[Neuroprotective effect of N-methyl-D-aspartate receptor antagonists in an experimental glaucoma model in the rat].

PURPOSE: To evaluate the neuroprotective effect of memantine and dizocilpine, which are noncompetitive open-channel blockers of the N-methyl-D-aspartate (NMDA) receptor, on glaucomatous optic neuropathy in an experimental glaucoma model in the rat. METHODS: Experimental glaucoma was induced in the right eyes of 30 Wistar albino rats by intracameral injection of India ink followed by laser trabecular photocoagulation 4 days later. The left eye served as a control. Either memantine, dizocilpine, or phosphate-buffered saline (PBS) was injected intraperitoneally just before trabecular photocoagulation. Five days later, 3% fast blue was injected into both superior colliculi. The eyes were enucleated another 3 days later and flat mounts of the retinas were prepared. Labeled ganglion cells were counted in the area 1 mm away from the optic disc. RESULTS: Five days after laser application, no significant intraocular pressure (IOP) change in the right eye was found among the 3 groups. In eyes treated with memantine or dizocilpine, significantly more ganglion cells were labeled. CONCLUSION: Systemically applied memantine and dizocilpine had a neuroprotective effect against experimental glaucomatous optic neuropathy in the rat.

Automated cerebrospinal fluid cytology: limitations and reasonable applications.

OBJECTIVE: To evaluate the precision and clinical applicability of the Bayer ADVIA 120 cytometer (Bayer Healthcare, Fernwald, Germany) for cerebrospinal fluid (CSF) cell count and differentiation. STUDY DESIGN: One hundred six analyses of CSF from 98 patients by the ADVIA 120 were compared with routine cell count and microscopic differentiation. Correlation coefficients were calculated. RESULTS: In general, the total cell counts of both methods correlated well. The best correlations were seen at higher cell counts, > or = 100 cells per microliter with < 100 erythrocytes per microliter. The best correlations of cell differentiation were seen for lymphocytes and neutrophils, while the results for monocytes and eosinophils were less precise. In some cases, considerable differences between automated and microscopic cell counts and differentiation were seen that were relevant to clinical decision making. The detection of pathologic cell types, such as hemosiderophages, mitoses and neoplastic cells, was not provided by automated cytometry. CONCLUSION: When experienced personnel are not available, a preliminary cell count and differentiation between neutrophilic and lymphocytic reactions by automated cytometry may be valuable in allowing initial therapeutic decision making. Since the detection of pathologic cell types is not provided and the precision at low cell counts is only moderate, a personal microscopic evaluation of each sample is still indispensable to avoid misdiagnoses.

[Influence of phospholipids and their combination with prothrombin complex and calcium ions on coagulation in mice and rabbits both normal and affected by acetylsalicylic acid (author's transl)].

The influence of phospholipids and their combination with prothrombin and calcium on coagulation both normal and affected by acetylsalicylic acid (ASA) was investigated by determination of the bleeding time of mice, thrombelastographic (TEG) measurements and counting of thrombocytes of rabbits. Bleeding times and times of the TEG were prolonged after oral application of ASA, while thrombus stability was not altered. The number of thrombocytes decreased. Bleeding- and TEG-times normalized after injection of phospholipids. This effect was increased and of longer duration when phospholipids were combined with calcium and prothrombin. Decrease of the number of thrombocytes caused by pretreatment with ASA normalized only after injection of phospholipids together with prothrombin and calcium. The number of thrombocytes was not negatively influenced by the therapy in animals with normal coagulation.

A comparison of haemocoagulation tests in the experimental endotoxin model DIC and in rats whole-body irradiated by 250 Gy.

Based on comparing results of tests followed in the present work between the endotoxin model of the disseminated intravascular coagulation (DIC) and irradiated groups of rats we consider that after the whole-body irradiation by a high dose of 250 Gy, the DIC occurs, in spite of the fact that the first stage--hypercoagulation condition, can be hardly observed. In the experimental endotoxin model, an increase of activated partial thromboplastin test (APTT) values and prolongation of the thrombin time was observed up to 24 hours in both endotoxin doses. After both endotoxin doses, the fibrinogen level was transiently decreased with its subsequent increase. The fibrin monomers correspond to decreasing the fibrinogen level. After the first dose, they were positive between the 3rd and 12th hours and after the second dose, the positivity was observed 6 hours after the application. The antithrombin III level was decreased after 12 hours in both endotoxin doses. The thrombocyte count was considerably reduced already from the 6th hour after administering endotoxin to the end of the experiment. Considerable changes of the thrombocyte aggregation were observed only 3 hours after administering the second dose. When comparing the resulting values of these tests with values observed in irradiated animals, then we can see a certain agreement in the nature of the changes after the exposure to 250 Gy. The fibrinogen level was transiently decreased 3 hours after irradiation, when considerable changes in the thrombocyte aggregation also occurred.

