[Success and complication rate of CT-guided marking of pulmonary nodules with coil wires for video-assisted thoracoscopic surgery (VATS)].

PURPOSE: To assess the success and complication rate of the CT-guided marking of pulmonary nodules for video-assisted thoracoscopic surgery (VATS). MATERIALS AND METHODS: Pulmonary nodules (mean diameter 9 +/- 5 mm, mean pleural distance 7 +/- 5 mm) were marked with a coil wire in 30 patients (20 males, mean age 57.6 +/- 15.5 years, 22 patients with a history of malignancy). The intended coil-nodule distance was < or = 10 mm. RESULTS: 81 % of nodules were not visible by thoracoscopy. The technical success rate of CT-guided marking was 86.7 %. The projected nodule-coil distance was achieved in 90 % of cases. The procedure had to be changed from thoracoscopy to thoracotomy in 4 patients due to coil wire marking problems: 2 x coil displacement, 1 x coil-nodule distance > 10 mm, unfavorable direction of wire. Histology was determined in all patients (70 % malignant, 30 % benign). Complications requiring therapy were not observed. CONCLUSIONS: The CT-guided marking of pulmonary nodules is a precondition for VATS if the nodule does not involve the visceral pleura in the majority of cases. The success rate is high with a low complication rate.

Delivering an AT service: a client-focused, social and participatory service delivery model in assistive technology in Ireland.

PURPOSE: This article outlines the development of an Assistive Technology (AT) service delivery model and suggests that a client-focused social and participatory service delivery model in AT can achieve the best results for people with disabilities and their carers. METHOD: It draws on case studies, which are part of the final evaluation report of the Aphrodite (European Horizon funded project 1998-2000) project to illustrate the success of a client-focused service. The aim of the project was to provide people with disabilities access to a local technical resource in the form of Technology Liaison Officers (TLOs). CONCLUSION: TLO's are people with physical disabilities, who have been trained in AT and who have achieved a certificate in AT, to act as a liaison between users and potential users of AT and the Central Remedial Clinic's (CRC) Client Technical Services (CTS) Department.

Funding community-based participatory research: lessons learned.

The California Breast Cancer Research Program (CBCRP) was created to fund innovative breast cancer research specifically addressing the needs of women in California. Beginning in 1997, the Program launched the Community Research Collaboration (CRC) Program, a Community-Based Participatory Research (CBPR) program intended to foster community-researcher collaboration on all aspects of the research process, essentially placing the community in the center of the research paradigm. The CBCRP conducted a process evaluation of the CRC Program to assess success and identify areas for improvement. The evaluation included community-researcher collaborations and the award process. The evaluation identified successes that speak to the effectiveness of the collaboration concept: empowering women to formulate and initiate research; involving underserved and hard-to-reach populations; addressing important and useful research questions; increasing communities' skills and expertise, and enabling lasting collaborations. The greatest weakness identified was the involvement of the broader community (beyond the group/organization involved) in these projects. The evaluation identified strengths in CBCRP's award process, including technical assistance and feedback, the emphasis on collaboration in the review process, and awarding funds directly to community groups. Barriers included: power imbalances due to the community's lack of experience in the dominant research funding culture; funding limits and award delays; and the increased service demand beyond the funding limits of the award that is created. The CBCRP has been able to incorporate many of the evaluation findings to improve the CRC Program.

PASBio: predicate-argument structures for event extraction in molecular biology.

