Use of Norplant in Pakistan: two years experience.

The acceptability and efficacy of norplant (levonorgestrel sub- dermal implants) was assessed over a period of two years among 445 married women aged 19 to 45 years. Twenty women were lost to follow-up while the remaining 425 were regularly followed-up. At the end of two years, 388 women were still continuing with norplant (91.2%). During this period, there was only one pregnancy (0.26%). There was no significant change in mean systolic or diastolic blood pressure. However, there was a significant increase (P < 0.001) in mean weight and decrease (P < 0.001) in the mean duration of menstrual cycle. Thirty-seven women (8.7%) got the implant removed. The major reason for removal (67.6%) was menstrual disturbances. Nine acceptors (18.59%) developed side effects, which were related to the type of symptoms often associated with steroid contraception. These findings are similar to those reported from other countries. The high efficacy (99.74%) and acceptability (continuation rate of 91.29%) as well as long acting (5 years) nature of norplant suggest that this method of contraception can safely be promoted in Pakistan.

A long-term study of the efficacy and acceptability of a single-rod hormonal contraceptive implant (Implanon) in healthy women in China.

OBJECTIVE: To investigate the contraceptive efficacy, cycle control and acceptability of Implanon, a new single-rod, progestogen-only contraceptive implant. METHODS: In a non-comparative, open, multicenter study, a contraceptive implant containing the progestogen etonogestrel was inserted into 200 healthy women. The study duration was 2 years, with an optional extension up to 4 years. RESULTS: Women were exposed to Implanon for 644.6 woman-years. There were no pregnancies during the study. Per 90-day reference period, the median number of bleeding-spotting days ranged between 18 and 21 and the median number of bleeding-spotting episodes was two. The mean overall incidence of prolonged bleeding fell markedly during the study, from 69% in the first reference period to 26% in the 16th period. The most common adverse events were related to disturbed bleeding pattern and amenorrhea. Heavy or prolonged bleeding caused 18 subjects to withdraw from the study. Only a few subjects discontinued the study early due to irregular bleeding (2%) or amenorrhea (2%). A slight increase in mean body weight was observed. The implant was removed in an average time of 2.9 min. CONCLUSIONS: Implanon demonstrated excellent contraceptive efficacy for 4 years of use and was well tolerated. The incidences of prolonged bleeding and amenorrhea both fell markedly with continued implant use. Because of its single-rod design, Implanon was quickly removed with few complications and proved to be a highly acceptable method of contraception.

Clinical performance and menstrual bleeding patterns with three dosage combinations of a Nestorone progestogen/ethinyl estradiol contraceptive vaginal ring used on a bleeding-signaled regimen.

OBJECTIVE: We examined the clinical performance of contraceptive vaginal rings (rings) delivering Nestorone (NES) progestin and ethinyl estradiol (EE). Ring removal times were signaled by menstrual events. Bleeding patterns, adverse events, patterns of use and continuation rates were the principal parameters evaluated. METHODS: In a two-stage 6-month trial, subjects were randomized to use rings releasing 50 microg/day of NES and either 10 (50/10) or 20 (50/20) microg/day of EE. Subjects were to keep rings continuously in situ until menstrual bleeding or prolonged spotting-signaled removal. Reinsertion was to occur 96 h later. After the randomized stage, an open-label 6-month trial of rings releasing 150 microg/day of NES and 15 microg/day of EE (150/15) began with the same menstrually signaled regimen. RESULTS: Two-hundred forty-six subjects participated in the trial. Six-month pregnancy rates ranged by ring dose from 1.3 to 3.9 per 100. For each ring dose combination, 6-month continuation rates were above 80 per 100. Bleeding and spotting (B+S) days in women with the 50 microg/day NES rings were similar in number to those experienced by cycling women not using contraception. Nevertheless, in the initial 90 days, fewer B+S days were reported by subjects with the 50/20 ring than by subjects with the 50/10 ring (p < .05). Throughout the trial, subjects using the 150/15 ring reported significantly fewer B+S episodes than did subjects with either 50 microg/day NES ring. CONCLUSION: Combined contraceptive rings used with a bleeding-signaled regimen led to few terminations attributed to bleeding problems and to acceptable continuation rates. The 150/15 ring appeared to induce fewer bleeding problems than did the lower-dose NES combination rings, but no important difference in 6-month continuation rates among the three doses was noted.

