Anencephalic infants and special relationships.

This paper investigates the scope and limits of parents' and physicians' obligations to anencephalic newborns. Special attention is paid to the permissibility of harvesting anencephalic organs for transplant. My starting point is to identify the general justification for treating patients in order to benefit third parties. This analysis reveals that the presence of a close relationship between patients and beneficiaries is often crucial to justifying treating in these cases. In particular, the proper interpretation of the Kantian injunction against treating persons as means only takes on a different light in the context of special relationships. The implications of this analysis for our responsibilities to anencephalic infants is clarified.

The origin of the non-specialized epithelial cells cultured from human parotid gland.

We investigated the origin of the non-specialized cells proliferated in primary explant culture of human parotid gland. The predominant cells in surface layer were morphologically epithelial origin with desmosomes and intercellular digitations. The cultured cells did not show any specific affinity to parotin isolated from bovine parotid gland, which seemed to be one of the marker for duct cell. However, it was demonstrated autoradiographically that DNA synthesis in the cells was rapidly induced by isoproterenol, a potent stimulator of acinar cell. From these results, the non-specialized epithelial cells in explant culture seemed to be derived from acinar cell in human parotid gland.

Globoid cell leukodystrophy: specialized contact of globoid cell with astrocyte in the brain of twitcher mouse.

Subplasmalemmal linear densities (Yajima et al. 1977 a) were the membrane specializations observed in globoid cells in globoid cell leukodystrophy (GLD) and in the cells of the mononuclear phagocytic system (Kawanami et al. 1980). In the spinal cord of the twitcher mouse, an authentic murine model of GLD, somewhat similar membrane specializations were noted in astrocytes, and on some occasions, a spot desmosome-like cellular contact was observed between globoid cells, which were likely to be mesodermal in origin, and astrocytes, which are of ectodermal origin. Possible significance of such apparent cellular contact is discussed briefly.

Attachment to a special object at the age of three years: behavior and temperament characteristics.

This study attempts to estimate the prevalence rate of attachment to a special object in a total population sample of three-year-olds drawn from an ongoing epidemiologic survey. The significance of object attachment at age three for the development of behavioral disturbance is assessed through two separate measures. Data from parental reports of temperament characteristics and observational codings of play sessions in the home setting are also presented. While the authors conclude that attachment to a special object is not related to behavioral disturbance, certain temperamental and play behavior characteristics were found to be significant.

Special classroom placements for EMR children: guidelines for community mental health personnel.

Community mental health personnel have become increasingly involved in public school interventions for mildly retarded children. Previous research, legal findings, and ethical principles regarding the most common intervention, special class placement, are reviewed. Advantages and disadvantages of special placement are presented. Finally, a process model is proposed as offering guidelines to the mental health professional who is involved in establishing treatment programs for EMR children in the public school.

Development and dynamics of regional specialization within the syncytial epidermis of the rat tapeworm, Hymenolepis diminuta.

The epidermis of the tapeworm Hymenolepis diminuta is a highly organized syncytium, composed of an outer layer of continuous cytoplasm, or ectocytoplasm, and an inner layer of nucleated cell bodies, or perikarya. The perikarya are in direct cytoplasmic continuity with the ectocytoplasm via narrow plasmalemma-bound bridges called internuncial process. Although distinct structural and functional differences are apparent between ectocytoplasm and perikarya, all of the perikarya along the body of the cestode are morphologically similar, as are all regions of ectocytoplasm. However, immunocytochemically distinct subpopulations of perikarya and regionally defined areas of ectocytoplasm were identified along the tapeworm strobila by the use of monoclonal antibodies raised against a preparation of isolated tegument. The different types of perikarya and the regionally specialized areas of ectocytoplasm were organized in a topographically precise manner along the body of the parasite. Examination of labeling patterns after colchicine treatment suggests that different types of perikarya are specialized for biosynthesis of specific tegumental molecules and for turnover or recycling of tegumental material. Furthermore, it appears that a 52 kDa polypeptide synthesized by one population of perikarya is transported through the syncytium and ultimately resorbed by a different population of tegumental perikarya. These data suggest that the syncytial epidermis of parasitic platyhelminthes exhibits a more complex organization of function than previously appreciated.

Phosphorylase activity as a histochemical marker of specialized tissue of heart.

Phosphorylase activity has been investigated histochemically under controlled conditions in the specialized tissue and the general myocardium of goat heart using glucose-1-phosphate as the substrate. It has been observed that the phosphorylase content of the nodal and conducting tissue is very high as compared to the general myocardium. The phosphorylase content of the cardiac neural elements is also high. Since the connective tissue which surrounds the nodal and conducting tissue has almost no phosphorylase activity, the nodal and conducting tissue is sharply demarcated by the histochemical reaction for phosphorylase. This reaction can, therefore, be used as a specific and expedient method for the localization of specialized tissue as an exploratory and confirmatory test.

Model film studies in enzyme histochemistry with special reference to glucose-6-phosphate dehydrogenase.

