BACKGROUND: Nurses' independent decisions about assessment, treatment, and nursing interventions for hospitalized patients are important determinants of quality of care. Physician peer implicit review of medical records has been central to Medicare quality management and is considered the gold standard for reviewing physician care, but peer implicit review of nursing processes of care has not received similar attention. OBJECTIVE: The objective of this study was to develop and evaluate nurse structured implicit review (SIR) methods. RESEARCH DESIGN: We developed SIR instruments for rating the quality of inpatient nursing care for congestive heart failure (CHF) and cerebrovascular accident (CVA). Nurse reviewers used the SIR form to rate a nationally representative sample of randomly selected medical records for each disease from 297 acute care hospitals in 5 states (collected by the RAND-HCFA Prospective Payment System study). SUBJECTS: The study subjects were elderly Medicare inpatients with CHF (n = 291) or CVA (n = 283). MEASURES: We developed and tested scales reflecting domains of nursing process, evaluated interrater and interitem reliability, and assessed the extent to which items and scales predicted overall ratings of the quality of nursing care. RESULTS: Interrater reliability for 14 of 16 scales (CHF) or 10 of 16 scales (CVA) was > or = 0.40. Interitem reliability was > 0.80 for all but 1 scale (both diseases). Functional Assessment, Physical Assessment, and Medication Tracking ratings were the strongest predictors of overall nursing quality ratings (P < 0.001 for each). CONCLUSIONS: Nurse peer review with SIR has adequate interrater and excellent scale reliabilities and can be a valuable tool for assessing nurse performance.

Planning is a routine human activity performed daily that often challenges confident professionals. The nursing process, adapted as a planning tool, provides the means to achieve institutional goals. The application of the nursing process to planning in one operating room setting resulted in the preparation of more than 1,500 surgical case supply preference lists in 5 weeks' time. The achievement facilitated timely implementation of an automated charging system for surgical cases through practically perfect planning.

The documentation of the nursing process is an important, but often neglected part of clinical documentation. Paper-based systems have been introduced to support nursing process documentation. Frequently, however, problems, such as low quality, are reported and it is still unclear if computer-based documentation systems can reduce these problems. We therefore introduced a computer-based nursing documentation system on four wards of the University Hospitals of Heidelberg. We systematically evaluated its preconditions and its effects in a pre-test post-test intervention study. We combined objective data (e.g., based on quality checklists) with subjective data drawn from questionnaires and interviews. In this paper, we present preliminary results, focussing on detailed results from the first two wards.
An intrasystem-intersystem model developed by Alfred Kuhn has been integrated into the nursing process model to serve as a model for patient-nurse interaction. In this conceptual model the patient and the nurse represent intrasystems with three components, the detector, selector and effector, which act to supply information about the environment, analyse internal values and preferences and initiate action. The intrasystems are connected through a specified set of relations as the patient and nurse interact to communicate information, transact values and organize a plan of care. An example is presented of how this model can be used to implement the nursing process in the community health setting.
Development of the Nursing Minimum Data Set for the Netherlands (NMDSN): identification of categories and items Rationale Currently, there is no systematic collection of nursing care data in the Netherlands, while pressure is growing from the profession, policy-makers and society to justify the contribution of nursing and its costs. A nursing minimum data set can provide data to demonstrate nursing's contribution to health care as it can be used to describe the diversity of different patient populations and the variability of nursing activities, and to calculate the associated nursing workload. Objective To identify categories and items for inclusion in the Nursing Minimum Data Set for the Netherlands. Design A multimethod, exploratory approach was used. This included interviews, document analysis, consensus rounds, seeking validation in the literature, and drawing up lists of most frequently occurring patient problems, interventions and outcomes of care. Eight hospitals, with a total of 16 wards, participated in the study. Results Relevant categories and items emerged after analysis and grouping of the material and included: five hospital-related items, six patient demographics items, seven medical condition items, 10 nursing process items, 24 patient problems, 32 nursing interventions, four outcomes of nursing care, and three complexity of care items. Almost every item could be located in the existing documentation systems, the lists of patient problems, outcomes and interventions, or in the literature. Conclusion A set of categories and items of nursing data has been identified. The content validity of this set is partly supported by its consistency with the literature, findings from practice and the judgement of potential users. Nursing outcomes need further development. The data set will be tested in practice to find out whether the categories and items are useful, and whether they can be minimized.
The purpose of this article is to share professional knowledge, practice, and educational opportunities related to energy-based nursing in order to broaden and improve the delivery of health care services. The holistic, theory-based approach places a patient's perceived needs first, and cares for the human body as well as the spirit. Energy medicine is an intricate part of the patient's expectation for health care. Watson's transpersonal-caring-healing model is explored (Watson, 1999). This model expands the view of the person to one that embodies energy that is comprised of spirit, a universal mind, and consciousness. The North American Nurses Diagnosis Association (NANDA) recognizes energy therapy as an intervention representing a specific theory: human energy field theory (HEFT). This therapy is related to the approved nursing diagnosis of energy field disturbance 1.8 (NANDA, 1995/1996). Healing touch (HT) is an energy-based therapeutic approach to healing that emphasizes caring for the whole person based on the HEFT. It is used in the nursing profession to influence changes in the human energy system; HT affects physical, emotional, mental, and spiritual health. The nursing process is evident throughout the curriculum. Nurse researchers report positive patients outcomes. The holistic nursing concept of energetic healing returns nurse professionals to the essence of nursing. Spinal cord injury (SCI) nurses will benefit by increasing their knowledge and awareness of energy therapy to increase patient satisfaction and improve outcomes for persons with SCI.
