Effect of chronic Giardia lamblia infection on epithelial transport and barrier function in human duodenum.

BACKGROUND: Giardia lamblia causes infection of the small intestine, which leads to malabsorption and chronic diarrhoea. AIM: To characterise the inherent pathomechanisms of G lamblia infection. METHODS: Duodenal biopsy specimens from 13 patients with chronic giardiasis and from controls were obtained endoscopically. Short-circuit current (I(SC)) and mannitol fluxes were measured in miniaturised Ussing chambers. Epithelial and subepithelial resistances were determined by impedance spectroscopy. Mucosal morphometry was performed and tight junction proteins were characterised by immunoblotting. Apoptotic ratio was determined by terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick-end labelling staining. RESULTS: In giardiasis, mucosal surface area per unit serosa area was decreased to 75% (3%) of control, as a result of which epithelial resistance should increase. Instead, epithelial resistance of giardiasis biopsy specimens was decreased (19 (2) vs 25 (2) Omega cm(2); p<0.05) whereas mannitol flux was not significantly altered (140 (27) vs 105 (16) nmol/h/cm(2)). As structural correlate, reduced claudin 1 expression and increased epithelial apoptosis were detected. Furthermore, basal I(SC) increased from 191 (20) in control to 261 (12) microA/h/cm(2) in giardiasis. The bumetanide-sensitive portion of I(SC) in giardiasis was also increased (51 (5) vs 20 (9) microA/h/cm(2) in control; p<0.05). Finally, phlorizin-sensitive Na(+)-glucose symport was reduced in patients with giardiasis (121 (9) vs 83 (14) microA/h/cm(2)). CONCLUSIONS: G lamblia infection causes epithelial barrier dysfunction owing to down regulation of the tight junction protein claudin 1 and increased epithelial apoptoses. Na(+)-dependent d-glucose absorption is impaired and active electrogenic anion secretion is activated. Thus, the mechanisms of diarrhoea in human chronic giardiasis comprise leak flux, malabsorptive and secretory components.

Gluten specific, HLA-DQ restricted T cells from coeliac mucosa produce cytokines with Th1 or Th0 profile dominated by interferon gamma.

Coeliac disease is precipitated in susceptible subjects by ingestion of wheat gluten or gluten related prolamins from some other cereals. The disease is strongly associated with certain HLA-DQ heterodimers, for example, DQ2 (DQ alpha 1*0501, beta 1*0201) in most patients and apparently DQ8 (DQ alpha 1*0301, beta 1*0302) in a small subset. Gluten specific T cell clones (TCC) from coeliac intestinal lesions were recently established and found to be mainly restricted by HLA-DQ2 or HLA-DQ8. Antigen induced production of cytokines was studied in 15 TCC from three patients, 10 being DQ2 and five DQ8 restricted. Cell culture supernatants were prepared by stimulation with gluten peptides in the presence of DQ2+ or DQ8+ Epstein-Barr virus transformed B cells as antigen presenting cells (APC). Supernatants were analysed for cytokines by bioassays, ELISA, and CELISA. Cellular cytokine mRNA was analysed semi-quantitatively by slot blotting and polymerase chain reaction (PCR). All TCC were found to secrete interferon (IFN) gamma, often at high concentrations (> 2000 U/ml); some secreted in addition interleukin (IL) 4, IL 5, IL 6, IL 10, tumour necrosis factor (TNF), and transforming growth factor (TGF) beta. The last TCC thus displayed a Th0-like cytokine pattern. However, other TCC produced IFN gamma and TNF but no IL 4, or IL 5, compatible with a Th1-like pattern. In conclusion, most DQ8 restricted TCC seemed to fit with a Th0 profile whereas the DQ2 restricted TCC secreted cytokines more compatible with a Th1 pattern. The TCC supernatants induced upregulation of HLA-DR and secretory component (poly-Ig receptor) in the colonic adenocarcinoma cell line HT-29.E10, most probably reflecting mainly the high IFN gamma concentrations. This cytokine, particularly in combination with TNF alpha, might be involved in several pathological features of the coeliac lesion. The characterised cytokine profiles thus support the notion that mucosal T cells activated in situ by gluten in a DQ restricted fashion play a central part in the pathogenesis of coeliac disease.

