[Urinary complications and sequelae of carcinoma of the cervix treatment (author's transl)].

The authors describe the sequelae and urinary complications of:--radiotherapy only in 524 patients, stage I to III treated between 1960 and 1974;--721 operations performed between 1968 and 1974, according to the protocol of treatment as used in the Fondation Curie. This reveals:--for irradiated patients, no iatrogenic damage to the upper urinary tract, whilst bladder complications were often present. No major complication was found in this group;--for operated patients, the surgical act was without harmful consequence in the 672 cases when the operation was foreseen in the initial protocol (0,6% of chief complications). On the other hand, surgery performed for persistent tumour or recurrence after exclusive radiotherapy showed 8 per cent of chief complications. In this group they analyse the factors that might influence ureterohydronephroses, especially the extent of node dissection and associated external irradiation.

Ectopic calcification as a late sequela of compartment syndrome. Report of two cases.

Ectopic calcification occurred in two patients as a late sequela of compartment syndrome. The clinical and roentgenographic appearance are sufficiently typical that the diagnosis should be made without difficulty. Leaving the wound open after incision and drainage of these lesions may lead to secondary infection. Closing the wound after decompression over suction drainage or needle decompression are treatment alternatives that may lead to more satisfactory results.

[The prevention of late psychological sequelae following therapeutic termination of pregnancy. The role of the doctor during consultations (author's transl)].

Legal termination of pregnancy only rarely has psychological sequelae, which furthermore are usually moderate and transitory. They are usually found in women who have been pressed to have a termination by third parties, in those women who are ambivalent as far as the termination is concerned, in those who submit to the operation because of a medical indication and in those women where there is a brittle psychological state before pregnancy. The author makes certain suggestions which should help to prevent psychological troubles developing in a certain number of cases.

Long-term sequelae of acute venous thrombosis.

The long-term sequelae of acute deep-vein thrombosis were examined prospectively in a series of 61 patients. The location and extent of the involvement were established by either contrast or isotope phlebography. The mean age of the patients was 47 years, with the duration of follow-up averaging 39 months (range, one to 144 months). Pain and/or swelling was noted in 67% of the patients. Pigmentation developed in 15 limbs (23%). Ulceration developed in three patients. The most important factor with regard to prognosis appeared to be the status of the distal deep veins. If these were found to be patent and competent by ultrasonic velocity studies, the long-term outlook was good, both with regard to symptoms and the development of pigmentation. Only 8% of those limbs with normal distal veins were found to have pigmentation during follow-up, as compared with 40% in those with either occlusion or incompetence of the valves.

[On the proof of late sequelae of bulbal and orbital contusion (author's transl)].

Contusion cataracts may develop long time after injury, without visible lens changes in the interval. The lens opacities appear like those known in senile cataracts if the eye was injured after the 45th year of age. Demonstration of other traumatic changes becomes then very important for the evaluation of the role of the accident as a cataract cause. As late sequelae of eye (and orbit) contusion, enophthalmos and supersensitivity of the pupil to adrenergic drugs can be found. Ultrasound exophthalmometry proved to be more accurate for checking the former than Hertel's optical exophthalmometry alone. The pupil was studied by the reaction following local instillation of one drop of 1% adrenaline solution (comparison with other eye). The results found in 2 patients are described.

[Sequelae of burns of the lips (author's transl)].

This report attempts to clearly define the polymorphic nature of sequelae from burns of the lips by introducing a simple classification based on the direct or indirect, and minor, moderate, or major nature of the affection. Understanding of the processes involved in scar formation is essential for developing a rational plan for surgical repair. However, the ideal situation would be to prevent scar formation by fixation-compression, the use of an orifice shaper, or by excision-grafting of facial burns associated with these procedures.

Sudden death: bereavement sequelae and interventions.

