Ophthalmic manifestations in Asian patients with systemic lupus erythematosus.

AIM OF STUDY: To determine the spectrum and prevalence of ophthalmic manifestations of systemic lupus erythematosus (SLE) in Asian patients. METHODS: We performed a standardised ophthalmic examination on consecutive Asian patients with SLE referred from a tertiary rheumatology unit to an ophthalmology department. RESULTS: Seventy patients were included in the study. There were 66 females (94%) and 4 males (6%). The mean (range) age of the patients was 32.9 (9-67) years. Five patients (7%) had ophthalmic symptoms while 65 (93%) were asymptomatic. Eighty-three eyes of 45 patients had abnormal Schirmer's #1 test and 27 of these eyes of 17 patients also had concomitant rose bengal staining of the cornea and/or conjunctiva. Seventeen eyes of 9 patients had retinal vascular lesions. Fourteen of these eyes had mild microangiopathic retinopathy with best-corrected visual acuity (BCVA) 6/12 or better and 3 had retinal vaso-occlusive disease with BCVA worse than 6/12. Twenty-eight eyes of 14 patients had cataract and 3 eyes of 2 patients had raised intraocular pressure. Twelve eyes of 7 patients had BCVA worse than 6/12 because of optic neuropathy (4 eyes), posterior subscapsular cataract (4 eyes), retinal vaso-occlusive disease (3 eyes) and phthisis bulbi (1 eye). None had any eyelid lesion, extraocular motility disorder or retrochiasmal disorder of vision. CONCLUSIONS: Asymptomatic dry eye is the most common ocular finding in patients with SLE. Sight-threatening complications of SLE include retinal vaso-occlusive disease and optic neuropathy.

Service to the broader community is an important component of Group Health Cooperative's (GHC's) tradition, values, and mission. The role and potential of community services in a staff model HMO requires consensus, careful planning and communication, and attention to results. This paper describes GHC's efforts to define, implement, and sustain its community services initiative.

Eye camps--providing medical coverage to the under privileged.

The eye camps make a very important contribution to both curative and preventive eye health care. In addition to serving the under privileged they also lessen the burden on the existing hospitals. Planning of such camps has been described vis-a-vis description of three camps done in far-flung areas.

Medicare, managed care, and vision services to the elderly.

Proportionately, fewer Medicare patients are enrolled in HMOs than in fee-for-service programs. Despite the advantages of selecting the HMO option, only 2.9 million beneficiaries are enrolled. Integrating Medicare into health care reform will be a major challenge to this administration. There is general recognition that it will be difficult to control health costs if people at least 65 years of age remain outside the system. With renewed marketing programs directed toward the elderly, we can expect further increases in Medicare HMO enrollment.

Vision adds value.

Both HMOs and vision service providers say vision will continue to gain popularity as an ancillary benefit. And some believe that carve-out eyecare plans that combine exams, corrective lenses, and medical benefits also will grow.

Chemical eye injuries in the workplace. Prevention and management.

1. The majority of ocular burns are related to accidents at work. Acute ocular chemical injuries require emergent recognition and management. 2. Copious irrigation of the eye, done immediately at the scene, is the most important factor in the long term prognosis of ocular chemical burns. 3. After irrigation has been completed and the eyes have been allowed to rest, visual acuity can be tested and referrals can be made to health care facilities and an ophthalmologist. 4. The nurse conducts health hazard assessments of the workplace, provides information about workplace chemicals and their risks, and ensures proper safety protective equipment and emergency supplies. Practicing emergency procedures such as irrigation is important. 5. The nurse monitors and analyzes injury exposure episodes and trends, along with coordinating referrals, treatments, and follow up care for workers with ocular chemical burns.

[Pseudophakia and aptitude of aviation personnel].

