Immediate aesthetic provisionalization for the dental implant patient.

Providing an acceptable provisional restoration for the patient undergoing tooth extraction in conjunction with bone grafting and dental implant treatment can present a challenge for the restorative dentist. The unnatural appearance and function of the typical temporary is often unacceptable to the patient whose treatment requires long-term temporization. This article describes a technique for using heat-cured acrylics to provide immediate aesthetics in a provisional bridge for such patients.

Postoperative analgesia in dental day case surgery. A comparison between Feldene "Melt' (piroxicam) and diclofenac suppositories.

A randomised, double-blind placebo-controlled trial was conducted to compare the analgesic efficacy of piroxicam as Feldene "Melt' with diclofenac suppositories following day case wisdom tooth extraction. Piroxicam was found to be equivalent to diclofenac in speed of onset and efficacy when given 1 h prior to surgery with no significant adverse effects. It is therefore possible to confer the benefits of postoperative analgesia from a non-steroidal anti-inflammatory drug whilst avoiding the inconvenience of the rectal route of administration.

Implant site development and alveolar bone resorption patterns.

Resorption of the alveolar ridges after tooth extraction can mandate site development (preparation) by augmentation, grafting, or both before implants can be placed. The fabrication of successful implant prostheses generally can be accomplished only if the artificial teeth are placed in the same position as was occupied by the natural teeth they are replacing. A knowledge of resorption patterns is necessary for satisfactory implant placement.

[Brephoplasty of an inflamed alveolus with bone chips (histological research)].

Bone tissue regeneration and abatement of inflammatory processes in the well after tooth extraction were examined in experimental dogs treated with iodoform and crushed brefobone preserved in 0.5 percent formalin. Histologic study has confirmed the efficacy of crushed brefobone employment for the inflamed well plasty, for such treatment was conducive to a sooner abatement of the inflammation and regeneration of bone tissue in the well.

[Endocarditis lenta caused by Lactobacillus salivarius subsp. salicinicus (author's transl)].

After two tooth extractions performed without antibiotic cover endocarditis lenta occurred in a ten-year-old girl. The causative organism isolated was Lactobacillus salivarius subsp. salicinicus, the first such reported case. The child has a small, haemodynamically insignificant, ventricular septal defect. A cure was achieved after long-term administration of penicillin G in high doses, at first combined with ampicillin. There were no complications.

Hypoxaemia during outpatient dental anaesthesia.

Eighty children, ASA grade 1, who had outpatient general anaesthesia for tooth extraction were studied. An inhalational induction was performed, with patients receiving halothane, nitrous oxide and either 33% or 50% oxygen. Oxygen saturation was measured throughout the procedure using the Ohmeda Biox 3700 pulse oximeter. No significant difference in the incidence of hypoxaemia was found between the two groups. There was no association between the grade of surgeon or anaesthetist and the incidence of hypoxaemia.

Hyper-responsiveness to DDAVP for patients with type I von Willebrand's disease and normal intra-platelet von Willebrand factor.

Desmopressin (DDAVP) has gained wide acceptance as the drug of first choice in the treatment or prevention of haemorrhages in the majority of patients with von Willebrand's disease (vWd). However, data concerning the clinical effectiveness of DDAVP refer generally to mild vWd, with factor VIII and vW factor levels usually above 20% of normal. In 14 patients with type I vWd characterized by very low plasma levels of factor VIII coagulant activity (VIII:C) and vWf, measured as ristocetin cofactor activity (lower than 20% and 3% of normal respectively), but with a normal intraplatelet content of vWf, a test infusion of DDAVP (0.4 microgram/kg) elicited a very marked increase of VIII:C and vWf and normalized the bleeding time. All these patients subsequently underwent tooth extraction after DDAVP infusion. The incidence of bleeding was remarkably low, with only two minor late bleeding episodes easily stopped by repeating DDAVP infusion. Compared to the cases of type I vWd with unknown intraplatelet vWf content reported in the literature, this subgroup of patients had a more marked, albeit short-lived, increment of VIII:C and vWf.

[Etiology and prevention of hemorrhages after stomato-surgical interventions].

Haemorrhages may occur as complications after tooth extractions or other oral surgical procedures. Therapeutical difficulties will happen especially in patients with haemorrhagic diathesis of unknown aetiology. A survey of rare congenital and acquired forms of disturbance of clot formation is given in tabular form. The local particularities of the course of intra-oral blood clotting are pointed to. On the basis of six years of clinical experience, a drug combination for local application is recommended for the prevention of postoperative bleeding.

[Incidence of radicular cysts in a series of 125 chronic periapical lesions. Histopathologic study].

The incidence of radicular cysts in 125 chronic periapical lesions was studied in 36 cases of periapical surgery (28.8%), and to 89 tooth extractions (71.2%). Histopathology revealed 18 radicular cysts (14.4%) and 107 lesions corresponding to chronic apical periodontitis (C.A.P.) or granulomas (85.6%). Cholesterol clefts, a fibrous capsule and presence of a cavity were more common in the cysts, significant differences being observed with respect to chronic apical periodontitis.

Replacement therapy in platelet-type von Willebrand disease.

