Multicentric ovarian and extraovarian cystadenofibroma.

A case of multicentric ovarian and diffuse pelvic cystadenofibroma is described. Despite multicentricity, the histologic findings are completely benign. Electron microscopy was performed and possible mesothelial origin is discussed.

Tamoxifen responders selection by computerized contact thermography in benign breast disease.

Initial thermic breast values recorded by contact thermography in 29 premenopausal patients with benign breast disease were compared to those recorded 4 and 24 hours after a single 20 mg tamoxifen oral administration. Programmes worked out by the thermographic Prometheus computerized system processed the thermic values. The data obtained were analyzed and compared to the clinical effects resulting from 2 or 3 tamoxifen therapeutic cycles subsequently administered to these patients. The results obtained so far are indicative of the possibility to differentiate by thermographic means the responders from non-responders to subsequent tamoxifen-therapy for breast benign disease.

Differences in the pattern of endogenous lectins from spontaneous rat mammary tumors.

Three spontaneously occurring rat mammary tumors, a fibroadenoma and two tubulopapillary adenocarcinomas, differing in their degree of histologic differentiation and malignancy, were investigated. They revealed significant differences in the pattern of carbohydrate-binding proteins with specificities for alpha- and beta-galactosyl, alpha-mannosyl, and alpha-fucosyl moieties, respectively. Analysis was performed by affinity chromatography on resins with immobilized sugars or glycoproteins and by polyacrylamide gel electrophoresis in the presence of sodium dodecyl sulfate. The patterns were divided into categories according to the dependence of binding activity on the presence of Ca2+ and to dependence on extraction conditions. In comparison with both adenocarcinomas, the fibroadenoma contained alpha- and beta-galactoside-binding proteins at apparent molecular weights of 74,000 and 130,000. These proteins were not found in either adenocarcinoma. Comparison between the two adenocarcinomas revealed qualitative differences for all classes of carbohydrate-binding proteins under investigation. These differences between tumors of the same tumor class (mammary tumors) and tumor type (tubulopapillary adenocarcinoma) allow for the proposal that the pattern of endogenous carbohydrate-binding proteins may be potentially useful as a diagnostic marker.

Ovarian endometrioid carcinoma with an adenofibromatous pattern.

We report the case of a 66-year-old postmenopausal woman who underwent unilateral oophorectomy for an ovarian endometrioid carcinoma with an unusual adenofibromatous stromal reaction pattern. She is presently alive, well and without clinical evidence of tumour two years after surgery. Adenofibromatous or cystadenofibromatous reaction is a peculiar reactive ovarian stromal change most probably induced by the invading neoplastic epithelial cells. No definite conclusions have been reached regarding the prognostic significance of this special stromal tissue reaction in cases of endometrioid carcinoma of the ovary.

Steroid receptors in benign breast disease, gross cystic disease and fibroadenoma.

The benign breast pathology embraces a wide variety of anatomo-clinical-pathological conditions producing confusion in nomenclature. The Authors collected three different types of BBP and investigated the hormonal receptor status for each. The following concentrations of ERc were found: 1-6 fmol/mg in BBD; less than 2 fmol/mg in GCD; 12-18 fmol/mg in the cytoplasm and 29-37.5 fmol/mg in the nucleus in FA. In FA, PgR was found in concentrations of 43.5-50 fmol/mg in the cytoplasm and 0.2-10 fmol/mg in the nucleus. Even if we consider these three histo-pathological entities (BBD, GCD, FA) separately, no correlation can be seen between the presence of receptors and benign breast disease. The only observation we can make is that the fibroadenomas contain more easily identifiable receptor concentrations than BBD and GCD.

Cytology of argyrophilic carcinoma of the breast.

Aspirations of breast lesions from 57 patients were studied by evaluating Grimelius-stained smears in order to determine the cytologic features of argyrophilic carcinoma and the reliability of argyrophilia as a characteristic of malignancy. The cytologic preparations were compared with histologic material. Sixteen benign lesions, 24 carcinomas correctly diagnosed by cytology and 5 carcinomas with technically inadequate smears were argyrophil negative. Five of 12 carcinomas with equivocal cytology were argyrophilic. There was perfect to case-to-case correlation between argyrophilia seen on histologic slides and on smears. The smears of the 5 argyrophilic carcinomas and 2 of the argyrophil-negative carcinomas with equivocal cytology shared features in common not seen in the other 50 smears: elongated cells with eccentric, round-to-oval nuclei and granular or opaque cytoplasm. These features can alert the pathologist to the possibility of malignancy in smears with equivocal cytology. A positive stain for argyrophilia will further increase the index of suspicion.

