[Estradiol-progesterone interaction in normal and pathological human breast cells].

In most target tissues of the female genital tract, an adequate cell differentiation can be obtained with the successive and synergistic action of estradiol (E2) and progesterone (P), essentially because the progesterone receptor (PR) synthesis implicates the previous action of E2 via its E2 receptor (ER). In normal breast, E2 stimulates the growth of the ductal system whereas the development of acini depends on P secretion. In other words, when E2 plus P are secreted by the ovaries in balanced proportions, the two hormones permit a complete and harmonious development of the mammary gland. The antiestrogenic activity of P is carried out through the decrease of ER resynthesis and stimulation of 17 beta-hydroxysteroid dehydrogenase enzyme activity, which transforms E2 into its less active metabolite estrone (E1) in the target cells. These biochemical events are well documented concerning the endometrium. They have also been observed in normal mammary cells in primary cultures as well as in breast fibroadenomas with high epithelial cellularity. Moreover, data from literature indicate that E2 could be both a direct and indirect factor of cell multiplication in cancerous cell lines. P as well as progestins have the opposite effect. Recent results from this laboratory indicate that E2 and P also have antagonistic effects on the cell multiplication of normal human mammary cells in primary culture. Therefore, the hypothesis that a lack of P during a long period of the female genital like could be a factor in the promotion of breast cancer must be considered.

[Merkel cell carcinoma with metastasis to the bone marrow].

The information concerning the concept of Merkel cell-neuron as a mechanoreceptor is revised. We are presenting an autopsy case in which a Merkel cell carcinoma infiltrated bone marrow and provoked its hypoplasia. An analysis of the literature let us see that the behavior of this neoplasia can be quiescent, but also could be very aggressive, with development of distant metastasis to lymph node, skin, liver, brain, bone and lung. These tumors can give origin to a paraneoplasic syndrome.

[Features of the extracellular matrix of fibroadenomas and leaf-shaped tumors of the human breast (immunomorphologic study)].

Fibronectin and collagen I, III, IV, V distribution in preexisting and newly formed connective tissue of fibroadenomas and leaf-like human breast tumour was studied by the immunofluorescent method. Extracellular matrix of a newly formed connective tissue was characterized by the predominance of collagen type III, the presence of collagen type IV, a high content of fibronectin in a leaf-like tumour. Such characteristics of the extracellular matrix structure in benign tumours are interpreted as a manifestation of desmoplasia.

[Pathology of breast tumors occurring before the age of 30 in Brazzaville].

In 42 months, 175 patients (171 women and 4 men) were followed for mammary tumor in the Department of Medicine and Cancerology of l'Hopital General of Brazzaville. Among them, 63 files were retained for the study, corresponding to women under the age of 30. The study of these cases enabled the authors to establish the relatively important frequency of large tumors (larger than 5 cm), and the similarity of the general aspects of the pathology of mammary tumors, as compared to european studies.

Cytophotometric measurements of nuclear DNA content values in human breast lesions.

Quantitative measurements of nuclear DNA content values have been evaluated in cytological smears from 33 women with normal breast, fibrocystic disease, fibroadenoma and different types of breast carcinomas. The DNA amount for normal breast was found to be in the diploid region. For fibrocystic disease and fibroadenoma the cytochemical data were established to range from diploid to tetraploid regions, with peaks at 2 C and 3 C, respectively. For the different types of malignant breast lesions the quantitative DNA amounts were presented in the various regions of the histograms: from diploid to tetraploid for papillar carcinomas and metaplastic carcinomas--fusiformcellular and epidermoid types, with peaks at 2 C; from diploid to octaploid for intracanalicular carcinomas with peaks at 2 C, 4 C and 6 C. Dynamic changes in the pathway from the normal to cancer have been determined after quantitative cytochemical measurements of various benign and malignant breast lesions.

Mammary masses in the adolescent female.

Disease of the breast is uncommon in the adolescent female. Of 25,650 surgical specimens examined at The Children's Memorial Hospital in Chicago from 1974 through 1985, only 40 lesions were of adolescent breast origin in females. The ages of the patients ranged from 12 to 22 years. The most common lesion was fibroadenoma (19) followed by gigantomastia and cystosarcoma phyllodes. A malignant lesion is rare in this age group and both instances in this series had predisposing factors (for example, prior regional radiation in childhood). The findings of this study, plus that of the literature reviewed, emphasize conservative treatment with observation followed by simple excision when the patient nears completion of the development of the breast in most instances.

[Clinicopathological study of 9 cases presenting axillary masses only].

A clinicopathological study of 9 cases presenting axillary mass alone was reported. Histological diagnosis revealed 3 occult breast cancers, 2 apocrine gland adenocarcinomas of the skin, a carcinoma and a fibroadenoma in the axillary accessory breast, a neurogenic tumor in the axilla, and a benign lymphadenopathy. There were no clinical characteristics helpful in preoperative differential diagnosis. As the axilla includes the main lymphatic routes from the breast, the axillary tail of the breast and accessory breast, breast-associated tumors should be kept in mind in the management of patients with axillary mass as a sole clinical feature.

Altered monocyte function in patients with benign breast disease.

Monocyte migration, lysozyme production and phagocytosis was studied in 34 patients with fibroadenosis, 28 patients with fibroadenoma and 48 healthy female controls. In patients with fibroadenosis and fibroadenoma, monocyte migration and phagocytic activity were significantly reduced when compared to controls (P less than 0.001). Conversely, lysozyme production by monocytes from patients with benign breast disease was significantly higher than in controls (P less than 0.001). In 20 patients with benign breast disease, there was no significant difference in monocyte function before and 3 months after operation. The observed impairment of monocyte function in fibroadenosis and fibroadenoma would not appear to be the result of abnormal blood biochemistry or due to a direct serum inhibitor, but is probably related to an intrinsic cellular defect. Further studies are required to evaluate the significance of impaired monocyte function in the pathophysiology of benign breast disease.

The establishment and operation of a computerized data bank in benign breast disease. (Preliminary results on 297 cases).

The principles of setting up and utilization of a data bank for breast benign pathology and medical gynecology are described. In presenting these principles the authors make an analysis of the loading phases with the two afferent programs for adding, changing and wiping out information. With respect to utilization, two applicative programs for computing the efficiency of the selective hormonal treatment (regarding each and all patients) are analyzed. The paper provides the necessary instructions which enable the specialist physician to work directly with the computer. The principles are exemplified by the results obtained with a first series of 297 menstruated patients with several clinical forms of benign breast disease (unique or multiple cysts, lesions corresponding to an adenoma or fibroadenoma, mammary lumps) hormonally treated in an average series of 3 therapeutic cycles.

Breast pathology in MRI. First results.

The authors present the first results of the assessment of MRI (0.35 T) with adapted surface reel in the exploration of mammary pathology in 90 cases. On a morphological level, MRI permits a new analysis of signs of dystrophy. As far as MRI is concerned, no specific sign of mammary neoplasia seems to exist. A classification is suggested, taking into account morphology and parameters of relaxation.

[Mullerian duct cysts. A case report].

Muellerian duct cysts are rarely occurring, cystic lesions in the true pelvis, differing in size, derived from or continuous with the prostate and seminal vesicles. They mostly manifest themselves between the ages of 30-40 years. The most common symptoms are related to irritability and obstruction of the lower urinary tract as well as the rectum. Therapy is troublesome because of incomplete resection resulting in frequent recurrences and iatrogenic damages including incontinence and impotence. One case of Muellerian duct cyst with a history of several operations is reported.
