The accuracy of enzyme-linked immunosorbent assay and latex agglutination progesterone test for the validation of estrus and early pregnancy diagnosis in dairy cattle.

The accuracy of the enzyme-linked immunosorbent assay (ELISA) and the latex agglutination (LA) on-farm progesterone kit for detecting estrus and diagnosing early pregnancy was investigated in this study. Italian Friesian dairy cows (n=82) from 6 dairy herds were used for the collection of foremilk samples at the time of breeding and at 19, 21, and 23 days post insemination. Pregnancy status was ascertained by uterine palpation per rectum 40 to 60 days post insemination. Progesterone levels were affected by herd, percentage of milk fat, and the day of testing x diagnosis interaction. Validation of estrus by qualitative on-farm tests was 74.6% (LA) and 100.0% (ELISA) accurate using 0.5 ng/ml of progesterone as the RIA estimate for estrus. The accuracy rate for early pregnancy diagnosis by RIA was 68.4 to 83.8% for day 19 and day 21, respectively, while the detection rate for nonpregnancy was 84.6 to 100% on day 19 and day 21, respectively, as compared with uterine palpation per rectum. The average accuracy rate for early pregnancy diagnosis ranged from 84.7 to 92.3% for the LA and ELISA tests, respectively; the nonpregnancy rate was correctly predicted 93.9% to 68.2% for the LA and ELISA tests, respectively.

Diagnosis of pregnancy in moose using a bovine assay for pregnancy-specific protein B.

Blood samples were collected from 26 moose (Alces alces ) and evaluated for the presence of an antigen that cross-reacted with antisera to bovine pregnancy-specific protein B (P-SPB). The objective of this study was to determine if the P-SPB radioimmunoassay (RIA) was a reliable indicator of pregnancy in these animals. In the first year of the study calf production the following summer was used as the index of previous pregnancy. In the second year all females were subjected to palpation per rectum after chemical immobilization. Seven of the 10 cows sampled in the first year were also sampled in the second year. All animals determined pregnant by rectal palpation were positive for P-SPB; however, P-SPB was not detected in males.

Follicular dynamics prior to and during superovulation in heifers.

The present ultrasonographic study examined the relationship between certain follicular parameters and the superovulatory response in gonadotropin-stimulated heifers. Thirty heifers received a total of 35 mg FSH twice daily for 4 d and 0.75 mg cloprostenol were given to induce luteolysis and estrus at 72 h after the initial FSH injection. Transrectal ultrasonography was performed once daily from 1 or 2 d before the initial FSH injection and until the day of estrus. The number of small (2 to 4 mm), medium (5 to 9 mm), and large (>/=10 mm) size follicles as well as the diameter of the large follicles were recorded. Embryos were recovered non-surgically 6 or 7 d after estrus, and the number of corpora lutea was determined by palpation per rectum. Heifers with >2 or </=2 corpora lutea were classified as responders or nonresponders, respectively. All follicular categories were affected by treatment with FSH in the responders (P<0.0001), while in the nonresponders only small follicles and the total number of follicles showed a change after treatment (P<0.05). All follicular categories were different between the 2 groups (P<0.005). There was no effect of corpus luteum location (ipsilateral vs contralateral) on any of the follicular categories (P>0.05). The number of large follicles and the sum of medium and large follicles were positively correlated (r=0.43 and r=0.54, respectively; P<0.05) with the number of corpora lutea palpated on the day of embryo recovery (6 to 7 d after estrus). In conclusion, there was an effect of the day relative to initiation of FSH treatment on all follicular categories in heifers responding positively to superovulation, and there was no effect of side (left or right ovary) or of corpus luteum diameter (ipsilateral or contralateral).

Relationships between seminal vesicle size and copulatory activity in bulls.

Two experiments were designed to investigate the hypothesis that dimensions of the seminal vesicle are positively correlated with copulatory behavior in bulls. In Experiment 1, thirty Santa Gertrudis bulls (16.5 mo old) were used to study the relationship between copulatory activity and seminal vesicle length and width (palpation per rectum), seminal vesicle thickness (ultrasonography), and volume. The bulls showed a low level of copulatory activity (6.0+/-1.5, 0.8+/-0.3 and 0.3+/-0.1 for mean number of mounts, intromissions and ejaculations, respectively) in 2 tests. The mean number of mounts but not the mean number of intromissions or ejaculations differed (P<0.01) among bulls. No significant correlations were found between seminal vesicle size and any of the variables of sexual behavior. In Experiment 2, 16 beef bulls (18 mo old) were used to study the relationship between copulatory activity and seminal vesicle length (palpation) or dorso-ventral thickness (ultrasonography). Differences were detected (P<0.01) among bulls in mean number of mounts, intromissions and ejaculations achieved in 2 serving capacity tests. No significant correlations were found between copulatory activity and seminal vesicle length or thickness. These data do not support the hypothesis of a positive relationship between seminal vesicle length or thickness with copulatory activity, even for bulls with marked diffences in intensity of sexual behavior.

