Two-step orchiopexy with pelviscopic clip ligation of the spermatic vessels.

Ten boys with nonpalpable testes located in the peritoneal cavity underwent celiopelviscopy and clip ligation of the spermatic vessels with the intent of augmenting vasal collateral blood flow. After intervals in excess of 6 months, 7 patients underwent vas-based orchiopexy following division of the occluded spermatic vessels. Six boys had good results as judged by size of the relocated testes. In 1 boy the testis atrophied. He had undergone previous inguinal-only exploration before referral to us for celiopelviscopy and at stage 2 orchiopexy a silk suture was found on the vas deferens.

The change of vascular reactivity to angiotensin II and norepinephrine in the two-kidney, one-clip renovascular hypertensive rabbit.

The change of the response to angiotensin II (AII) and norepinephrine (NE) was evaluated in vivo and vitro in the chronic phase of two-kidney, one-clip renovascular hypertensive rabbits. In the constricted group, systemic blood pressure (BP) was significantly higher and plasma renin activity (PRA) was significantly lower than in the control group. Subpressor doses of AII and NE injections in the constricted group produced significant elevations of BP. In the aortic, renal and iliac arterial strips, the reactivity to AII (10(-10) to 10(-8) M) and NE (10(-10) to 10(-7) M) was significantly increased in the constricted group as compared to the control group. In the constricted group, sodium loading produced BP elevation with decreased PRA and shifted the dose-response curves by AII and NE to the left, whereas sodium restriction decreased BP and shifted the curves to the right. In the control group, altered sodium intake did not affect BP but affected only the dose-response curves by AII. These results suggested that in the chronic phase of renovascular hypertensive rabbits, the increased reactivity to AII and NE may contribute to the maintenance of hypertension.

Participation of central norepinephrine in the development of two-kidney, one clip Goldblatt hypertension.

Two series of experiments were conducted to ascertain whether the central catecholamines would participate in the elevation of blood pressure in two-kidney, one clip (2K-1C) Goldblatt hypertensive rats. The effect of 6-hydroxydopamine (6-OHDA) injected intraventricularly (250 micrograms/kg.) before the induction of hypertension (acute phase) or after the establishment of hypertension (chronic phase) was examined in 2K-1C rats and sham rats. The evidence has been presented: 1) that the hypothalamic norepinephrine content of 2K-1C rats in acute phase is 50 per cent lower (p less than 0.001) than that of sham rats, but norepinephrine content in the pons is similar in these animals, 2) that the content of dopamine in hypothalamus or pons is similar in 2K-1C rats and normotensive age-matched sham rats in acute phase, 3) that the content of these monoamines either in hypothalamus or pons in 2K-1C rats in chronic phase is almost identical with that in sham rats, 4) that administration of 6-OHDA markedly reduced the development of hypertension in 2K-1C rats in the acute phase, but did not affect their sustained hypertension in the chronic phase. These findings suggest that hypothalamic norepinephrine participates in the development of hypertension in 2K-1C rats in the acute phase but is not connected with the maintenance of their established hypertension in the chronic phase.

Complications with the four-loop lens of Binkhorst (the iris-clip lens).

The iris-clip (four-loop) lens of Binkhorst has been and continues to be the best intraocular lens on the market. It is recommended for primary or secondary implantation in an eye with a suitable iris sphincter and anterior chamber. Complications may occur, but these can be minimized by appropriate treatment and, particularly, by case selection.

The platinum clip (Platina) intraocular lens.

The platinum clip (Platina) intraocular lens is reserved for the patient with an active mobile pupil when some type of fixation device is necessary to avoid the complication of dislocation. A surgical technique is described in which the surgeon fixates the lens with the cataract wound closed (closed-eye system), thereby allowing the procedure to be carried out safely.

Management of hemorrhage from severe scalp lacerations with Raney clips.

