["Fast-track surgery": Perioperative management].

The multimodal therapeutic concept of fast-track surgery is directed against the pathophysiologic functional changes following elective surgery. This concept has been proven to reduce postoperative morbidity and convalescence. This benefit is based on an interdisciplinary approach by surgeons, anaesthesiologists, nurses, and physiotherapy staff to optimise perioperative care in order to decrease surgically-induced stress. Fast-track surgery after elective colorectal surgery has been shown to reduce the rate of postoperative complications and shorten hospital stay significantly. A prerequisite for successfully implementing this concept is the willingness of the participating surgeons to abandon conventional traditions. In addition to abdominal procedures, the basic concept of fast-track surgery has been successfully instituted in other surgical fields, such as urology.

Increased plasma levels of soluble CD23 in haemorrhagic fever with renal syndrome; relation to virus-specific IgE.

In 15 consecutive patients hospitalized with nephropathia epidemica, a European form of haemorrhagic fever with renal syndrome (HFRS) caused by Puumala virus, plasma concentrations of soluble CD23 (sCD23) and Puumala virus-specific IgE were determined. In the acute phase of illness, 11/15 patients had increased sCD23 levels (> 91 U/ml), whereas in convalescence, values of 8/10 patients were normalized. Maximal sCD23 values were correlated to maximal concentrations of Puumala virus-specific serum IgE (r = 0.597; P = 0.025). The results are compatible with a known ability of sCD23 to augment IgE production.

Bassini repair compared with laparoscopic repair for primary inguinal hernia: a randomised controlled trial.

OBJECTIVE: To compare the effectiveness of open and laparoscopic primary inguinal hernia repair. DESIGN: Randomised controlled trial. SETTING: University hospital, The Netherlands. SUBJECTS: 87 patients had 103 open repairs and 88 patients had 114 laparoscopic repairs between November 1993 and July 1995. INTERVENTIONS: Laparoscopic repair by the transabdominal preperitoneal (TAPP) technique and open repair by the Bassini technique. MAIN OUTCOME MEASURES: Recurrence, morbidity, pain, and duration of convalescence. RESULTS: Operating time was longer for laparoscopy (mean (SD): 82 (28) compared with 45 (15) minutes p < 0.001). Patients in the Bassini group had higher postoperative pain scores (mean (SD)VAS: 2.9 (1.6) compared with 2.0 (1.6) p=0.002), used more analgesics (median total intake: 2 (0-54) compared with 0 tablets (0-42) p=0.008), and needed a longer convalescence time (mean (SD) time to return to work: 22 (12.6) compared with 14 (10.1) days p < 0.001; mean (SD) time to return to physical activities: 27 (12.6) compared with 17 (12.2) days p < 0.001). Mean follow up was 24 months. Recurrence rates were 21% (22/ 103) after Bassini and 6% (7/114) after laparoscopic repair (p=0.001). CONCLUSION: Laparoscopic hernia repair is a safe operation, which has obvious advantages over the Bassini repair in terms of pain, use of analgesic drugs, resumption of activities, and recurrence. A disadvantage of the laparoscopic repair is the longer operating time.

Cholera-like illness due to Aeromonas sobria.

A Thai woman from Bangkok was admitted to a hospital in Paris for a cholera-like illness. A culture of her "rice-water" diarrhea was negative for Vibrio cholerae and enterotoxigenic Escherichia coli but was positive for Aeromonas sobria. This strain produced enterotoxin, cytolysin, proteolysin, hemolysin, and a cell-rounding factor. Acute -and convalescent-phase sera showed an increase in neutralizing antibodies to enterotoxin, cytolysin, and hemolysin. The enterotoxin, which was labile at 100 C, induced an accumulation of fluid in the rabbit ileal loop model and was not neutralized by antiserum to cholera toxin. Suckling mouse assays and rabbit permeability skin tests were negative, and the Y1 mouse adrenal cell assay produced not true cytotonic effect. This report, the first of an infection due to A. sobria alone, provides evidence that A. sobria is an enteric pathogen of humans that can cause a toxin-mediated, life-threatening illness.

Technical and methodologic considerations for performance of indirect calorimetry in ventilated and nonventilated preterm infants.

