A 34-year-old man with digital ischemia is reported. Angiography revealed thromboembolic occlusions of the proper digital arteries of the index, middle, and ring fingers and a tortuous ulnar artery in Guyon's canal. Though hypothenar hammer syndrome was suspected, there was no relevant occupational history. Magnetic resonance imaging and magnetic resonance angiography demonstrated an anomalous muscular sling around the ulnar artery immediately adjacent to the hook of the hamate. The ulnar artery showed mural thrombi in its tortuous segment. These findings were confirmed during operative exploration. After thrombectomy and embolectomy the involved segment of the ulnar artery was replaced by an autologous vein graft. Postoperatively there was complete resolution of the symptoms. Only during convalescence did it become clear that the patient was a passionate golfer.

RATIONALE: Tablets sold as ecstasy often contain not only 3,4-methylenedioxymethamphetamine (MDMA) but other compounds well known to cause dopaminergic neurotoxicity, such as (meth)amphetamine. Furthermore, the use of ecstasy in the Netherlands is often combined with the use of amphetamine. However, little is known about the effects of ecstasy use or the combination of ecstasy and amphetamine use on dopamine (DA) neurones in the human brain. OBJECTIVES: This study was designed to investigate the effects of ecstasy as well as the combined use of ecstasy and amphetamine on the density of nigrostriatal DA neurones. METHODS: [123I]beta-CIT SPECT was used to quantify striatal DA transporters. Striatal [123I]beta-CIT binding ratios of control subjects ( n=15) were compared with binding ratios of ecstasy users ( n=29) and individuals with a history of combined ecstasy and amphetamine use ( n=9) after adjustment for age. RESULTS: Striatal [123I]beta-CIT binding ratios were significantly lower in combined ecstasy and amphetamine users compared to sole ecstasy users (6.75 versus 8.46, respectively: -20.2%, P=0.007). Binding ratios were significantly higher in ecstasy users when compared to controls (8.46 versus 7.47, respectively: +13.2%, P=0.045). CONCLUSIONS: These initial observations suggest that the sole use of ecstasy is not related to dopaminergic neurotoxicity in humans. In contrast, the reported use of amphetamine by regular users of ecstasy seems to be associated with a reduction in nigrostriatal DA neurones.

RATIONALE: 3,4-Methylenedioxymethamphetamine (MDMA) is known to affect brain serotonin (5-HT) neurons in experimental animals. However, its effects on humans are more difficult to infer. Serotonin is implicated in the body's ability to modulate the effects of pain and to regulate mood. OBJECTIVE: The aim of this research is to test nociceptive responses and mood in MDMA users as an index of central 5-HT function. METHOD: Measurements of pain tolerance were obtained using the cold-pressor test for 15 polydrug users who regularly use MDMA, 3-4 days after the most recent usage, and ten matched polydrug users who do not use MDMA. A rating on mood was obtained for each participant using the Nowlis Mood Adjective checklist. RESULTS: Measurements of pain tolerance and mood were significantly lower in the MDMA group. A positive correlation between mood and pain tolerance was found in the MDMA group, whereas no correlation was found between these variables in the non-MDMA group CONCLUSION: This study found an association between pain tolerance and MDMA usage and confirmed the association between MDMA and depressed mood. The current results suggest that MDMA, at least in the short term, may cause serotonin-mediated alterations in pain sensitivity.
/ The travel cost method was used to estimate the economic value of seven major power boating resources in Pennsylvania. A significant relationship between number of visitor trips and cost per visitor trip existed for five of the seven water bodies. The annual total value for those five resources was $396 million, which was 2.5 times greater than the total out-of-pocket expenditures of approximately $157 million visitors spent to visit them. Research results can help resource managers plan and craft programs and policies that are founded on economic values of the natural resources involved.
OBJECTIVE: This pilot study was designed to measure tooth wear in a sample of 13 subjects who regularly use amphetamine-like drugs (Ecstasy, amphetamines) and compare the observed wear with a matched sample of nondrug users. DESIGN: The two groups, both composed of 13 undergraduate students, were matched for age and sex. Other factors influencing tooth wear were controlled by matching the groups on their responses to a questionnaire asking about recognised common causes of tooth wear. The participants teeth were examined and the degree of wear scored according to a tooth wear index. RESULTS: Severity of occlusal tooth wear of the lower first molar teeth was significantly greater in the drug user group than in the control group (P<0.05). No other statistically significant differences between the groups were found. CONCLUSION: Regular use of amphetamine-like drugs could be associated with increased posterior tooth wear.
This report describes the case of a 59-year-old woman with a history of non-Hodgkin's lymphoma who developed bacteremia with Vibrio vulnificus. The patient had been swimming in the unusually warm Baltic Sea in the summer of 2002. She presented with symptoms of septicemia and severe bullous necrotizing skin lesions of the extremities. Blood culture revealed Vibrio vulnificus as the pathogenic organism. Under treatment with cefotaxime and gentamicin, she recovered slowly without further complications. Vibrio vulnificus is a marine bacterium that is present in aquatic ecosystems worldwide, especially when water temperatures exceed 20 degrees C. Infections with Vibrio vulnificus are uncommon in Europe, and most cases are reported from subtropical or tropical regions.

The effect of physical exercise on lipid and apoprotein levels was studied in 31 healthy fertile women (mean age, 39.7 +/- 2.3 years) working as civil servants and leading a mostly sedentary way of life (group 1). A control group consisted of 31 age-matched women (mean age, 39.2 +/- 2.4 years) with a comparable life-style (group 2). Group 1 performed physical exercise for at least 30 minutes three times per week. They also climbed a 500-m hill at least once per week. The study lasted 6 months, ie, from May to November 1990. Changes in maximum oxygen consumption (Vo2max), body weight, body mass index (BMI), waist to hip ratio (WHR), and levels of lipids and apolipoproteins (apos) A-1 and B were compared between the two groups of subjects. During the May-November period, the control group showed an increase in body weight (P < .02), total cholesterol, high-density lipoprotein (HDL) cholesterol, HDL3, and low-density lipoprotein (LDL) cholesterol (P < .01) and a decrease in HDL2 (P < .05). In contrast, group 1 did not show any increase in total cholesterol, and their body weight decreased (P < .01). Very-low-density lipoprotein (VLDL) cholesterol and triglyceride levels decreased (P < .02), as did LDL cholesterol and HDL2 levels (P < .05), whereas HDL cholesterol and HDL3 levels increased (P < .01). There were no statistically significant changes in WHR and apo A-1 level. The findings indicated possible seasonal variations in lipoprotein levels in group 2.(ABSTRACT TRUNCATED AT 250 WORDS)