A 23-year-old man was assaulted about the head and face with a wrench, sustaining multiple severe scalp lacerations. Massive bleeding from the lacerations ensued, resulting in refractory hypovolemic shock. Emergency department control of this hemorrhage was accomplished by the application of Raney clips to the edges of the scalp lacerations. Following stabilization and radiologic studies, the patient was admitted to the ICU, where his scalp wounds were managed definitively. The remainder of his hospital course was unremarkable, and he was discharged on the third hospital day to be followed in the surgical clinic as an outpatient.

The antihypertensive effect of garlic (Allium sativum) in the rat two-kidney--one-clip Goldblatt model.

A study was designed to elucidate the antihypertensive effect of garlic in the two-kidney--one-clip (2K-1C) Goldblatt model. Since the hypertension in this model depends largely on the increasing concentrations of vasopressor agents (prostaglandins and angiotensin II) there is a strong indication that reducing these factors could contribute in controlling the pathological rise in blood pressure. In this study, single or multiple doses of 0.5 ml of aqueous extract of garlic were given orally to 2K-1C rats. The data shows that the single dose of garlic used had a maximum antihypertensive effect 2-6 h after administration. The residual effect of this single dose continued for up to 24 h. The multiple dose of garlic appeared to be effective in restraining the expected rise in blood pressure that normally occurs in 2K-1C rats. Switching multiple-dose treatments halfway through the experiment further strengthened the findings. The study suggests that garlic does have an effective antihypertensive ability, and may be used as a supplementary and natural remedy in cases of unilateral renovascular hypertension.

Effects of N omega-nitro-L-arginine and N-acetyl-L-cysteine on the reversal of one-kidney, one-clip hypertension.

The present study evaluated whether nitric oxide (NO) synthesis blockade or potentiation (with N omega-nitro-L-arginine or N-acetyl-L-cysteine, respectively) modulates the systemic and renal responses to unclipping in anesthetized one-kidney, one-clip hypertensive rats (1K-1C). Cardiac output was measured by thermodilution. In time-control rats, mean arterial pressure (MAP) decreased from 197 +/- 8 mm Hg to 139 +/- 4 mm Hg 3 h after unclipping, and cardiac index (CI) decreased by 35%, with a transient rise in sodium and water excretion and no changes in total peripheral resistance (TPR), glomerular filtration rate (GFR), or renal plasma flow (RPF). Administration of N omega-nitro-L-arginine methyl ester (NAME, 10 micrograms/kg/ min) blunted the hypotensive (from 190 +/- 6 mm Hg to 157 +/- 3 mm Hg), diuretic and natriuretic responses and potentiated the decrease in CI (40%) observed after unclipping, whereas TPR increased by 103%. Also, in rats given NAME, GFR and RPF decreased by 20% and 45%, respectively, at the end of the experiment. The effect of N-acetyl-L-cysteine (NAC, 300 mg/kg), a sulfhydryl group donor that may protect NO from free radical destruction by forming an S-nitrosothiol compound, was also evaluated. NAC potentiated the depressor response to unclipping (from 180 +/- 5 mm Hg to 97 +/- 3 mm Hg), and GFR and RPF increased by 80% and 35%, respectively. These effects of NAC appear to be NO dependent, as they were blocked by simultaneous administration of NAME. However, no significant differences were observed among groups in cumulative excretion of sodium and water, demonstrating that the hemodynamic effects of NAME and NAC after unclipping are due to mechanisms other than renal excretory changes. The results of the present study indicate that the cardiovascular depressor effects of unclipping are modulated by endothelium-derived nitric oxide.

Effects on regional renal blood flow when unclipping a two-kidney, one-clip hypertensive Wistar rat during renal nerve stimulation.

