[Mucosynechial conjunctivitis and bilateral corneal ulcers in Lyell's syndrome].

The patient, aged 44, presented fever and chills with altered general condition and received an analgesic-antipyretic treatment with salicylic acid and pyrazolon derivatives. A bullous eruption that followed was labelled incipient Lyell syndrome. Both cornea presented ulcers with a tendency to perforation and mucosynechial conjunctivitis. Pathogenic Staphylococcus albus was isolated from the conjunctival secretion. The ocular phenomena improved under mydriatic treatment and trophic medication. Lyell's syndrome is the result of a severe medicamentous toxemia. Stress is laid on the role of Staphylococcus aureus, phage type 71, in the process of epidermal necrosis with detachment of the epithelium and ulceration of the mucosa.

[An etiological study of acute hemorrhagic conjunctivitis in Beijing area in 1984].

Acute hemorrhagic conjunctivitis (AHC) has been widespread in China since 1971. An etiological survey of the sporadic outbreak in Beijing was done in 1984. Conjunctival swabs and sera were collected at the Eye Clinic of Peking Union Medical College Hospital and 10 strains of virus were isolated from 46 cases by culture on the HeLa cell line. 8 strains were identified as Enterovirus 70 by homologous antiserum J 670/71 obtained from the California State Laboratory of Viral and Rickettsial Diseases. 2 other strains defied neutralization by this antiserum and were eventually identified as Adenovirus type 3 and type 7. 30 paired sera were examined for neutralization antibodies to EV 70 (J 670/71). Among these, 23 (77%) showed fourfold or higher rise in titer. Furthermore, neutralization antibodies to EV70 (J 670/71) and the EV70 isolated from 7 patients with AHC by paired sera also showed a fourfold or higher rise in antibody titer, indicating that EV70 was the etiologic agent responsible for the 1984 outbreak in Beijing.

Microbiological studies in conjunctivitis.

A total of 102 cases of clinically diagnosed cases of acute conjunctivitis were included in the present study. The sample was collected from the inflamed conjunctiva and was subjected to conventional bacterial and fungal cultural studies. The allergic element was found out by studying the smear after Giemsa staining and demonstrating increased number of eosinophils. The data revealed that 32 samples had an evidence of bacterial infection, 14 showed isolation of fungi and one displayed an allergic etiology. Thirty cases showed presence of mixed infection in different combinations while 25 samples were sterile on conventional culture media. Microbiological studies are essential for confirmation of clinical diagnosis and to institute an appropriate treatment.

Water, trachoma and conjunctivitis.

The incidence of eye infections in a community is generally accepted as an indicator of the adequacy of water supply for their needs. However, discrepancies in the published results from various studies seem to challenge this view. We have reanalysed the published data on trachoma in relation to the most relevant indicators of water accessibility, using prevalence ratios as the single parameter for risk assessment. A definite trend emerges from this review: the incidence of infectious conjunctivitis is not sensitive to differences in water accessibility; on the other hand, a reduction in the risk of trachoma is consistently associated with better access to water. This conclusion may support the efforts of WHO and other multilateral and bilateral agencies to sustain the commitment towards the water supply sector beyond the International Drinking Water Supply and Sanitation Decade.

[Conjunctivitis of bacterial origin in children. Local antibiotic treatment with tobramycin collyrium].

A total of 122 children, from one to twelve years old, with suspected bacterial conjunctivitis, were treated with Tobramycin 0.3% eye drops. The follow up control was at the 3d (+2), the 7th (+2) day and a third control was performed around the 15th day, in case of a longer therapy. All patients showed a significant remission of the sign and symptoms already at the first control. No local or systemic side effect was noticed. Locally the product was well tolerated.

Contact lens induced giant papillary conjunctivitis: a retrospective study.

In this retrospective epidemiological study, records from the Contact Lens Department of SUNY College of Optometry were randomly selected and reviewed. An association between contact lens induced giant papillary conjunctivitis (GPC) and a FDA classified sub-category was found. Younger patients were shown to have a higher risk of developing GPC. Gender and tear film break-up time were not found to be associated with the condition. The mean replacement time for hydrogels was 10.8 +/- 9.2 months with no significant differences among contact lens polymer types. The GPC phenomenon was almost exclusively bilateral with a mean onset time of 31.4 months after commencing lens wear. A model for the development of GPC based on tear film interactions with the hydrogel lens surface is presented. A model for the tear film's interaction with the hydrogel contact lens in situ is offered.

Molecular epidemiology of adenoviral conjunctivitis in Sapporo, Japan, and Manila, the Philippines.

DNA restriction endonuclease analysis was carried out on 48 adenovirus isolates from the conjunctival swabs of patients with acute conjunctivitis. Samples were obtained in Sapporo, Japan, and Manila, the Philippines, from October 1983 to October 1984. We identified new genomic variants among the isolates of Ad3 in Sapporo, and Ad4 and Ad11 in Manila, and these were designated as Ad3f, Ad4b, Ad11a and Ad11b, respectively. Ad7 isolates in Manila were identical to Ad7b reported by Wadell et al. In Sapporo, no Ad7 was isolated during the period of this study. One isolate in Sapporo, which was neutralized by both Ad34 and Ad35 antisera, showed identical restriction patterns to Ad35 prototype strain (Holden) with SmaI, BamHI and HpaI. However, with HindIII, the pattern was identical to that of Ad34 prototype strain (Compton). This isolate was tentatively designated as Ad34/35. Ad8 isolates in Sapporo were identical to Ad8B detected there from 1977 to 1981, while Ad8 isolates in Manila were prototype (Trim) strains. In both cities Ad19 isolates were all identical to Ad19a and Ad37 isolates were all prototype (GW). Genomic variants of Ad isolates in both cities are as follows: (Table: see text).

