Clinical study on the reasons for and location of failures of metal-ceramic restorations and survival of repairs.

PURPOSE: A recently introduced technique, the Cojet system, using SiOx (Al2O3 coated with silisic acid), provides ultrafine mechanical retention by sandblasting, as well as a chemicophysical bond between the metal-ceramic and the composite resin. This study determined the reasons for and locations of failures of metal-ceramic restorations and evaluated the survival of the intraoral silica-coating system used for the repair of failed restorations. MATERIALS AND METHODS: A total of 153 patients possessing 289 fractured crowns were involved in this study; 255 of these fractures were on fixed partial dentures, whereas 34 were on single crowns. The mean observation period was 34.6 months. The materials used for the repair process were ESPE-Sil for silane; Visiogem, Sinfony, and Dentacolor as opaquers; and Pertac II Aplitip, Sinfony, and Charisma as repairing composite resins. RESULTS: The majority of the failures (65%) occurred in the anterior region. Sixty percent of the failures were observed at the labial, 27% at the buccal, 5% at the incisal, and 8% at the occlusal regions. The fractures were mainly in the maxilla (75%), predominantly at the labial surface. The overall cumulative survival rates of the repairs (89%) showed that the first failures happened mostly from 1 week to 3 months after the repair, without any difference between the composite resins. CONCLUSION: The original failures happened mainly during chewing function or because of accidents, iatrogenic factors, and surgical operations, whereas secondary failures after repair using intraoral silica coating and composite resin were due to trauma, chewing function, or lack of rubber dam application.

The proximal precinct in direct posterior composite restorations: interproximal integrity.

Resourceful efforts of researchers and clinicians have made for an ever-expanding restorative repertoire. The search for improved operative techniques must continue as the objectives of an ideal direct restoration reach beyond natural aesthetics and include pulpal health considerations, occlusal stability, anatomical restitution, marginal perfection, and interproximal integrity. This article focuses on the proximal precinct and provides solutions to the challenge of restoring proximal physiological form to contiguous surfaces when utilizing the direct composite resin technique.

[Study of self-threading pins in the preservative restoration of teeth with large area defects].

OBJECTIVE: To investigate the application of self-threading pins in the preservative of teeth with large area defects. METHODS: A follow-up study of 42 out of 50 patients with large area defects in vital teeth were restored by self-threading pins. RESULTS: Among the cases that had been followed up for 5 years, except 2 upper molars which were extracted because of fraction, other cases achieved good effects. CONCLUSION: Self-threading pins are very suitable to restore large area defects of teeth with healthy pulp, and can be widely used because of significant clinical effects.

Microleakage and damage to adjacent teeth when finishing Class II adhesive preparations using either a sonic device or bur.

PURPOSE: To detetmine the damage to adjacent tooth surfaces and the quality of the marginal seal of a resin-based composite restoration when a sonic preparation device (Sonicsys) was used to finish the preparation outline. MATERIALS AND METHODS: Three general practitioners prepared 60 Class II box-type cavities in extracted teeth placed in an artificial jaw in a phantom-head. In 30 preparations a bevel was made with a bur, in the other 30 restorations the margins were finished using the sonic preparation device. All preparations were restored with a total-etch technique and a posterior composite (SA primer-Photo Bond-Clearfil PhotoPosterior). Microleakage was tested and the adjacent teeth were inspected for iatrogenic damage. RESULTS: No statistically significant differences in microleakage were observed between the experimental and control groups. In the control group, 80% of the approximal surfaces of adjacent teeth were damaged while only 30% of the adjacent surfaces were damaged in the experimental group (P < 0.05, Chi-square test).

[Use of a monomaxillary curved splint in maxillofacial surgery].

We describe a prefabricated monomaxillary arch, fitted with perpendicular supports with a main bar which provides dento-maxillary setting in the antero-posterior direction and also vertically since it is possible to attach it to the free edge of the teeth. The indications concerning the use of this arch are given in detail. It offers the advantage of being readily adaptable and is therefore easy to use in an emergency.

Common dental emergencies.