Just the phrase "sudden death' carries its sense of trepidation and even horror. So fear-inducing is its reality and prospect, that we have attempted to sanitise it by the colloquialisms such as a "sudden death play off' in sport, and even (recalling my days in military service) as the name of a particularly ferocious mix of alcoholic beverage. One of the reasons why the phrase is so evocative is the force of the unexpected bereavement that follows sudden death. This paper purports to examine, using a systems approach, the psychological sequelae of the emotionally involved survivors of sudden death, and to relate this to possible intervention strategies.

Surgical experience with coronary arterial sequelae of Kawasaki disease in children.

The coronary arterial sequelae due to Kawasaki disease have been treated surgically in five children ranged from 6 to 9 years old. The procedures were aortocoronary bypass surgery in all cases, coronary artery aneurysmectomy in one case and left ventricular aneurysmectomy in one case. The early results of operated cases have been good with the graft patency of 86% confirmed by angiography one month after surgery. The characteristic features of coronary artery damage in patients with Kawasaki disease are coronary artery aneurysms, which manifest wall irregularity, thrombus, calcification and stenosis. The significant stenosis of coronary artery could often be observed at the inlet or outlet of the aneurysm in major coronary artery branches. Although the early results of aortocoronary bypass surgery in the children with Kawasaki disease have been good, long-term follow up is mandatory to investigate the status of autogenous saphenous vein grafted in children.

[Long-term sequelae of neonatal group B streptococcal septicemia/meningitis (author's transl)].

In recent years group B streptococci accounted for nearly 40% of all cases of neonatal septicemia in our intensive-care-unit. Nineteen of 38 babies did not survive the acute illness (mortality 50%). Six of 18 surviving children showed abnormalities related to the septicemia/meningitis (morbidity 33%). The severity of chronic complications ranged from minor neurological problems to marked retardation, deafness, blindness, and epilepsy. In our series there seemed to be a relation between the severity of complications during the acute illness (meningitis, convulsions), and later neurological sequelae. Early detection and early treatment were found to be most important for the final outcome.

[Sequelae of the resection in peptic ulcer (author's transl)].

Clinical symptomatology of early and late sequelae after resective surgery in peptic ulcer may be dramatic or insidious; insidious changes for instance may occur in the gastric mucosa or they may be due to chronic deficiency of elementary dietary components. Loss of pylorus function and duodenal-gastric reflux are the most important causes for these symptoms.

Sequelae of transcallosal surgery.

The Sequelae of transcallosal surgery were assessed clinically in 15 children Isolated transection of the corpus callosum does not leave any pronounced when there are no lesions of the cerebral hemisphere. In cases with disturbances of some centers or analyzers, especially in the proprioceptive analyzer in the cortex of the parietal lobe, loss of recent memory, hypokinesia, ideomotor apraxia and astereognosia may develop. The sings are mixed and it is difficult to classify them. We can speak of disjunctive syndromes. The transcallosal approach is suitable in the case of lesions in the anterior part of the third and lateral ventricles.

[Sequelae of vagotomy in peptic ulcer (author's transl)].

Sequelae of vagotomy in peptic ulcer are dysphagia, dumping syndrome, gastric emptying disturbances, diarrhoea, and functional disturbances of liver, bile ducts and pancreas. All these disturbances, except dysphagia, are less pronounced after high selective vagotomy than after surgical procedures including drainage techniques. Relapsing ulcer is the most frequent complication; this is rarely a direct consequence of vagotomy, but it is more likely due to non-complete transection of the nerve or to transections not indicated.

Classification and surgical management of the severe sequelae of septic hips in children.

The treatment of the acute state of septic arthritis of the hip in children has been clearly outlined in the current literature. The treatment of residual anatomic deformity is less well understood; no classification or comprehensive treatment program has been documented. The following classification of the sequelae of septic hips in children is based on the presence or absence of a capital femoral epiphysis and hip stability. This classification defines the case material and outlines the anatomic problem requiring solution. Of ten patients with severe destruction of the femoral head followed for an average period of 11.2 years, there were eight satisfactory and two unsatisfactory results. Both unsatisfactory results were Type III hips with pseudarthrosis of the neck. Both had an average of five surgical procedures, compared with 2.2 procedures for patients with satisfactory results. The complications were leg-length discrepancy, hip instability, and scoliosis.