Effects of increasing doses of a bolus injection and an intravenous long-term therapy of taurolidine on subcutaneous (metastatic) tumor growth in rats.

BACKGROUND: Experimental studies have shown that taurolidine suppresses intraperitoneal tumor growth following local application in rats. In opposite, a single intravenous therapy affected neither intraperitoneal nor subcutaneous growth of malignancies. Thus, an intravenous long-term therapy with taurolidine was investigated in rats after administration of a subcutaneous tumor load. VEGF and TNFalpha production and their effects on tumor growth have not been elucidated so far. METHODS: VEGF and TNFalpha levels of rat colon adenocarcinoma cells (DHD/K12/TRb) were analyzed in the supernatant undergoing treatment of increasing taurolidine doses in vitro. Besides the cell experiments rats were treated intravenously. At the beginning of the operation, 10 000 colon adenocarcinoma cells were applied subcutaneously at the back of the rats. Then the animals (n = 80, BD IX rats) were randomized into eight groups and underwent a standardized midline laparotomy for 30 min. At the end of the operation the animals were given either a bolus (1 ml Ringer's solution) or a long-term intravenous therapy (7 days, eight-hourly 1 ml 1%, 2%, or 3% taurolidine) were performed. For long-term therapy, a jugularis vein port catheter system was placed and left for 1 week. The influence on subcutaneous tumor growth, animal growth, general side effects and leukocyte/granulocyte levels were analyzed. Total tumor weights were determined 4 weeks after cell application. RESULTS: The VEGF and TNFalpha levels decreased rapidly after taurolidine therapy with low doses in vitro. The subcutaneous tumor growth showed a downtrend of tumor weight (P = 0.075) with a statistical significance in solid tumor counts (P = 0.04) at the back of the animals. A slight and temporary depression in animal growth was observed only in long-term therapy groups. Independent of the therapeutic agents and the application forms, the operation itself caused a slight leukopenia shortly after the operation compensated by a moderate leukocytosis in the following course. Fast injections of taurolidine led to a reduction of breathing rate. CONCLUSIONS: Only the intravenous long-term therapy of 3% taurolidine led to a slight downregulation in subcutaneous tumor growth. The changes of leukocyte counts were not affected by taurolidine. Fast injections have to be avoided. The findings prompted us to start new experiments to determine the influence of increasing doses of taurolidine on progressive tumor growth in rats.

The platelet level indicates the erythropoietic capacity for preoperative autologous blood donation.

UNLABELLED: The erythropoietic capacity for preoperative autologous blood donation (ECPABD) shows marked inter individual variability. This study was performed to evaluate factors useful to predict individual ECPABD from data available before the first donation. The subjects consisted of 74 adult patients who received autologous blood donation, with a mean of 61 +/- 12.8 yr (SD). We classified the patients into four groups using our criteria for evaluating the ECPABD and investigated the relationships among age, disease, pre-platelet count, and the rate of platelet increase. RESULTS: (1) Advanced age and the status of disease were not distinctly correlated with low ECPABD. (2) Patients with a high pre-platelet levels had a low ECPABD regardless of the haemoglobin. (3) Patients in which the platelet count increased in accordance with the level of collection exhibited low pre-platelet counts and high ECPABD. CONCLUSION: In patients with high pre-platelet levels, we reduced the amount collected, considered early use of recombinant human erythropoietin and reevaluated the application of autologous blood donation.

Application of laser scanning for the rapid and automated detection of bacteria in water samples.

It is widely accepted that the heterotrophic plate count method may not support the growth of all viable bacteria which may be present within a water sample and that alternative procedures using 'viability markers' may yield additional information. In this study, ChemChrome B (CB), which is converted to a fluorescent product by esterase activity, was used to stain viable bacteria (captured by membrane filtration) from potable water samples. The labelled bacteria from each sample were subsequently enumerated using a novel laser scanning instrument (ChemScan). Analysis of 107 potable water samples using this procedure demonstrated the presence of a significantly greater number of bacteria than were detected by culture (z-test, P < 0.05). The mean number of bacteria isolated by culture on R2A agar incubated at 22 degrees C for 7 d was only 25.2% of the total number of viable bacteria detected using the CB/ChemScan viability assay. Further analysis of 81 water samples using a 5-cyano-2,3,4-tolyl-tetrazolium chloride (CTC) viability assay also demonstrated the presence of many viable bacteria which were not capable of growth under the culture conditions employed in this study. However, the results indicate that ChemChrome B has the ability to stain a significantly greater number of heterotrophs than CTC (z-test, P < 0.05). In contrast, six potable waters were identified in which the CTC viability assay resulted in counts greater than those obtained using CB. The ChemScan instrument was successfully used for rapid and accurate enumeration of labelled micro-organisms, allowing information on the total viable microbial load of a water sample to be determined within 1 h. Furthermore, the ChemScan system has the potential for use in detecting specific organisms labelled with fluorescently-labelled antibodies or nucleic acid probes.