The response to infusions of cryoprecipitate and factor VIII concentrate was studied in a patient with platelet-type von Willebrand disease (vWD) who showed lack of the large multimers of von Willebrand factor in plasma, increased platelet aggregation with low concentrations of ristocetin, and in vitro platelet aggregation by normal plasma. The cryoprecipitate and factor VIII concentrate to be infused induced platelet aggregation when added to patient platelet-rich plasma at concentrations higher than 0.86 U/ml and 3 U/ml of factor VIII-related antigen (VIIIR:Ag), respectively. Administration of cryoprecipitate (41.9 U VIIIR:Ag/kg body weight) was followed by a shortening of the bleeding time, and hemostasis was achieved during tooth extractions. Factor VIII concentrate (70.2 U VIIIR:Ag/kg) failed to correct the prolonged bleeding time and proved ineffective to control the gum bleeding. No significant diminution of the platelet count was observed following any infusion. These results indicate that cryoprecipitate is hemostatically effective and safe when infused in such a dosage, but factor VIII concentrate is not effective in platelet-type vWD in analogy to what is observed in various types of vWD.

Histopathologic observation of seven removed endosseous dental implants.

Seven failed endosseous dental implants that were removed from humans were examined histologically. Results suggest that the hollow-basket portion or the vent should be located as far from the gingiva as possible, and that an adequate healing period is needed for implant placement following tooth extraction. In addition, findings also suggest that the hollow-basket implant is not suitable for a healing socket, that periodic inspection is necessary for two-stage implants to evaluate adaptation of the embedded junction between the abutment and implant in the gingiva, and that there is risk of coating separation from hydroxyapatite-coated implants and possibilities of coating resorption.

Changes in bone mineralization pattern: a response to local stimulus in maxilla and mandible of dogs.

A pilot study was carried out in order to verify the pattern of changes in mineralization of bone in the maxillas and mandibles of dogs which had a tooth extraction or luxation. Bone mineral content was determined using computerized microdensitometry. Significant changes in patterns of mineralization were found for alveolar bone, cortical bone and trabecular bone at the sites adjacent to the area of operation. These findings suggest that the three envelopes of jaw bones of the dogs are influenced by Regional activation phenomenon (RAP). These results have important implications for the design of clinical studies of periodontium. A more detailed study should elucidate the cellular mechanisms by which these changes occur.

Cardiovascular and neuroendocrine responses during acute stress induced by different types of dental treatment.

Patients demonstrate a physiological stress response during dental checkups and treatment. Local anesthesia and tooth extraction activate the adrenal cortex to produce cortisol. Changes in adrenaline or noradrenaline concentrations have been reported in plasma and urine after drilling and filling or extraction, and anticipation of a dental checkup increases blood pressure. Both diastolic and systolic blood pressure rise still further during restorative treatment without local anaesthesia and during extraction. In a study by the same authors, no significant changes in blood pressure were observed during restorative treatment with local anaesthesia, which suggests that the pain experienced by the patient contributes to the rise in blood pressure. Dentists must be aware that this increased blood pressure may induce cardiovascular complications during dental therapy.

Fibrin sealant in surgery of patients with a hemorrhagic diathesis.

The utilization of fibrin sealants in patients with hemostatic disorders has hitherto been very limited. Our experiences from 40 major and 8 minor surgical procedures, 10 circumcisions and 118 tooth extractions in 106 patients with hemophilia A, B or von Willebrand's disease, almost exclusively of the severe form, respectively, demonstrate the benefits regarding reduction of blood loss and requirements for systemic replacement therapy. The literature on the use of fibrin sealants in this group of patients, which mainly concerns dental procedures, is reviewed and compared with our data. The importance of the composition of the glue is also discussed in detail.

An evaluation of the McGill pain questionnaire for use in dental pain assessment.

Pain following tooth extraction was assessed in 60 patients using the McGill Pain Questionnaire and the Anxiety Differential, along with other measures of patient expectations and behavior. The McGill Pain Questionnaire demonstrated efficacy in assessing postextraction pain with subscale measures relating to situational anxiety, recovery time and use of analgesics. Significant overlap among the subscales (7--58% common variance) was found suggesting further scale refinement is indicated. Pain descriptive words used and mean subscale scores were similar to those previously reported for toothache pain.

Neutropenia after single-dose clindamycin for dental prophylaxis.

A 68-year-old man with stable chronic myelogenous leukemia received a single dose of clindamycin before having a tooth extracted. He was neutropenic 6 days later, with an absolute neutrophil count of 945 cells/mm3. His neutrophil count returned to normal within 2 weeks. Clindamycin has been implicated in drug-induced neutropenia; however, a review of the literature produced only three reports of this reaction. Only one provided the duration of the neutropenia. To our knowledge, this case report is only the second that provides the duration of the clindamycin-induced neutropenia. Clinicians should be made aware of this potential adverse event.

[Prospective evaluation of the risk of hemorrhage. Measures for the prevention of postoperative bleeding in dental surgical procedures].

The authors concern themselves with the problem of a possible prospective hemorrhage. The history of the patients proved to be the most efficient means to prophylaxis: From 250 patients, seven had histories of hemorrhages, mainly after tooth extractions. In the most prominent case, hematologic examination showed a hemophilia A, which was unknown to the patient. Examination of the family uncovered another case of this disease. Two cases could be interpreted as caused by medication. Preopertive selection uncovered only one patient with abnormal Quick value. Practical instruction for pre-, intra-, and postoperative prophylaxis against bleeding may contribute to reduce unexpected and life endangering hemorrhages to minimum.

[Influence of extensive information on readiness for surgery].

207 patients who already had had one lower wisdom tooth extracted surgically were interviewed about their readiness for accepting the same kind of surgery again. 9% of this cohort refused the repeated (fictitious) surgery. After extensive information about four serious post-/intraoperative complications 10%--only one percent more--now refused surgery. However, there were shifts within the "yes" and "no" groups. The overall outcome of this study whose figures may only be applied to other types of surgery with great reservations and care, demonstrate the trend that comprehensive and detailed information does not have any great influence on the attitude of the patient towards surgery.
