Cortical excitability threshold for distal limb muscles in transcranial magnetic stimulation. Method--normative values.

Transcranial magnetic stimulation was carried out in 30 healthy subjects, aged 20 to 55 and 156 to 180 cm tall, in whom cortical excitability threshold was determined for musculus (m.) abductor digiti minimi (ADM) and m. extensor digitorum brevis (EDB). The lowest stimulus intensity causing in the muscle a reproducible potential of an amplitude of 0.1 mV was adopted as the threshold value. The intensity of the stimulus was noted as a percentage of maximum stimulator output, defined as 100% (Magstim 200). In the stimulation of cortical areas for ADM, a circular coil (2.0T-530 V/m) was used while the in case of EBD, a double-cone coil (1.4T-660 V/m) was used. The threshold was noted while the muscle was relaxed and during weak voluntary contraction. The application of the double-cone coil allowed us to obtain for foot muscles similar threshold values as for hand muscles with a circular coil. It was confirmed that the threshold declines with voluntary contraction of the muscles under investigation. Normative values were prepared, with the norm, limits of the norm, outlier values and pathological markers being distinguished.

Odor thresholds of microbially induced off-flavor compounds in apple juice.

Microbially derived off-flavor is a major problem in apple juice production as it diminishes the sensory quality of the juice significantly. Fifteen relevant off-flavor compounds that are formed in apple juice, for example, by the strains Alicyclobacillus acidoterrestris and Actinomycetes (Streptomyces ssp.) were investigated with respect to their sensory relevance. The odor threshold values (i.e., detection and recognition values) were determined for all compounds in the matrix apple juice. Odor threshold values for fenchyl alcohol are reported here for the first time. The obtained values were set in relation to the limits of detection and quantification of a previously published GC-MS method. Eight tainted apple juice samples were analyzed for the presence of the 2 strains and the 15 off-flavor compounds. Both strains could be found in the samples; the presence of Streptomyces ssp. as spoilage bacteria of apple juice is reported for the first time. In samples with distinct off-flavor, 2-isopropyl-3-methoxypyrazine, 2-isobutyl-3-methoxypyrazine, 2-methylisoborneol, 1-octen-3-ol, fenchyl alcohol, geosmin, and guaiacol as well as 2,6-dibromophenol were determined in concentrations higher than the detection threshold.

[Destruction of confidence in established jurisprudence--1.1 promille as the new limit value of absolute inability to drive?].

The author, presiding judge of the fourth criminal panel of the German Federal Court, declares himself for a reduction of the threshold value concerning the absolute driving unfitness from the current 1.3 blood alcohol standard to 1.1. With his contribution, which for the same reason appears in different special periodicals, he tries to call a broad public's attention to the impending change in jurisdiction so that relevant drivers can prepare for it in time.

Use of the signal-averaged QRS duration for diagnosing left ventricular hypertrophy in hypertensive patients.

We analysed the diagnostic performance of the signal-averaged QRS duration for the detection of left ventricular hypertrophy in 100 consecutive outpatients (62 men and 38 women; mean age, 49.8 +/- 11.8 years) with essential hypertension and compared the results with some of the currently employed electrocardiographic criteria. Forty-eight healthy subjects (24 men and 24 women; mean age, 46.4 +/- 12.1) with normal physical, electrocardiographic and echocardiographic findings served as a control group to derive normal reference values for signal-averaged QRS duration. Twenty-six (26%) hypertensives (22 men and 4 women) had left ventricular hypertrophy echocardiographically defined as a left ventricular mass > or = 261 g in men and > or = 172 g in women or left ventricular mass index > or = 125 g/m2 in men and > or = 112 g/m2 in women. The signal-averaged QRS duration was different in patients with than in those without left ventricular hypertrophy (102.1 +/- 10.8 vs. 95.8 +/- 8.4 ms; P < 0.01). Also, in the group with left ventricular hypertrophy QRS duration was longer, although not significantly different, in men than in women (103.5 +/- 10.7 vs. 94.2 +/- 8.8 ms; P n.s.). The correlation between the signal-averaged QRS duration and left ventricular mass was weak but statistically significant in men (r = 0.34; P < 0.05) in women (r = 0.30; P < 0.05) and in men and women together (r = 0.42; P < 0.01). Partition values of filtered QRS duration > or = 114 ms in men and > or = 107 ms in women were used to diagnose left ventricular hypertrophy as these values were above the upper limits in our control men and women when 95% confidence intervals were calculated. These criteria were insensitive (12%) but highly specific (99%) for left ventricular hypertrophy. The use of a single threshold value of filtered QRS duration > or = 111 ms in both sexes combined improved sensitivity modestly (15%) while maintaining a good specificity (95%). Also, we tested the following standard electrocardiographic criteria: the Sokolow-Lyon index, the Romhilt-Estes point score > or = 4 points and > or = 5 points, the Cornell voltage criteria, the sum of QRS voltages in all 12 leads > 175 mm, and the QRS duration > 90 ms. Sensitivities ranged from 4% to 58% and specificities from 74% to 99%.(ABSTRACT TRUNCATED AT 400 WORDS)

