What data integration means to the practicing dentist.

Practitioners all have been confronted with unscheduled, emergency care patients. Their immediate concern is usually pain relief, and they may not always provide accurate or complete information about their health history. They may be distracted by pain, lack the understanding of their health to answer appropriately, or by intention or oversight omit or provide faulty information. Faced with omissions and inconsistencies, practitioners make an extra effort to explore their health history and, if indicated, consult their physician. Although the information that is developed creates a more complete and more accurate picture of a patient's health, there is always a small risk of omission or misrepresentation of a significant item. Although chances are that this misrepresentation will have little impact on how the practitioner handles the case, there is that one case in a thousand that can ruin one's day and possibly one's practice.

Universal application-specific integrated circuit for bioelectric data acquisition.

Use of highly integrated application specific circuits (ASICs) in bioelectric data acquisition systems promise important new insights into the origin of a large variety of health problems by providing light-weight, low-power, low-cost medical measurement devices that allow long-term studies. They also promise significant cost reduction in medical care, as patients in principle become mobile and do not have to be hospitalized for observation. We report on the development and successful implementation of a universal ASIC, designed to meet key characteristics of a broad variety of bioelectric signals in terms of their dynamic range, sampling rate and input referred noise; e.g. electrocardiogram (ECG), electroencephalogram (EEG) and, most constringently, evoked potentials (EPs). Our approach for the first time makes cost-effective use of state-of-the-art microelectronics in medical measurement equipment, thus offering to replace discrete, single application devices used at present.

The impact of symptom interpretation on physician utilization.

This article explores physician consultation decisions among a sample of elderly people living in community settings. The analysis integrates the insights of research on illness behavior, particularly symptom interpretation and lay consultation, with previous specifications of the Health Behavior Model. Data were gathered through personal interviews and health diaries completed on a daily basis for 21 consecutive days by a probability sample of older people living in community settings. Results indicate that uncertainty regarding potential seriousness, level of pain or interference, and causal attributions were the most consistently significant predictors of physician contact.

Plasma leptin levels and coronary artery calcification in older adults.

CONTEXT: Leptin is associated with adiposity and insulin resistance and may play a direct role in vascular calcification. It is unclear, however, whether leptin is an independent predictor of atherosclerotic burden. OBJECTIVE: The aim of this study was to examine the association between plasma leptin and coronary artery calcification (CAC) in an ethnically diverse cohort of older adult men and women free of clinical cardiovascular disease. DESIGN: This was a cross-sectional study with data collection between January 2002 and February 2004 as part of the ADVANCE Study. SETTING: The study was conducted at an integrated health care delivery system in Northern California. PARTICIPANTS: Participants included 949 men and women aged 60-69 yr old. INTERVENTIONS: There were no interventions. MAIN OUTCOME MEASURE: The main outcome measure was CAC by multidetector row computed tomography. RESULTS: In ordinal logistic regression, plasma leptin levels were positively associated with extent of CAC independently of age, race/ethnicity, and smoking status in women (odds ratio of higher CAC for the sex-specific upper tertile vs. lower tertile = 1.81; 95% confidence interval, 1.10-3.00) but not in men (odds ratio = 1.29; 95% confidence interval = 0.89-1.86). However, this association was explained by metabolic risk factors and adiposity measures. CONCLUSIONS: Our findings support a role of leptin on vascular calcification in women but, in our sample of older adults, the association between leptin and CAC was not independent of other cardiac risk factors.

Body Area Network BAN--a key infrastructure element for patient-centered medical applications.

The Body Area Network (BAN) concept enables wireless communication between several miniaturized, intelligent Body Sensor (or actor) Units (BSU) and a single Body Central Unit (BCU) worn at the human body. A separate wireless transmission link from the BCU to a network access point--using different technology--provides for online access to BAN data via usual network infrastructure. BAN is expected to become a basic infrastructure element for service-based electronic health assistance: By integrating patient-attached sensors and control of mobile dedicated actor units, the range of medical workflow can be extended by wireless patient monitoring and therapy support. Beyond clinical use, professional disease management environments, and private personal health assistance scenarios (without financial reimbursement by health agencies/insurance companies), BAN enables a wide range of health care applications and related services.

The emerging role of the health information management professional in data quality and analysis of an electronic medical record system.

The role of the health information management professional is changing with the advancement of the electronic medical record system (EMRS) and electronic financial database systems. The Medical ARchival System, a longitudinal EMRS developed at the University of Pittsburgh Medical Center, was used to describe the methodology that the health information management professional should use when performing data quality evaluations and data analysis of an EMRS. Specific examples of steps used to integrate databases to collect the data and generate appropriate reports and tables are also described and discussed.

Morbidity recording in primary medical care and its use for community diagnosis.