UNLABELLED: Because of sensorial disruptions, aphakia post-cataract surgery is a cause of unfitness for the job of aeronautics flying personnel. Its correction thanks to intraocular lenses and a correct functional check-up permit to reconsider the fitness through a derogation given by the competent authorities. EQUIPMENT AND METHODS: The authors realized a retrospective study on the 5 last years. 27 flying personnel, 24 to 76 years old, went through a cataract surgery with implantation. The check-up includes a chemical exam completed by a morphoscopic, coloured and spatial study. RESULTS AND DISCUSSION: The files are more or less well-documented according to their origin. The flying personnel have an average of 4,010 flying hours. The average post-operative hindsight is 30 months. 3 wear intraocular lenses of rear chamber among which 1 is multifocal. 7 were examinated at the CPEMPN with satisfying morphoscopic, coloured and luminous sense compatible with the fitness. 4 are declared permanent unfit (1 professional pilot with bad results, 1 private pilot with other pathologies, 1 inexperienced stewardess getting through the admission visit with insufficient post-operative hindsight, 1 professional pilot declared unfit for its military activity in the reserve). 4 are qualified with restriction. 20 are qualified without restriction. CONCLUSION: The correction of aphakia with intraocular lenses permits in most cases to obtain good functional results compatible with the flying aptitude.

[Neonatal care of premature infants].

Perinatal intensive centers deal with preterm infants of the region. Therefore, the rate of preterm deliveries at the Department of Obstetrics and Gynecology, Medical University of Pécs was high in 1997, about the double of the national rate (18.7%). Among prematures the rate of very low birth weight (VLBW) infants (less than 1500 g) was about 40%, moreover, the rate of extremely low birth weight infants (less than 1000 g) was 39% within the latter group. Related to the new regulation concerning preterm delivery the number of immature infants (weighing less than 750 g) with poor outcome increased. The most characteristic disease of preterm infants is the idiopathic respiratory distress syndrome (IRDS). This could be prevented by maternal steroid treatment, a therapy accepted worldwide, however, not so common in Hungary. Direct fetal steroid therapy may be applied also in selected cases. In the treatment of IRDS surfactant products made a breakthrough. Several factors (maternal tocolytic and fluid therapy, Cesarean section) influence the cardiopulmonary adaptation which may be monitored with impedance cardiography. Late hyponatremia and metabolic acidosis may develop in 1/3 of VLBW infants. Besides the fluid therapy the introduction of enteral feeding must be planned carefully and examination of gastric emptying with ultrasound is useful. The development of osteopenia may be prevented by passive exercise of the preterm infant. Anemia is common and human recombinant erythropoietin is available for treatment. Screening of vision, hearing, and neurodevelopmental follow up are integral parts of neonatal care. As a result of the above mentioned preventive and therapeutic interventions neonatal mortality has decreased significantly in Hungary in the past decades.

A learning-service community partnership model for pediatric student experiences.

Nurse educators are attempting to revise the nursing curriculum to keep pace with the changing focus of health care and prepare graduates for practice in a community-based, community-focused health care system (1,2). An important aspect of the change in emphasis in nursing education is the need to address the primary and chronic health care needs of individuals, families, and aggregate populations.

The National Eye Institute Visual Function Questionnaire: experience of the ONTT. Optic Neuritis Treatment Trial.

PURPOSE: To describe the health-related quality of life, measured with the National Eye Institute Visual Function Questionnaire (NEI-VFQ), of patients several years after the onset of optic neuritis, according to their neurologic and visual status; to assess the relationship between the NEI-VFQ subscales and clinical measures of visual function; and to assess the internal consistency reliability of the NEI-VFQ subscales. METHODS: The NEI-VFQ was administered to 244 patients 5 to 8 years after treatment for an episode of acute optic neuritis as part of the Optic Neuritis Treatment Trial. Visual acuity, visual field, contrast sensitivity, and color vision were measured at the same time as questionnaire completion. RESULTS: The NEI-VFQ scores generally were lower than those reported for a disease-free group. Reported dysfunction was greater when multiple sclerosis was present and when visual acuity was abnormal, supporting the construct validity of the NEI-VFQ. Rank correlations between the NEI-VFQ subscales and clinical measures of visual function were moderate at best. Internal consistency reliability was generally high for most of the NEI-VFQ subscales. CONCLUSIONS: These findings add support to the use of the NEI-VFQ as a valuable measure of self-reported visual impairment.