Antigenic and restriction endonuclease analyses of new adenovirus types 19 and 37 causing acute conjunctivitis.

DNA restriction endonuclease cleavage analysis and serum neutralization test were performed in strains of adenovirus types 19 (Ad-19) and 37 (Ad-37) isolated from patients with acute conjunctivitis in Sapporo (Japan), Kaohsiung (Taiwan) and Pusan (South Korea) during the period of 1977 to 1984. Ad-19 and Ad-37 could be roughly differentiated, but a fair degree of cross reactivity was noted by serological test. By DNA restriction endonuclease analysis, Ad-19 isolates, Ad-19 prototype strain and Ad-37 strains were clearly differentiated. It was suggested by DNA cleavage patterns that both Ad-19 and Ad-37 isolates in East Asia are the same as those currently found in Europe and the USA. A new subtype of Ad-37 was found by DNA cleavage analysis among the isolates obtained in Sapporo and Kaohsiung in 1980.

Immune responses in monkeys to lenses from patients with contact lens induced giant papillary conjunctivitis.

The clinicopathologic findings in contact lens-induced giant papillary conjunctivitis (GPC) suggest that the syndrome is the result of a complex immunological process, an idea supported by the presence of elevated tear concentrations of IgG and IgE in GPC. Several groups of investigators have proposed that GPC may be due, in part, to the coating of the contact lens. To test this hypothesis we undertook development of an animal model of GPC in cynomolgus monkeys. Two soft contact lenses from patients with GPC, two from asymptomatic contact lens wearers, and two clean, unused lenses were each placed in one eye and held in place with a partial tarsorrhaphy. Tears from the two monkeys with GPC lenses showed increased levels of IgG (43 +/- 10 micrograms/mL), IgA (54.3 +/- 12.8 micrograms/mL) and IgE (7.7 +/- 3.3 IU/mL) 35-75 days post-lens placement. While the tears from the two monkeys with clean lenses, and the two monkeys with lenses from asymptomatic contact lens wearers had elevated levels of IgG compared to the contralateral control eye without a lens, the tear IgE levels remained normal. Histopathology studies of tarsal conjunctival biopsy material from the monkeys with GPC lenses showed an intense round cell infiltrate at the epithelial-stromal junction. Mast cells were seen in the epithelial layers. These studies suggest that some factor (or factors) in the lens coating from GPC patients was able to induce a local tear IgE response and histopathological changes in monkeys. These changes are similar to the histopathological and immunological findings in human patients with GPC.

Treatment of ligneous conjunctivitis.

A case of ligneous conjunctivitis, which presented in the acute form and converted to a mixed form after several months is described. Treatment with fibrinolysin was of value in reducing the reformation time of the membranes and in decreasing the viscosity of the serofibrinous discharge. The use of cryosurgery appeared to be of value in eliminating the membranes and granuloma and in markedly reducing the production and viscosity of the discharge.

[Studies on new adenovirus types of subgenus D causing conjunctivitis--antigenic and restriction endonuclease analysis of adenovirus types 19 and 37].

Neutralization test and DNA restriction endonuclease analysis were performed in strains of adenovirus types 19 (Ad-19) and 37 (Ad-37) isolated from patients with acute conjunctivitis in Sapporo, Kao-Hsiung (Taiwan) and Pusan (Korea) in the period of 1979 and 1984. Although Ad-19 and Ad-37 were differentiated, they were related to each other in neutralization test. There was no difference between prototype strain and isolates in Ad-19 and Ad-37 strains. By DNA restriction endonuclease analysis Ad-19 isolates were clearly distinct from Ad-19 prototype strain (AV-587) and Ad-37 strains. And it was suggested that these Ad-19 isolates from East Asia were as same as isolates from Europe named Ad-19a. Ad-37 isolated strains were as same as Ad-37 prototype strain (GW) by SmaI, SacI, SalI, BamHI, EcoRI, XhoI, HindIII restriction analysis. But some strains were distinct from prototype strain by only HindIII restriction analysis, and they were new subtype of Ad-37 which had never been reported.

A new clinical sign for the diagnosis of allergic conjunctivitis: a brief communication.

A clinical sign of the lower eyelid to be referred to as the "Mag Sign" or "Crescent Sign" is described, which when present is highly suggestive for conjunctivitides of allergic origin mostly due to aeroallergens (IgE-mediated type I allergic reaction). It is hoped that this sign will facilitate the diagnosis and treatment of allergic conjunctivitis.

The effect of topically applied fluorescein on fluorescent monoclonal antibodies in the diagnosis of chlamydial conjunctivitis.

Fluorescein-conjugated monoclonal antibodies have now been developed for use in the diagnosis of chlamydial conjunctivitis. This article discusses a clinical trial involving the use of such antibodies. We found that topical application of fluorescein before obtaining a specimen did not result in a statistically significant rate of false positives in a control group of patients.

Giant papillary conjunctivitis associated with rigid gas permeable contact lenses.

Giant papillary conjunctivitis (GPC) is an external ocular inflammatory disorder associated with contact lens wear. GPC seems to occur less frequently with hard than with soft contact lens wear. The management of soft contact lens related GPC has included refitting with hard lenses, specifically the newer rigid gas permeable (RGP) lenses. We retrospectively studied 14 RGP lens wearers in whom GPC developed. Three patients had had GPC associated with soft contact lens wear and had been fitted with RGP lenses in an effort to restore contact lens tolerance. The interval of time between the initiation of RGP lens wear and the onset of GPC was inversely related to the lens oxygen transmissibility (DK value). Ophthalmologists should be aware of the association between GPC and rigid gas permeable lens wear.