Plasma concentrations of progesterone, 17beta-estradiol and androstenedione and superovulatory response of Nelore cows (Bos indicus) treated with FSH.

Progesterone (P(4)), 17beta- estradiol (E(2)) and androstenedione (A(4)) plasma concentrations were correlated with palpated corpora lutea (CL), recovered embryos and viable embryos in 13 Nelore cows induced to superovulate with FSH, starting on Day 10 of the estrous cycle. Administration of FSH increased the number of ovulations and recovered embryos. Plasma P(4), E(2) and A(4) levels on Day 0 and of P(4) on Days 10 and 11 of the cycle were not correlated with the superovulatory response. Determination of CL by palpation per rectum was used to estimate the number of recovered embryos. Plasma P(4) levels higher than 1 ng/ml on the induced estrus day (Day 14) had an adverse effect on the embryo viability rate. Plasma E(2) concentrations on Day 14 were positively correlated with the number of viable embryos collected, a correlation that has not been previously reported. The present data indicate that plasma P(4) and E(2) concentrations in FSH-PGF2alpha-treated Nelore cows are useful for the identification of 2 different populations of Nelore donors and are correlated with superovulatory response and, particularly, with the number of viable embryos.

Influence of management factors on pregnancy attrition in dairy cattle.

The present study was conducted to determine the influence of management factors on pregnancy attrition in dairy cattle. Data from 3162 diagnosed pregnancies in parous cows and 1050 in heifers at 9 commercial dairy herds in northeastern Spain were used. Pregnancy diagnosis by palpation per rectum was performed from 30 to 70 d post insemination. Pregnancy attrition was registered when pregnancy diagnosis resulted negative in a second palpation carried out between 120 and 150 d following insemination. Overall proportion of pregnancy losses was 7.9% (9.6% in parous cows and 2.8% in heifers). Data analysis was performed by multiple logistic regression methods. For all animals, effect of time of pregnancy diagnosis was shown (Odds ratio = 0.97 for 1 d increase; P = 0.0042). Conceptus loss in heifers was lower than in parous cows (Odds ratio = 0.28; P = 0.0001), and a higher proportion of pregnancy attritions was detected in animals inseminated in spring, summer and winter, compared to those inseminated in autumn (P < 0.04). Herd effect on pregnancy attrition was also significant. Similar results were observed in the subanalysis for parous cows and, furthermore, no effect of lactation number and of interval from previous calving to pregnancy was shown in this group. In heifers, no effect of time of pregnancy diagnosis and of insemination season on pregnancy attrition was shown, and only a herd effect was observed. Our data suggest that the influence of parity status (heifer vs cow) could affect the proportion of pregnancy attrition rather than early diagnosis, and, in pregnant cows, adaptation to seasonal changes associated with temperature decreases seem more efficient.

Effect of reproductive disorders previous to conception on pregnancy attrition in dairy cows.

This study was undertaken to determine whether reproductive disorders previous to conception influence pregnancy attrition in dairy cows. Reproductive disorders were registered and pregnancy diagnoses were performed as a part of a reproductive health program at 9 commercial dairy herds in northeastern Spain. Data from 3022 diagnosed pregnant lactating cows were used. Pregnancy diagnosis by palpation per rectum was performed from 33 to 70 d post insemination. Pregnancy attrition was registered when the pregnancy diagnosis was negative at the second palpation carried out between 120 and 150 d following insemination. Data analysis was performed by multiple logistic regression methods. Pregnancy attrition rates were 2.6 and 1.8 times higher in cows with previous pyometra and retained placenta, respectively, than in cows without these disorders. No effect of endometritis, ovarian cysts and repeat breeding was shown on pregnancy attrition. Our results suggest that additional efforts to reduce the risk of retained placenta and pyometra should decrease the incidence of pregnancy attrition in dairy cows.

Tumor necrosis factor-alpha as a biochemical marker of pain and outcome in temporomandibular joints with internal derangements.

