmid profile except in one case where plasmids of two different profiles were observed in strains from the same outbreak.

[Soft-tissue profile changes through therapy with the Herbst hinge appliance].

The posttreatment tissue profile changes that can be observed after treatment with the Herbst therapy were investigated in a class II malocclusion mixed dentition group. Profile photographs of two groups of 18 patients were compared. One group was treated with the Herbst appliance, the other served as an untreated control. The treated cases were retained by means of an activator after the active treatment period. After this retention period, new profile photographs were taken and compared to the controls. All treated patients displayed a significant improvement of the soft tissue profile at the end of the active treatment period. After retention a relapse tendency could be observed. Due to the large range of observed treatment effects, no correlation between soft tissue profile and skeletal changes could be found.