Resuscitation from out-of-hospital cardiac arrest: implications for cardiac enzyme estimation.

BACKGROUND: diagnosis of precipitating myocardial infarction is essential for management of victims of out-of-hospital cardiac arrest, since investigations and treatment are determined by the underlying cause. Skeletal muscle and myocardial damage from external cardiac massage and defibrillation may complicate biochemical diagnosis of myocardial infarction. OBJECTIVES: (a) to examine the relationship between cumulative defibrillation energy and serum levels of cardiac troponin T and MB creatine kinase (MB-CK) mass in out-of-hospital cardiac arrest survivors without electrocardiographic evidence of myocardial infarction; (b) to reassess diagnostic thresholds for myocardial infarction using MB-CK mass and troponin T in this setting. METHODS: 77 victims of out-of-hospital cardiac arrest were studied. Serum was obtained for MB-CK mass, CK and troponin T estimation on the first 4 days of admission. Patients were divided into three groups using electrocardiographic criteria: group 1, myocardial infarction; group 2, no evidence of infarction; and group 3, equivocal electrocardiograms. Correlation coefficients were calculated for highest recorded levels of the biochemical markers versus defibrillation energy. Receiver-operating characteristic plots were used to determine optimum biochemical diagnostic thresholds for subjects in groups 1 and 2. RESULTS: using predefined criteria, 27 patients had myocardial infarction, 34 did not have myocardial infarction and 16 had equivocal electrocardiograms. Significant correlations were found for defibrillation energy versus log troponin T (r = 0.42, P < 0.05), log MB-CK mass (r = 0.51, P < 0.01) and total CK (r = 0.68, P < 0.001) in group 2. Within groups 1 and 2, MB-CK mass and troponin T provided additional diagnostic value over MB-CK fraction (P < 0.001). Diagnostic accuracy was not improved by adjusting for shock energy. The optimum threshold value was 4 ng/ml for troponin T (sensitivity 88%, specificity 95%), 60 ng/ml for MB-CK mass (sensitivity 88%, specificity 88%) and 8% of total CK for MB-CK fraction (sensitivity 74%, specificity 82%). These values should be interpreted with caution, since this study is limited by the exclusion of patients with uncertain electrocardiographic diagnoses into group 3. CONCLUSIONS: skeletal muscle and myocardial damage occurs in survivors of out-of-hospital cardiac arrest and is related to the duration of resuscitation. This complicates biochemical diagnosis of underlying myocardial infarction. Specific high diagnostic threshold values for MB-CK and troponin T are needed to optimise diagnostic accuracy. The use of MB-CK fraction leads to greater diagnostic error because of the variability of muscle CK release after resuscitation.

Excessive length of iliac arteries in athletes with flow limitations measured by magnetic resonance angiography.