This paper deals with the progress made over the last few years in our understanding of enzyme cytochemical staining methods as studied using a fundamental approach with the aid of a model system of thin gel films. Although model films with a matrix of polyacrylamide have been mostly used, the properties and possible applications of other matrices are also reviewed. The chemical aspects of the entrapment of enzyme molecules into a matrix are summarized. Special attention has been paid in model film studies to the principles of the trapping reaction of a diffusable precursor resulting from the enzymatic conversion of a substrate. They are considered here as they concern the cytochemical demonstration of acid phosphatase activity with a lead salt. The effect of fixatives on different enzyme activities, the diffusion rate of substrates and chromogenic compounds to the enzyme site, and enzyme kinetics under cytochemical conditions are also discussed, since they are factors which influence the final results of the staining procedures. The advantage of model film studies in enabling the direct correlation of cytochemical and biochemical results is outlined with special reference to the cytochemical determination of glucose-6-phosphate dehydrogenase with Tetra Nitro BT. A method for determining enzyme activities in the soluble fraction of isolated cells after incorporation in model films is described for the first time. This method has proved to be highly appropriate for microscopical observations of glucose-6-phosphate dehydrogenase activity in single cells, because it results in a good morphology and no formazan precipitates outside the cells. On the other hand, this type of model film forms a bridge between fundamental model film studies using purified enzyme and quantitative enzyme cytochemistry performed in situ.

Assessment and treatment of Spanish-speaking sex offenders: special considerations.

While there is an expanding body of literature pertaining to the special considerations related to the treatment of Hispanic patients whose primary language is Spanish, there is virtually no information reported to aid in the assessment and treatment of Hispanic and primarily Spanish-speaking sex offenders. Beyond the obvious problems posed by differences in language, there are a host of transcultural factors that may impact on the evaluation and engagement of this patient population when they are also largely psychologically unsophisticated and from a low socioeconomic level. The experiences in modifying and implementing a special program to accommodate the needs of this patient group is presented. Reticence to discuss sexuality in a group setting, the relationship between therapists and patients as effected by rules of social etiquette, and the reluctance to criticize other members in the group were prominent features attributed to religious and cultural factors. The authors consider it a necessity to be sensitive to these subtle nuances in order to provide adequate treatment.

A special type of small granule-containing cell in the abdominal para aortic region of the frog.

Light and electron microscopic studies have been made of a special type of small granule-containing cell (termed Type IV cell) in the frog abdominal para aortic region. These cells contain numerous dense granular vesicles (100--150 nm in diameter) and are considerably smaller (10--20 mu) than neighbouring nerve cells, although they have many features in common with them. They do not resemble chromaffin cells as do Types I, II and III cells. The cell bodies are completely ensheathed by satellite cells and are isolated from neighbouring cells of the same type. Type IV cells have long processes which usually become incorporated in bundles containing 2--20 processes, including some cholinergic nerve fibres, and are loosely enveloped by perineurium. The termination of the processes of Type IV cells do not appear to form efferent synapses on nerve cells at least within the para aortic region or in paravertebral sympathetic ganglia. A close topographical relationship is not found between these processes and blood vessels. It is suggested that the small Type IV granule-containing cells in the frog abdominal para aortic region are not interneurons or neurosecretory cells, but are a special type of sympathetic nerve cell.

Stability of the behavioral specialization of neurons.

The constancy of the association of the activity of the neurons of the limbic cortex and food-procuring behavior has been analyzed in neurons specially selected on the basis of the criterion of length of recording in experiments in rabbits. Comparison of the activity in the first and second halves of the period was carried on the basis of the average frequency for the time of each half of the recording, the average frequency in each of 10 distinctive acts of cyclical behavior, as well as on the basis of the probability of the presence of activation in these acts. It was demonstrated that behavioral specialization, defined as the presence of 100% activation of the cell in specific acts, did not change in the course of the recording, and was a stable information-containing characteristic of the association of the activity of the neuron with the animal's behavior.

Membrane specializations in the first optic neuropil of the housefly, Musca domestica L. I. Junctions between neurons.

Thin section and freeze-fracture replicas of the first optic neuropil (lamina ganglionaris) of the fly Musca were studied to determine the types, extent and location of membrane specializations between neurons. Five junctional types are found, exclusive of chemical synapses. These are gap, tight and septate junctions, close appositions between retinular (R) axons and capitate projections (in which an epithelial glial cell invaginates into an R axon). Junctional types and their cellular associations follow: gap junctions, between lamina (L) interneurons, L1-L2; tight junctions, between L1-L2; L3-L4; L4-epithelial glial cell; and R7-R8. Septate junctions, between L1-L2, L3-L4, L3-beta, L4-beta, alpha-beta, and an unidentified fibre making septate junctions with L1 and L2. Close appositions are found between R axons in the distal portion of the optic cartridges of this neuropil prior to extensive R chemical synapses with L1, L2. These loci (seen in freeze-fracture replicas) have rhomboidal patches of hexagonally arrayed P face particles. Intermembranous clefts between R axons are about 50 A and are invariably electron lucent. These points of near contact between R terminals are probably the sites of low electrical resistance measured by Shaw (1979). Capitate projections are for the first time revealed in freeze fracture surfaces. Here epithelial glia send many, short, mushroom-shaped processes invaginating into R axons forming a tenacious structural bond. All four membrane leaflets (P and E faces of R axon and glial membrane) in the capitate projection possess particles in higher densities than in the surrounding nonspecialized regions. The known, general functions of each membrane specialization were correlated with the functional capacities of those lamina neurons possessing them in an effort to interpret better the integrative capacity of this neuropil. These data provide some fine structural bases for a putative 'blood-brain' barrier between lamina and haemolymph, between lamina and peripheral retina, and possibly between lamina and second optic neuropil.

