Neonatal herpes simplex meningoencephalitis associated with fetal monitor scalp electrodes.

Two full-term unrelated infants developed herpetic lesions at the site of scalp fetal monitor electrodes several days after uncomplicated labor and vaginal delivery. The mothers had been asymptomatic during pregnancy. In addition to the scalp vesicles, one infant had unilateral herpetic conjunctivitis; the other infant had vesicles on the face without ocular involvement. Both babies had cerebrospinal fluid (CSF) pleocytosis and elevated protein. Herpes virus was cultured from the vesicles and CSF. Computed tomography showed low density areas in the frontotemporal regions. On follow-up, seizures persisted and neurologic development was impaired. These patients illustrate the potential risk of infection when internal fetal monitoring is used in low risk pregnancies.

Reliability and accuracy of localization by scalp ictal EEG.

One hundred forty-four scalp ictal EEGs from 54 patients were analyzed independently by three electroencephalographers for side and lobe of seizure onset. Observers did not know the patients' identities. Accuracy was determined by depth EEG. We found 58 to 60% agreement between observers for lobe, and 64 to 74% for side, of seizure onset; 21 to 38% agreement with depth EEG for lobe, and 46 to 49% agreement for side, of seizure onset; best accuracy for lateralization of seizure onset in temporal lobe seizures, but erroneous in 3 to 17%. More formal criteria are needed before scalp ictal records can be used reliably or accurately for localization.

Scalp hair loss caused by octreotide in a patient with acromegaly: a case report.

We used octreotide to treat a woman with acromegaly and observed pituitary adenoma shrinkage after 5 months. Diffuse scalp hair loss occurred after 5 months, resulting in the discontinuation of treatment. After the cessation of octreotide, the hair loss stopped and hair growth resumed. Since bromocriptine did not effectively decrease the GH level of the patient, we decided to perform transsphenoidal surgery. After resection of the pituitary adenoma, her GH and IGF-1 levels were normalized. Although octreotide-induced scalp hair loss has not been well recognized, we should pay more attention to this side effect.

From the heart to the brain: a study of heartbeat contingent scalp potentials.

Heartbeat perception has become the most widely studied example of visceral perception. In the present study scalp potentials contingent to the visceral event "heartbeat" were investigated. Scalp potentials, averaged time-locked to the EKG-R-wave, were studied at Fz, Cz, and Pz under four conditions: resting (baseline), heartbeat discrimination task, signal detection task, and heartbeat discrimination task after physical exercise. 22 subjects were assigned to the two groups "good" and "poor" perceivers, according to their performance in an initial heartbeat perception test. Event related potentials (ERPs) of "good" perceivers were more stable across conditions than those of "poor" perceivers. Peak latency within the range of 200 to 300 ms differed significantly between conditions. A principal component analysis performed on the ERP averages extracted five components. Subsequent ANOVAs across factor scores yielded significant main effects for the "groups" factor, experimental conditions and electrode sites. The strongest effects occurred over the frontal region in the latency range of 250-400 ms (following the EKG-R-wave). These were found to be not due to artifactual EKG influences.

Scalp paraffinoma underlying squamous cell carcinoma.

We report the case of an 84-year-old man with multiple squamous cell carcinomas located on his bald scalp, arising in association with underlying paraffinoma. Histologically, poorly differentiated, acantholytic squamous cell carcinomas were located above characteristic pseudocystic spaces. Carcinomas have been reported in association with penile and breast paraffinomas, but we are unaware of any reports of squamous cell carcinoma arising over a scalp paraffinoma.

Relationship of temporal lobe seizures to sleep and arousal: a combined scalp-intracranial electrode study.

