Longevity of pimonidazole adducts in spontaneous canine tumors as an estimate of hypoxic cell lifetime.

The longevity of pimonidazole adducts in tumors was quantified as an estimate of the lifetime of hypoxic cells. Pimonidazole was given before irradiation to 12 dogs bearing spontaneous tumors, and tumors were biopsied 24, 48 and 72 h later. Pimonidazole antigen was quantified in the biopsies using ELISA and immunohistochemistry. Pimonidazole antigen was detectable in the initial biopsy in all dogs. In 5 dogs the amount of detectable antigen decreased to less than 50% of the initial amount, in 5 other dogs the amount of detectable antigen decreased to an amount between 50 and 100% of the initial amount, and in 2 dogs the amount of antigen appeared to increase relative to the initial amount. Tumors with high initial adduct concentration were characterized by greater decreases in adduct concentration than tumors with low initial adduct concentration. Immunohistochemically, labeled cells were present in 11 of 12 tumors. The geographic area in tumor biopsies labeled immunohistochemically with pimonidazole adducts (labeled area fraction) tended to decrease over time in 6 dogs, remain stable in 4 dogs and seemingly increase in 1 dog. There was no relationship in individual tumors between the relative change in antigen concentration and the relative change in labeled area fraction. Hypoxic cells which bind pimonidazole may persist for days during fractionated radiation therapy, and the potential exists for them to exert a negative effect on the host.

Issues affecting the longevity of the continuous peritoneal dialysis therapy.

Continuous ambulatory peritoneal dialysis (CAPD) is one of the accepted dialysis modalities worldwide. However, the longevity of the CAPD technique and the patient survival associated with this modality have not been established. We review our 16 year experience in one treatment center in 224 patients who underwent CAPD. Overall, these patients survived on CAPD for a mean of 6.6 years; a 50% survival was reached at a mean of 5.5 years. Ultrafiltration loss was the most prominent cause of withdrawal from CAPD among patients in whom the technique was successful for more than 6 years. Countermeasures to decrease the CAPD-associated morbidity are crucial to prolonging the success of the technique, and thus increasing the duration of patient survival.

Lessons learned from clinical nurse specialist longevity.

This paper provides insight into issues related to longevity and retention of experienced clinical nurse specialists (CNS). To date, this phenomenon has received little attention in both clinical and administrative literature. To prepare for a national presentation on this component of advanced practice, the author questioned 12 professional colleagues who had practised in their role for over 10 years in the same setting. The CNS responses are described and critiqued and suggestions for support of veteran CNS are provided by the author.

The longevity of space maintainers: a retrospective study.

METHODS: This retrospective study investigated the longevity of 301 space maintainers fitted in 141 patients aged 3.4-22.1 years in the Department of Pediatric Dentistry at Leeds Dental Institute between 1991 and 1995. RESULTS: Failure occurred in 190 space maintainers (63%), of which 36% were due to cement loss, 24% breakage, 10% design problems, and 9% were lost. Using the life table method, the median survival time (MST) for space maintainers was found to be 7 months. Band and loop (B&L) appliances had the highest MST of 13 months, while the lower lingual holding arch (LLHA) had the lowest of 4 months. Unilateral space maintainers survived longer than bilateral space maintainers (MST of 13 months vs. 5 months). Left B&Ls had a MST of 16 months, compared to only 4 months for right B&Ls, Gender, age, arch in which the appliance was placed, the operator planning it, fixed vs. removable, and adequacy of pretreatment assessment did not have a significant effect on survival time.

Müllerian inhibiting substance improves longevity of motility and viability of fresh and cryopreserved sperm.

Müllerian inhibiting substance (MIS) is a hormone present in seminal plasma, but its role is unknown. In this study, the effects of MIS on sperm survival in fresh and cryopreserved specimens were investigated. Fresh sperm motility and viability (n = 12) were evaluated in specimens after 0, 0.5, 1, 3, 5, and 22 hours of incubation in the presence or absence of MIS. Motile and nonmotile sperm were evaluated in Cell-Vu counting microscope slides, and viability was assessed by eosin-nigrosin exclusion. Sperm cryopreserved for 2 weeks in TES (N-Tris[hydroxymethyl]methyl-2-aminoethanesulfonic acid)-Tris-glycerol-egg yolk buffer and 4% glycerol with or without MIS were thawed at room temperature (n = 6) and were evaluated for motility and viability using identical methods to those used with fresh sperm. The effects of MIS were examined by coincubation with monoclonal anti-MIS antibody (6E11; n = 6). In fresh and cryopreserved sperm incubated with MIS, both motility and viability were higher than in the absence of MIS (P < 0.03; Wilcoxon signed rank test) at 5 and 22 hours. Coincubation with anti-MIS antibody eliminated the effects of MIS. Longevity of sperm motility and viability are improved both in fresh and cryopreserved sperm in the presence of MIS and may have potential for use in assisted reproductive technology.

[Pacemaker implantation data registry. Supervision of the function of pacemaker and longevity].

To prolong the expected lifetime of pacemakers, more and more pulse generators with reduced output and electrodes with high impedance are used nowadays. Therefore it becomes necessary to measure carefully the physiological parameters as threshold, intracradial potential or the electrode-myocard impedance. When evaluating the threshold it is important to know the output properties of the pacemaker to be used and to take into consideration that the pacemaker itself also will influence the size of the intracardial potential. As no reliable data about the longevity of pacemaker electrodes is available, the total electrode impedance should be measured. Thus at least insulation defects can be detected and impending pacemaker failure be avoided. To observe long term changes in those parameters it is advisable to keep a comprehensive implantation protocol.
