[Characteristics of the organization of the psychosocial and vocational rehabilitation of patients with juvenile schizophrenia (a clinico-psychological study of the slowly progressing form with a favorable outcome)].

Clinical catamnestic and experimental psychologic techniques allowed the authors to single out a range of personality and psychosocial structures in patients with juvenile slow progredient neurosis-like schizophrenia with favorable outcome up to "virtual recovery". Influencing these structures with respect to the patients' age could promote their general and special activities, heighten their level and quality of adaptation to major spheres of social functioning. This provided a basis for establishing the major goals of social rehabilitative and psychic corrective aid to the juvenile patients.

[Medical and vocational rehabilitation of patients with implanted pacemakers].

Medical and occupational rehabilitation following pacemaker implantation is discussed with special reference to objective quality of medical and technical data, on which occupational recommendations are to be based. A comprehensive analysis of whether an individual with an implanted pacemaker can drive a car is presented.

[Clinical and social aspects of vocational rehabilitation of patients after heart valve prosthesis].

Clinical and social factors that determined the effect of occupational rehabilitation were analyzed in 71 patients after prosthetic valve implantation. In addition to objective factors, resuming work depended on a number of subjective ones as well, as the number of working individuals was virtually similar among the preoperative and follow-up patients, while the number of invalids increased, in spite of the fact that functional condition had improved by 1 or 2 functional classes, according to NYHA, in 52.8% of postoperative patients. There were no significant differences in physical working capacity, as evidenced by bicycle ergometry, between working postoperative patients and those who failed to resume working.

Tissue pressure management in the vocational setting.

A significant problem that confronts a spinal cord injured person who returns to work is how to design the work station and tasks so that pressure sores do not develop and compromise the worker's productivity. In this study, reach was investigated as a mechanism to cause weight shift during task execution. It is shown that by using an extended reach motion, it is possible to cause effective weight redistribution. By incorporating this type of motion in task design for mobility impaired workers, it should be possible to reduce their risk of developing pressure sores with a minimal effect on job performance.

[Outcome, course and prognosis of the vocational status of psychiatric patients hospitalized for the 1st time--results of a multiple factor study].

In the framework of a prospective longitudinal investigation, the occupational course of first-admission psychiatric patients was analyzed for the first year (1980) and the fifth (1984) post-discharge from the clinic. Findings indicate that, to quite an important degree, tendencies towards occupational disintegration had existed already immediately after the first in-patient treatment episode. This applied almost totally independent of the clinical diagnosis given at the time of the first hospitalization. Only persons with substance abuse illness had been affected clearly less often, the situation four years later being however comparable. Except for the groups of neurotic and personality disorders, the occupational situation had by then however tightened drastically for all of the diagnostic groups, a structural analysis making it clear that this development had initiated already in the course of the first year. Thus, all patients who had been without work over the entire first year of the five-year period, had already reached the "terminal point" in their employment history. As far as diagnoses are concerned, our study confirms the experience that a schizophrenic illness will by no means always entail a straight course toward an unfavourable outcome. The investigation at the same time however also indicated that non-schizophrenic illnesses take a more unfavourable course than had been expected. In relation to earlier longitudinal catamnestic studies of persons with schizophrenia, the findings for our schizophrenic patients moreover were markedly worse, this discrepancy being presumed to be accounted for by the different methods applied, different selection criteria, as well as differences in economic activity at the time.(ABSTRACT TRUNCATED AT 250 WORDS)