STUDY OBJECTIVES: The role of arousal from sleep in promoting epileptic seizures is controversial. To examine the question of whether seizures precede or follow arousals from sleep, we defined the timing of temporal lobe seizures in relation to sleep and arousal using combined scalp-intracranial electrodes. DESIGN: Retrospective review of 67 sleep-related mesial temporal lobe seizures in 14 subjects. SETTING: Inpatient epilepsy monitoring laboratory. PATIENTS: Subjects with medically refractory mesial temporal lobe seizures undergoing epilepsy surgery evaluations. INTERVENTIONS: None MEASUREMENTS AND RESULTS: Electroencephalographic (EEG) and/or polygraphic recordings and videotapes were independently reviewed to determine intracranial electrode seizure onset times and time of initial arousal from sleep. In 60 seizures in 13 subjects, intracranial ictal onsets always preceded clinical arousals from sleep. Electrographic signs of arousal in the scalp EEG, defined by the presence of sustained alpha or theta activity, either coincided with or followed, but never preceded, intracranial ictal onsets. In seven seizures in one subject with known seizures upon awakening, intracranial ictal onsets always followed clinical arousals and electrographic signs of arousal from sleep. Seven of the 14 subjects had electrooculogram and chin electromyogram monitoring; in these subjects, no seizures occurred during REM sleep with the majority occurring during NREM stage 2 sleep. CONCLUSIONS: Most sleep-related temporal lobe seizures occurred during NREM sleep and preceded arousals, supporting the premise that processes involved in the initiation and maintenance of NREM sleep play a greater role in facilitating temporal seizures than those involved in promoting REM sleep and arousal. However, arousal from sleep may provoke seizures in exceptional cases.

[Status of scalp hair and therapy of alopecia in men in Switzerland].

A community-based interview and a questionnaire of men visiting the dermatologist for treatment of hair loss were conducted in Switzerland, to characterize the significance of scalp hair and self-perception of hair loss in Swiss men, and to evaluate current treatment of hair loss. 508 men, aged 15-74 years, regardless of the degree of hair loss, were interviewed by telephone, and 308 patient questionnaires were completed by 19 dermatologists. The questions addressed by the interview were: degree of self-rated hair loss, time invested for hair care, use or reasons for rejecting hair growing agents, relevant criteria for scalp hair, self-assessment with respect to different "hair communication types". The questionnaire analysed the causes of hair loss, prior and current treatment modalities, and follow-up at the dermatologist. Respondents rated their hair loss on a 5-point, textual scale that ranged from 'no hair loss' to 'bald areas'. 43% reported hair loss to some extent. For 42% a full head of hair was very important, especially for men under 29 years, who invested more time for hair care and had not lost hair. Of men with hair loss, 26% previously applied hair growing agents. Of men consulting the dermatologist for hair loss, 90% had androgenetic alopecia. 37% were previously treated: prior treatment was in 59% minoxidil, in 4% finasteride (Propecia), in 7% Aminexil, in 7% dietary supplements, and in 6% conducted by the hair dresser. In 79% treatment was switched to Propecia: of these, 73% adhered to the follow-up consultations at the dermatologist.

Sources of the scalp-recorded amplitude-modulation following response.

The scalp-recorded amplitude-modulation following response (AMFR) is gaining recognition as an objective audiometric tool, but little is known about the neural sources that underlie this potential. We hypothesized, based on our human studies and single-unit recordings in animals, that the scalp-recorded AMFR reflects the interaction of multiple sources. We tested this hypothesis using an animal model, the unanesthetized rabbit. We compared AMFRs recorded from the surface of the brain at different locations and before and after the administration of agents likely to enhance or suppress neural generators. We also recorded AMFRs locally at several stations along the auditory neuraxis. We conclude that the surface-recorded AMFR is indeed a composite response from multiple brain generators. Although the response at any modulation frequency can reflect the activity of more than one generator, the AMFRs to low and high modulation frequencies appear to reflect a strong contribution from cortical and subcortical sources, respectively.

Ringworm of the scalp in primary-school children in Alexandria: infection and carriage.

A total of 510 children from a primary school in Alexandria were examined for tinea capitis, and samples were taken from their scalps. Specimens were examined by direct microscopy and were cultured. Diagnosis was by clinical and mycological findings. Clinical evidence of pediculosis capitis was found in 54.1% of children, more commonly in girls. Dermatophytes were isolated from 7.4% of scalp samples (2.9% confirmed cases, 4.5% carriers). A further 2% were suspected cases as they were negative by mycology. Most of the children were under 10 years and none was aware of having the infection. All isolates were identified as Trichophyton violaceum.

Scalp ringworm among Black children in South Africa and the occurrence of Trichophyton yaoundei.