Lymphocytic infiltration and survival in rectal cancer.

Lymphocytic infiltration was assessed semiquantitatively in 447 specimens of rectal cancer. Corrected five year survivals for pronounced, moderate, and little lymphocytic infiltration were 92%, 65% and 36%, respectively. Grading was shown to be reproducible in an intraobserver study (observed agreement 81%, expected agreement 34%, kappa coefficient 0.72). In the multivariate survival analysis of grade and stage related variables lymphocytic infiltration was the only grade related variable to be accepted within the prognostic model. This model also included the number of lymph node metastases and the extent of tumour spread. It is suggested that the conservation of the normal interactive traffic between epithelium and mucosa associated lymphoid tissue (MALT) signals a low grade growth. This is supported by the association between lymphocytic infiltration and the expression of secretory component by malignant epithelium.

Antibody to coliform antigens in urine samples from patients with symptoms of urinary tract infection.

An enzyme linked immunoadsorbent assay (ELISA) was used to detect IgG, IgM, and IgA antibody in urine to a crude antigenic mixture prepared from six arbitrarily chosen strains of common coliform urinary pathogens. In a group of patients with symptoms of urinary tract infection 24 of 80 (30%) urine samples were positive on culture by conventional methods, 58 of 80 (72.5%) were positive for polymorphs on microscopy, and 72 of 80 (90%) were positive for antibody by ELISA. Secretory component was detected in 28 of 80 (35%) samples, and this is consistent with local antibody production. The results show that antibody can be detected in a wide range of urine samples using a simple antigen mixture, and this may form the basis of a useful test to diagnose urinary tract infection.

Use of statistical evaluation of antigen profiles in differential diagnosis between colonic and ovarian adenocarcinomas.

A study was carried out to determine whether it was possible to classify colonic and ovarian adenocarcinomas by their antigen profile. Colonic and ovarian adenocarcinomas were immunostained with a panel of antibodies which have a limited specificity for colon (parlam-4, 19.9, anti-secretory component) and ovary (OV-TL3 and OC125) and the most discriminatory antibodies were selected by stepwise linear discriminant analysis. For frozen material OV-TL3 and OC125 were the best classifying antibodies. Although OC125 had better discriminative power, for paraffin wax embedded material parlam-4 was selected as the best classifying antibody. OC125 had no additional effect on the classification of a tumour. These antibodies were subsequently tested on an independent test set of primary and metastatic adenocarcinomas of colonic and ovarian origin. When ovarian posterior probabilities of less than 0.1 and greater than 0.9 were selected as cut off points for a positive identification of colonic or ovarian origin (jackknifed classification method), no adenocarcinoma was incorrectly identified as ovarian carcinoma in frozen material. The same trend was noticed for paraffin wax embedded material. Statistical analysis of antigen profiles can be helpful in defining the colonic or ovarian origin of an adenocarcinoma when routine microscopy does not yield a definitive result.

Suspected nasopharyngeal carcinoma in three workers with long-term exposure to sulphuric acid vapour.

Sulphuric acid vapour has been suspected of being an industrial carcinogen. In this study, a cluster is presented of three patients with nasopharyngeal carcinoma (NPC) who worked in the same building of a telecommunications conveyance station in southern Taiwan with long term exposure to sulphuric acid vapour concentrations as high as 0.18 mg/m3. All three workers were diagnosed with NPC within a 5 month period between September 1992, and March 1993. Compared with 19 other healthy workers from the same building, these three workers with NPC had worked significantly longer in this building than had the others (mean (SD) (years): 12.7 (0.6) v 7.4 (4.4); p = 0.01). With an in situ nucleic acid hybridisation and immunostaining method for colocalised Epstein-Barr virus (EBV) and secretory component (SC) protein among biopsy specimens of these three patients with NPCs, it was found that some tumour cells did not contain EBV and SC protein staining signals. These results indicate that EBV infection is not the only risk factor for NPC and long term exposure to relatively low concentrations of sulphuric acid vapour may be associated with the development of NPC.

