[ Pulmonary inflammatory pseudotumor in a child].

The Inflammatory Pseudotumor is a no neoplastic process characterised by an irregular growth of inflammatory cells. A case of 10 years old male is described, he was asymptomatic, diagnosed by a casual chest XR, with a spheric mass in the inferior lobe of the right lung. An excisional biopsy was done with the result of "Inflammatory Pseudotumor". The Inflammatory Pseudotumor is a rare process that can be found in different regions of the human body, with most frequent localization, in the lung. The image tests are not specific for the diagnosis that only can be done by biopsy. The recommended treatment is the complete resection.

[The late recurrence of ovarian cancer seventeen years after radical surgery].

The study presents a very interesting case of ovarian cancer with long-term survival. The patient was primary treated by conservative surgery without chemotherapy. Cystadenocarcinoma mucinous G-1 was recognized. Ovarian cancer relapsed after eight years and the second operation was performed. During next, radical operation other histological typ of ovarian cancer (mixed, sero-mucinous cystadenocarcinoma) was recognized. Eight years after second surgery and seventeen years after primary surgery ovarian cancer again recurred. There were a very diffuse neoplastic process at the abdominal cavity. Although patient was treated by surgery and chemotherapy, she died. The study presents new opinions of pathogenesis of ovarian cancer and origin of recurrences, including theory of multifocal neoplasia of mesothelium.

[Dynamics of changes in enterochromaffin cells during tumor growth].

A distinct correlation was found between changes in enterochromaffin cells producing highly active chemical substances--melatonin and serotonin, and stages of the neoplastic process. In early stages of tumor development a marked hyperplasia of enterochromaffin cells was noted, that was associated with enhancement of their histochemical properties. In advanced forms of cancer and chronic leucosis the number of cells is markedly reduced with simultaneous diminishing of their staining by specific histochemical methods. Such kinetics of changes of enterochromaffin cells appears to be related with definite antiblastic properties inherent to them and their products (melatonin and serotonin).

[Role of genealogical studies for the prevention of malignant neoplasms].

Based on clinico-genealogical studies of 219 cancer patients, whose relatives suffered neoplastic processes, it was concluded that a hereditory factor plays a definite role in cancer morbidity. It seems to be rational to single out the relatives of the proband as a high-risk group in malignant tumor morbidity and to keep them under a dispensary control.

[Derivatives of cholecalciferol in the treatment of psoriasis].

The review of the literature is devoted to derivatives of calciferol (vitamin D3), representatives of a new group of antiproliferative drugs employed in psoriasis, ichthyosis, rheumatism and some neoplastic processes. Results are submitted of histo- and biochemical, physiological, and clinical trials of the synthetic derivative of vitamin D calcipotriol, a possibility of its combination with physiotheraputic modes of treatment is reported together with recently identified side effects. Novel analogues of the drug are listed, with their merits and demerits pointed out. The authors conclude that further studies are warranted into abnormalities of metabolism of calcium in psoriasis to develop efficacious modes of correction of dermatosis involving the use of representatives of the given group of medicinal agents.

Tumour's angiogenesis--the function of VEGF and bFGF in colorectal cancer.

The main objective of this paper was to present the view on the vascular endothelial growth factor (VEGF) and basis fibroblast growth factor (bFGF) in the angiogenesis in the colorectal career. The analysis includes the works of these scientists, who proved the relationship between VEGF and bFGF expression and the advancement level of colorectal cancer and the survival of the patients ill with this disease. It was stated that the highest levels of these factors were connected with the advancement of neoplastic process, especially when metastases coexisted at the same time. It was also proved that the higher levels of VEGF and bFGF gave worse prognosis as far as survival was concerned. Apart from this, other factors effecting angiogenesis and inhibiting factors were also presented.

[Neck mass changes of the nontumorous and noninflammatory nature: the value of radiodiagnosis].

The results of X-ray examination of 25 (2.8%) out of 899 patients were analyzed. The changes of their neck mass were due to the conditions of non-tumoral and non-inflammatory nature. These states were not always clinically clear and were not infrequently considered as a neoplastic process. The causes of them can include developmental anomaly of neck organs, post-radiation and postoperative alterations, atherosclerosis, degenerative-dystrophic alterations in the cervical portion of the vertebral column etc. To establish the character of the neck mass changes it is necessary to perform a complex radiation examination, its first and sometimes the only stage being USI. Choice of the following methods (CT, MRT and so on) depends on the results of USI and the clinical picture.

