New perspectives in the management of cranio-mandibular ankylosis.

Cranio-mandibular ankylosis is characterized by the formation of a bony mass, which replaces the normal articulation. Although the bony mass represents the pathology responsible for the ankylosis, it is not a neoplastic process capable of continued growth. For this reason excision of this mass is not necessary for the release of ankylosis. An osteotomy performed inferior to the base of the ankylotic mass converts this into a situation akin to that of a subcondylar fracture. The technical details of this conceptually new approach to the management of TMJ ankylosis is described and the advantage of this technique over the conventional ones, particularly in cases of recurrent ankylosis, is discussed.

[Clinical approach to inflammatory syndromes in the aged].

Primary care physicians have often to provide care to elderly patients presenting with non specific general complaints such as anorexia, weight loss and fatigue associated with biological inflammatory tests (increased erythrocyte sedimentation rate, increased CRP, anemia of inflammatory origin). In elderly patients, inflammatory diseases of unknown origin are most often related to an infectious illness (particularly bacterial endocarditis or tuberculosis), a systemic autoimmune disorder (temporal arteritis, polymyalgia rheumatica or ANCA positive vasculitis) or a neoplastic process. A methodological clinical approach is discussed and the most valuable complementary tests are proposed.

Application of cytology in diagnosing benign changes and breast cancers.

On the basis of cytology performed in a group of 386 women and 14 men admitted to the Breast Disease Outpatient Unit in the area of Sucha Beskidzka, a structure of changes in breast benign and malignant neoplasms was defined. The diagnostic value of cytology based on FNB and a secretion from mammary glands were evaluated. Expression of p53 and nm23 protein expression in the cells of the most common cancers was studied for prognostic values. The study demonstrated 81% of benign changes and 19% of cancers. With respect to cancers, cytology confirmed its usefulness since the findings were compatible in 94.44% with histopathologic results. Moreover, application of proliferation markers (p53 and nm23) allowed to evaluate the degree of malignancy and prognosis for further neoplastic process.

[Radical surgical treatment of Barrett's esophagus].

In the light of modern scientific knowledge, pathogenesis of Barrette's esophagus (BE) is presented as an integral neoplastic process. Indications to operation and policy of radical surgical treatment in BE, including in combined esophageal diseases (burn and peptic strictures, reflux esophagitis, short esophagus) were developed. From 1990 to 2000 radical operations were performed in 14 patients (11 males, 3 females) with BE. Mean age was 54.1 years. About 80% patients had clinical signs of reflux esophagitis at the moment of hospitalization. In all the patients the diagnosis of BE was verified in microscopic examination of operative material. 4 patients with cylindrical metaplasia associated with ulcers and esophageal peptic strictures were operated, 3 patients--with epithelium dysplasia, 7 patients--with adenocarcinoma. All the patients underwent extirpation of thoracic portion of esophagus with one-stage plasty with isoperistaltic gastric tube, supplemented with mediastinal and abdominal lymphodissection in cancer. Affection of the regional lymph nodes was revealed in 43% patients with esophageal adenocarcinoma. 50% patients had complications in postoperative period. There were no cases of hospital lethality. The expansion of indications for early surgical treatment of BE at the stage of meta-, and dysplasia, before adenocarcinoma development, is thought valid.

[Epiploic arteries. Normal and pathological radiological appearances (author's transl)].

The blood supply of the greater omentum was studied using selective digestive angiography in 450 cases. Comparison of the radiology and the anatomical appearances showed that their are two independent systems, right and left, the size of which is variable from one subject to another. These blood supplies are united by variable anastomoses which are difficult to demonstrate by arteriography. However, the classical anastomotic arterial circle of the greater curvature, between the right and left gastro-epiploic arteries was never found. The pathology of the greater omentum is of two types: Tumoral pathology: the demonstration of epiploic metastases is the most frequent finding in pathological cases. Its practical and prognostic importance is very great for it contra-indicates laparotomy. Associated pathology: the greater omentum reacts to any infective or neoplastic process of the abdominal cavity, such as "sentinel loop". These topographic changes should orient analysis of the films towards a given region and lead one to suspect some disease yet undiagnosed or not very obvious on arteriography.

[Chemotherapeutic combinations of mutually potentializing drugs. 1-Application to the treatment of breast cancers].