Blood pressure (BP) is rapidly normalized when removing the obstruction from the renal artery of a two-kidney, one-clip renovascular hypertensive rat (unclipping). This study tested whether efferent renal nerve stimulation (ERNS) of the unclipped kidney affects this drop in BP or the associated changes in diuresis-natriuresis and regional renal blood flow. Three groups of anesthetized renovascular hypertensive Wistar rats were studied: 1) W(C) (time control); 2) W(UC) (unclipped after 30 min); and 3) W(UC+NS) (unclipped after 30 min, with ERNS at 5 Hz for 2 h). Renal excretion and regional hemodynamics (laser Doppler) were monitored in the unclipped kidney. Medullary and cortical blood perfusion increased by 84% and 95%, respectively, in W(UC) 30 min after unclipping (P < .001) but only with 8% and 9%, respectively, in W(UC+NS) (P = NS). Unclipping induced a marked increase in diuresis-natriuresis that was largely unaffected by ERNS. In W(UC) and W(UC+NS) BP returned to normotensive levels within 4 h. However, during the first 30 min, average BP decreased significantly less in W(UC+NS) (9%, 20 mm Hg) than in W(UC) (16%, 35 mm Hg) (P < .05). ERNS at 5 Hz effectively prevented the increase in medullary blood perfusion but did not affect the fall in blood pressure or the pressure diuretic/natriuretic response seen after unclipping. The results suggest that both the reduction in BP and the pressure-induced increase in diuresis/natriuresis seen when unclipping the 2K,1C renovascular hypertensive rat occurs largely independently of ERNS and an increase in medullary blood perfusion.

Molecular lipophilicity potential by CLIP, a reliable tool for the description of the 3D distribution of lipophilicity: application to 3-phenyloxazolidin-2-one, a prototype series of reversible MAOA inhibitors.

The capacity factor of eleven derivatives belonging to a prototype series of 3-phenyloxazolidin-2-one, reversible MAO inhibitors, was measured and compared to the calculated log Pcalc using the CLIP package. We demonstrate that this Molecular Lipophilicity Potential (MLP) approach is a valuable tool to estimate log Pcalc of such compounds.

Microtubule capture: IQGAP and CLIP-170 expand the repertoire.

Rho GTPases regulate microtubule capture near the cell cortex to polarize cells. What is surprising is the repertoire of interactions between proteins at the ends of microtubules and their cortical targets. The microtubule tip protein CLIP-170 has now been found to interact with the Cdc42/Rac effector IQGAP and mediate transient capture of microtubules.

Laparoscopic ligation of hypogastric arteries using the Hulka clip to arrest massive vaginal bleeding due to stage IIb cervical carcinoma.

A woman experienced severe vaginal bleeding from an exophytic, eroded cervical carcinoma. When vaginal tamponade and blood transfusion were ineffective in stopping the hemorrhage, bilateral laparoscopic ligation of the hypogastric arteries was performed. A Hulka clip was used to collapse the artery walls. Two months post-operatively a second, minor episode of bleeding occurred. This might have been avoided by ligating the ovarian-uterine anastomosis.

Three-vessel occlusion using a micro-clip for the proximal left middle cerebral artery produces a reliable neocortical infarct in rats.

We demonstrate a new three-vessel occlusion model of temporary focal ischemia in Sprague-Dawley rats (n=13). Under a surgical microscope, bilateral carotid arteries and the left middle cerebral artery (MCA) were occluded for 2 h using a snare for the neck and a micro-clip for the MCA. The reduction of regional cerebral blood flow (rCBF) monitored by laser Doppler flowmetry was 28% on average compared to the preischemic control value (100%) during ischemia. At that level of CBF reduction, there was no thrombus formation in the proximal MCA trunk, and a full recovery of rCBF was confirmed in the ischemic core within 5 min of reflow. Two days later, the cerebral infarct was limited to the neocortex, and the size and variability of the infarct are considered feasible for the assessment of any therapeutic challenge against the ischemic damage due to temporary focal ischemia in this model.

CLIP: similarity searching of 3D databases using clique detection.

This paper describes a program for 3D similarity searching, called CLIP (for Candidate Ligand Identification Program), that uses the Bron-Kerbosch clique detection algorithm to find those structures in a file that have large structures in common with a target structure. Structures are characterized by the geometric arrangement of pharmacophore points and the similarity between two structures calculated using modifications of the Simpson and Tanimoto association coefficients. This modification takes into account the fact that a distance tolerance is required to ensure that pairs of interatomic distances can be regarded as equivalent during the clique-construction stage of the matching algorithm. Experiments with HIV assay data demonstrate the effectiveness and the efficiency of this approach to virtual screening.