OBJECTIVE: Previous studies have demonstrated the presence of tumor necrosis factor-alpha (TNF) in human temporomandibular joint (TMJ) synovial fluid. The present study continues the investigation of the role of TNF in TMJs with internal derangements. MATERIALS AND METHODS: Synovial fluid was obtained from 18 TMJs in 12 patients undergoing either arthroscopy (14 joints) or arthrotomy (four joints) for internal derangements. Standardized clinical data were collected preoperatively, intraoperatively, and postoperatively. RESULTS: When pain on palpation was absent, the mean preoperative TNF level was 14 +/- 6 ng/mL. When pain on palpation was present, the mean TNF level was 42 +/- 39 ng/mL (significant difference at P = .05). When the surgical outcome was poor, the mean preoperative TNF level was 26 +/- 9 ng/mL. When the outcome was within the stated guidelines for a favorable result, the mean TNF level was 12 +/- 7 ng/mL (significant difference at P = .05). In addition, a significant reduction (P = .05) in TNF following joint lavage (preoperative, 48 ng/mL to postoperative, 7 ng/mL) was found. CONCLUSIONS: The finding of a positive correlation between preoperative pain and TNF values suggests a biochemical basis for the origin of the pain associated with internal derangements. The relationship between preoperative TNF levels and surgical outcome suggests that the prognosis for surgery may be predicted by measuring biochemical markers of joint disease.

Pericranial muscle tenderness and pressure-pain threshold in the temporal region during common migraine.

Twenty-six patients were examined during attacks of common migraine as well as during headache-free interval. Pericranial tenderness was scored blindly by a systematic manual palpation on both occasions by the same observer. Pressure-pain threshold (PPT) in a fixed location over the temporal muscle was determined by the use of a pressure algometer. A 28% increase in total tenderness score was observed during attacks (P less than 0.01). During unilateral attacks, tenderness scores were significantly higher on the ipsilateral side as compared to the contralateral (P less than 0.01). A positive correlation was observed between tenderness on the two sides (P less than 0.05) and the two occasions (P less than 0.01). PPT showed no changes during migraine attacks and there was no difference in PPT between the ipsilateral and contralateral side. A positive correlation was observed between PPT on the two sides and the two occasions (P less than 0.01). PPT was not correlated to the tenderness scores obtained by manual palpation. The absence of a decrease in PPT and the presence of several tender areas in multiple regions, particularly where pain was spontaneously reported to be located, suggest the presence of either a multi-focal peripheral pathological process or referred pain from other structures in the head and neck region.

Diagnosis of displaced ulnar collateral ligament of the metacarpophalangeal joint of the thumb.

In a prospective study of 24 consecutive patients with posttraumatic instability of the metacarpophalangeal joint of the thumb, clinical examination including instability tests and palpation of displaced ulnar collateral ligaments was used to separate the patients into two groups--nondisplaced and displaced ruptures. Palpable displaced ruptures were treated surgically, whereas nonpalpable ruptures were interpreted as nondisplaced and were treated with plaster, irrespective of the instability. At follow-up 1 year later both groups showed similar results with respect to stability, strength, and function. Our results indicate that a clinical examination with palpation of the torn ligament ends identifies displaced ruptures of the ulnar collateral ligament, i.e., those cases needing surgery. Nondisplaced ruptures might be treated nonoperatively.

The practise of breast self-examination over a six-year period following teaching.

The null hypothesis is that there is no difference in the performance of breast self-examination (BSE) with respect to three teaching methodologies. A total of 614 women were randomly assigned to a group receiving content only, one receiving content plus supervised practise on a model with implanted lumps or one receiving content plus instruction on their own breasts. It has been established that all groups increased significantly their frequency of practise and level of confidence (Alcoe SY, Gilbey VJ. McDermot RSR, Wallace DG. The effects of teaching breast self-examination: reported confidence and frequency of practise over a six-year period. Patient Educ Couns 1994; 23: 13-21). This paper considers the steps undertaken during BSE and the changes likely to be noticed through factors labelled Technique, Observation, Inspection and Palpation. The factors have been created from 35 items reflecting multiple aspects of the procedure as reported at four follow-ups. The group wherein teaching involved practise on their own breasts achieved higher scores than the other groups. Scores for Technique and Inspection are better maintained over time than are those for Observation and Palpation.

Characterization of secretion of thymosin alpha 1 and thymosin beta 4 during prepuberty, estrus and pregnancy in the bovine female.