PURPOSE: Kinking of the iliac arteries can cause flow limitations in endurance athletes. Such kinking may be treated by surgical release of the iliac arteries. However, when the length of the iliac artery is excessive, this may not be effective. Because threshold values of excessive length of the iliac arteries are unknown, normal values for endurance athletes were obtained and abnormalities encountered in these patients are reported. METHODS: Forty-three endurance athletes (46 symptomatic legs) with flow limitations in the iliac arteries were examined using magnetic resonance angiography (MRA) with hips extended and flexed. The ratio of vessel length to straight-line distance was determined for the common and external iliac arteries. Sixteen national-level cyclists (32 reference legs) served as a control group. RESULTS: For the common iliac artery, length ratios were significantly (P < 0.05) higher in the symptomatic legs than in the reference legs (symptomatic legs: 1.1 +/- 0.12, 1.22 +/- 0.19, reference legs 1.05 +/- 0.04, 1.11 +/- 0.05 with extended and flexed hips, respectively). For the external iliac artery, only in the position with hips flexed, the ratios in the symptomatic legs were significantly higher than in the reference legs (symptomatic legs: 1.11 +/- 0.09, 1.44 +/- 0.23, reference legs 1.08 +/- 0.05, 1.32 +/- 0.13 with extended and flexed hips, respectively). A small proportion of symptomatic legs had extremely high length ratios. CONCLUSION: MRA is effective for determining vessel length. The ratio of vessel length to straight-line distance with extended and flexed hips is a good measure for excessive vessel length and achieves extreme values in a small subgroup of patients. Further prospective study is warranted to define maximal vessel length ratios, which still allow benefit from surgical release of the iliac arteries.

[Does the highest judicial ordinance for a new limit value for absolute driving incapacity (BGH 4 StR 297/90, Beschl. v. 28 June 1990) allow unequal treatment of alcohol intoxicated drivers?].

The jurisdiction in the BRD knows a threshold value of the blood alcohol concentration from which on the actual absolute driving unfitness without any further evidence will be assumed. This threshold value has been lowered by the highest german criminal court (BGH) from so far 1.3% to now 1.1 g%. As the determination of the alcohol content takes place in two different procedures as a mean value out of four or five single values, the highest court orders a dispersion control--the standard deviation from 0.03 g% or 0.04 g%. The authors proof that therefore a dissimilar treatment of motor traffic participants under the influence of alcohol may be possible. The reason is to be traced in the different distributions of a single values around the mean value and in the incorrect application of the standard deviation.

Stereopsis impairment during smooth pursuit eye tracking.

Coherent motion of random dot pattern across a stationary stereograting at 1-5.5 deg/s causes an impairment in perceiving the stereograting which is associated with optokinetic nystagmus. This study was aimed towards understanding the cause of the impairment. We tested two alternative hypotheses: (1) that the impairment is caused by efferent-afferent interactions and (2) that it is due to a temporal integration process, the effect of which is expressed at the temporal resolution limit of the stereoscopic mechanism to disparity alternation. The first hypothesis was rejected on the basis of modified displays and experimental conditions which clearly showed that in these displays stereopsis was not impaired in the presence of optokinetic nystagmus. In testing the second hypothesis, we first determined, for the original display, the threshold values of spatial frequency and angular velocity at which stereopsis ceased. We found for these values spatial frequency x angular velocity = 7.2 cycles/s, i.e., a constant limiting alternation rate for all angular velocity values tested. The averaging effect at the critical alternation rate was demonstrated by bisecting the display, each part having a different disparity value. The perceived depth levels of the two parts were different and are in accordance with an averaging process explanation. It is, therefore, argued that the cause of the impairment is a temporal integration process which averages the alternating disparity values of the moving dots.

Influenza A and B epidemic criteria based on time-series analysis of health services surveillance data.

Many countries now have epidemiological surveillance systems using health services-based indicators that allow detection of influenza epidemics. However, there is no accepted criterion for defining an influenza epidemic. An epidemic criterion has been developed, based on a time-series analysis of health services-based indicators collected on a weekly basis by a surveillance network implemented in the Paris region since 1984: the Groupe Regional d'Observation de la Grippe (GROG). For each new season, an epidemic threshold is independently defined for each health services-based indicator as the upper limit of the one-sided confidence interval of the expected value calculated from the weekly differences between the observed number of events and those predicted by a SARIMA model fitted on the non-epidemic data of previous seasons. Epidemic criteria for influenza A and B are then defined from the combination of both viral indicators and epidemic thresholds of individual health services-based indicators. Among health indicators, sick-leave data collected from GP's or the Health Insurance system, emergency home medical visits, and influenza-like-illness reported by GP's are the most sensitive indicators for the early recognition of epidemics. The exceeding of the above mentioned thresholds combined with virological data allows the specific detection of influenza A or B epidemics. This time-series method of analysing surveillance data provides early and reliable recognition of these epidemics.