[Survey on the prevalence of sequelae of poliomyelitis of the lower limbs in school-age children in rural areas of the Department of Niamey, Niger].

An investigation of lower limb motor disorders among school age children in three rural areas of Niger has demonstrated a prevalence rate of approximately seven per thousand in the 5-9 and 10-14 years age-groups, with no significant difference in rates for boys and girls. Although severe (bilateral) paralysis is encountered only among non-attenders, no significant difference in overall prevalence is found with regard to school attendance status. Eight per cent of affected children are unable to walk and a further eight per cent need crutches or other aids. Patient histories indicate that over 90 per cent of cases occur before the child's fourth birthday; there is no indication of a modification in incidence over the past nine years. Extrapolation from the observed data indicates an annual incidence rate for paralytic poliomyelitis of the order of 45 per hundred thousand population, i.e. about 2,500 cases a year for the whole of Niger, approximately 2,000 of whom survive with a permanent disability. These rates and figures are similar to those calculated for several other african countries and are seven to ten times higher than the figures gathered from routine reports of poliomyelitis in Niger. The total number of children unable to walk unaided among the 5-14 years age-group can be estimated in 1981 at 1,600, half of whom are paralysed in such a way that they are unable to walk at all. Other preventable causes of paralysis of the lower limbs encountered include i.a. tuberculosis of the spine, sequelae of cerebrospinal meningitis and, second only to paralytic poliomyelitis in importance, trauma to peripheral nerves through faulty injection technique.

[Effect of alcohol on interhemispheric functional relations in patients with late sequelae of cranio-cerebral injuries].

Using the backward masking method the time of perception of visual stimuli in the right and the left field of vision was examined. It was found that in patients with craniocerebral injury sequelae, as well as in healthy subjects the time of perception of literal stimuli was shorter in the left field of vision, i.e. the information entering the right hemisphere was processed more rapidly. Under the action of alcohol the asymmetry of the perception time disappeared in the patients, and became inverse in the healthy subjects, i.e. the time of perceiving letters in the left field of vision got longer than in the right field. Under the action the right hemisphere seemed to lose its advantage in the speed of the stimuli perception. It is supposed that the right hemisphere is more sensitive to alcohol.

Etiology, pathogenesis and sequelae of chronic hepatitis.

Chronic hepatitis is the commonest clinical picture with liver involvement and regarded as the link between acute liver injury due to viruses or toxic noxae, and irreversible, complete hepatic cirrhosis. For reasons associated with diagnosis, prognosis and therapy it is essential that a distinction should be made based on the etiology and stage of the disease but assessment is only partly possible by clinical or biochemical procedures. This paper states etiology, pathogenesis and sequelae of chronic hepatitis on the basis of our cases.

[Secondary amputation of the penis in the sequelae of local therapy with iridium 192 for carcinoma (author's transl)].

The author reports 4 cases of severe delayed irradiation complications which required partial or complete secondary amputation of the penis for sequelae of local therapy with Iridium. After reviewing the modes of spread of carcinoma of the penis and the various therapeutic possibilities in the light of these 4 cases, the author raises the question as to whether such conservative treatment by interstitial radiotherapy is well-founded. He would tend to suggest immediate surgery giving an identical result at the price of much shorter treatment and which would be free of the complications reported following radiotherapy.

Characteristics of women recruited to a long-term study of the sequelae of induced abortion. Report from a joint RCGP/RCOG Study co-ordinated by the Royal College of General Practitioners' Manchester Research Unit.

One thousand, five hundred and nine general practitioners and 795 gynaecologists are co-operating in a large, long-term, controlled, prospective study of the sequelae of induced abortions. The ;case' (6,349 patients) and control (8,132 patients) groups are comparable, and the patients who had induced abortion are also comparable in age, marital status and parity with those in the national abortion statistics. The study should, therefore, give a sound basis for evaluating the long-term hazards of the operation.