The objective of the present study was to characterize secretion of thymosin alpha 1 (A1) and thymosin beta 4 (B4) during different stages of ovarian function and pregnancy in bovine females. One hundred and thirty-five prepubertal heifers averaging 9 mo of age at the time the study was initiated were used. Estrous detection was conducted twice daily using intact bulls fitted with marking harnesses. Blood samples were collected at first behavioral estrus and at the time of rectal palpation for pregnancy. Heifers were weighed at the beginning of the study and every 56 days and additional blood samples were collected at those times. Plasma was harvested and radioimmunoassays were conducted to quantify concentrations of thymosin A1 and B4. Mean concentrations of thymosin A1 and thymosin B4 were determined during prepuberty, estrus during which conception did not occur, estrus during which conception did occur, pregnancy and in heifers that failed to attain puberty. Mean plasma concentrations of thymosin A1 and B4 were greater at estrus during which conception occurred than at any other period (P less than .01). Regressions of days prepubertal and days post conception on thymosin A1 and B4 were analyzed. During the 100 days prior to puberty, thymosin A1 and B4 did not change in the heifers that ultimately conceived. However, in the heifers that were never determined to be pregnant by rectal palpation, concentrations of both thymosin A1 and B4 increased linearly during prepuberty (P less than .01). A decline in thymosin A1 and B4 concentrations occurred in a quadratic fashion from conception through the first 100 d of pregnancy (P less than .01).(ABSTRACT TRUNCATED AT 250 WORDS)

Piriformis muscle syndrome: an underdiagnosed cause of sciatica.

This is a retrospective review of 26 patients with sciatica due to the piriformis muscle syndrome. Most patients had pain in the buttock area and sciatica, and most experienced difficulty walking and sitting, even for short periods of time. Reproduction of the sciatica upon deep palpation, either by gluteal or rectal route, was diagnostic. Reproduction of sciatica occurred in 92% of the patients upon deep digital palpation and in 100% of the patients upon rectal or pelvic examination. Other signs were helpful but not consistent. After the appropriate diagnosis, the treatment was relatively easy and rewarding. This study emphasizes that the diagnosis of piriformis muscle syndrome is clinical; without the appropriate clinical examination, it can be easily misdiagnosed.

The postpartum buffalo. II. Acyclicity and anestrus.

Prolonged postpartum acyclicity (absence of ovarian cyclic activity) and anestrum (absence of overt estrous signs) are major sources of economic loss to buffalo breeders. Studies on the epidemiology of these two problems are highly recommended to achieve successful control. Review of the available literature on controlled studies in dairy buffaloes revealed that first ovulation as detected by rectal palpation and progesterone analysis occurred between 28-71 and 24-55 days, respectively, after calving. Postpartum estrus in the same studies occurred between 44 and 87 days. Reports concerned with data compiled from breeding records of research stations, breeding farms and small holders where estrus is a subjective measure, gave much longer periods. Also data from Egypt, India and Pakistan indicate that only 34-49% of buffaloes showed estrus during the first 90 days after calving and 31-42% remained anestrus for more than 150 days. In swamp buffaloes both postpartum ovulation and estrus are more delayed than in dairy buffaloes. The role of suckling, nutrition, body condition score at calving, milk yield, parity, season of calving and other minor factors were discussed. First postpartum ovulation is frequently followed by one or more short estrous cycles (<18 days). Long anovulatory and anestrous periods due to prolonged inter-luteal phase were reported to occur after short cycles. Also long anestrous periods due to cessation of cyclic activity (true anestrus) for 3 or more weeks and prolonged luteal activity for 28 days or more were described to occur in about 25 and 8-11% of the buffaloes, respectively, after the first or second ovulation. These cycle irregularities certainly impose difficulties on estrus detection programs in postpartum buffaloes. Four main forms of anestrus i.e. true anestrus (inactive ovaries and small and medium sized anovulatory follicles), subestrus, prolonged luteal activity and ovarian cysts in addition to pregnancy are reviewed in this article. Differentiation between true anestrus and subestrus is particularly important in buffaloes because of their weak estrous signs. However, the accuracy of a single rectal palpation of the ovaries is limited with an overestimation of the frequency of true anestrus due to misdiagnosis of the corpus luteum. Early detection and treatment of vesicular adenitis in bulls.

The objective of this study was to determine whether early detection and treatment with tilmicosin would reduce the number of yearling bulls culled due to vesicular gland disease at the time of their first semen test. Bulls (n=2207) of 15 breeds at 17 performance test stations were examined by transrectal palpation at 9-12 months of age and assigned to three treatment groups: (1) positive treated group, receiving subcutaneous injections of tilmicosin every second day for three treatments (2) positive untreated control group, (3) negative untreated control group. Transrectal palpation of the glands was done again at a pre-sale semen test 28-70 d (mean=42.8 d) after the first examination. Semen was evaluated for the presence of pus and/or leukocytes by light microscopy. The proportion of bulls with vesiculitis was 4.4% (97/2207). At the pre-sale semen test the number of bulls with vesiculitis had decreased to 1.3% (29/2207); however, seven of these were new cases that developed after the first examination. Therefore, there was a recovery rate of 75/97 (77.3%) in the original group of bulls positive for vesiculitis. At the pre-sale test, there was no difference in the proportion of bulls with enlarged glands in the positive treated group (15/66) versus the positive untreated group (7/31) and no difference in the proportion of bulls with leukocytes or pus in their semen samples. The results of this experiment did not indicate any advantage in detection and treatment of vesiculitis before a year on age due to a high rate of spontaneous recovery.