27 patients suffering from disseminated carcinoma of the breast with at least two visceral metastases, and two had become resistant to conventional chemotherpy and hormones, received a combination of, in the present trial, vincristine followed by cyclophosphamide with 5-fluoro-uracil. Chemotherapy was administered intermittently: each cycle of treatment lasted 6 days and was followed by a period without treatment of 25 days. Haematological tolerance was satisfactory. No serious incidents occurred during two years use of the combination. 20 out of 27 patients showed objective tumour regression of more than 50 p.cent lasting for more than 6 months, whilst 9 showed apparent complete regression of the malignant lesions. There was one complete failure. Chemotherapy was continued in all cases after regression of the neoplastic process was obtained.

Angiogenesis as a prognostic factor in invasive carcinoma of the uterine cervix.

The aim of the study was to evaluate angiogenesis as an independent prognostic factor and to determine the correlation between the angiogenic index (AI) and histologic grade of the neoplastic process in patients operated on for invasive carcinoma of the uterine cervix. Angiogenesis was assessed with immunohistochemical technique using a monoclonal antibody against human factor VIII--(F8/86 M0616, DAKO, Denmark). A positive correlation was revealed between the intensification of angiogenesis and the incidence of lymph node involvement and survival rate.

[Urgent interventions for invasive thyroidal carcinoma].

There was summarized the experience of treatment of the thyroid gland cancer, invading trachea (larynx) and other neighboring structures of the neck in 42 patients, whose state was estimated as severe one and neoplastic process--as nonoperable. Optimal surgical methods were applied depending on clinical signs of thyroidal carcinoma. In 36 patients the palliative resection of thyroid gland (in 6--with tracheostomy addition) was done, in 6--intervention was restricted by tracheostomy formation. The method of tracheostomy formation in cases of massive spreading of tumor along trachea is adduced.

[Diagnosis and surgical treatment of adrenocortical carcinoma. A review of the literature and report of two cases].

Adrenocortical carcinomas (ACC) are rare and represent only 0.05 to 0.2% of all cancers. They can be hormonally active, appearing clinically by one or more endocrine syndromes, or can be hormonally non-active. Commonly most of the cases with AC are diagnosed when the neoplastic process have spread out of the suprarenal gland (stage III or IV). Computed tomography and magnetic resonance imaging are the most useful diagnostic methods of ACC but the latter is more accurate, especially in estimation of the local invasion of the tumor. Surgery is the main and the most effective method for treatment of both primary and recurrent ACC, and in selective cases--of metastasis. The chemotherapy with mitotane has a limited role and is indicated in cases of inoperable ACC (primary or recurrent) and/or presence of metastasis. Two cases of ACC treated in our department are reported. Case I: a 26-years-old female with ACC in stage II, which was diagnosed incidentally by ultrasound investigation for other consideration. Although the urine levels of free cortisol were elevated, the woman had no endocrine symptoms. Case II: a 50-years-old female with a second recurrence ACC appeared 5 years after a resection of the first recurrence tumor and 6 years after an operation for the primary tumor. Problems in diagnose and surgical treatment of these cases are discussed.

Conization as treatment of carcinoma in situ of the uterine cervix.

A series of 343 consecutive cases with the preoperative diagnosis of carcinoma in situ of the uterine cervix treated by cervical conization is presented. In 93% of these patients cervical cytology returned to normal. The follow-up period ranged from 1 to 5 years. There was significantly greater chance for an apparent cure in cases in which histologic evaluation indicated free margins of resection (98%) than in those where the margins were involved by the neoplastic process (70%). Colposcopic evaluation to determine the site for punch biopsies and adequate resection margins could greatly enhance the success of therapeutic conization. The urgent need for conservative management of cervical intraepithelial neoplasia because of its predominance among young women is discussed.

Laparoscopic fenestration of a simple hepatic cyst.

Hepatic cysts are usually asymptomatic, most being detected incidentally during imaging. Distinction has to be made between a simple hepatic cyst from more sinister types such as a parasitic cyst and a cyst occurring as part of a neoplastic process. Intervention in a simple hepatic cyst is only indicated when they become symptomatic or when they grow at a rapid rate. Surgery is the mainstay of treatment, with laparoscopic intervention becoming more and more widely accepted. We present here the first case of laparoscopic fenestration of liver cyst performed in our hospital and also review the literature on this modality of treatment.