An artificial neural network for predicting the incidence of radiation pneumonitis.

A method to predict radiation-induced pneumonitis (RP) using an artificial neural network (ANN) was investigated. A retrospective study was applied to the clinical data from 142 patients who have been treated with three-dimensional conformal radiotherapy for tumors in the thoracic region. These data were classified, based on their treatment outcome, into two patient clusters: with RP (Np=26) and without RP (Np= 116). An ANN was designed as a classifier. To perform the classification, a patient-treatment outcome with RP was assigned a value of 1, and a patient treatment outcome without RP was assigned a value of -1. The input of the ANN was limited to the patient lung dose-volume data only. A volume vector (VD) that describes patient lung subvolumes receiving more than a set of threshold doses was used as the network input variable. A zero value was used as the threshold to set the output value into -1 or 1. Three ANNs (ANN_1, ANN_2, and ANN_3), each with three layers, were trained to perform this classification function and to show the effect of training data on the ANN performance. Radial basis function was applied as the hidden layer neuron activation function and a sigmoid function was selected as the output layer neuron function. Backpropagation with a conjugate gradient algorithm was used to train the network. ANN_1 was trained and tested by using the leave-one-out method. ANN_2 was trained by randomly selecting 2/3 of the patient data, and tested by the remaining 1/3 of the data. ANN_3 was trained by a user selecting 2/3 of the patient data, and tested by the remaining 1/3 of the data. The predictive accuracy was verified as the area under a receiver operator characteristic (ROC) curve. The correct classification rates of 73% for RP cases, and 99% for non-RP cases were obtained from ANN_1. The corresponding correct classification rates of 44% for RP cases, and 89% for non-RP cases were obtained from ANN_2. From the ANN_3 test phase, the corresponding correct classification rates of 55% for RP cases, and 95% non-RP cases were achieved. The area under ROC curve was 0.85+/-0.05, 0.68+/-0.10, and 0.81+/-0.09 for ANN_1, ANN _2, and ANN_3, respectively, within its asymmetric 95% confidence interval. The sensitivity was 95%, 57%, and 71%, and the specificity was 94%, 88%, and 90% for ANN_1, ANN_2, and ANN_3, respectively. Preliminary results suggest that the ANN approach provides a useful tool for the prediction of radiation-induced lung pneumonitis, using the patient lung dose-volume information.

Nuclear morphometry as an important prognostic factor in stage I renal cell carcinoma.

Although 60% of Stage I renal carcinoma patients die from tumor within 5 years postoperatively, a considerable percentage survive that period. Nuclear grading can help to predict the outcome, but many of the patients are Grade 2, and the prognosis of this subclass is uncertain. Therefore, nuclear morphometry was carried out in 41 patients with Stage I renal cell carcinoma. Of these, 24 died within 5 years and 17 have survived that period. Using a mean nuclear area of 32 micron 2 as the decision threshold, none of the 24 short-term survivors are below that threshold and three of the long-term survivors exceed that value (18% false-positives) (99% confidence limit). Separate analysis with sets for learning and testing and Grade 2 patients gave similar results. The results show the essential prognostic value of morphometry in this set of patients with Stage I renal cell carcinoma.

Assessment of myocardial area at risk by technetium-99m sestamibi during coronary artery occlusion: comparison between three tomographic methods of quantification.