Dominant-negative PKC-epsilon impairs apical actin remodeling in parallel with inhibition of carbachol-stimulated secretion in rabbit lacrimal acini.

We investigated the involvement of PKC-epsilon in apical actin remodeling in carbachol-stimulated exocytosis in reconstituted rabbit lacrimal acinar cells. Lacrimal acinar PKC-epsilon cosedimented with actin filaments in an actin filament binding assay. Stimulation of acini with carbachol (100 microM, 2-15 min) significantly (P < or = 0.05) increased PKC-epsilon recovery with actin filaments in two distinct biochemical assays, and confocal fluorescence microscopy showed a significant increase in PKC-epsilon association with apical actin in stimulated acini as evidenced by quantitative colocalization analysis. Overexpression of dominant-negative (DN) PKC-epsilon in lacrimal acini with replication-defective adenovirus (Ad) resulted in profound alterations in apical and basolateral actin filaments while significantly inhibiting carbachol-stimulated secretion of bulk protein and beta-hexosaminidase. The chemical inhibitor GF-109203X (10 microM, 3 h), which inhibits PKC-alpha, -beta, -delta, and -epsilon, also elicited more potent inhibition of carbachol-stimulated secretion relative to Gö-6976 (10 microM, 3 h), which inhibits only PKC-alpha and -beta. Transduction of lacrimal acini with Ad encoding syncollin-green fluorescent protein (GFP) resulted in labeling of secretory vesicles that were discharged in response to carbachol stimulation, whereas cotransduction of acini with Ad-DN-PKC-epsilon significantly inhibited carbachol-stimulated release of syncollin-GFP. Carbachol also increased the recovery of secretory component in culture medium, whereas Ad-DN-PKC-epsilon transduction suppressed its carbachol-stimulated release. We propose that DN-PKC-epsilon alters lacrimal acinar apical actin remodeling, leading to inhibition of stimulated exocytosis and transcytosis.

Migrating action potential complex of cholera: a possible prostaglandin-induced response.

Distal ileal loops of New Zealand white rabbits exposed to cholera toxin demonstrated the presence of a highly organized myoelectric pattern defined as the migrating action potential complex (MAPC). We investigated the mechanism by which cholera enterotoxin stimulates MAPC activity. Certain anti-inflammatory drugs have altered the secretory component of cholera diarrhea. We investigated effects of these anti-inflammatory drugs on the MAPC. Indomethacin, 5.0 mg/kg iv, abolished all MAPC activity. Indomethacin, 1.5 mg/kg iv, or acetylsalicylic acid, 150 mg/kg given intragastrically, altered propagation velocity and at times its direction of propagation, but did not abolish the MAPC. An infusion of prostaglandin F2alpha, 2 microng/kg per min intraluminally, induced MAPC activity similar to that of the cholera complex. Indomethacin, 5.0 mg/kg iv, produced no significant changes in number of complexes or propagation velocity. These observations suggested that inhibition of the cholera complex by indomethacin may result from the alteration of prostaglandin synthesis and that prostaglandins may initiate the motility component of cholera diarrhea.

Oscillatory insulin secretion in perifused isolated rat islets.

After a step-function increase in glucose concentration, insulin secretion by perifused isolated rat islets of Langerhans showed oscillations superimposed on the well-known first- and second-phase secretory components. The oscillations were sustained for the length of the experiment and corresponded to at least four cycles. This established the existence of an oscillatory pacemaker with a narrow dispersion of periodicities intrinsic to the islets and showed that synchronization of islet action could be achieved by a step-function increase in glucose concentration. The observed period of 16 min is similar to the period of oscillatory insulin secretion in a number of intact organisms. This argues for identity of pacemakers in vivo and in isolated islets. This means that neural or other forms of interislet communication are not prerequisites for oscillatory insulin secretion. Theophylline increased the length of the oscillatory period, suggesting the periodicity of the pacemaker of insulin secretion can be metabolically regulated. This observation also provided a basis for explaining fine tuning of oscillatory periods by the nervous system.