BACKGROUND: The exploitation of information extraction (IE), a technology aiming to provide instances of structured representations from free-form text, has been rapidly growing within the molecular biology (MB) research community to keep track of the latest results reported in literature. IE systems have traditionally used shallow syntactic patterns for matching facts in sentences but such approaches appear inadequate to achieve high accuracy in MB event extraction due to complex sentence structure. A consensus in the IE community is emerging on the necessity for exploiting deeper knowledge structures such as through the relations between a verb and its arguments shown by predicate-argument structure (PAS). PAS is of interest as structures typically correspond to events of interest and their participating entities. For this to be realized within IE a key knowledge component is the definition of PAS frames. PAS frames for non-technical domains such as newswire are already being constructed in several projects such as PropBank, VerbNet, and FrameNet. Knowledge from PAS should enable more accurate applications in several areas where sentence understanding is required like machine translation and text summarization. In this article, we explore the need to adapt PAS for the MB domain and specify PAS frames to support IE, as well as outlining the major issues that require consideration in their construction. RESULTS: We introduce PASBio by extending a model based on PropBank to the MB domain. The hypothesis we explore is that PAS holds the key for understanding relationships describing the roles of genes and gene products in mediating their biological functions. We chose predicates describing gene expression, molecular interactions and signal transduction events with the aim of covering a number of research areas in MB. Analysis was performed on sentences containing a set of verbal predicates from MEDLINE and full text journals. Results confirm the necessity to analyze PAS specifically for MB domain. CONCLUSIONS: At present PASBio contains the analyzed PAS of over 30 verbs, publicly available on the Internet for use in advanced applications. In the future we aim to expand the knowledge base to cover more verbs and the nominal form of each predicate.

[Standardization and quality control in the evaluation of proliferation parameters in T1T2, N0N1, M0 breast cancer: multicentric retrospective study II. DNA-ploidy and S-phase fraction].

As part of a clinical research project, proliferative parameters were studied in primary breast cancer: standardization and technical validation of thymidine kinase (TK), thymidylate synthase (TS) and protein tyrosine kinase (PTK) are described. A total of 633 frozen tumor specimens, available in four institutions, was analyzed in three flow cytometry laboratories for DNA content and percentage of S-phase cells (%S) measurement. 1) The standardization step consisted in developing a common protocol for sample preparation; then, common cell suspensions were analyzed in order to perform an inter-laboratory control. Objective guidelines were elaborated to interpret DNA histograms in breast carcinoma. 2) DNA-aneuploidy was observed in 61% of cases of the retrospective series. Compared with DNA-aneuploid tumors, mean %S was significantly lower in case of DNA-diploidy (respectively: 6.4% and 2.2%, p < 0.001). When compared between the four institutions, %S distributions did not differ significantly. 3) %S is strongly correlated with TK, TS and PTK and high percentages were also observed in high grade tumors or tumor without hormone receptors. These results show that a standardization in using flow cytometers and DNA software allows multicenter studies.

[Experience in the development of information systems at the canton level].

In the aim of need and necessity of stronger orientation and more efficient aim realization 35 Strategy SZO (health information system) country members should have the health information system able to provide information support to their National strategy "Health for All until 2005". Such a health information system should provide the information support for the planning process, monitoring and health evaluation, health care and work in public-health services sector. The experiences in the work of information system organization in Tuzla Canton Public Health Institute should be seen from two aspects: Aspect of the information system significance in the framework of the social-medical, epidemiological, hygiene-ecological and other activities of the Institute; Aspect of the information system development within the Public Health facilities of Canton in relation to the automatic data processing important for registration, tracking and recognition of success and population health care needs. From the aspect of establishing the information system in the area of Tuzla Canton and the Public Health Institute role, there is a need for adequate high technology technical equipment, with the task to enable practical realization of defined projects and in such a way, varied acceleration of work giving as a result usable information. The canton Public Health Institute needs as follows: necessary resources for free information system work; to provide free computer access to all; to ensure connection with other PHI through communications. In this way the canton Public Health Institute information system would be connected with other information systems in the field of health. In would be a base for development of integrated health information system. The information system is desirable aim for the work of Public Health Institute, method of selection and leading content of modernization. Analysing the Bill of Law on Health Care, a defectiveness has been noticed, since the information system is not regulated. Therefore, the need to be issued by Lay is of great importance. A statement that all projects on local information systems, started before war or during the war have been stooped, joins to the aforesaid, and also a series of independent information system in different health segments, which are not connected into one, unique information system. In any case, this disparaged the quality of medical decision making and therefore, providing of high-quality services at all levels of health work.

[Ibn Sīnā or the control of medical reasoning].