The aim of this study was to directly compare three currently used quantitative methods of analysis of technetium-99m sestamibi images in patients with selective balloon-induced transmural ischaemia. The area at risk (AR) was assessed in 19 patients undergoing single-vessel percutaneous transluminal coronary angioplasty by injecting the 99mTc-sestamibi at the time of coronary artery occlusion during balloon inflation. After imaging, the patients were classified according to localization of the perfusion defect as having anteroseptal (group I, 11 patients) or posterolateral defects (group II, eight patients). The planimetric technique based on polar maps, proposed by Verani et al. (J Am Coll Cardiol, 1988) (method A), the method described by Tamaki et al. (Circulation, 1982) (method B) and the technique validated by O Connor et al. (Eur J Nucl Med, 1990) (method C) were tested. Three threshold values of 45%, 50% and 60% of the maximum left ventricular count were used to define the limits of the perfusion defect. The mean values of the AR calculated by the three techniques with the original cut-off level (method A=16.5%+/-12.9; method B=10.4%+/-7.6%; method C=29.6%+/-15.7%) were statistically different (one-way analysis of variance: P<0.001; paired t test: method A vs B, P=0.003; method B vs C and method A vs C, P<0.0001). There was no significant difference between the mean values of the AR estimated by the three methods using the same cut-off levels. The use of 60% of the maximum left ventricular count provided the best correlation between the techniques (method A vs B, r=0.95; method B vs C, r=0.92; method A vs C, r=0.95). Nevertheless, a difference >10% between the values of AR assessed by the three methods was found in four subjects. There was no significant difference between the three methods in the evaluation of AR in the subjects of group I and group II. Reproducibility was good for all methods. It is concluded that the three methods of analysis of the AR by 99mTc-sestamibi SPET imaging showed comparable performance and good reproducibility using the same cut-off level. The location of perfusion defect does not affect the comparability of the three techniques. We suggest the use of a cut-off level of 60% for all three methods in the assessment of the AR by 99mTc-sestamibi SPET imaging.

Transmission of encephalomyocarditis virus (EMCV) among pigs experimentally quantified.

Two types of transmission experiments were performed to estimate the basic reproduction ratio R(0), indicating the level of encephalomyocarditis virus (EMCV) transmission among pigs. In a first experimental set-up with nine separate pairs, one randomly chosen piglet per pair was inoculated with a Belgian (myocardial) EMCV strain (B279/95, 10(3)TCID(50)/ml oronasally) and placed back into the pen. In the second experiment with two separate groups of five piglets, two piglets in each group were inoculated at the start. During the experiments, viraemia in blood and excretions was measured as well as the serological response against EMCV antigen. After death or euthanasia, the piglets were checked for heart lesions and virus isolation was done on various tissues. In both the experiments, the majority of the inoculated piglets either died with typical heart lesions (five out of nine and three out of four resp.), or produced high levels of neutralising antibody. EMC virus was isolated from the hearts of all piglets that died during either one of the experiments. The pairwise experiment revealed a point estimate for R(0) of 2.0 (95% confidence interval (CI)=0.37-10.74), while the group experiment resulted in a R(0)-value of 0.71 (95% CI=0.08-4.93). Combining the information from both experiments results in an estimate for R(0) of 1.24 (95% CI=0.39-4.35). Since R(0) has values around the threshold value of 1, the spread of EMCV due to contacts between pigs will in most cases be limited, but due to chance processes may lead to large outbreaks as well.

Evaluation of post-polymerase chain reaction melting temperature analysis for meat species identification in mixed DNA samples.

Real-time uniplex and duplex polymerase chain reaction (PCR) assays with a SYBR Green I post-PCR melting curve analysis were evaluated for the identification and quantification of bovine, porcine, horse, and wallaroo DNA in food products. Quantitative values were derived from threshold-cycle (C(t)) data obtained from serial dilutions of purified DNA. The limits of detection in uniplex reactions were 0.04 pg for porcine and wallaroo DNA and 0.4 pg for cattle and horse DNA. Species specificity of the PCR products was tested by the identification of peaks in DNA melting curves, measured as the decrease of SYBR Green I fluorescence at the dissociation temperature. The peaks could be distinguished above the background even at the lowest amount of template DNA detected by the C(t) method. The system was also tested in duplex reactions, by use of either single-species DNA or DNA admixtures containing different shares of two species. The minimum proportions of each DNA species allowing the resolution of T(m) peaks in the duplex reactions were 5% (cattle or wallaroo) in cattle/wallaroo mixtures, 5% porcine and 1% horse in porcine/horse mixtures, 60% porcine and 1% wallaroo in porcine/wallaroo mixtures, and 1% cattle and 5% horse in cattle/horse mixtures. A loss in the sensitivity of the method was observed for some DNA combinations in the duplex assay. In contrast, the results obtained from SYBR Green I uniplex and duplex reactions with single-species DNA were largely comparable to those obtained previously with species-specific TaqMan probes, showing the suitability of that simpler experimental approach for large-scale analytical applications.

