[VDT pulse magnetic field enhances teratogenic effect of ara-c in mice].

Impregnated NIH mice were irradiated 4 hrs daily on days 16th-17th of gestation with 15.6 kHz pulse magnetic field at 40 microT peak to peak. Significant enhancement of ara-c induced teratogenic potency by the magnetic field can be observed. The incidence of harelip fetuses bearing harelip and/or cleft palate was significantly higher in the combined exposure group, in which mice were exposed to the magnetic field and given an intraperitoneal injection of ara-c on the 9th day of gestation, than that of mice treated with ara-c alone.

Issues in the etiology of recurrent spontaneous abortion.

Recurrent spontaneous abortion is seen often. Although many causes of this condition have been identified, there is still a large proportion in which no cause can be found. This review highlights some of the newly described proposed causes. Several reports have linked reproductive inefficiency to hypersecretion of luteinizing hormone (LH) in the follicular phase. It is apparent that the presence of inappropriate elevation of LH, and not the ultrasonographic appearance of polycystic ovaries, is the critical factor in determining the risk of abortion. Consequently, by lowering the LH levels one can expect a better reproductive outcome. This hypothesis awaits testing through clinical trials. The role of ultrasonography in evaluating the luteal phase, by assessing the image of the endometrium, has been studied by many investigators. This non-invasive technique is not yet able to replace the endometrial biopsy. The association between antisperm antibodies in the female serum and recurrent spontaneous abortion requires re-evaluation in light of more recent evidence; however, the role of semen abnormalities, especially severe oligospermia in this disorder, points towards a factor in the male partner that should be identified. Oligomenorrhea has been identified by mathematical modeling as a potential risk factor for recurrent abortion, although the mechanism has not been clearly elucidated. Chromosome heteromorphisms, particularly inv (9)(p11 q12), appear to be associated, more frequently, with recurrent abortion. Similarly, associations were observed with factor-XII deficiency and high titers of immunoglobulin G (IgG) antibody to Chlamydia trachomatis.(ABSTRACT TRUNCATED AT 250 WORDS)

Point of care technology: impact on people and paperwork.

A systematic evaluation of a bedside computer system for nursing documentation indicates that point of care technology has a positive direct impact on the efficiency, effectiveness, and satisfaction of the nursing staff, and a positive indirect influence on other members of the health care team in their delivery of patient care.

Redesigning nursing practice. The partners in patient care model.

Several nursing practice models use nurse extenders/partners to extend nurses' time. The authors discuss the development, implementation, and evaluation of the "partner in patient care" (PIPC) model. Unique aspects of the model are emphasized, including use of a collaborative process to generate the design and guide the implementation; staff participation throughout the design and implementation of the project; and use of evaluation at each phase of the implementation process to improve on the outcomes.

Autosomal recessive alobar holoprosencephaly with cyclops in three female sibs: prenatal ultrasonographic diagnosis at 18th week.

A couple who were first cousins were referred for genetic counselling because of a previous spontaneous abortion and two female stillbirths with cyclopia. In the 18th week of the mother's fifth pregnancy a holoprosencephalic fetus was diagnosed by ultrasonography. The pregnancy was terminated. Postmortem computed tomography and pathological examination revealed that the female fetus had alobar holoprosencephaly with cyclopia. Chromosomal analysis of the fetus was normal.

[Computer display work and rest position of the vergence system].

Subjects with different characteristics of the vergence system resting state (close or distant resting vergence, large and small increase upon depression of the eyes) worked with a VDU in a relatively high and a relatively low position for 1 1/2 hours. The directions of gaze were about -5 degrees and -35 degrees below the horizontal. A shorter observation distance was chosen with the lower VDU position than with the higher one, and it increased during the course of the work. In the high VDU position the increase was particularly strong for subjects with a distant resting vergence. The discrepancy between the angle of convergence during visual work and the resting vergence was larger in subjects with a distant resting vergence than in subjects with close one, and it was larger with the higher than with the lower VDU position (except in subjects with a minimal effect of eye depression on resting vergence). However, corresponding differences in visual fatigue as assessed by means of a questionnaire could not be found. Nevertheless the majority of subjects preferred the low VDU position. A number of additional consequences of a low VDU position support the conclusion that it would be advantageous for many users to tilt the display backward and to sink it below the level of the table surface.

A clear policy: eyesight testing.

Like it or not employers are now required to provide employee eye and eyesight tests on request. Following last month's article on the Display Screen Equipment Regulations Hazel Montague looks at this specific duty and describes how one company has taken a proactive approach to setting up a vision screening programme.