This article analyses the context in which Ibn Sīnā produced his works and the terminologic and conceptual material used by him to write his medical and philosophical texts and to become generally acknowledged as a sage or learned scholar. Between the prince and the theologian, he is a physician-philosopher who uses reason to create his own space. The Qānūn fī'l-tibb is the consequent expression of medical reasoning typical of the early XI century; if, on one hand, it is indebted to objective conditions, we remark also that it is structured around an epistemological project which organizes its expression and transmits its knowledge in a technical, codified language.

Speech recognition and the clinical microbiology laboratory.

Keyboard entry of large amounts of diagnostic data is a laborious task. Any transcription errors that occur may pass undetected by 'non-technical staff' employed to input these data. The object of this project was to design a system, based on a Marconi 'Macrospeak' Speech Recogniser, for 'on line' laboratory entry of data generated from the microbiological examination of specimens of urine. Six parameters considered important to the success of the system were assessed: accuracy, speech recognition, reproducibility, speed, user friendliness, and cost effectiveness. The system performed well under the test conditions examined. 'On line' hands free entry of diagnostic data, using speech recognition, may be a practical alternative to more conventional means of data entry.

[Echocardiography--a stethoscope of the 21st century].

The goal of this article is to provide a report of a current potential of an ultrasound examination of heart and to outline future development of echocardiography and its use in clinical practice. Since echocardiography appearance in the middle of the 20th century till today, the method has undergone a very dynamic development. From a simple one-dimensional image (M-mode), through two-dimensional image (2D) complemented with Doppler imaging of blood flows, echocardiography developed into currently up to date methods such as contrast echocardiography, tissue Doppler imaging, assessment of myocardial strain and strain rate, methods based on detection of ultrasonic backscatter (acoustic densitometry, automated border detection, colored image of wall motion--"color kinesis") and others. Possibility of an ultrasonic assessment of heart significantly supplemented transesophageal echocardiography and newly emerging three-dimensional imaging of heart structures. As a routine method to prove myocardial ischemia and viability a stress echocardiography is currently used. Its results significantly improved after technical innovations enabling simultaneous monitoring and evaluation of images at various projection displays and at various levels of stress. A big promise for further bigger use of echocardiography has become a miniaturisation of devices enabling one man to carry the device to a patient outside a health care facility. These small, more affordable ultrasound devices can soon become real stethoscopes of the 21st century.

The learning unit "Orthodontic set-up" as a new-media module in teaching.

The present study examines the extent to which computer-assisted learning units provided independently of place and time are used in self-study as a supplement to the classical classroom instruction of dental students. Indications as to whether such teaching modules improve training in orthodontics should be obtained from this. Attention was focussed on the implementation and evaluation of the "Orthodontic set-up" teaching module, which can be accessed in the Internet and Intranet of the university. The didactic arrangement offered classical university courses in parallel (four lectures on the subjects of occlusion, function, diagnostics, and therapy) in addition to the electronically communicated teaching contents. In addition, intensive supervision during the production of the set-up was guaranteed. The use of this multimedia learning concept was in general assessed positively by 63 surveyed students in the 2002/03 winter semester. The results revealed on the one hand the intensity of use and features of the acquisition of knowledge (use types), and on the other hand, in terms of professional relevance, the contents were found to be well explained, didactically attractive, and understandably presented. However, numerous drawbacks were also mentioned (technical and time problems; qualification deficits). The experience gained in this project should encourage more future investment in the development of alternative university didactic models.

Imaging techniques: state and future.

Technical progress has produced a variety of novel imaging techniques (digital projection radiography, X-ray-transmission computed tomography, sonography, single photon emission computed tomography, positron emission tomography, magnetic resonance imaging), and has enormously extended the diagnostic possibilities in industrialized countries. Further technological developments will be considered under three aspects: --Improvement of current imaging procedures and opening-up of new applications --introduction of new imaging techniques --progressive use of computer technology in imaging diagnostics [digital image generation, image processing, image archiving and communication systems (PACS)] Finally, several consequences emerging from these aspects will be discussed.