67Gallium accumulation in hepatic actinomycosis.

A case of actinomycosis involving the liver is reported. The liver scan with 99mTcS colloid showed multiple areas of focal abnormalities. 67Gallium citrate liver scan showed these areas to concentrate intensely. This pattern of differential hepatic scintigraphy is seen in pyogenic abscesses as well as malignancy of the liver. Actinomycosis may mimic the clinical picture of a malignant neoplastic process, making the differential diagnosis of 67Ga-positive lesions of the liver more difficult.

Phosphatases XII. Isoenzymes of alkaline phosphatase and radionuclear investigation (85Sr) of patients with neoplastic affection of the skeleton.

In a group of 30 patients with neoplastic processes, 13 were found to have a significantly increased serum alkaline phosphatase activity. This finding correlated with the results of investigation with 85Sr in 7 patients, but owing to a concomitant hepatal symptomatology, it proved of differential diagnostic value in only one of them. On the other hand, the activity of bone isoenzyme of alkaline phosphatase was significantly altered in 15 patients. In 14 of them the increase in bone isoenzyme activity corresponded to an X-ray and a radionuclear finding of a tumor process in the bones. This activity was within the normal range of values in only one patient with a positive result of the 85Sr investigation. An agreement between the results of determination of bone isoenzyme activity of alkaline phosphatase and those of radionuclear investigations was found in 27 patients. In addition, a correlation was established between the increased activity of bone isoenzyme and that of intestinal isoenzyme of alkaline phosphatase. Enzyme investigation can be suitably utilized, alongside radionuclear examination, for early detection of a bone process.

Oral lesions with a papillary surface texture: clinical and pathological correlations.

Oral mucosal lesions with a papillary surface texture are a heterogenous group of conditions with widely varying clinical behaviour. The aetiology is variable and includes genetic, infective, traumatic or neoplastic processes. A biopsy is required to establish a diagnosis and the microscopic examination must be coordinated with the clinical findings. This paper reviews the clinical and histopathological features of papillary or verruciform lesions.

Prognosis of giant cell tumor of bone. Histopathological analysis of 15 cases and review of the literature.

BACKGROUND: The purpose of this study is the evaluation of certain markers determining the prognosis of giant cell tumor of bone. METHODS: A clinicopathological analysis of 15 cases of giant cell tumor is presented. RESULTS: Giant cell tumor of bone mainly affects women with an average age of 34 years and is located in the upper and distal ends of the long tubular bones (93.3%). 60% of the tumor biopsies was Grade I, 33.3 % Grade II and 6.7% Grade III. The recurrence rate was 33.3% and the malignant transformation was 13.3%. CONCLUSIONS: Prognosis of giant cell tumor of bone is likely to be mistaken if it is based only on the histological picture. On the contrary, considering important parameters, such as tumor location, size and tumor aggressiveness, frequency of recurrences and type of surgical management, one may predict in most cases the clinical outcome of the neoplastic process.

Minor salivary glands: causing major problems.

Tumours arising in the parapharyngeal space (PPS) are rare and account for approximately 0.5% of all head and neck neoplasms. These neoplastic processes represent a wide variety of both benign (80%) and malignant lesions arising from the diverse range of structures within and surrounding the PPS. The PPS is typically conceptualized as a potential neck space in the shape of an inverted cone with its base at the skull base and apex at the greater cornu of the hyoid. Because of this unique structure, lesions must often grow to a considerable size before symptoms become apparent and clinical detection is possible. A rare case of mucoepidermoid tumour of the minor salivary glands arising in the prestyloid parapharyngeal space is described. The complex anatomical and pathological considerations within this region present a substantial challenge to the head and neck surgeon in the evaluation and management of these lesions.

[A case of carcinoma adenoides cysticum in the external auditory canal].

The authors present a case of a woman aged 31 with carcinoma adenoides cysticum at external auditory canal. The tumor was surgically removed; after 9 month a recrudescence was ascertained but there were no metastasis to other organs. The tumor was once more surgically removed. Now it has been a year of observation and no renewal of neoplastic process was noticed.