Oral contraception and epilepsy: findings in a large cohort study.

There is no good evidence that oral contraceptives (OCs) increase the risk of epilepsy in unaffected women or increase the frequency of seizures in those suffering from the disorder. Nonetheless, manufacturers' literature continues to include warnings about OCs and epilepsy. We have analyzed the available data about epilepsy in the Oxford-Family Planning Association contraceptive study that includes 17,032 women followed for periods of up to 26 years. In total, 82 women (with no record of having suffered epilepsy when recruited to the study) were referred to hospital for diagnosis or treatment of epilepsy during the follow-up period. No association was found between OC use and the occurrence of the condition. Referral to hospital was, however, more frequent in women of low social class and in those with a high body mass index.

Sexual function during the puerperium.

Of 328 women interviewed at a postnatal clinic, 166 (50.6%) had resumed sexual intercourse, the average interval after delivery being 5 weeks postpartum. Of the various social and clinical factors examined, resumption of coitus was related only to marital status, vaginal laceration, and continuing vaginal bleeding. Of the sexually active group, 89% were practicing effective contraception.

Feasibility and use of school-based condom availability programs in Tijuana, Mexico.

Condom availability programs (CAPs) may increase access to condoms and contribute to increased condom use among youth. This study describes the characteristics, degree of satisfaction, and gender differences among users of a CAP implemented in two high schools in Tijuana (Baja California, Mexico). A kiosk distributing free male condoms was set up in two high schools in Tijuana. Kiosk users (N=570) were more likely to be male, young, and/or enrolled in a lower SES school. Most kiosk users were either sexually active or planning to have sex. Females were less likely to request condoms and to continue using the kiosk, but more likely to request information on other contraceptive methods. Results demonstrate the feasibility of implementing CAPs in Mexican schools and suggest that these programs can improve adolescents' condom access.

Experimental study of cervical blockage induced by continuous low-dose oral progestogens.

An investigation correlating scanning electron microscopic observations with sperm penetration tests carried out on cervical mucus under the influence of low-dose continuous progestogen (Norgestrienone) is presented. The results demonstrate that such type of contraceptive is involved in drastic alterations of mid-cycle cervical mucus at the macromolecular level. The meshwork which constitutes the infrastructure of the cervical secretion appears to be greatly tightened as a result of the treatment, thus giving the woof a general appearance typical of cervical mucus in the late luteal phase. The immobilizing effect of such modified mucus on spermatozoa is demonstrated and the duration of effectiveness after the administration of a last pill on the morning of day 13 is determined.

Components of stably high fertility in three areas of west Africa.

The constancy of fertility levels in Ghana, Senegal and southwest Nigeria since 1970 is separated into its nuptiality and marital fertility elements. The age-specific changes in the two components are examined and these show that the apparent stability in observed total fertility rates is essentially the outcome of the offsetting impact of increased marital fertility below age 25 and above age 40, over the effect of the increasing proportion of women remaining single up to 25 yr of age. Continuity in traditional fertility behaviour and stable nuptiality has remained operative over the broad middle segment of the reproductive lifespan of women in the three areas. The paper thus concludes that West Africa is likely to continue to display stably high fertility for many years into the next century. The social and cultural conditions behind this trend are discussed with special reference to the continued high demand for children and the low and only slowly rising contraceptive prevalence level.

Single monthly administration of the anti-progestagen Org 31710 in users of the 75 microg desogestrel progestagen-only pill: effects on pituitary-ovarian activity.