In-depth indexing of monograph literature for an on-line retrieval system: a pilot project.

One of the unique features of the State University of New York (SUNY) Biomedical Communication Network is the availability of depth-indexed monograph information for on-line retrieval. Approximately 8,000 titles from the Upstate Medical Center Library collection in Syracuse, New York, were indexed in depth, chapter-by-chapter, as to subject content using the control vocabulary Medical Subject Headings. Detailed indexing of monographic conference proceedings was considered of particular importance to the project. This attempt to make monograph literature more readily accessible to medical library patrons is interesting from both a philosophical and a technical point of view. This paper will discuss both of these aspects, giving an overall description of the nature of this innovative project.

Preemployment physical evaluation.

There is a growing trend toward using preemployment tests to select employees for physically demanding jobs. Women are, in increasing numbers, entering physically demanding occupations that were traditionally dominated by men. Under current Federal employment law, it is illegal to disqualify an employee for a job because of race, color, religion, sex, national origin, and with the recent passage of the American Disabilities Act (ADA), handicap. Because of gender differences in strength, body composition, and VO2max, preemployment tests for physically demanding jobs tend to screen out more females than males. Employers are using preemployment tests not only to enhance worker productivity, but also to minimize the threat of litigation for discriminatory hiring practices and to reduce the risk of musculoskeletal injuries. The primary ergonomic methods used in industry to reduce the risk of back injuries are preemployment testing and job redesign. When a test results in adverse impact, the validity of the test must be established. Validity in this context means that the test represents or predicts the applicant's capacity to perform the job. Criterion-related, content, and construct validation studies are the means used to establish validity. The validity of preemployment hiring practices for physically demanding jobs has been decided in the courts. The most common reason for ruling an employment practice invalid is the failure to show that the test measured important job behaviors. Much of this litigation has involved height and weight requirements for public safety jobs. The courts have generally ruled that using height and weight standards as a criteria for employment is illegal because they were not job related. If fitness tests comprise part or all of the preemployment test, it is essential to demonstrate that the fitness component is related to job performance. Although there are many factors to consider when establishing a cut score, there is a growing trend toward establishing the cut score on the basis of the job's physical demands, defined by VO2max and strength. This literature is limited because most validation studies are not published. They more typically take the form of a technical report to the governmental agency or company that funded the project. There are published preemployment validation studies for outdoor telephone craft jobs involving pole-climbing tasks; firefighters; highway patrol officers; steel workers; underground coal miners; chemical plant workers; electrical transmission lineworkers; and various military jobs.

[Vascular imaging with spiral-CT. The path to CT-angiography].

Spiral CT is a technique that allows for high-quality two-dimensional angiographic projections and 3D imaging of vascular structures. The authors present the technical and methodological principles of the technique, including scan parameters and parameters of contrast application for various clinical imaging tasks. They present their experience with over 150 clinical cases using spiral CT angiography. Suitable applications of this technique include congenital anomalies, aneurysms, dissections, stenoses, thrombi and vascular tumor involvement. Given a problem-adapted examination technique, pathologic changes in vessels of as little as 2 mm can be visualized. In some cases with complex vascular anatomy, spiral CT angiography can be superior to arterial angiography.

A stochastic population projection system based on general age-dependent branching processes.

"Algorithms for a stochastic population process, based on assumptions underlying general age-dependent branching processes in discrete time with time inhomogeneous laws of evolution, are developed through the use of a new representation of basic random functions involving birth cohorts and random sums of random variables. New algorithms provide a capability for computing the mean age structure of the process as well as variances and covariances, measuring variation about means. Four exploratory population projections, testing the implications of the algorithms for the case of time-homogeneous laws of evolution, are presented. Formulas extending mean and variance functions for unit population projections...are also presented. These formulas show that, in population processes with non-random laws of evolution, stochastic fluctuations about the mean function are negligible when initial population size is large. Further extensions of these formulas to the case of randomized laws of evolution suggest that stochastic fluctuations about the mean function can be significant even for large initial populations."