Return of axonal and glial membrane specializations during remyelination after tellurium-induced demyelination.

We have studied remyelination of rat peripheral nerves after tellurium-induced demyelination using thin section and freeze-fracture techniques. In rats fed a 1% tellurium diet, regions of demyelination were readily identified by myelin debris and the presence of large denuded axons. Remyelination occurred despite continued tellurium ingestion. However, the demyelinated axons underwent a more rapid remyelination if tellurium was removed from the diet. Remyelination proceeded as described for myelination in the normal developing animal. Sites destined to become nodes of Ranvier were identified as patches of intramembranous particles in the axonal E-face. Early terminal loops of the remyelinating Schwann cell were found adjacent to these particle patches. As wrapping proceeded, terminal loops of myelin, along with associated rows of dimeric-particles characteristic of the axonal P-face, were wound into a paranodal location. This winding of the membrane specializations and associated terminal loops resulted in the reformation of morphologically normal paranodes. The size of the nodal E-face particle patch increased in concordance with increases in the number of paranodal loops until an annulus of particles was obtained as seen in the normal node. The thin section and freeze-fracture morphology of remyelinated fibres was indistinguishable from the morphology of control fibres. These observations are discussed with respect to proposed functions of membrane specializations in myelination and nerve conduction.

Tissue culture of epithelial cells from esophageal specialized columnar epithelium (Barrett's esophagus).

A procedure is described for the tissue culture of epithelial cells from Barrett's specialized columnar epithelium. The method employs use of nonenzymatic disaggregation techniques and use of a specialized growth medium, M-19. Successful growth can be achieved in 60-70% of cases. Characteristics of four cultures are presented including PAS, Alcian blue, and cytokeratin staining properties. Ultrastructural studies showed the presence of distended rough endoplasmic reticulum, abundant Golgi apparatus, glycogen granules, and cell-cell junctional complexes. Colony formation in anchorage-independent growth in soft agar was not observed in any culture. The cultured cells were not capable of indefinite growth. Thus, they do not behave as fully transformed cells. Availability of this culture system should be of use to the study of the biology of this metaplastic premalignant lesion.

A study of HMO physicians' receptivity to special programs for sociomedical and behavioral problems.

The purpose of this study was to determine HMO physicians' receptivity to special organized programs dealing with sociomedical and behavioral problems. The study population consisted of full-time physicians in a large prepaid group practice HMO, and the data were obtained in 1977 by means of a self-administered structured questionnaire. Most physicians favored special organized services for alcoholism, drug abuse, obesity, disturbances in sexual relations, and the like. Except for alcohol and drug abuse, favoring organized services for one problem did not correlate highly across problem areas. Specialty, AMA membership, and political orientation were the main characteristics that differentiated physicians on their receptivity to organized programs for sociomedical problems. Social background and professional training and experience may be more important than the practice setting in influencing physicians' receptivity to these types of services.

Increasing simple toy play in profoundly mentally handicapped children: II. Designing special toys.

The purpose of this study was to increase simple toy play in profoundly mentally handicapped children by presenting them with specially designed toys. Twenty children (mean chronological age 14 years, mean mental age less than 1 year) were observed while playing with the special toys, which emitted stimuli (vibration, light, or sound) when appropriately operated. They were also observed with the toys when the stimuli were unavailable. There were large individual differences, but, in general, children interacted significantly more with the experimental toys than with the control toys and engaged in significantly less stereotyped behavior when the experimental toys were available. The implications for clinical and research work are discussed.

An interface to display special physiologic signals on patient monitors.

OBJECTIVE: The objective of this study was to develop an interface to allow special physiologic signals (e.g., in a research setting) to be displayed on the invasive pressure channel of conventional clinical monitors. The interface accepts single-ended high-level signals for display using the pressure channel of patient monitors, which use strain-gauge transducers employing direct current (DC) excitation. METHODS: By studying the electronic circuitry common to most clinical invasive pressure measurement systems (Wheatstone bridge, differential input instrumentation amplifier) it was possible to develop an interface to convert high-level single-ended signals into the low-level differential signal needed for input to an invasive pressure channel. RESULTS AND CONCLUSIONS: The device is useful when it is desired to display signals from special transducers on regular patient monitors. Schematic diagrams and sample results are provided.