Arachidonic acid stimulates mucin secretion in prairie dog gallbladder.

Mucin glycoprotein secretion from prairie dog gallbladder explants was studied in 24-h organ culture using [3H]glucosamine as a precursor. Indomethacin caused a reversible dose-dependent inhibition of both mucin release (50% inhibition between 10(-6) and 10(-5) M indomethacin) and 6-ketoprostaglandin F1 alpha (6-keto-PGF1 alpha) release (66% inhibition at 10(-7) M). Sepharose 4B chromatography of secreted [3H]-glucosamine-labeled glycoproteins revealed that indomethacin inhibited release of a high-molecular (greater than 10(6) daltons) mucin-type glycoprotein. Addition of sodium arachidonate (10(-4) M) to organ culture medium caused an approximate two- to fivefold increase (P less than 0.02) in mucin release compared with control and an increase in the secretory component of total glycoprotein synthesis from 37.5% in control explants to 75.2% (P less than 0.01). The stimulatory effect of arachidonate on mucin secretion was blocked by indomethacin. We incubated explants for 1 h with [14C]arachidonate and studied prostaglandin and thromboxane products in medium by high-performance liquid chromatography. The only conversion product identified was 6-keto-PGF1 alpha, the stable breakdown product of prostacyclin. Release of gallbladder mucin is significantly inhibited by indomethacin and increased by arachidonate. Mucin secretion may be regulated in part by endogenous levels of prostacyclin, the major cyclooxygenase product in prairie dog gallbladder epithelium.

Cholecystokinin in the regulation of intestinal motility and pancreatic secretion in dogs.

Peptidal (CR-1409) and nonpeptidal (L-364,718) cholecystokinin (CCK) receptor antagonists were used to determine the possible involvement of CCK in the fasted and fed intestinal motility patterns and the related alterations in pancreatic secretion. Dogs were implanted with electrodes along the small bowel and with chronic pancreatic fistulas. In fasted dogs, the typical migrating motor complex (MMC) cycles and accompanying fluctuations in pancreatic secretion were recorded. Neither of the CCK antagonists affected these motor and secretory components of the MMC. Feeding interrupted the MMC and increased spike activity at all levels of the small bowel, and this was accompanied by a significant increase in pancreatic secretion and in plasma hormone [gastrin, CCK, and pancreatic polypeptide (PP)] levels. Both CCK antagonists significantly reduced the postprandial spike activity but failed to restore the fasted pattern. Exogenous gastrin and CCK, as well as bombesin, induced fedlike motility patterns accompanied by marked pancreatic protein secretion. These effects were completely reversed to the fasted patterns during intravenous infusion of CCK antagonists. In contrast, cholinergic stimulation (bethanechol) induced a fedlike pattern that was more resistant to CCK antagonists. We conclude that CCK does not play a major role in the fasted motility pattern and related fluctuations in pancreatic secretion but may be partly involved (by itself and by released PP) in the induction of the fed motility pattern and the postprandial stimulation of the exocrine pancreas.

Base secretion in rat distal colon: ionic requirements.

To evaluate the ionic requirements of colonic base secretion, segments of rat distal colon were studied under short-circuited conditions. Net base flux was composed of an active secretory component and a diffusive component. Studied in the absence of a transepithelial HCO3- concentration gradient, active base secretion was dependent on the HCO3- concentration of the bathing solution but was not influenced by the CO2 tension or pH. Base secretion appeared to saturate with a Km of 33 +/- 9 mM and was inhibited by ouabain. The diffusive component was characterized by an apparent permeability coefficient to HCO3- of 8.9 +/- 0.9 x 10(-6) cm/s. In addition to requiring HCO3- on the serosal surface, net base secretion was inhibited by reducing the Na+ concentration in the serosal medium and the Cl- concentration in the mucosal medium. These data suggest that colonic base secretion involves HCO3- entry across the basolateral surface, energized by the Na+ gradient, and HCO3- exit across the apical surface in exchange for Cl-.

Protein transport across the lung epithelial barrier.

