BACKGROUND: Most antiobesity drugs act centrally to reduce appetite or increase satiety. Orlistat is the first in a new class of drugs targeted at a single dietary component, in this case dietary fat. OBJECTIVE: To review the clinical actions and efficacy of orlistat and to discuss its place in overall weight management. DISCUSSION: Orlistat is best used in long term weight management as an adjunct to dietary modification and increased physical activity. The reduction in fat absorption results in a slow but sustained weight reduction and improved metabolic parameters such as reduced total, and LDL cholesterol. Side effects are minimised by maintaining a low fat diet.
Authors revealed considerable decrease substance of vitamins at patients with chronic tonsillitis in depending on degree of difficulty illness and presence of complication. Content of vitamins B1, B2 and C is decreased more than rest. Results of immune investigations register lowering phagocytic activity of leucocyte and cell immunity at patient with chronic tonsillitis in the presence of the complication.

OBJECTIVES: To evaluate the consumption of energy and nutrients and to identify the risk of inadequate intakes in the Canary Island population (1997-98). DESIGN: Cross-sectional epidemiological study. METHODS: A dietary survey was conducted in a representative sample of the Canary Island population (n = 1747; 821 men and 926 women) aged 6 to 75 years. Two 24-hour recalls were utilised as the dietary survey instrument, carried out over non-consecutive days. Spanish Food Composition Tables were used and data was adjusted for intraindividual variability. RESULTS: The mean daily intakes were 1760 kcal for energy, 73 g for protein, 228 g for carbohydrates, 15 g for fibre and 62 g for fat (24 g SFA, 25 g MUFA and 8 g PUFA). Energy and nutrient consumption decreased with age excluding vitamins A. C and folate. Nutrient density increased with age, with the lowest intakes seen in children and adolescents. As for social class, decreased calorie intake was observed in the lowest category, with the highest income level showing the greatest intakes for vitamin A, B12, niacin and folate. Noteworthy findings include an overall low calorie intake, and elevated risks of inadequate intakes (percentages of the population with intakes below 2/3 of the RDI) for vitamins D (92.5%), E(87.4%), A(74%), folate(44.7%), iron(30.1%) magnesium (14.9%) and vitamin C (5.4%). Risk for inadequate protein intake was not observed.

Inclusion of vitamin-containing soft drink and oil solution of beta-carotene in the diet of the 4-14 years old children suffering from gastrointestinal diseases lead to vitamin and antioxidant status improvement with simultaneous positive effects on their clinical condition. Polyhypovitaminosis frequency had decreased. Thus, there is every reason to fortify diets of the patients suffering from gastrointestinal diseases with vitamins-antioxidants.

SETTING: Bugando Medical Centre, a referral consultant hospital in the lake zone to which all inmates from Butimba prison are admitted and treated. OBJECTIVE: To determine the extent of open (smear-positive) tuberculosis and other types of tuberculosis among prisoners with tuberculosis. DESIGN: A retrospective cohort study. Case notes of 501 prisoners from January 1994 to December 1997 were retrieved and reviewed. RESULTS: The proportion of open tuberculosis in this study was high, with 204 prisoners (40.7%) having smear-positive tuberculosis. Co-infection was recorded in the majority of patients; HIV/AIDS was recorded in 25.9% of cases. The mean length of imprisonment at the time of diagnosis was 19 months. CONCLUSION: The proportion of open, smear-positive tuberculosis among prisoners admitted with tuberculosis is high. Intervention measures specifically targeting this population are urgently needed in order to contain tuberculosis disease in the prison population.
Surveys of 28 children have shown that 64 per cent of the children suffering from ulcerous disease of the stomach and the duodemun or chronic duodenitis have a combined deficiency of vitamin E, vitamins of the B group and beta-carotene. Diet No. 1 and No. 5 could not provide daily needs for vitamins. The patients were found to have a lower antioxidant activity of the blood serum, whereas the contents of hydroperoxides, double bonds and malonic dialdehydes were higher than in healthy children. The POL index in children with high beta-carotene deficiency was significantly higher than that in ailing children supplied with this carotenoid. Correlation analysis has revealed a reverse relationship between concentration of vitamin E and beta-carotene in blood serum and POL products, i.e. the better a child is supplied with these vitamins, the lower is the level of POL products. When the drink with the whole set of vitamins and an oil solution of beta-carotene was included in the diet of the children, it was found that hypovitaminosis frequency lowered with positive effects on their clinical condition and improved POL indexes. So there is a reason to fortify diets of children suffering from gastrointestinal disorders with vitamins-antioxidants.
