Arthroscopic treatment of capitellum fracture of the humerus.

This case report presents a type I Hahn-Steinthal capitellum fracture treated by screw fixation under arthroscopic control. It is the first published case of the use of this technique. The 3 types of capitellum fractures and different methods for treatment are described: functional treatment, open surgery, and arthroscopic surgery. The arthroscopic approach allows a precise reduction and fixation of the articular fragment with a better evaluation of associated lesions compared with the open surgery. Arthroscopic fixation minimizes the damage to periarticular soft tissues and has a lower morbidity compared with open surgery.

[Can ultrasonography be used to measure torsion angle of the humerus? Comparison of ultrasonographic, computerized tomographic and anthropometric methods].

Sonographic measurements on 40 cadavaric humeri of the angle of torsion according to Harland had been performed. The results were referred to different computertomographic and anthropometric methods. Sonographic measurements proofed to be accurate and were easily reproducible. Comparing sonographically detected angles with the anthropometric results there was shown a mean difference of 28.9 degrees +/- 7.8 degrees. We conclude that sonography is able to give accurate and reproducible results that do not exactly reflect the angle of torsion.

[Sonographic determination of the humerus retrotorsion angle].

In patients with habitual dislocation of the shoulder and post-fracture malrotation the measurement of humeral torsion is important in preoperative diagnostics. An easy method of ultrasonic measurement of humeral torsion is proposed. The correlation with anthropometric, radiological and CT-measurement is good, what could be shown in 20 macerated humeri. In 111 patients with normal glenohumeral joints the average of humeral retrotorsion was 61 degrees. 86% had a torsion between 40 and 80 degrees.