Alveolar lining fluid normally contains proteins of important physiological, antioxidant, and mucosal defense functions [such as albumin, immunoglobulin G (IgG), secretory IgA, transferrin, and ceruloplasmin]. Because concentrations of plasma proteins in alveolar fluid can increase in injured lungs (such as with permeability edema and inflammation), understanding how alveolar epithelium handles protein transport is needed to develop therapeutic measures to restore alveolar homeostasis. This review provides an update on recent findings on protein transport across the alveolar epithelial barrier. The use of primary cultured rat alveolar epithelial cell monolayers (that exhibit phenotypic and morphological traits of in vivo alveolar epithelial type I cells) has shown that albumin and IgG are absorbed via saturable processes at rates greater than those predicted by passive diffusional mechanisms. In contrast, secretory component, the extracellular portion of the polymeric immunoglobulin receptor, is secreted into alveolar fluid. Transcytosis involving caveolae and clathrin-coated pits is likely the main route of alveolar epithelial protein transport, although relative contributions of these internalization steps to overall protein handling of alveolar epithelium remain to be determined. The specific pathways and regulatory mechanisms responsible for translocation of proteins across lung alveolar epithelium and regulation of the cognate receptors (e.g., 60-kDa albumin binding protein and IgG binding FcRn) expressed in alveolar epithelium need to be elucidated.

Tubular localization of adaptation to dietary phosphate in rats.

The renal tubule of rats adapts its transport capacity for inorganic phosphate (Pi) in response to changes in dietary Pi intake. Tubular localization of this adaptation was studied in rats pair-fed for 10 days a low (LPD), i.e., 0.2 g/100 g, or a high (HPD), i.e., 1.8 g/100 g, phosphorus diet. Free-flow micropunctures under acute Pi infusion were made at elevated but similar filtered loads of Pi: LPD, 6.07 +/- 0.29 (n, 32); HPD 5.34 +/- 0.24 (n, 33) micromol/kidney per min. Fractional excretion of Pi (FEPi) for the whole kidney was: LPD, 0.26 +/- 0.02 (n, 32); HPD, 0.67 +/- 0.02 (n, 33) (P less than 0.001). The fraction of filtered Pi found at various puncture sites suggests that part of the adaptation to dietary Pi occurs in the early proximal tubule. An effect in the later portion of the proximal convoluted tubule cannot be excluded in the present experiments. Furthermore, dietary Pi affects markedly the difference in Pi delivery between distal tubule and final urine. This could be due to a greater adaptive response of deep nephrons. Alternatively, the results could also indicate that an adaptation with a secretory component takes place along the terminal part of the nephrons.

Augmented bidirectional HCO3 transport by rat distal tubules in chronic alkalosis.

Free-flow micropuncture studies show both augmented net HCO3 reabsorption in the distal tubule of rats with chronic metabolic alkalosis and higher HCO3 delivery to this nephron segment. The present studies in rats used in vivo microperfusion of surface distal tubules to investigate whether the augmented net reabsorption 1) was due to decreased HCO3 secretion and/or to increased proton secretion or 2) depended on the higher HCO3 delivery to the distal tubule. Artificial perfusates were designed to simulate in situ deliveries of HCO3 to the distal tubules of both alkalotic and control animals and to represent extremes of in situ Cl deliveries. Rather than being decreased, both measured and calculated HCO3 secretion were higher in the alkalotic animals for each perfusate used. Similarly, calculated proton secretion (difference between net HCO3 reabsorption and calculated HCO3 secretion) was higher for the alkalotic animals using each HCO3-containing perfusate. Augmented net HCO3 reabsorption by alkalotic animals was more clearly demonstrated using higher HCO3 deliveries and Cl-free perfusates. These studies demonstrate that both the reabsorptive and secretory components of net HCO3 transport are increased in the distal tubule of animals with chronic metabolic alkalosis.

In vivo adaptation of bicarbonate reabsorption by rat distal tubules during acid loading.

