An open label study of clobetasol propionate 0.05% and betamethasone valerate 0.12% foams in the treatment of mild to moderate acne keloidalis.

Acne keloidalis (AK) is a disease affecting primarily African American men. Topical steroids are a widely accepted treatment of AK; however, no studies have been published investigating their effectiveness. The purpose of this open-label study was to assess the efficacy and tolerability of clobetasol propionate 0.05% and betamethasone valerate 0.12% foams in the treatment of AK in 20 African American patients. These patients were treated for 8 to 12 weeks using a pulsed-dose regimen. We found topical clobetasol propionate foam to be effective in improving AK, and our patients found the foam vehicle to be cosmetically acceptable.

Keratitis-ichthyosis-deafness syndrome in association with follicular occlusion triad.

Keratitis-Ichthyosis-Deafness syndrome is a rare congenital disorder of the ectoderm caused by mutations in the connexin-26 gene (GJB2) on chromosome 13q11-q12, giving rise to keratitis, erythrokeratoderma and neurosensory deafness. We report the case of a 31-year-old black male diagnosed as having KID syndrome. Sequencing analysis showed a heterozygous missense mutation D50N (148G > A) in the GJB2 gene. In addition to the classical features of vascularizing keratitis, erythrokeratoderma and congenital deafness, our patient presented a follicular occlusion triad with hidradenitis suppurativa (HS, alias acne inversa), acne conglobata and dissecting cellulitis of the scalp, leading to cicatricial alopecia and disfiguring, inflammatory vegetations of his scalp. Conservative therapy such as a keratolytic, rehydrating and antiseptic external therapy, antibiotic, antimycotic and retinoids were only of moderate benefit, so we finally chose the curative possibility of surgery therapy of the axillar papillomas and of the scalp. The inflammatory papillomatous regions of the axillae and of the scalp were radically debrided. Clean granulation was awaited and covered in a second session with a mesh graft from the thigh, achieving a satisfactory result. To our knowledge, only one case of KID syndrome occurring in association with follicular occlusion triad has been reported before.

The importance of socio-economic status and individual characteristics on the prevalence of head lice in schoolchildren.

Pediculosis is a common infestation in schoolchildren but little is known about the factors influencing its prevalence. The aim of this study was to determine the prevalence of head lice in schoolchildren in Ghent and to investigate the independent association between individual characteristics of the child, socio-economic status (SES) of the family and head lice. The prevalence of head lice at baseline and 14 days after treatment advice was determined by the wet combing method in a total of 6,169 schoolchildren age 2.5 to 12 years from Ghent (Belgium). Age, sex, educational level and hair characteristics of the child, SES of the family, and number of children in the family was collected by the school health department. The prevalence of head lice was 8.9%. The only statistically significant factors at the child level are SES, the number of children in the family, hair length and hair colour. Treatment failure was recorded in 41% of the children positive at baseline screening and was significantly related to hair colour and SES. This study demonstrated that the prevalence of head lice is determined by clustering of children rather than by characteristics of the child. The management of head lice should take a community-based approach equally involving families, schools, health care professionals and the government.

Metastatic gastric adenocarcinoma presenting as an enlarging plaque on the scalp.

We present a case report of metastatic gastric adenocarcinoma in which the cutaneous metastases were the first sign of disease recurrence. A 73-year-old man presented with painless red plaques on his scalp and forehead. He was diagnosed with gastric carcinoma with metastases to perigastric lymph nodes 3 1/2 years earlier. Histopathology results revealed signet-ring cells consistent with gastric adenocarcinoma. The patient failed to respond to treatment with intralesional interleukin 2, previously reported to be effective, and expired 7 months later.

Natural history and treatment of scalp psoriasis.

The scalp is a common site of involvement at onset and throughout the course of psoriasis. Worldwide census studies report that approximately half of the psoriasis population are afflicted with psoriasis of the scalp. We describe the frequency, clinical aspects, and treatment of scalp psoriasis in childhood, adulthood, and in the geriatric age groups. Measures of prevention and control are reviewed.