Total tritium measurement in atmosphere.

Measurement of tritium in the atmosphere is of strong interest wherever this radionuclide is used. Therefore, a method is proposed for the joint measurement of burnable tritium, independently from its physico-chemical form, and of tritiated water. The method consists of transforming the tritiated molecules of the gases present in the air volume into tritiated water by burning them together with a known quantity of hydrogen. The water vapor is condensed and added to a liquid scintillator. The scintillator is also able to dissolve conventional filters so that the tritium attached to particulate and concentrated on these filters can be jointly measured, as will be discussed in a future report. The overall detection limit of the method is approximately 64 Bq m-3 for a combustion period of 10 min (which corresponds to sampling an air volume of 15 L) and a counting period of 10 min. This limit, much lower than the derived air concentrations in the most unfavorable cases, allows the application of the method for safety purposes. Moreover, the method can be integrated into a general procedure for the measurement of tritium in different chemical forms, to be applied in case of necessity.

Internal calibration for a total body counter.

Whole body counting is used to monitor the ingestion of radioactivity by radiation workers when unsealed sources are used in biomedical or industrial applications. The whole body counter at Lucas Heights Research Laboratories has been running for four decades, and radiation workers are frequently monitored for radioactive contamination. In a retrospective study of total body potassium (TBK) values in radiation workers, it was found that the results varied markedly with time. In that TBK is a slowly varying quantity in healthy individuals, variation in TBK is therefore indicative of instability in the counting system. We conclude that TBK can be used as an internal calibration for the whole body counter. The activity of radioisotope contaminants can therefore be expressed relative to the value of the potassium activity in a given subject.

[Major eosinophilia in a premature infant induced by topical ketoconazole].

INTRODUCTION: Ketoconazole is a member of a newer group of imidazole antifungals. Except for treatment of oral candidiasis, there is no reported experience of use in neonates. Thus, its use in neonatalogy must be considered experimental. Herein we report a major eosinophilia in a very low birth weight neonate induced by transcutaneous resorption of topical ketoconazole. CASE REPORT: A 26 week premature was intubated and ventilated for a hyaline membrane disease. At more than 2 weeks, he developed diaper dermatitis with Candida albicans. Treatment consisted of local application of ketoconazole. After 6 days of application, WBC showed a major eosinophilia [20,000/microL]. Discontinuation of the drug was followed by a prompt normalization of the eosinophil count. The challenge test by topical ketoconazole reproduced eosinophilia, implicating this drug, which, to our knowledge, has not been described previously to cause eosinophilia in very low weight neonates. DISCUSSION: Peripheral eosinophilia is an uncommon finding present in a relatively limited number of conditions in children. A minor eosinophilia often seen in premature neonates may in part be due to intubation. This case illustrates the potential danger of percutaneous application of drugs in newborn and infants as it has previously been pointed out way various chemicals. Any deterioration of the corneal layer such as observed in many dermatosis prompts an increase in the cutaneous permeability. Immaturity of the premature skin may have aiso played a role. Dermatologists should be aware of potential toxic topical ointments before prescribing them to very low weight babies.

[Application of Testsimplets for classification of sperm morphology].

Testsimplets is a pre-stained, ready-to-use slides for differential blood cell counts. We have been using this Testsimplets slide to classify sperm morphology since 1978. Its characteristics is that it is very easy to prepare specimens quickly and we can see sperm morphology clearly for the classification of the sperm. We regard this Testsimplets slide as a supravital staining. The disadvantage is that we can not preserve specimens for more than 24 hrs. Extensive use of the Testsimplets slide is a useful method for the classification of sperm morphology in the infertility clinic.

Flow system for automated analysis of maize pollen.

Pollen grains are haploid gametes of uniform shape and size, and can be obtained in large quantity. If appropriate traits are used, they can be an excellent material for investigation of rare but important biological events like intracistronic recombinations or mutations induced by very low level of mutagens. This advantage will be further improved, if the laborious counting and examination can be made automatically. For automation of pollen analysis, techniques of flow analysis and image analysis would be applicable. Flow analysis with a optical detector was tested using maize pollen. Pollen grains were transported by gentle suction through a glass capillary which was placed under a microscope. Interruptions of the light path by pollen grains were detected by a silicon photocell after optical magnification and converted into electric pulses. The frequency distribution of pulse height was examined by a multichannel pulse height analyzer. 10(6) pollen grains would be counted and classified within about 30 min for a pollen suspension dilute enough for separation of each pulse. The flow system tested seems promising for detection of Wx mutant pollen in a wx pollen population after iodine staining if illumination of sample particles is improved.