[Venous malformations: clinical characteristics and differential diagnosis].

Venous malformations (VM) are localized defects of blood vessels that are due to vascular dysmorphogenesis. These slow-flow lesions can affect any tissue or organ. Clinically, a cutaneous VM is characterized by a bluish mass that is compressible on palpation. Phleboliths are commonly present. Symptoms depend on location and size. VM are often sporadic and isolated, however, they can be associated with other malformations and be part of a syndrome; Klippel-Trenaunay (capillary-lymphatico-venous malformation with limb hypertrophy) is the most common. Glomuvenous malformation (GVM) is another type of venous anomaly. In contrast to VM, GVM is often painful on palpation and not compressible. Clinical diagnosis of VM is often made in the presence of a bluish cutaneous lesion: however, other lesions can mimick VM. The most frequent anomalies are a blue naevus, a hemorrhagic lymphatic malformation, a sub-cutaneous hemangioma or even the presence of dilated superficial normal veins due to underlying venous stenoses. This chapter will detail the clinical characteristics of venous anomalies and their differential diagnosis.

Inter- and intrarater reliability of a back range of motion instrument.

OBJECTIVE: To examine the interrater and intrarater reliability of a back range of motion (BROM) instrument when measuring lumbar spine active planar motions and pelvic inclination. DESIGN: Single-group repeated measures for inter- and intrarater reliability. SETTING: Academic institution. PARTICIPANTS: Ninety-one participants (61 women, 30 men; mean age, 28 y) without a current complaint of low back pain volunteered. INTERVENTION: Two examiners measured pelvic inclination and all lumbar motions by using the BROM device. Subjects alternated between examiners for 4 complete trials; examiners remained blinded to the measurements. MAIN OUTCOME MEASURES: Intraclass correlation coefficients (ICCs) were used to determine intrarater and interrater reliability. Regression analysis was performed to determine the role palpation played in sagittal plane measurement error. RESULTS: Intrarater reliability for side bending was good (ICC range, .85-.83), lumbar forward flexion and pelvic inclination was good to fair (ICC range, .84-.79), and extension and rotation was fair to poor (ICC range, .76-.58). Interrater reliability was fair to poor for all lumbar motions and for pelvic inclination (ICC range, .79-.55). Less than 2% of the variation in sagittal plane measurements was explained by consistency of palpation for device placement. CONCLUSIONS: The BROM provides a reliable means of measuring lumbar forward flexion, side bending, and pelvic inclination when performed by the same examiner in asymptomatic subjects.

Impact of ramipril in patients with evidence of clinical or subclinical peripheral arterial disease.

AIMS: The aim of this study was to assess the prognostic importance of peripheral arterial disease (PAD) as evaluated by ankle blood pressure index (ABI), and the impact of ramipril on the prevention of major cardiovascular events in PAD patients included in the Heart Outcomes Prevention Evaluation (HOPE) study. METHODS AND RESULTS: Patients were randomized to treatment with ramipril or placebo and followed for 4.5 years. Ankle brachial blood pressure index was measured, mainly by digital palpation of the foot pulse, at baseline in 8986 patients. The ABI was subnormal (< or =0.9) in 3099 patients and normal in 5887 patients. A low ABI was a strong predictor of morbidity and mortality during the follow-up even in patients with no clinical symptoms of PAD (n=6769). This was so for the primary outcome of the study; ABI>0.9:13.1%, 0.6-0.9: 18.2% and <0.6: 18.0% (P<0.0001) and for mortality from all causes: in those with a normal ABI it was 8.5%, in those with ABI >0.6-0.9, 12.4% and 14.2% in those with an ABI lower than 0.6 (P<0.0001). Ramipril reduced the risk of clinical outcomes in those with a clinical history of PAD as well as in the patients with subclinical PAD. CONCLUSIONS: The ABI even if measured simply by palpation of the foot arteries is a strong predictor for future cardiovascular events and for all-cause mortality. Ramipril prevented major cardiovascular events in patients with clinical as well as subclinical PAD.