Application of germline IGH probes in real-time quantitative PCR for the detection of minimal residual disease in acute lymphoblastic leukemia.

Large-scale clinical studies on detection of minimal residual disease (MRD) in acute lymphoblastic leukemia (ALL) have shown that quantification of MRD levels is needed for reliable MRD-based risk group classification. Recently, we have shown that 'real-time' quantitative PCR (RQ-PCR) can be applied for this purpose using patient-specific immunoglobulin (Ig) and T cell receptor (TCR) gene rearrangements as PCR targets with TaqMan probes at the position of the junctional region and two germline primers. Now, we tested an alternative approach on 35 immunoglobulin heavy chain (IGH) gene rearrangements, by designing three germline JH TaqMan probes to be used in combination with one of six corresponding germline JH primers and one allele specific oligonucleotide (ASO) primer complementary to the junctional region. In nine cases in which both approaches were compared, at least similar (n = 4) or slightly higher (n= 5) maximal sensitivities were obtained using an ASO primer. The ASO primer approach reached maximal sensitivities of at least 10(-4) in 33 out of 35 IGH rearrangements. The reproducible range for accurate quantification spanned four to five orders of magnitude in 31 out of 35 cases. In 13 out of 35 rearrangements the stringency of PCR conditions had to be increased to remove or diminish background signals; this only concerned the frequently occurring JH4, JH5 and JH6 gene rearrangements. After optimization of the conditions (mainly by increasing the annealing temperature), only occasional aspecific amplification signals were observed at high threshold cycle (CT) values above 42 cycles and at least six cycles above the CT value of the detection limit. Hence, these rare aspecific signals could be easily discriminated from specific signals. We conclude that the here presented set of three germline JH Taq-Man probes and six corresponding germline JH primers can be used to develop patient-specific RQ-PCR assays, which allow accurate and sensitive MRD analysis in almost all IGH gene rearrangements. These results will facilitate standardized RQ-PCR analysis for MRD detection in large clinical studies.

Effects of PMCA and SERCA pump overexpression on the kinetics of cell Ca(2+) signalling.

The dynamic interactions of the main pathways for active Ca(2+) transport have been analysed in living cells by altering the expression of their components. The plasma membrane (PMCA) and the endoplasmic reticulum (ER) (SERCA) Ca(2+) pumps were transiently overexpressed in CHO cells, and the Ca(2+) homeostasis in the subcellular compartments was investigated using specifically targeted chimaeras of the Ca(2+)- sensitive photoprotein aequorin. In resting cells, overexpression of the PMCA and SERCA pumps caused a reduction and an increase in ER [Ca(2+)] levels, respectively, while no significant differences were detected in cytosolic and mitochondrial [Ca(2+)]. Upon stimulation with an inositol 1,4, 5-trisphosphate (IP(3))-generating agonist, the amplitude of the mitochondrial and cytosolic Ca(2+) rises correlated with the ER [Ca(2+)] only up to a threshold value, above which the feedback inhibition of the IP(3) channel by Ca(2+) appeared to be limiting.

Applicability of biomarkers in the early diagnosis of prostate cancer.

Early diagnosis of prostate cancer can increase the curative success rate for this disease. Although serum prostate-specific antigen measurement is regarded as the best conventional tumor marker available, there is little doubt that it has great limitations. The threshold above which biopsies are indicated has now decreased to a serum prostate-specific antigen value of 3 ng/ml, which results in a negative biopsy rate of 70-80%. This can readily be explained by the fact that prostate-specific antigen is not specific for prostate cancer. Clinicians need more sensitive tools to help diagnose prostate cancer and monitor progression of the disease. Molecular oncology is playing an increasing role in the fields of diagnosis and therapy for prostate cancer and has already been instrumental in elucidating many of the basic mechanisms underlying the development and progression of this disease. The identification of new prostate cancer-specific genes, such as DD3PCA3, would represent a considerable advance in the improvement of diagnostic tests for prostate cancer. This could subsequently lead to a reduction of the number of unnecessary biopsies.
