A retrospective study was conducted in which several parameters of postoperative recovery were compared between 2 groups of patients: 25 patients who had lateral retinacular release surgery in which the holmium:yttrium-aluminum-garnet (Ho:YAG) laser was used, and a control group of 17 patients who had the same surgery in which electrocautery was used. Postoperative recovery times of patients treated with the Ho:YAG laser were significantly decreased as compared with the recovery rates of patients in the control group. Morbidity also was reduced in the group for whom the laser was used. The results of this study indicate that the 2.1-micron-wave-length, pulsed Ho:YAG laser can be used safely and effectively to perform lateral retinacular release surgery.
Dislocation of the proximal tibiofibular joint is a rare injury. Diagnosis is initially often missed. Physical examination and radiographic findings of a 22-year-old football player are reported. Classification and therapeutic options are discussed.
Recurrent dislocation of the proximal tibiofibular joint is a rare injury. It has been reported in parachutists and water-skiers. A technique for reconstructing the joint by using an iliotibial band fascial graft is described. This technique is recommended for recurrent dislocations of a nonarthritic proximal tibiofibular joint.
Tibial tuberosity fracture is uncommon. We reviewed five patients with the injury, presenting over a two-year period. All of them were adolescent boys who sustained the injuries during sport. They were treated with open reduction and internal fixation using cancellous screws with additional tension band wiring for comminuted fragments. Results were excellent, with complete union of fracture site, full range of movement by three to five months and no evidence of complication on follow-up for thirty months.
The frequency of recurrent dislocation of the patella is increased by the young age of onset, the presence of patella alta and permanent subluxation of the patella. The natural history of the patellar syndrome appears to be fairly favourable in everyday activities, but discomfort is often marked in the intermediate term in sports activities. Isolated femoropatellar osteoarthritis rarely causes major functional impairment of gait. Medial or lateral femorotibial osteoarthritis occurs secondarily. Painful cases often present with subchondral anomalies of the lateral patellar facet, lateral tibial osteophytosis and patella alta.
Medial femorotibial osteoarthritis (MFTO) is characterised by progressive deterioration resulting, in 6 out of 10 cases, in an inability to walk after 10 to 15 years. At the surgical stage, AP films show subchondral bone wear on the edge of the medial tibial plateau with lateral tibial dislocation and lateral films show obliqueness of the medial tibial plateau possibly exceeding 10 degrees and accompanied by anterior subluxation of the tibial or even rupture of the anterior cruciate ligament. Lateral femorotibial osteoarthritis (LFTO), more frequent in women, occurs an average of 7 years later than MFTO, at an average age of 70 years. Instability is often severe. 72% of cases develop clinical valgus deformity of 10 degrees to 20 degrees after an average of 13 years. Recurvatum is also possible. The combination of femoropatellar osteoarthritis, generally lateral, is observed in 23 to 41% of cases, depending on the author, while osteoarthritis of the hip is observed in 10% of cases and chondrocalcinosis is present in 20% of cases. These figures must be interpreted in relation to the usual ageing of the subjects.

Twenty-one patients with isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament were treated in our hospital during the period 1965 to 1976. Twelve of the patients had fresh injuries and nine, injuries with delayed union or non-union. Twelve of the twenty-one patients were treated conservatively and nine, by surgical repair by means of staple fixation. Detachment of the posterior horn of the meniscus with an avulsed and displaced bone fragment was found in five of the nine patients operated on. All the patients were followed for an average of four years and eight months. Satisfactory results were obtained in every instance.
Thirty-nine patients with symptomatic osteoarthrosis (OA) of the patellofemoral joint were treated with 42 anterior tibial tubercle elevations, also known as the Maquet procedure. The patients all had symptomatic OA of the patellofemoral joint that fell into one of three categories: old patellar fracture, chronic patellar subluxation, or postpatellectomy pain. In the latter group, the OA was manifested by erosions of trochlear articular cartilage from the articulating quadriceps tendon-patellar ligament suture line. All 39 patients had a 1.5-2.5-cm tibial tubercle elevation with medial displacement, as necessary, to centralize their patellofemoral mechanisms. Follow-up period averaged 6.1 years. Seventy-nine percent (33 of 42 procedures) had good to excellent results. There was a major complication rate of 7%. Six of the nine failures were attributable to social/psychiatric reasons. Previously unrecognized tibiofemoral OA was the reason for poor results in two of the other failures, and one failure was unexplained. Patients with long-standing symptoms caused by patellofemoral OA should preoperatively be psychologically evaluated and diagnostically arthroscoped before the Maquet procedure is carried out.

Over 100 surgical procedures for recurrent patellar dislocation is described in the literature. Soft tissue operations being less traumatic are specifically indicated in children and adolescents. This paper deals with three of them: Ali Krogius, Garlicki-Salamon and Blauth-Schwarz procedure. Material included 31 patients aged 8-21 years. Thirty-five operation were done: Ali Krogius procedure 14 times, Garlicki-Salamon 13 times and Blauth-Schwarz procedure in 8 cases. The four grade Hall and Michelli scale was used to assess the results. Excellent results were achieved in 12 cases, good in 19, fair in 4 and 2 were poor. Blauth-Schwarz procedure rendered the best results, the worst were accomplished with Ali Krogius operation.
The authors describe a case of superior dislocation of patella that was irreducible by closed methods. Extensive review of the literature revealed that this is the first case reported in which an open reduction was needed to reduce a superior dislocation of the patella to unlock the osteophytes, which were blocking the closed reduction.
We report four cases of Larsen's syndrome spanning three generations of a Saudi Arabian family. The patients had the classic features of Larsen's syndrome, without spinal involvement.