OBJECTIVE: To evaluate and refine indirect calorimetry measurement techniques so that accurate metabolic measurements can be performed in mechanically ventilated and convalescing preterm infants who require supplemental oxygen. DESIGN: Laboratory validation of an indirect calorimeter; clinical and laboratory assessments of technical problems in performing metabolic measurements; and clinical indirect calorimetry studies in mechanically ventilated and nonventilated preterm infants. SETTING: Neonatal intensive care unit (ICU) in a tertiary care university hospital. PATIENTS: Level II and level III mechanically ventilated (n = 10) and nonventilated (n = 14) neonatal ICU patients who required FIO2 levels ranging from 0.21 to 0.42. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: System calibration was assessed by combustion of 100% ethanol; the mean respiratory quotient was 0.667 +/- 0.001 (SEM). In addition, oxygen consumption (Vo2) and CO2 production (Vco2) were simulated by CO2/nitrogen infusions within the range expected for 0.5- to 7-kg infants. Mean relative errors were 0.6 +/- 0.3% and 1.8 +/- 0.3% for expected Vo2 and Vco2 values, respectively. In 27 mechanically ventilated patients with no audible endotracheal tube leak, measured endotracheal tube leak ranged from 0.0% to 7.5%. Fluctuations in FIO2 during mechanical ventilation were monitored in 30-min studies, using wall-source (n = 27) or tank-source (n = 11) supplemental oxygen. Mean FIO2 variation was 0.00075 +/- 0.00013 vs. 0.00011 +/- 0.00001 using wall-source and tank-source oxygen, respectively. Some of the difficulties of obtaining accurate measurements in supplemental hood oxygen studies were overcome by using tank-source vs. wall-source oxygen and a unique hood design. CONCLUSIONS: Accurate indirect calorimetry studies can be performed in both ventilated and nonventilated infants weighing as little as 500 g, providing that sufficient attention is paid to technical and methodologic measurement details.

Vitamin A supplementation of South African children with severe diarrhea: optimum timing for improving biochemical and clinical recovery and subsequent vitamin A status.

BACKGROUND: Vitamin A has well-recognized benefits for prevention of diarrhea, but the impact of therapeutic doses given during diarrhea on biochemical and clinical outcomes is less clear. We investigated these potential therapeutic benefits within a study of the optimum time for vitamin A supplementation to promote vitamin A status. METHODS: Young children with severe diarrhea were randomized to receive 60 mg of retinol as retinyl palmitate during acute diarrhea or once symptoms had resolved, usually after 5 to 8 days. Either during acute diarrhea or after its resolution, children not receiving vitamin A were given identical placebo. On Days 0 and 3 we measured urinary neopterin, plasma retinol and acute phase proteins and intestinal permeability by the lactulose/mannitol test. Eight weeks after discharge children returned to hospital for a modified dose response test of vitamin A stores. RESULTS: Most children presented with watery diarrhea and grossly abnormal intestinal permeability and immune activation markers. At 8 weeks plasma retinol concentrations of children receiving vitamin A during acute diarrhea, compared with those receiving it in early convalescence [0.67 (95% confidence interval, 0.58 to 0.76) micromol/l vs. 0.68 (95% confidence interval, 0.59 to 0.79) micromol/l], and the proportion of children with deficient vitamin A stores (7 of 34 vs. 8 of 34) did not differ. Clinical features, lactulose/mannitol tests and urinary neopterin concentrations on Day 3 also did not differ significantly when vitamin A was given early or late. CONCLUSIONS: Even when it was given during severe diarrhea, a large dose of vitamin A improved vitamin A stores in a population in whom vitamin A deficiency is a public health problem. Vitamin A did not significantly improve early clinical or biochemical recovery from severe diarrhea.

A randomized study of ciprofloxacin versus ceftriaxone in the treatment of nursing home-acquired lower respiratory tract infections.