Endocrine and ultrasound effects were studied of an intermittent (every 28 days) oral administration of 150 mg of the anti-progestagen Org 31710 during the continued daily use of 75 microg desogestrel (DSG) for progestagen-only contraception. A randomized, double-blind, placebo-controlled two-centre study was conducted in 50 healthy volunteers. Serum luteinizing hormone (LH), follicle stimulating hormone (FSH), oestradiol and progesterone concentrations, and follicle number and size were studied, as well as endometrial thickness, which was assessed by transvaginal sonography at least twice weekly during a single medication cycle (cycle 3-5). Forty-eight women were evaluated (Org 31710, n = 25; placebo, n = 23). Seven ovulations were observed in the treated group versus none in the placebo group. LH concentrations were higher on days 9 and 11 and oestradiol concentrations lower on day 3 in the treated group, irrespective of whether ovulation occurred. No parameter could predict ovulation. Endometrial thickness was greater on cycle days 7-13 and 19 in the treated group. However, within the Org 31710 group, no significant differences were found in volunteers who did or did not ovulate. Observed differences may be attributed to a competitive effect of Org 31710 with progestagen-induced suppression of the pituitary-ovarian axis, altered oestradiol feedback mechanisms, and/or altered receptor availability.

Hysterectomy in the United States.

Hysterectomy is the most frequently performed major operation in the United States--and one of the most controversial. Although there are large regional differences in hysterectomy rates, the nationwide rate declined between 1975 and 1980. Leiomyomas, dysfunctional uterine bleeding, and pelvic relaxation are among the most frequent indications for hysterectomy. One fourth to one half of all women who undergo hysterectomy develop some morbidity, with fever and hemorrhage the most common types. Late sequelae may include the residual ovary syndrome, depression, and an increased risk of cardiovascular disease. The mortality rate appears to be about one death per 1000 operations. Neither prevention of cancer nor contraceptive sterilization appears to justify elective hysterectomy in asymptomatic women. Hysterectomy will continue to be an important component of women's health care. Ongoing peer review activities, improvements in surgical technique, and continued research should help ensure that each hysterectomy performed is both appropriate and safe.

Antischistosomal action of atorvastatin alone and concurrently with medroxyprogesterone acetate on Schistosoma haematobium harboured in hamster: surface ultrastructure and parasitological study.

Aiming to study the influence of long-term administration of lipid lowering agents (atorvastatin; AV), and to study the action of combined treatment with injectable contraceptive (medroxyprogesterone acetate; MPA) on tegumental ultrastrucutre and survival of Schistosoma worms, this study was established. AV (0.9 mg kg-1) was administered orally for 49 successive days to Schistosoma heamatobium-infected hamster starting from day 35 post-infection (pi). Another group of infected hamster was administrated MPA intramuscularly (0.1 ml kg-1) at days 7 and 35 pi followed by AV treatment regimen. Both treatment regimens significantly affected the surface ultrastructure of the male worms more pronouncedly than the female ones. Combined treatment was more severe in action compared to single one. The combined treatment was characterized by losing of spines and damaging of tubercles throughout the tegument, severe erosion and peeling and appearance of deep crakes in different parts of the tegument. Moreover, mild to sever destruction to the oral suckers of both female and male worms was noticed. On the other hand, both treatment regimens significantly reduced numbers of recovered S. haematobium worms and tissue egg load. Oogram pattern was affected only in case of combined treatment with high percentage of dead eggs. In conclusion, AV, if given continuously for long time, has a pronounced antischistosomal action especially when accompanied with contraceptive intake. These promising results may encourage further investigation with the intention of their possible application on treatment of schistosomiasis as a complement strategy to praziquantel chemotherapy.

Use and acceptance of the "Paper Pill" among an urban clinic population.

A randomized, crossover study comparing the acceptability of two presentations of the same oral contraceptive, a paper formulation and the conventional tablet, was carried out among two-hundred-and-seventy women attending two urban family planning clinics in Mexico City. During the first three cycles of use, the percentage of women discontinuing the medication for side effects was almost identical in the two groups. Although the paper formulation failed to demonstrate any major advantage in terms of continuation or acceptability, it was preferred by a majority of the women who began oral contraception with its use.