We carried out in vivo microperfusion experiments in acid-loaded rats to characterize the adaptive response of the unidirectional components secretory flux (Jsec) and reabsorptive flux (Jreab)] of distal tubule bicarbonate reabsorption and to test the hypothesis that Jreab is dependent on bafilomycin A1-sensitive H(+)-adenosinetriphosphatase activity. During 18 h of severe acidosis there was a significant decrease in Jsec (-15 +/- 3 vs. -38 +/- 5 pmol.min-1.mm-1, P < 0.05) and a significant increase in Jreab (37 +/- 6 vs. 0 +/- 5 pmol.min-1.mm-1, P < 0.05), which was insensitive to 10(-5) M bafilomycin A1, 10(-5) M Sch-28080, and 3 mM amiloride. After 3 days of acid loading, these same inhibitors reduced Jreab by approximately 60%. However, when water flux was completely inhibited by isosmotic perfusion, a significant Jreab (15 +/- 2 pmol.min-1.mm-1) resistant to 10(-5) M bafilomycin A1 persisted, as in severe acidosis. In reabsorbing distal tubules of overnight-fasted rats, Sch-28080 elicited no inhibition, whereas bafilomycin A1 and amiloride had significant effects (28 +/- 5, 24 +/- 4, respectively, vs. 50 +/- 4 pmol.min-1.mm-1 for fasted rats, P < 0.05). Thus, although Jsec is reduced in the transition from mild to severe metabolic acidosis of 18-h duration, the predominant effect is a stimulation of bafilomycin A1-resistant Jreab.

Mesangial deposition of J chain-linked polymeric IgA in IgA nephropathy.

Renal biopsy specimens from 8 patients with IgA nephropathy (Berger's disease) were examined immunocytochemically at the light and electron microscopic levels with peroxidase-labeled antibodies to IgA, IgM, J chain and secretory component. In 2 of the 8 specimens heavy deposits of IgA, but no IgM, were found in the mesangium. After acid-urea treatment of these tissue sections, J chain, a subunit of polymeric immunoglobulins, was identified in a distribution similar to that of IgA. In the remaining 6 specimens, small amounts of IgM in addition to denser deposits of IgA and J chains were found. We conclude that the IgA deposits in at least some patients with Berger's disease consist of IgA polymers linked by J chain.

Immunomorphological characteristics of human Peyer's patches.

Human follicle-associated epithelium (FAE) was found not to express the secretory component (SC) or polymeric immunoglobulin (pIg) receptor, and is therefore unable to transport pIgA to the gut lumen. However, human FAE (except the M cells) was positive for MHC class II (HLA-DR) determinants. It may therefore perform class II-restricted uptake and presentation to T cells of antigens that have been adequately processed, whereas the function of the M cells is perhaps limited to transport of particulate or undergraded luminal antigens to subjacent antigen-presenting cells (APC). A large number of APC of the inter-digitating cell phenotype (positive for HLA-DR and S-100 protein) and relatively few L1-positive reactive macrophages were found in PP between the follicles and beneath the FAE, compared with villous mucosa. Also, there were significantly more intra- and subepithelial T cells in PP than in distant villi, and the T cells were concentrated adjacent to the M cells. The proportion of the CD4+ subset (putative helper T cells) was much higher in FAE (40%) than in villous epithelium where the CD8+ subset (putative suppressor T cells) predominated strikingly (90%). This disparity might reflect differences in capacity for positive and negative immune regulation at the two sites. The relatively few B cells terminating with Ig production in PP apparently belonged to relatively mature memory clones as they showed a large proportion of IgG immunocytes and reduced J-chain expression. Conversely, both IgG and IgA immunocytes in distant mucosa showed a high percentage of J-chain positivity (80-100%); such expression was also considerable (45-60%) in mesenteric lymph nodes (MLN) in contrast to peripheral lymph nodes (PLN) and palatine tonsils. Moreover, there was a decreasing percentage of IgA2 immunocytes in the order of PP (52%), distant ileal mucosa (40%), MLN (32%), PLN (11%), tand tonsils (5%). These results support the notion that migration of relatively immature memory B-cell clones takes place from PP through MLN preferentially to distant intestinal mucosa.