OBJECTIVE: To compare the efficacy and safety of ciprofloxacin and ceftriaxone in patients with nursing home-acquired lower respiratory tract infections requiring initial hospitalization. DESIGN: Prospective, randomized trial. SETTING: Extended care nursing homes affiliated with a teaching hospital. PATIENTS: Fifty patients aged 60 years or older with normal or mildly impaired renal function admitted to the hospital for treatment of lower respiratory tract infections. INTERVENTIONS: Twenty-four patients received initial therapy with intravenous ciprofloxacin, 200 mg every 12 hours (19 patients) or 400 mg every 12 hours (5 patients) during the acute phase followed by 750 mg orally every 12 hours during the convalescence phase. Twenty-six patients received initial therapy with intravenous ceftriaxone, 2 g every 24 hours during the acute phase followed by 1 g administered intramuscularly every 24 hours during the convalescent phase. The total duration of therapy was 14 days. MAIN OUTCOME MEASUREMENTS: Successful outcome was defined as resolution or marked improvement in clinical signs and symptoms of lower respiratory tract infection upon completion of the treatment course. RESULTS: Twelve (50%) of the ciprofloxacin-treated and 14 (54%) of ceftriaxone-treated patients had successful outcomes. Recurrent oropharyngeal aspiration was the reason for treatment failure in most patients refractory to either antibiotic. Mortality during therapy was 8% in each group. From 21 satisfactory sputum specimens collected, S. pneumoniae was the most common isolate, followed by H. influenzae and other Gram-negative bacteria. Ciprofloxacin therapy was well tolerated; ceftriaxone therapy was discontinued in two patients (8%) due to adverse reactions (intramuscular pain and drug fever). CONCLUSIONS: Sequential intravenous/oral ciprofloxacin appears to be as safe and effective as sequential intravenous/intramuscular ceftriaxone. The optimal dosage of intravenous ciprofloxacin in this patient population appears to be 400 mg every 12 hours; however, additional clinical and pharmacokinetic studies with this regimen are warranted.

Detection of Japanese encephalitis virus immunoglobulin M antibodies in serum by antibody capture radioimmunoassay.

An assay for detecting human immunoglobulin M (IgM) antibodies to Japanese encephalitis (JE) virus was developed by using the antibody capture solid-phase radioimmunoassay approach (JE IgM ACRIAAA). Heavy-chain-specific goat antihuman IgM was first bound to the wells of a polyvinyl microtiter plate, and successive steps involved sequential binding of test sample IgM, acetone-extracted mouse brain JE antigen, and (125)I-labeled flavivirus hyperimmune human IgG. Among 20 patients hospitalized in Bangkok with clinical diagnoses of acute encephalitis, and with acute flavivirus infections proven by hemagglutination inhibition (HAI) serology, 16 had detectable (positive/negative [P/N] ratio, greater than 3.0) JE IgM ACRIA antibodies in the acute-phase serum specimen, and 19 had such antibodies in the convalescent-phase serum specimen. Convalescent patient sera regularly had higher P/N values than the corresponding acute-phase sera (mean +/- 1 standard deviation = 13.0 +/- 9.3 with acute-phase sera and 25.8 +/- 19.6 with convalescent-phase sera). JE virus-infected patients with HAI serological responses indicative of a primary flavivirus infection had higher JE IgM ACRI P/N responses than did those patients whose serological response indicated past exposure to other flaviviruses. None of 70 serum specimens from healthy Thai adults and children with serum JE HAI antibodies had detectable JE IgM ACRIA activity (P/N ratios all less than or equal to 3.0). Biological false-positives with low P/N ratios (range, 3 to 15) were found in sera from patients with acute or recent infections with flaviviruses other than JE virus but could be differentiated by the fact that these sera gave higher P/N ratios with homologous antigens than with JE virus. False-positive reactions with low P/N ratios (range, 3 to 6) due to serum rheumatoid factor activity were differentiated by testing with control antigen. The JE IgM ACRIA technique permits a rapid, accurate diagnosis of acute JE virus infections in both patients with and those without previous exposure to other flaviviruses.

Patterns of concerns and demands experienced by spouses following coronary artery bypass surgery.

The purpose of this study was twofold: first, to describe spouses' concerns and demands related to their partner's recovery from coronary artery bypass surgery (CABS) prior to discharge and at 1, 3, and 6 weeks following discharge; and second, to describe how spouses' concerns and demands change from the time of hospitalization through 6 weeks after discharge. The study used a descriptive panel design. Ninety females (82.6%) and 19 (17.4%) males constituted the sample. Respondents' mean age was 60.7 years. Manifest content analysis revealed that spouses' concerns and demands changed from week to week. The study findings support a convalescence trajectory and lend support for paying close attention to the timing of interventions after surgery and during early convalescence.

The role of polyneuropathy in motor convalescence after prolonged mechanical ventilation.