The prevalence of tinea capitis among 153 children who were selected by overt signs of scalp infestation from among 658 Black schoolchildren, was studied in two rural areas of the Transvaal. In both areas (Eastern and Northern Transvaal), Trichophyton violaceum was the predominant dermatophyte species which was isolated. Microsporum audouinii was isolated in both areas from a total of 4 children, but M. canis was the cause of only 1 infection, and that was in the Eastern Transvaal. Five infections caused by T. yaoundei, all of which occurred in the Northern Transvaal, were recorded. They are the first documented cases of endothrix scalp ringworm caused by this species in South Africa.

Red scalp hair turning dark-brown at 50 years of age.

We report on a man whose scalp hair was reddish since childhood but changed into dark-brown after the age of 50. His pubic hair and beard remained carrot coloured. A high level of arsenic in his scalp hair seemed to be a possible cause of the change from pheomelanogenesis to eumelanogenesis.

Segmented heterochromia in black scalp hair associated with iron-deficiency anemia. Canities segmentata sideropaenica.

A newly recognized disorder of black scalp hair is characterized by the irregularly alternating segmentation of hair into dark and light bands. A 15-year-old girl had segmented heterochromic scalp hair in association with iron-deficiency anemia. The clinical and laboratory investigations support the view that low serum iron levels play a critical role in reducing eumelanogenesis and in the possible failure of melanin transfer. The segmented heterochromic hair recovered completely after iron supplementation, which coincided with increased eumelanogenesis in the recovered hair. This clinical experience indicated participation of iron in the kinetics of melanogenesis within the follicular melanocytes.

Ringworm of the scalp in children.

Scalp scrapings were collected from 651 children in the Cape Peninsula and along the Western Cape coast to determine which dermatophytic fungi occur in these areas. Fungi were isolated from 61% of specimens, by far the commonest being Trichophyton violaceum. Only half of these specimens showed fungal elements on direct microscopy. Among patients with scaly scalp lessons of varying severity the isolation rate was 64%, but dermatophytic fungi were also isolated from 16% of 50 asymptomatic children. Carriers of fungi without obvious ringworm may play an important role in the spread of the disease.

Minerals in blood sera and scalp hair in patients with lepromatous leprosy and tuberculoid leprosy.

Scalp hair and blood sera of three groups of individuals were studied for mineral contents by flame photometric and colorimetric procedures. Patients in one of the three groups were suffering from lepromatous leprosy, the second group comprised patients with tuberculoid leprosy and the third group consisted of normal controls. The results obtained revealed changes in the mineral content of the scalp hair of those suffering from lepromatous leprosy and tuberculoid leprosy. A possible relationship may exist between hair and serum minerals.

Haemophilus influenzae septic thrombophlebitis following use of a "scalp-vein" needle.

Indolent Haemophilus influenzae type B septic thrombophlebitis developed in a 14-year-old boy two weeks after completing a course of intravenous antibiotics administered via a "scalp-vein" needle for an unrelated infection. Presumably, the primary disease (common variable immunodeficiency) contributed to the simultaneous occurrence of this uncommon complication of scalp-vein needle use, with an unusual pathogen.

[The diagnostic value of petrous bones in the fetal scalp (author's transl)].

We studied the orientation of the petrous bones in the fetal scalp in 200 X-ray pelvimetries. It is easy to measure the petrous bones' orientation when the fetal scalp is studied with Thoms view. It is possible to measure the height of the head in the pelvis in the lateral view. We have been able to measure asynclitism in flexion using a calculating machine. It seems to us that these findings are useful for the practical conduct of labour and can be of helpful prognostic value.

[Intestinal parasitic diseases and tinea of the scalp in Dakar school population: influence of environmental factors on the infestation level].

We carried out a study during February 1993 in a french and three coranic schools in order to evaluate the incidence of the economical level on the prevalence of intestinal parasites and tinea's scalp. Infection rate by intestinal parasites were 67.4% and 84% respectively in french and coranic schools with significant differences: X2 = 10.8. Infection rate by dermatophytes were 10.6% and 48.8% respectively in francophone and coranic schools with significant differences: X2 = 29.3. Ascaris was predominant through all ages in coranic schools, while amaeba were the main parasites in the upper ages in the french school. Bad environmental features observed in coranic schools as well as in popular areas are risk factors to the infection and contribute to increase the prevalence of intestinal parasitic diseases and tinea's scalp.