Differential survival of patients with AIDS according to the 1987 and 1993 CDC case definitions.

OBJECTIVE: To evaluate the impact of the 1993 Centers for Disease Control and Prevention (CDC) revised classification system for human immunodeficiency virus and expanded surveillance case definition for acquired immunodeficiency syndrome (AIDS) on the number of cases and on survival of patients with AIDS. DESIGN: Retrospective analysis of data from a prospective cohort study of patients treated with zidovudine. PATIENTS: A total of 3515 patients enrolled in the Italian National Registry of Zidovudine-Treated Patients between July 1987 and December 1991 were analyzed. MAIN OUTCOME MEASURES: Numbers and survival probability estimates (using the Kaplan-Meier method) for patients satisfying the 1993 CDC case definition compared with patients fulfilling the 1987 CDC classification. Multiple regression analysis was also used to analyze the combined effect of independent variables on survival. RESULTS: According to the new classification system, the number of AIDS cases in the study population would increase by 188%. While the median survival of patients meeting the 1987 definition was 24 months, at the end of 57 months 53% of patients meeting the 1993 definition were still alive. Among the patients meeting the laboratory criteria for AIDS diagnosis using the new definition (CD4+ lymphocyte count < 0.20 x 10(9)/L [200/microL]), the presence of an AIDS-defining illness was a strong independent predictor of death. CONCLUSIONS: The application of the new definition results in a considerable increase in the number of cases. Survival for patients classified according to the 1993 definition is much longer than for those classified with the 1987 definition. Clinical status plays a major role in predicting survival outcome among patients whose CD4+ lymphocyte counts meet the new case definition.

[Effect of post-emergence application of two herbicides to winter wheat on the soil microflora (author's transl)].

In a field experiment the herbicides U 46 PD-Fluid (dichlorprop salt) and Tribunil (methabenzthiazuron) were applied in April 1976 post-emergence to winter wheat cultivated in 3 different soils. After 4 days, 1, 2 and 3 months samples were taken from 0-5 and 5-10 cm depth of a loamy sand soil. Dehydrogenase activity and respiration were investigated. Microbial populations were counted after 4 days only. In all 3 soils decomposition of straw in 10 cm depth was estimated using nylon gauze bags. The application of U 46 DP-Fluid stimulated bacterial and algal but reduced fungal and actinomycete populations at 0-5 cm depth. About 10% increase in the dehydrogenase activity was recorded during the whole season. Soil respiration investigated immediate after taking samples was stimulated on the 4th day after herbicide application, but reduced after 3 months. Storage of the soil samples at 5 degrees C for 10 months showed similar or opposite results depending on the incubation method used. A somewhat increased decomposition of the straw was also observed in the loamy sand until June. The application of Tribunil at the same depth stimulated populations of actinomycetes and algae but did not affect the other microflora. Reduced dehydrogenase activity was observed during the dry month of June, but the same remained unchanged at the other time of sampling. In freshly drawn soil samples, reduced soil respiration was observed after 1 and 3 months and this remained unaltered at other times. The storage at 5 degrees C of the soil samples revealed inhibition only after 1 month and at the other times a stimulation or no influence could be observed and this was found to be dependent on the incubation method. Straw decomposition was not changed following the application of Tribunil. In 5-10 cm depth, however, the above mentioned trends were only scarcely observed.

Endodontic treatment with application of Er:YAG laser waveguide radiation disinfection.

OBJECTIVE: The objective of this study is to examine the ability of Er:YAG laser radiation. Using a movable waveguide helps to obtain an antibacterial effect, not only in root canal walls but also in the surrounding tissues. BACKGROUND DATA: Conventional endodontic treatment is not fully effective due to microbial colonization of root canal walls dentin in premolars and molars. Various laser systems seem to be effective to kill the remaining microbial content in the root canal. The problem is in the flexibility of laser system tips. MATERIALS AND METHODS: The Er:YAG laser system was designed with a fluorocarbon polymer-coated silver hollow glass waveguide. Root canal systems of 44 premolars and molars were treated endodontically, using a step-back technique; 10 teeth were then treated with calcium hydroxide paste, and 22 teeth were irradiated by a movable waveguide. Before and after treatment, the colony-forming units were counted to determine 21 various microorganisms. RESULTS: Classical enlargement and shaping of the root canal is effective in 60%. Application of calcium hydroxide prepares sterile root canal in 80%. Er:YAG laser irradiation via movable waveguide (energy of 100 mJ, 30 pulses, repetition rate 4 Hz) can ensure residual disinfection of the root canal. CONCLUSION: Application of Er:YAG laser radiation through a flexible waveguide helps to attain antibacterial effect, not only in the root canal walls, but also in the surrounding tissues. Therapeutic doses of laser radiation guarantee one-step disinfection, including of anaerobic microorganisms.