OBJECTIVE: To test the hypothesis that prolonged motor recovery after long-term ventilation may be due to polyneuropathy and can be foreseen at an early stage by electromyography (EMG). DESIGN: Cohort study with an entry period of 18 months. Polyneuropathy was identified by EMG studies in the intensive care unit (ICU). During a 1-year follow-up, amount of time was recorded to reach a rehabilitation end point. SETTING: The general ICU of a community hospital. PATIENTS: Fifty patients younger than 75 years who were receiving mechanical ventilation for more than 7 days. MAIN OUTCOME MEASURES: A rehabilitation end point was defined as return of normal muscle strength and ability to walk 50 m independently. RESULTS: In 29 of 50 patients, an EMG diagnosis of polyneuropathy was made in the ICU. Patients with polyneuropathy had a higher mortality in the ICU (14 vs 4; P = .03), probably related to multiple organ failure (22 vs 11; P = .08) or aminoglycoside treatment of suspected gram-negative sepsis (17 vs 4; P = .05). Rehabilitation was more prolonged in 12 patients with polyneuropathy than in 12 without polyneuropathy (P = .001). Of nine patients with delays beyond 4 weeks, eight had polyneuropathy, five of whom had persistent motor handicap after 1 year. In particular, axonal polyneuropathy with conduction slowing on EMG indicated a poor prognosis. CONCLUSIONS: Polyneuropathy in the critically ill is related to multiple organ failure and gram-negative sepsis, is associated with higher mortality, and causes important rehabilitation problems. EMG recordings in the ICU can identify patients at risk.

The central region of Switzerland did not participate in the development of psychiatry in the 19th century. It was not before 1909 that the providing for insanes was taken in hand. At that time the capuchin Rufin Steimer founded the first catholic infirmary for mental diseases under the name "Franziskusheim" in the canton of Zug. Brethren of charity came to conduct it. Steimer realized there a project which formerly had seemed impossible. But he was no expert, and his hospital could not be perfect. It was merely a place for taking persons in custody. Because this sanatorium was restricted for men, one had no provision for women until 1927. In this year, the congregation of Sisters of the Holy Cross in Menzingen inaugurated a catholic women's infirmary in an aristocratic villa called "Meisenberg". Both hospitals came under the common direction of professor Johann Baptist Manser. Then they were extended, brought up to date and got modern institutions for treatment. His succesors maintained the high standard. The Franziskusheim as the greater of them is now the psychiatric centre of the cantons of Uri, Schwyz, and Zug. A new building has been opened in March 1994. Both homes are private institutions till now and have preserved their religious background. The gradual rehabilitation of convalescents, the employment of persons with physical or mental defects and the care for sufferings from alcoholism or drugs are great duties of the present time. The above-mentioned cantons have therefore founded places of care and a Service of Social Medicine. There exist now houses for transitory or permanent lodging, special factory-places and particular workshops. These installations, self-help-groups and especially the "Foundation Phoenix" for advice and protection are giving useful and meritorious assistance.

Gastric acid secretion and enteric infection in Bangladesh.

In developing countries many enteric infections are caused by acid-sensitive pathogens. Failure of the gastric acid barrier to infection has been reported in cholera but gastric acid secretion has been little studied in other enteric infections. We therefore studied basal and stimulated gastric acid in 185 Bangladeshi men admitted to hospital for the treatment of enteric infection. Patients with dysentery (amoebiasis, n = 24 and shigellosis, n = 19) and culture-negative diarrhoea (n = 69) had similar mean gastric acid levels (basal, 3-5 mmol/h; stimulated, 11-17 mmol/h), which remained stable in those patients studied throughout 12 weeks of convalescence. In contrast, patients with secretory diarrhoea caused by cholera or enterotoxigenic Escherichia coli (ETEC) had low gastric acid levels (P < 0.05 compared with other groups) (cholera, n = 34: basal mean 1.8 mmol/h [SD = 2.2], stimulated mean 7.9 mmol/h [SD = 6.4]; ETEC, n = 39: basal mean 2.7 mmol/h [SD = 2.8], stimulated mean 9.4 mmol/h [SD = 7.5]). Cholera patients' gastric acid level rose during convalescence to similar levels to the dysentery patients'. Low gastric acid level was associated with severe disease in patients with cholera (P < 0.02) or ETEC (P < 0.05). Gastric acid level fell with increasing age (P < 0.007) but this did not account for the differences between groups. Gastric acid levels were not associated with Giardia duodenalis or Strongyloides stercoralis co-infection, fever, use of tobacco, or chewing betel nut. Cholera and secretory diarrhoea caused by ETEC may, therefore, partly result from a reduction in gastric acid level which does not occur during dysentery. Factors which impair gastric acid secretion may predispose to diarrhoeal disease in developing countries.

A standard protocol for blood pressure measurement in the newborn.

