Practical assessment and evaluation of mental health problems following a mass disaster.

Almost all individuals who experience a severe trauma will develop symptoms of posttraumatic stress disorder (PTSD) shortly after the traumatic event. Although the natural history of PTSD varies according to the type of trauma, most people do not develop enduring PTSD, and, in many of those who do, it resolves within 1 year without treatment. To the extent that is possible, maintenance of normal daily activities is believed to help patients cope more successfully in the aftermath of major trauma. In the case of a disaster such as the Asian tsunami, the whole community is involved, and it is impossible to continue with normal daily activities. To improve overall outcome after trauma, it would be optimal to identify individuals at increased risk for developing PTSD. This article describes screening and assessment tools for posttrauma mental health problems, particularly PTSD, and examines in more detail instruments that can be used in rapid field assessment of individuals who may be affected or who have already been identified and require monitoring. Self-rated instruments are most appropriate, but the choice of instrument will depend on the local situation and availability of appropriately validated questionnaires. The article also addresses important aspects of training nonmedical personnel in screening and assessment.

An Intelligent 802.11 Triage Tag for medical response to disasters.

When medical care is initiated at a mass casualty event, the first activity is the triage of victims, which is the grouping by victims severity of injury. Paper triage tags are often used to mark victims' triage status and to record information on injuries and treatments administered in the field. In this paper we describe the design and development of an"Intelligent Triage Tag" (ITT), an electronic device to coordinate patient field care. ITTs combine the basic functionality of a paper triage tag with sensors, nonvolatile memory, a microprocessor and 802.11 wireless transmission capabilities. ITTs not only display victims' triage status but also signal alerts, and mark patients for transport or immediate medical attention. ITTs record medical data for later access offsite and help organize care by relaying information on the location of the victims during field treatment. ITTs are a part of the Wireless Information System for Medical Response in Disasters (WIISARD) architecture.

Wireless distribution systems to support medical response to disasters.

We discuss the design of multi-hop access networks with multiple gateways that supports medical response to disasters. We examine and implement protocols to ensure high bandwidth, robust, self-healing and secure wireless multi-hop access networks for extreme conditions. Address management, path setup, gateway discovery and selection protocols are described. Future directions and plans are also considered.

A simple fluorogenic method to detect Vibrio cholerae and Aeromonas hydrophila in well water for areas impacted by catastrophic disasters.

The colony overlay procedure for peptidases (COPP) is a simple, fluorogenic assay that can rapidly detect and quantify Vibrio cholerae and Aeromonas hydrophila in well water. Cleavage of the substrate L-lysyl-7-amino-4-trifluoromethylcoumarin by enzymes present in Vibrio and Aeromonas species produces fluorescent foci on cellulose acetate membranes exposed to long-wave ultraviolet light. Vibrio cholerae O1, O139, O155, and A. hydrophila were readily detected using this procedure, whereas Enterobacteriaceae and other non-Vibrionaceae pathogens did not produce fluorescence. The assay is practical for assessing the relative safety of well water in areas that have experienced catastrophic devastation from natural disasters, acts of war, or civil strife and may help curb outbreaks of cholera and other enteric illnesses in affected areas. In tropical climates, the procedure may be adapted for use in areas without electricity.

Triage of disaster-related neuropsychiatric casualties.

Existing triage algorithms consider, for the most part, only the primary casualty with physical trauma. Algorithms fail to appreciate the primary, secondary, or tertiary neuropsychiatric casualty. Research advances on neuropsychiatric casualties must link with the mandates of emergency medical services and disaster management to improve triage sensitivity and specificity. Early recognition and management of neuropsychiatric casualties will diminish the potential for long-term consequences. Expanded triage algorithm supplements are proposed to improve the recognition of those victims at risk.

Flugtag-88 (Ramstein Air Show Disaster): an Army response to a MASCAL.

A successful response to a mass casualty event must include trained personnel, adequate facilities, and abundant supplies. The medical staff must strictly adhere to triage and classification standards. Landstuhl Army Regional Medical Center (LARMC) triaged, within 30 minutes, 120 casualties from the Ramstein Air Show Disaster--14 patients were admitted, 86 were stabilized and transferred to German facilities, 16 were treated and released, and 4 were expectant. There were 6 subsequent transfers to LARMC. Seven patients were operated upon that day, 7 patients were eventually transferred to Brooke Army Burn Unit, and 3 patients underwent subsequent delayed reconstructive surgery.

[Stomatological identification after air disasters].

The paper describes experiences of stomatological identification after two air disasters and shows potentialities and limits of an odontologic(al) comparison. The application of a 16-bit-computer is pointed out as the essential improvement of the work.

[Aviation support of measures for rendering medical care to the victims of natural disasters, accidents and catastrophes].

The unpredictable character of natural calamities and technological accidents makes some specific demands on the organization and planning of the wide range complex of social measures to abolish their awful and tragic consequences. Analysing and summarizing the reviews and reports of international congresses on the emergency and catastrophic medicine and their own experience, the authors accentuate attention on the advantages and capabilities of aerial transport in delivering of medical personnel, all necessary equipment to the zones of mass sanitary losses and its really essential role in rapid aeromedical evacuation of injured. The authors scrutinize in details the structural scheme of organization of patient's search-and-rescue work and suggest, that main principles of planning and management of search-and-rescue operations in aviation could be applied in a decision of problems, which arise in medical assistance to the victims of the natural disasters and accidents.

Cholera epidemic and natural disasters; where is the link.

In May 1985, a cyclone and tidal surge devastated Sandwip, an island off the Southern Corner of Bangladesh. Within one week after this disaster a cholera epidemic broke out. It resulted in 12,194 registered cases and 51 deaths. The factors contributing to the occurrence of the epidemic are analyzed.

Toxic chemical disasters and the implications of Bhopal for technology transfer.

The dramatic disaster in 1984 at Bhopal, India, may be overshadowed in total impact by less immediate health effects characterized by long latency, cumulative damage, and subtle impairments. Transfer of chemical technology must be accompanied by transfer of the corresponding infratechnology, toxicology, only then can the process of technology transfer be managed with fewer risks, fewer costs, and fewer tragic surprises.

Scene investigation, identification, and victim examination following the accident of Galaxy 203: disaster preplanning does work.

Galaxy Airlines Flight 203 crashed following takeoff from Reno-Cannon International Airport on 21 Jan. 1985. Sixty-eight persons on board the aircraft perished in the initial crash and resultant fire which followed. Two victims expired as a result of crash injuries within subsequent days and one passenger survived. A community disaster response plan was in place and had been practiced by local government agencies before this incident. The successes of this preplanned response, as well as methods of actual recovery, identification, and examination of the victims is presented.

[Organization of emergency health services in disasters].

Since 1982 the St. Radboud hospital, Nijmegen, has a contiguity plan for peace-time disasters which describes the way to deal with great numbers of patients of traffic-, environmental- or industrial-calamities. The function of the commander, the separate hospital services, the medical and nursing triage team and administrative services are clearly delineated. Also is indicated how to organise communication between hospital, municipality, police and fire-brigade. The plan also provides possibilities for information towards press and relatives of the victims.