Irradiation and prolactin effects on rat mammary carcinogenesis: intrasplenic pituitary and estrone capsule implants.

The implantation of silicone capsules that contained estrone and that were adjacent to grafts of anterior pituitary tissue in the spleens of adrenalectomized glucocorticoid-deficient inbred F344 rats resulted in high circulating prolactin (Prl) levels without the untoward effects of chronic hyperestrogenism or of grafts of Prl-secreting pituitary tumors. All peripheral serum estrone titers were below the titers in sera of proestrous untreated intact rats. Peripheral serum estrone and Prl levels were, however, a function of capsule surface area over the capsule sizes tested (12-74 mm2); the elevated Prl levels persisted for as long as 700 days. In adrenalectomized glucocorticoid-deficient female rats, both 5 Gy gamma-irradiation alone and intrasplenic pituitary-estrone implants alone induced mammary carcinomas; the combination of these treatments induced a greater incidence of first carcinomas and reduced first carcinoma latency. There were, however, no marked differences in tumor incidence or latency due to differences in estrone capsule size. Finally, ovariectomy reduced first carcinoma risk in irradiated, pituitary-estrone-implanted rats but did not change the time of maximum risk. Ovarian secretory activity thus persisted in such rats and ovarian hormones synergized with Prl in mammary carcinoma induction.

[5MHz realtime sonography of the breast. 2. Examination technic and diagnostic value].

The diagnostic value of mamma-sonography using realtime technique was evaluated in a prospective study based on the experience of 452 pathomorphologically confirmed sonographic results. Large cysts (greater than 1 cm) were correctly classified as 97%, while small cysts (less than or equal to 1 cm) were recognized in 84%. Eighty-six percent of the fibroadenomas were visualized; 50% of these lesions were classified as "benign" and 34% as "equivocal--probably benign". In 70% of the patients with mastopathic alterations, no abnormalities were detected sonographically, except when cystic lesions (greater than 3 mm) were present. Fifty-percent of the in-situ-carcinomas and small carcinomas (less than or equal to 1 cm) were recognized, while 97% of the larger carcinomas (greater than 1 cm) were demonstrated. The frequency of the different echographic criteria was analyzed with regard to the pathohistological results.

[Fibroadenoma of the paraurethral glands. Immunohistochemical study].

A sub-urethral tumour presenting the architecture of a fibroadenoma and of which the tubular structures were lined with a transitional type epithelium, was described under the term of fibroadenoma of the para-urethral glands. The immunohistochemical study described contributes to bring supplementary arguments in favour of this hypothesis, in showing the presence of numerous argyrophil cells, rich in serotonin, as well as some cells marked by an anti-prostatic acid phosphatase serum. We noted also, with surprise, the presence of some rare cells stained by the anti-calcitonin serum.

Multiple forms of plasminogen activator in human breast tumors.

Total plasminogen activator (PA) activity, tissue-type PA (t-PA) activity, urokinase-like PA activity, and immunoreactive t-PA were measured in benign breast tumors (fibroadenomas), primary breast carcinomas, axillary node metastases, and chest wall recurrences. Total PA activity did not differ significantly in the different types of tumors. However, benign tumors contained predominantly t-PA activity. Urokinase-like PA activity was significantly higher in the malignant tumors compared with the benign group. Both t-PA activity and immunoreactive t-PA were significantly lower in chest wall recurrences compared with primary carcinomas. The ratio of t-PA to urokinase activity was significantly decreased between stages 1 and 3 in the primary tumors. Also, immunoreactive t-PA levels were significantly lower in stages 2 and 3 compared with stage 1. No correlation was found between PA (either total or its different forms) and tumor grade, histological type, or the presence or absence of axillary node metastases.