[Behavior of SCC marker level in cervix erosion, cervix dysplasia and cervical pre-invasive carcinoma].

The work presents the results of studies on the behaviour of SCC antigen in 52 patients aged 21-70 years, with pathologic changes involving the uterine cervix and classified to the groups of erosion, dysplasia and carcinoma in different stage of advancement. The SCC antigen was determined in patients blood sera by radioimmunologic method, on the basis of Abbott Firm testes. An increase in concentration was observed exceed the norm (2 ng/ml) in cases of CIN and CIS. The rise of marker depended on the stage of the neoplastic process advancement in the uterine cervix.

Lymphoproliferative disorder of granular lymphocytes. A heterogeneous disease.

Lymphoproliferative disorders of granular lymphocytes (LDGLs) represent a family of diseases that are morphologically similar but diverse with regard to immunophenotype, function, and clonality. In this article, we report three informative cases and propose a modification of the current classification of LDGLs. Our first case is an example of natural killer cell LDGLs (CD2+, CD3-, CD16+, CD57+/-). Based on a review of the literature, we suggest that natural killer cell LDGLs can be divided into two subgroups (types 1 and 2) according to the expression of CD57. Reduced expression of CD57 may distinguish between patients with a poorer prognosis. The remaining two cases illustrate examples of T-cell LDGLs (CD2+, CD3+, CD8+, CD57+) that differ mainly in their expression of CD16. The CD16+ T-cell LDGLs (type 1) usually show a clonal rearrangement of the T-cell receptor-beta chain gene, whereas CD16- T-cell LDGLs (type 2) may show a germline configuration, suggesting a reactive rather than a neoplastic process. Pathologists should differentiate LDGLs from other chronic lymphoproliferative diseases, since most cases evolve slowly and aggressive cytoreductive therapy is usually unwarranted.

Nucleolar organizer region-associated proteins in cancer cells. Quantitative investigations on gliomas, meningiomas, urinary bladder carcinomas and pleural lesions.

Using a modified silver staining technique, we investigated nucleolar organizer region-associated proteins (AgNORs) in paraffin sections of 156 neoplastic tissues and other lesions, including gliomas (n = 41), meningiomas (n = 20), urinary bladder carcinomas (n = 58), and neoplastic and reactive lesions of the mesothelium of the pleural cavity (n = 37). We found significant differences in the mean number and area of AgNORs per nucleus between nonanaplastic and anaplastic astrocytomas. In meningiomas AgNOR analysis may be useful to distinguish between mostly benign tumors (grade 1 tumors) and atypical ones. Urinary bladder carcinomas exhibited a statistically significant increase in both AgNOR number and area as the grade of malignancy increased. Diagnostically useful differences in the AgNOR configuration between inflammatory and neoplastic processes were found for mesothelial lesions. In general, a higher grade of malignancy correlated with an increase in the AgNOR number. This was accompanied by an increase in the total AgNOR area per nucleus, irrespective of whether the size of the individual AgNORs had changed.

Gd-DTPA enhancement of the cisternal portion of the oculomotor nerve on MR imaging.

PURPOSE: To describe a radiographic finding--enhancement of the cisternal portion of the third cranial nerve on postcontrast MR--and to correlate it with patients' clinical symptoms and ultimate diagnosis. MATERIALS AND METHODS: Thirteen consecutive patients with enhancement of the cisternal portion of the third cranial nerve on postcontrast MR were retrospectively identified; 50 control patients referred for pituitary microadenomas were also retrospectively reviewed. FINDINGS: The enhancement was bilateral in six patients and unilateral in seven patients. Four of the six patients with bilateral enhancement had unilateral oculomotor nerve palsies; none had bilateral third cranial nerve palsy. Five of the seven patients with unilateral enhancement had ipsilateral third nerve palsies. Of the nine patients with third nerve palsies, the pupil was involved in four patients. Follow-up studies were available in six patients, four of whom had third nerve palsy. Resolution of the enhancement correlated with resolution of the symptoms in two patients. The patients' underlying diagnoses were lymphoma (four), leukemia (one), viral meningitis (one), neurofibromatosis (two), inflammatory polyneuropathy-HIV related (one), ophthalmoplegic migraine (one), Tolosa-Hunt syndrome (one), coccidioidomycosis (one), and diabetes (one). No enhancement was seen in any of the controls. CONCLUSION: Enhancement of the cisternal segment of the third cranial nerve is always abnormal, revealing an underlying inflammatory or neoplastic process. However, it is not always associated with clinically apparent oculomotor nerve dysfunction.