Dental caries, a bacterial disease of teeth characterized by destruction of enamel and dentine, is often the underlying cause of dental pain. When a carious lesion impinges on the dental pulp, pulpitis follows and, ultimately, necrosis of the pulp occurs. Untreated necrosis may lead to a localized abscess or a spreading infection into the surrounding soft tissue that results in cellulitis. Immediate treatment involves antibiotic therapy for cellulitis, perhaps with drainage of abscesses, while definitive treatment requires root canal therapy or extraction of the involved tooth. Pericoronitis is an inflammation of the soft tissue overlying a partially erupted tooth. Localized cases respond to irrigation. Secondary cellulitis can develop. Definitive treatment may require surgical extraction of the underlying tooth or excision of the gum flap. Avulsion of a permanent tooth secondary to trauma is a true dental emergency. The tooth should be reimplanted on the spot, and the patient should be seen immediately by a dentist for splinting and antibiotic prophylaxis. Most dental problems can be prevented with regular dental care and steps to minimize risks of oral trauma.

Oral piercing: the hole story.

Oral and perioral jewelry is increasingly being viewed as an acceptable fashion statement in our society. Accordingly, dental professionals are being presented with questions by their patients as to the risks associated with this form of body piercing. This article reviews recommendations for individuals with, or contemplating, an oral piercing, common locations of piercings, risks associated with the piercing procedure, types of jewelry used, and post-piercing home care instructions. Current trends in the piercing industry will be discussed, and the recommendation made that dental professionals advocate for improved safety standards at body modification studios.

Endodontic treatment during childhood and adolescence. A survey of 19-year-olds living in the city of Malmö, Sweden.

The aim of the present study was to determine the prevalence and reasons for endodontic treatment in a sample of 19-year-old adolescents living in the city of Malmö, Sweden. Dental records and available radiographs of all 19-year-olds (1,971) registered at the 20 Public Dental Service (PDS) clinics in Malmö were reviewed and data were collected on the following: endodontic treatment, number and type of teeth involved, and diagnosis leading to treatment. Endodontic treatment had been performed in 180 (9.1%) patients. The percentage of 19-year-olds at a PDS clinic who had undergone such treatment varied between 1.2% and 25.2%. One tooth had been treated endodontically in 138 (76.7%) of these patients. In three patients four teeth and in two patients five teeth had needed treatment. The molars were the teeth most frequently treated (46.5%), followed by the anteriors (39.5%), and the premolars (14.0%). The most commonly treated tooth was the maxillary central incisor and the most common diagnosis was caries (56.4%) followed by trauma (22.2%) and tooth developmental disturbances (1.6%). In conclusion, endodontic treatment seems to be fairly common among 19-year-olds in the city of Malmö. The great variation seen between the different PDS clinics most likely reflected variations in the socio-economic structure of the catchment areas of the clinics.

[Gingivo-dental sparing strategy of orthodontic treatment (fitting with permanent dentures)].

Development of inflammatory process in the marginal part of the mucosa is caused not only by epithelial injury during tooth preparation and dental plaque accumulation, but by irregular contour and position of the artificial crown edge as well. Clinical, x-ray, and cytoenzymological studies showed that orthodontic treatment with cermet and polyceramic dentures with fixation of the supporting elements at the level of the gingival edge in tissues adjacent to the tooth causes the slightest pathological changes. The compensatory adaptive processes in this area of the buccal mucosa are the most pronounced if preparation of hard dental tissues is combined with closed currettage of periodontal pouches, which was confirmed by remote clinical results and laboratory findings in patients with slight and medium-severe periodontitis.

Atypical localized deep pocket due to a cemental tear: case report.

A 71-year old patient with a history of minimal periodontal attachment loss presented with an asymptomatic, non-bleeding, isolated deep pocket on the mesio-facial of a central incisor. Such localized, deep pockets are most frequently associated with a root fracture or an abscess, but in this case an atypical ledge-like projection determined to be a cemental tear was detected at the base of the 6 mm pocket. Periodontal flap surgery revealed a partial detachment of the cementum. Uneventful resolution of the pocket was achieved following root planing to remove the cemental fragment and placement of a bone graft and resorbable membrane. This article reviews the nature, predisposing factors, causes, clinical significance, and treatment of cemental tears.