Primary medical care now plays an important role in integrated community health services. For the planning and evaluation of such services, health administrators require information on morbidity in the community. While useful data on morbidity are generated in the primary care services themselves, these data are not yet utilized to their fullest. Common difficulties encountered in recording and analyzing data in primary care are briefly reviewed. The methods and results of a pilot study in 13 Kupat Holim clinics in Israel are presented. The study used a classification of diseases and symptoms specially designed for the needs of primary care but based on and cross-indexed with the International Classification of Diseases. The data produced gave a meaningful picture of the patterns of disease encountered in primary care and revealed some interesting differences between the communities served by the clinics. The system of recording was acceptable to both physicians and coders.

[Breeding, advice and ecopathology. A didactic approach to situations involving breeding diagnosis in swine production].

Innovation, quality, variety and flexibility of production require simultaneously an improvement of professional skills and an increase in cooperation. However, this cooperation should not be reduced to a mere data exchange, but must extend to a co-construction of new representations and new action schemes. In this way, health advising integrates production and knowledge apprenticeship, which we studied using a didactic approach (from the knowledge point of view). In this article, we will establish that animal ecopathology, by the standardization of the breeding description through diagnostic activity, has contributed to the strengthening of the cooperation between the breeder and the practicien.

Effect of integrated research programs on health care systems and costs.

We examined the effect of research program size on clinical costs (current dollars/work unit) and productivity (work units/ employee) in Department of Veterans Affairs (DVA) hospitals. Our data demonstrate that research programs have a small, almost negligible effect on clinical costs. The data also show that average acute medical service length-of-stay was inversely related to research program size. The indirect cost of research may have been almost entirely offset by an increased efficiency, as reflected in the shortened length of stay. This unexpected benefit of research may be due to the influence of research opportunities on the recruitment and retention of highly trained clinicians, including medical specialists. In the past, recruitment and retention was part of the justification for the DVA research program. Although this justification no longer appears to be considered politically acceptable, our data strongly support the notion that research programs can and do enhance the efficiency of medical care. Furthermore, research activities help professional staff maintain their critical thinking and analytic skills. We discuss the implications of these observations on cost containment in health care systems.

Risk assessment for benefits analysis: framework for analysis of a thyroid-disrupting chemical.

Benefit-cost analysis is of growing importance in developing policies to reduce exposures to environmental contaminants. To quantify health benefits of reduced exposures, economists generally rely on dose-response relationships estimated by risk assessors. Further, to be useful for benefits analysis, the endpoints that are quantified must be expressed as changes in incidence of illnesses or symptoms that are readily understood by and perceptible to the layperson. For most noncancer health effects and for nonlinear carcinogens, risk assessments generally do not provide the dose-response functions necessary for economic benefits analysis. This article presents the framework for a case study that addresses these issues through a combination of toxicology, epidemiology, statistics, and economics. The case study assesses a chemical that disrupts proper functioning of the thyroid gland, and considers the benefits of reducing exposures in terms of both noncancer health effects (hypothyroidism) and thyroid cancers. The effects are presumed to be due to a mode of action involving interference with thyroid-pituitary functioning that would lead to nonlinear dose response. The framework integrates data from animal testing, statistical modeling, human data from the medical and epidemiological literature, and economic methodologies and valuation studies. This interdisciplinary collaboration differs from the more typical approach in which risk assessments and economic analyses are prepared independently of one another. This framework illustrates particular approaches that may be useful for expanded quantification of adverse health effects, and demonstrates the potential of such interdisciplinary approaches. Detailed implementation of the case study framework will be presented in future publications.

Designing an ambulatory clinical practice for outcomes improvement: from vision to reality--the Spine Center at Dartmouth-Hitchcock, year one.

Development of a new program for diagnosis and treatment of spine-related problems provided a unique opportunity to design and implement a new model for delivery of health care incorporating outcomes measurement and improvement. Key features include: application of microsystem thinking and interdisciplinary practice; integration of a uniform outcomes measurement tool, the Dartmouth Clinical Value Compass; and touch pad technology for data collection. This, for the first time, provided clinically meaningful point-of-service data and aggregated information for improvement. A further advantage was the ability to integrate a clinical research program within this microsystem. A multisite clinical research trial, the Spine Patient Outcomes Research Trial (SPORT), modeled on the Spine Center microsystem and funded by The National Institute of Arthritis, Musculoskeletal and Skin Diseases and the Office of Research on Woman's Health, the National Institutes of Health, and the National Institute of Occupational Safety and Health, the Centers for Disease Control and Prevention, is currently underway. The significant problems we face today cannot be solved by the same level of thinking that created them.

Socioeconomic marginality and health services utilization among Central Harlem substance users.

The article examines whether decrements in socioeconomic measures in a poor, substance using population predict changes in health services utilization. The sample consisted of 658 "hard drug" (crack, powder cocaine, and heroin) users drawn from Central Harlem in New York City during 1998 and 1999. Chain referral and social networking were used in order to gain access to hidden users. The sample was stratified according to operational measures indicating socioeconomic marginality, one calculated using indices of income, education, and employment and another designed to measure lived homelessness. Rates of self-reported utilization of 10 health services were compared across strata. In this sample socioeconomic marginality reflected by low levels of income, education, and employment sometimes predicts greater rates of health services utilization and, in other cases, it predicts lower rates. When the sample is stratified according to an operational measure of homelessness, the gradient of greater utilization and self-reported morbidity for the homeless is more marked and consistent. Results are supportive of a public health model of drug user treatment that recommends that it occur as part of an integrated strategy addressing poverty, homelessness, violence, and related social problems.