[The surgical and combined treatments of locally disseminated cardioesophageal cancer].

The article deals with the experience in the treatment of more than 1,000 patients with cardioesophageal carcinoma in 1965-1989. The authors note increase of surgical activity and widening of indications for operative treatment at the cost of patients of the old age groups and those with locally-extending forms of the neoplastic process. In distinguishing the term cardioesophageal carcinoma the authors insist on the community of the surgical tactics and operative methods in carcinoma of the cardia extending to the esophagus and in esophageal carcinoma spreading to the stomach. Improvement of the results of tre treatment the authors attribute to the perfection of the methods of lymphodissection and formation of esophageal anastomoses. The authors claim that the development of effective methods of combined treatment underlies the prospects of improving the late-term results. Experience in preoperative hypoxyradiotherapy, adjuvant postoperative chemotherapy, and preoperative regional intraarterial polychemotherapy is analysed.

Treatment of Paget's disease of bone with mithramycin.

The hypothesis that Paget's disease of bone is a low grade neoplastic process led us to use the cytotoxic antibiotic mithramycin in its treatment. The dramatic effects observed on the serum calcium and alkaline phosphatase, and urinary hydroxyproline are compatible with the concept that mithramycin is primarily toxic to osteoclasts. Subjective and objective clinical effects establish this agent as useful in the treatment of Paget's disease despite its observed toxicity to other organ systems.

[Effects of the antibacterial peptide cecropins from Chinese oak silkworm, Antheraea pernyi on 1, 2-dimethylhydrazine-induced colon carcinogenesis in rats].

OBJECTIVE: To gain insight into the putative anticancer effect of the antibacterial peptides, cecropins, from Chinese oak silkworm, Antheraea pernyi, on the cancer cells and 1,2-dimethylhydrazine (DMH)-induced colon carcinogenesis in rats. METHODS: Growth inhibitory effect of the cecropins on normal human gastric epithelial cell line (GES-1) and human colon adenocarcinoma cell line (LS-174T) was observed using a microculture tetrazolium (MTT) colorimetric methods. Male Wistar rats were divided into 4 groups. Group1 was given on a weekly basis cecropins from Antheraea pernyi (3,000 Ua/ml) by gavage at 2 doses of 10 ml/kg body weight and exposed to subcutaneous injection of DMH at the dose of 20 mg/kg body weight. Group 2 was received weekly DMH only. Group 3 was given the cecropins by gavage at the same dose as in group 1. Group 4 was weekly exposed to subcutaneous injection of EDTA (1 mmol/L). All treatments were completed in a course of 18 weeks and the experiment was finished at week 33. RESULTS: MTT assay showed selective cytotoxic activity of the cecropins against the human colon adenocarcinoma cells line. The viability of the cancer cells was about 54% and 100% for the normal cells. There was a significantly lower incidence of large intestinal tumors in rats gavaged with cecropins (65%, P<0.01), but the tumor burden (tumors/tumor-bearing animal) and tumor mass index were comparable between the groups (P>0.05). CONCLUSION: The cecropins possess effective anti-tumor activity with no cytotoxicity against normal eukaryotic cells, and impede the neoplastic process in murine large intestines.

[Effect of hepatic resection on development of liver metastasis].

In the early stages of metastasis, development of the disease is dependent on growth factors produced by the host. There are clinical situations associated with an increase in these factors, such as partial resection of metastasized liver. Given the important role of hepatotrophic factors in liver regeneration, we have studied the effect of partial hepatectomy on the development of residual micrometastases in the liver, and on the neoplastic process as a whole. We used a murine model in which a rabdomiosarcoma was established by subcutaneous inoculation of syngeneic tumor cells in male Wag rats. Subsequently, the primary tumor was resected and/or a 40% hepatectomy was performed. The effect of these two surgical procedures on the tumor process was analyzed on the 25th and 35th days post-inoculation, and the percentage of regenerating hepatocytes was assessed. Both the tumorectomy and liver resection, when not combined, produced an increase in regional adenopathies without modifying the evolution of metastasis in the liver. However, when tumor excision and partial hepatectomy were performed simultaneously, there was a net increase in the metastatic process. In addition to a rapid spread of the disease (lung, mediastinum, retroperitoneum), the number of liver metastases increased by 300%. This development coincided with a steep rise in the percentage of regenerating hepatocytes, which nearly doubled that of the group subjected only to liver resection. We conclude that liver resection, alone or combined with excision of the primary tumor, may enhance tumor progression, both locally and at the metastasic level.