Synthesis and crystal structure of two new discrete, neutral complexes of manganese and zinc using a rigid organic clip.

Two discrete neutral dimanganese(II) and tetrazinc(II) complexes were synthesized from a rigid organic clip and the corresponding metal acetates. The compounds were characterized by elemental analysis and single crystal X-ray diffraction study. The manganese species is a dinuclear discrete product with two disordered acetates bridging two manganese centers, while the zinc one consists of two octahedral and two tetrahedral Zn(II) centers with both bridging acetates and triply micro(3)-hydroxides. Variable temperature magnetic measurement reveals the existence of weak antiferromagnetic interaction (J = -1.6 cm(-)(1); H = -2JS(1).S(2)) within the manganese complex.

Heterometallic compounds assembled from ferrocene-containing bisthiosemicarbazone clips.

Heteropolynuclear organometallic compounds have been constructed by using the ferrocene-based ligand H(2)L, [NH(2)SCNHN=C(CH(3))(C(5)H(4))](2)Fe. Reaction of the ligand H(2)L with the cobalt(II) salt gave a tetranuclear helicate Co(2)Fe(2) (1) with two ferrocene-based clips wrapped around the two cobalt atoms. The chiral helicates recognized the neighbors with same chirality through intermolecular hydrogen bonding between the thiosemicarbazone moieties to form a one-dimensional chiral channel. Reaction of the ligand H(2)L with the cadmium(II) salt afforded a tetranuclear dimeric compound Cd(2)Fe(2) (2), with two symmetric related parts bridged through two sulfur atoms. Reaction of the ligand H(2)L with the copper(I) salt, in the presence of triethylamine, resulted in a hexanuclear compound Cu(4)Fe(2) (3) with a crownlike Cu(4)S(4) octagon similar to that of S(8) sited inside and two ferrocene-containing ligands positioned outside. Electrochemical measurements were displayed to investigate the redox communications between the ferrocene moieties through metal centers.

Cyclic vinylogous TTF: a potential molecular clip triggered by electron transfer.

Cyclic vinylogous tetrathiafulvalenes (TTFs) where the two dithiole rings are linked through the outer sulfur atoms with an alkyl chain of various lengths were synthesized by oxidative intramolecular coupling of bis(dithiafulvenes). Upon oxidation, these systems exhibit large molecular movements associated with electron transfers. Their electrochemical behaviors were investigated together with the X-ray crystallographic structures of several derivatives and compared with molecular geometry calculations. Dependent on the length of the alkyl chain, either a stretch or a clip movement can be observed. These conformational modifications were found to be fast and fully reversible and required only low oxidation potentials. The movements seem to be almost insensitive to the nature of the substituents or its steric hindrance on the central conjugated spacer.

Selective activation of two sites in RNA by acridine-bearing oligonucleotides for clipping of designated RNA fragments.

Artificial enzymes for selective scission of RNA at two designated sites, which are valuable for advanced RNA science, have been prepared by combining lanthanide(III) ion with an oligonucleotide bearing two acridine groups. When these modified oligonucleotides form heteroduplexes with substrate RNA, the two phosphodiester linkages in front of the acridines are selectively activated and preferentially hydrolyzed by lanthanide ion. This two-site RNA scission does not require any specific RNA sequence at the scission sites, and the length of clipped RNA fragment is easily and precisely controllable by changing the distance between two acridine groups in the modified oligonucleotide. The two-site scission is also successful even when the substrate RNA has higher-order structures. By using these two-site RNA cutters, RNA fragments of predetermined length were obtained from long RNA substrates and analyzed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). Single nucleotide polymorphisms in homozygous and heterozygous samples were accurately and easily detected in terms of the difference in mass number. Multiplex analyses of in vitro transcripts from human genome were also successful.