High prevalence of bedbugs Cimex hemipterus and Cimex lectularis in camps for internally displaced persons in Freetown, Sierra Leone: a pilot humanitarian investigation.

The prevalence of bedbugs Cimex hemipera and C. lectularis was investigated in camps for the internally displaced persons (IDPs) in Freetown, Sierra Leone. Two hundred and thirty eight rooms were searched during the day and at night, and 233 (98%) of those rooms in 30 booths were infested with different life cycle stages of bedbugs. There hundred and ninety-eight (68%) of the bedbugs were adults, 145 (24.8%) were nymphs of various instars, and 41 (7%) were clusters of eggs. Significantly (P > 0.05) more bedbugs were recovered during the night inspections 64.6% as compared to 35.4% during the day inspections. In addition, more adult bedbugs were recovered at night than during the day, a manifestation of their peak feeding period. Of the total of 570 adults and nymphs collected and identified, 320 (56.1%) and 250 (43.9%) were Cimex lectularis and Cimex hemipterus respectively. Clinical examination of 221 individuals living in the booths during 3 consecutive weeks of examinations and treatment for conditions suggestive of bedbug infestation (bites and skin reactions as well as treatments for other health and medical conditions) showed that 196 (86%) had wheals as a direct result of bedbug bites. The data of this pilot humanitarian investigation shows a high prevalence of bedbug infestation in these displacement camps. It is recommended that some control measures be instituted, like residual insecticide application along with integrating control methods within the primary health care system, because bedbugs are a source of great irritation and sleepless nights that could lead to stress.

[Quality control in anesthesiology. Results of a prospective study following the recommendations of the German Society of Anesthesiology and Intensive Care].

The German Social Law has required quality assurance (QA) procedures since 1989. The measures must be suitable to allow "comparing investigations". In 1992 the German Society of Anaesthesiology and Intensive Care Medicine published recommendations for QA in anaesthesia: most problems during an anaesthetic should be documented in a standardised manner, and thus, a list of 63 pitfalls, events, and complications (PECs) and five degrees of severity were defined. The goal of this study was to determine the frequency of PECs in anaesthesia and to correlate PECs with procedures and preoperative health status. MATERIALS AND METHODS. Demographic data, preoperative findings, type and duration of anaesthesia and operation, and kind and severity of PECs were integrated in an automatically readable anaesthetic data record (ARADR). During 12 months all anaesthetics in our department were documented by the ARADR; the records were read by a reading device and the data stored in a modern SQL database (Informix). Degrees of severity: I. PEC leads to reaction of anaesthetist, no impact for recovery room (RR); II. impact for RR, no impact on transfer to ward; III. significant prolongation of RR stay or additional monitoring on ward; IV. PEC leads to intensive care unit admission; V. disabling damage or death. RESULTS. In all, 18350 anaesthetics were recorded (9055 male, 9295 female); the median age was 41 years (1 day-99 years). In 4251 (23.2%) anaesthetics 5927 PECs occurred, 3412 of them involving the cardiovascular and 949 the respiratory system, the latter with a tendency to higher degrees of severity. PECs caused by technical equipment (126) or lesions caused by anaesthesists (342) had no fatal outcomes and were less severe. Patients in ASA class I had 12.3% anaesthetics with PECs, ASA II 23.3%, ASA III 33.8%, ASA IV 34.9%, and ASA V 58.5%. PECs of degrees IV and V showed a higher incidence in the higher ASA classes. There was no fatal PEC in an ASA class I patient and only one (of 13615) in an elective procedure. Emergency cases had more frequent and more severe PECs: 16 of 19 PECs of degree V were in ASA class IV and V patients and 15 in emergency situations, all of them in surgical patients. Patients with cardiovascular disease had a more frequent incidence of PECs by a factor of 1.39 to 5.93 than those without such disease. CONCLUSIONS. Standardised incident reporting by defined PECs seems a good way to describe problems in anaesthesia. The types of PECs in our study had a similar distribution to those in other investigations, but there was a tendency to less frequent fatal PECs in ASA classes I to IV and more frequent ones in ASA class V. We expect better comparability when multicenter studies are done using identical methods in the next few years. Perhaps different patients collectives with special risks will be detected; efforts in quality improvement could focus on these patients.

Coordinating the care of the chronically ill in a world of managed care.

The systems required to provide coordinated health care to the chronically ill within a managed care contract are complex. As an integrated health care delivery system assuming shared financial risk of enrollees in a Medicare + Choice contract, many care processes need to be created to meet the needs of the clients and administrators. Nursing case management and physician partnering are integral to the creation of the care model. The fiscal data demonstrated, for clients in this model, that there was an overall decrease in the inpatient length of stay, hospital days per thousand, and 30-day readmission rates.