Malignant lymphoma of the jaw bone.

BACKGROUND: Malignant lymphomas are neoplasms originating from lymphocytes, whose precursor cell or cells are generated during the multipotential differentiation of a stem cell. Extranodal localization most often involves the Waldeyer's ring, nasal cavity, maxillary sinuses, eyeballs, salivary glands, oral cavity, stomach or skin. Localization in the nervous system or facial bones is very rare. CASE REPORT: This article presents a rare case of a patient with B-cell lymphoplasmocytic lymphoma located in the jaw bones. The diagnosis was established after histopathological investigation of periapical lesions. Due to the localized growth of the malignant lymphoma, the patient underwent surgery combined with radiotherapy. The follow-up period amounted to 14 months and the patient now presents without recurrences or lymphadenopathy. CONCLUSIONS: All periapical lesions should be sampled for histopathological examination, possibly revealing a hidden neoplastic process.

Chromosome 10 and 17 deletions and p53 gene mutations in Thai patients with astrocytomas.

The tumor suppressor gene locus is known to be partly responsible for the tumorigenesis of sporadic gliomas, but the genetic events that drive the neoplastic process of this tumor remain largely unknown. We correlated the results of loss of heterozygosity (LOH) analysis on chromosomes 10 and 17 and a point mutation analysis of a tumor suppressor gene, p53, in 21 patients with astrocytomas at different stages. LOH was determined in tumor and leukocyte DNAs of primary human central nervous system tumors. The incidence rate of brain tumors corresponded to every p53-coding exon for single-strand conformation polymorphisms (SSCP) and the mutations were confirmed by sequencing. p53 mutations were found in 2 of 10 glioblastomas (20%) and in 1 of 8 low-grade astrocytomas (12.5%). Similarly, LOH on chromosome 10 was also found in 2 of 10 glioblastomas (20%) and 1 of 8 low-grade astocytomas (12.5%). Neither of the p53 mutations nor LOH on chromosome 10 was observed together in the tumor types analyzed. Interestingly, the p53 mutations were found in 29% of patients with LOH on chromosome 17. The fact that p53 mutation and LOH on chromosome 17 were found together only in glioblastomas, suggested that these genetic changes may accumulate during astrocytoma progression.

[Evaluation of endometrial biopsy in postmenopausal women before HRT].

UNLABELLED: One of the reasons for not commencing or withdrawal of HRT in women is their fear for breast and endometrial cancers. Does ultrasonographic valuation guarantee sufficiently patient's safety? Most investigators do not recommend further endometrial diagnostics with endometrial thickness less than 4 mm. Endometrial biopsy is advised in cases of irregular uterine bleedings with normal endometrial image or when any endometrial pathology is suspected. In Poland the most frequently performed procedure of endometrial diagnostics is D&C. Our proposition was to offer endometrial biopsies to all patients who were taken HRT for more than 5 years or, despite having strong climacteric ailments, refused to take it because of their cancerophobia. AIM OF STUDY: To evaluate the usefulness of aspiration endometrial biopsy in women after menopause qualified for HRT. MATERIAL AND METHODS: In 84 females, being postmenopausal (amenorrhoeic for at least 12 months) and qualified for HRT, aspiration endometrial biopsies were performed in outpatient clinics. Age of women ranged 46-63 years, mean 57.3. Attained results were compared to ultrasonographic endometrial evaluation. Such factors as patient's age, menopausal age (years from last menstrual period) and previous administration of HRT were taken into our account (maintaining a 3-month wash-out interval). In 74% of women we have got a material being sufficient for histological evaluation. Form clinical point of view the most important for us was to exclude a proliferative or neoplastic process within endometrium. One of endometrial polyps was omitted in USG study, we were surprised also while detecting endometrial carcinoma cells in endometrium below 3.5 mm. In one postoperative slide the cells of endometrial ovarian carcinoma (with infiltrated Fallopian tube) were detected. CONCLUSION: Endometrial biopsy seems to be a useful, effective and cheap method of endometrial diagnostics also in women after menopause.