OBJECTIVES: Improvements in neonatal care have resulted in increasing survival of extremely premature infants whose hospital course often runs into weeks or months. Some interventions during the acute care of these neonates, such as umbilical catheterization and use of steroids, not infrequently result in elevation of blood pressure (BP). It is, therefore, essential that these infants be monitored accurately for possible hypertension during their convalescence. Unfortunately, normative data on BP in this population are scant and comparison of data from various studies is hampered by methodologic differences in design. Studies in adults address the necessity for a restful state, adopting a comfortable position, and attempts to reduce the startle response to initial cuff inflation. Studies in the newborn using the oscillometric technique have not addressed these concerns. A standard BP measurement protocol was studied to determine the effect of ensuring a restful state, startle response to cuff inflation, and infant position on BP in clinically stable low birth weight infants after the first week of life. STUDY DESIGN: The Dianamap oscillometer was used to measure BP in infants with a birth weight <2500 g between 7 and 42 days postnatal age. Each infant was studied only once when they were clinically stable. BP was measured in two positions, prone and supine, in random order. Infants were studied at least 11/2 hours after their last feeding or medical intervention. An appropriate sized cuff was applied to the right upper arm and the infant was positioned according to randomization. The infant was then left undisturbed for at least 15 minutes or until the infant was sleeping or in a quiet awake state. Three successive BP recordings were taken at 2-minute intervals. The infant's position was then reversed and another 15 minutes of quiet time was allowed. Thereafter, a second set of three successive BP recordings were obtained. The most recent routine nursing BP measurement was also recorded. Data were analyzed using analysis of variance and are presented as means and standard errors of the mean. RESULTS: Sixty-four infants were studied. Birth weights ranged from 901 to 2423 g and gestational ages from 26 to 37 weeks. Overall, mean BP was significantly lower in the prone than supine positions (45.7 +/- 0.7 vs 47.8 +/- 0.8 mm Hg, P < .002). In either position, the first measurement was significantly higher than the third (average difference was 3 mm Hg, P < .003). In general, the relationships among position and order of measurement were similar for systolic and diastolic BP. Mean BPs obtained by routine nurse measurements were significantly higher than those in either position using our standard protocol (54.4 vs 47.0 or 49.1 mm Hg, P < .003). Moreover, the routine nurse measurements varied more widely than did those obtained using the standard protocol. The standard deviation for the routine mean BP measurements by nurses was 11.4 compared with 6.8 and 8.2 for the first measurements in the prone and supine positions, respectively, with the standard protocol. The mean BP measurements made in the supine position (the highest measurements obtained) using the standard protocol were also significantly lower than published values: 57 of 64 measurements were less than the average mean BP for age described by Tan (J Pediatr. 1988; 112:266-270). CONCLUSION: The statistically significant difference between the prone and supine position and among successive measurements in each position are not clinically relevant. The clinically significant differences between measurements obtained with this standard protocol and routine nursing measurements or published data are the result of ensuring a restful state after cuff application. We believe that measurements thus obtained are more representative of true resting BPs in these infants. (ABSTRACT TRUNCATED)

[Phlebitis of the right upper limb and thromboembolic pulmonary hypertension. Apropos of a case of heart-lung transplantation].

The authors report the case of a 25 year old man with unexplained pulmonary hypertension during his first hospital admission. However, a past medical history suggesting right arm vein thrombosis was obtained. Venography showed signs of thoraco-brachial compression. After heart-lung transplantation, pathological studies of the explanted organs showed changes of thrombo-embolic pulmonary hypertension. A spontaneous right arm venous thrombosis occurred during convalescence. The possibility of post-embolic pulmonary hypertension caused by venous thrombosis of the upper limbs is discussed.

Perinatal regionalization versus hospital competition: the Hartford example.

OBJECTIVES: The increasingly competitive health care environment may undermine effective traditional regional organizations. It is urgent to document the benefits of perinatal regionalization for the emerging health care system. We present a case study that illustrates many of the challenges to and benefits of perinatal regionalization in the 1990s. BACKGROUND: The controversy in Hartford was sparked by a proposed merger of two major pediatric services into a full-service children's hospital. Community hospitals reacted with plans to upgrade their obstetrics/neonatal facilities toward level II (intermediate) or II+ (intensive) neonatal intensive care units (NICUs). The fear that unrestricted competition would drive up overall health care costs prompted the hospital association and Chamber of Commerce to retain consultants to evaluate the number and location of regional NICU beds. METHODS: The consultant team interviewed stake-holders in area hospitals, health maintenance organizations, insurance companies, businesses, state agencies, and community groups, and analyzed quantitative data on newborn discharges. RESULTS: The existing system worked remarkably well for clinical care, training, referrals, and provider and patient satisfaction. There was a high level of inter-hospital collaboration and regional leadership in obstetrics and pediatrics, but strong and growing competition between their hospitals. Hospital administrators enumerated the competitive threats that obligated them to compete and the financial disincentives to support the regional structures. Business leaders and insurance executives emphasized the need to control costs. Analysis of discharge data showed marginal adequacy of NICU beds but maldistribution between NICUs, particularly between level III and level II units. The consultants recommended no new beds based on population projections, declining lengths of stay nationally, and substantial gains available from aggressive back-transport of convalescing infants. The consultants emphasized the need for all stakeholders to support the regional infrastructure (referral, transport, education, evaluation, quality assurance) and to modify competition when it impaired effective regionalization. CONCLUSIONS: Regionalization permits better care at lower cost, yet competition may disrupt this effective system. Active cooperation by stakeholders is vital. Substantial new research is required to define optimal regional organization.

[Construction of pharmacy network to support home care].

Today, as medical care specialization and medical fee system coverage are rapidly progressing, hospitals have no choice but to reduce the number of hospital-stay days for patients. As a result, patients are being placed in home for medical care. Home care entails the transfer of care to the home of patients who are highly dependent on medical care. Consequently, it is expected that the number of cases requiring medical treatment also will increase. To support this type of home care, pharmacies will promote the supply of not only oral drugs but also injectables for use in nutrient management and pain control as well, while pharmacists will promote an appropriate drug therapy for patients who are convalescing at home, by visiting patients and providing advice for drug management. However, the reality is far from that because there are fewer than 50 pharmacies in the nationwide scale that can supply injectables. Considering future home care development, it is urgent that a pharmacy network should be constructed and facilities be created for supplying drugs.

[Comparison of open and laparoscopic cholecystectomy in the treatment of acute cholecystitis].

The paper analyses and compares the results of the treatment of acute cholecystitis with open and laparoscopic approach in a seven-year period. From 1994 to 2000, 311 patients with clinical picture of acute cholecystitis were operated. Open cholecystectomy was done in 162 (52.09%) patients, and laparoscopy in 149 (47.91%). In both patient groups the time from the onset of clinical symptoms to the surgery, and preoperative workup and preparation, were the same. In patients with open surgery the procedure lasted 93 (+/-SD) minutes, and in those with laparoscopic surgery 114 (+/-SD) minutes. Those operated with open method received 5.83 (+/-SD) ampules and 3.75 (+/-SD) tablets of analgesics, and those with laparoscopic surgery 3.2 (+/-SD) ampules and 2.1 (+/-SD) tablets per patient. Antibiotics were administered to 149 patients with open surgery for 4.9 (+/-SD) days and to 68 of those with laparoscopic surgery for 2.29 (+/-SD) days. Patients with open surgery stayed in the hospital 9.55 (+/-SD) days and were on sick leave 43 (+/-SD) days, and those with laparoscopy spent 4.35 (+/-SD) days in hospital and were 16 (+/-SD) days on sick leave. In conclusion, better clinical results and faster return to everyday activities point to the significant advantage of laparoscopic cholecystectomy. Analysis of the results shows that total costs of treatment of working patients are significantly lower than of those with laparoscopic surgery, due to shorter hospital stay, shorter sick leave, and faster recovery.

Postoperative convalescence after inguinal hernia surgery: prospective randomized multicenter study of laparoscopic versus shouldice inguinal hernia repair in 1042 patients.

Interest in inguinal hernia surgery has increased significantly with the introduction of new operating techniques during the past decade. This multicenter study compared short-term results in patients treated by the laparoscopic transabdominal preperitoneal patch technique (TAPP; n = 518) and the Shouldice technique (n = 524). We evaluated demographics, operative data, complications, hospital stay, postoperative pain, use of cs, functional status, sick leave, and complaints up to 3 months postoperatively. The median operating time was shorter in the Shouldice group (55 vs. 65 min), but there were no significant differences in complication rates, and major complications were rare. The hospital stay was 1 day or less in over 98% of cases in both groups, but more operations were performed on outpatient basis in the Shouldice group. In the TAPP group postoperative pain and analgesic consumption were less, postoperative functional status was better, and sick leave was shorter (10 vs. 14 days). These results show that the two methods are equally safe and have few major complications. The TAPP operation is associated with less postoperative pain, better postoperative functional status, and shorter sick leave, but at the price of a longer operating time.
